Mostly caused by not scanning patients bar code on identification band and keying input into the Glucometer incorrectly.

Patient ID Not Found

Caused by not inputting a Health Care Provider (HCP) ID or the correct HCP ID after doing the blood glucose test.

Login to Essentris

Locate Health Care Provider assigned to patient & annotate.

Click on “Find Patient”

Search both [X] Active [X] Archived files

Click on “Administration Tab”

Search last four of patients SSAN “*1234”

Locate patient in list by matching SSAN and error date.

Login to Essentris

Locate patient in CHCS system.

Click on “Find Patient” and follow steps in preceding slide.

Focus on error date and try to match POCT SSAN with log

Click on “Administration Tab”

Check Admissions and Transfers Log Book for patient SSAN

Check 24 Hour Report Error Date with last four of pts SSAN

If a patient does not have an arm band, request one from Admissions.

Be extremely careful in keying in patient correct ID information.

Remember to input HCP ID after each test.

Scan Patient’s Bar Code

- Essentris- Administration Tab- Find Patient- Search last four of

SSAN- [ ] Active - [ ] Archive

- Match SSAN & error date

- Find HCP and annotate- Check Admissions &

Transfers Log Book

- Check 24 Hour Report- Check CHCS- Keys to correcting

error:- Last 4 of SSAN- Test Date / Time

- Remember to input HCP ID after each test.

Correcting Errors

Always scan patient bar code on their arm band.
