In its determination towards ensuring improved maternal, newborn and child health in Nigeria, the Nigerian Urban Reproductive Health Initiative (NURHI 2) project narrates one of its proven interventions - 72-Hour Clinic Makeover concept and process. The NURHI 2 project is built on the premise that demand for family planning is a requirement for increased contraceptive use, thereforeit is important to generate demand and ensure that supply meets the demand by improving access to quality FP services.

This compendium narrates a stepwise guide on how the 72-Hour Clinic Makeover concept is being implemented in the project supported states; Kaduna, Lagos and Oyo. It also contains information on the facilities that benetted from the 72-Hour Clinic Makeover in these states.

NURHI 2 envisions a Nigeria where supply and demand barriers to contraceptive use are eliminated and a positive shift in family planning social norms at the structural, service, and community levels that drive increases in Contraceptive Prevalence Rate (CPR) in Kaduna, Lagos, and Oyo states.

Family Planning/Child Birth Spacing has been proven to be one of the key factors to reduction in maternal and child mortality, making it a very vital intervention for improving the quality of life of women and children. With NURHI's proven interventions in the family planning landscape, there is a move by key stakeholders to successfully redirect and improve the health and economic indices of the country, through high impact MNCH interventions.

One of NURHI's proven intervention to address supply barriers at these three targeted levels to improve the quality of family planning services and expand access of these services to more men and women is the 72-Hour Clinic Makeover concept. The concept ensures that all NURHI 2 supported facilities have basic equipment and infrastructure using a cost-effective way. The methodology is to use direct labour through community-based artisans who are hired to provide services to give the facility a 'face lift' thereby providing a private and safe environment for condential counselling and family planning method administration. The overall aim is to provide quality family planning services and access to care in line with the National Family Planning Service Protocol.

This compendium contains the list, pictures and details of facilities that have benetted from the 72-Hour Clinic Makeover with the list of equipment and infrastructure. It also details the challenges and success stories recorded in implementing the process.

Our desire is that this innovation, if replicated appropriately, would cause a positive shift in providing an integrated, comprehensive, and holistic approach to improve the Nigerian health system at all levels of care.

Oyo State was created in 1976 but was split into Oyo and Osun States in 1991. The current Oyo State covers a total of 27,249 square kilometres and is bounded in the North by Kwara State, in the east by Osun State, in the south by the Ogun State and in the west by Ogun State and the Republic of Benin. Oyo State consists of 33 Local Government Areas further subdivided into 35 Local Community Development Areas. Based on the 2006 census, the population in Oyo State was estimated at 5,591,589. However, by 2015, the projected population of Oyo State had risen to 7,424,242 making it the fth most populous state in Nigeria (NDHS, 2013). This situation, in addition to several other factors ranging from inadequate health nancing to poor system planning and failure to respond to the needs of a growing population, contributed to dilapidation of public health facilities, a weakened health system and signicant reduction in the capacity to provide quality care and services to clients.

NURHI 2 is committed to contributing towards achieving the State's goal of increasing its CPR to 46.5 percent by 2020. The project uses a three-prong approach of Advocacy, Demand Generation and Service Delivery to increase local support for contraceptive use, while expanding demand and access to high quality and affordable family planning services.

72-Hour Clinic Makeover ProcessThe 72-hour clinic makeover activity is implemented from start to nish in close collaboration with the health regulatory agencies in the state to have a waiting area, counselling room and FP procedure room which is based on space available in each facility.The implementation takes place within 72 hours. It starts after close of work on Friday and is completed before resumption of work the following Monday.

At the beginning of the project, NURHI 2 selected the intervention sites and conducted a Performance Improvement Assessment (PIA) of these health facilities with the aim of understanding the current state of family planning service delivery and also identifying gaps in training, equipment, infrastructure, logistics, data documentation and reporting. The ndings from the PIA were used to develop the Performance Improvement Plan (PIP), which was shared with all key stakeholders at the LGA and State levels with the intent of getting them onboard to be key contributors towards implementation of the PIP and to encourage government to scale-up and replicate the process in other facilities.

Based on the PIP, and with input from the artisans, the scope of work and the bill of quantity for the infrastructural upgrade was developed. This process of engaging community artisans ensured cost effectiveness by paying for direct labour rather than the commonly used contracting method. It also ensured that community members played an active role in the 72-hour clinic renovation thereby stimulating interest in the welfare and maintenance of the health centres in their communities. The infrastructure renovated include tiling of oors, painting, plumbing, electrical repairs and carpentry works which could include repair of torn nets, xing of doors, repair of roof leakages/ceiling and in some instances demarcation of rooms. In some facilities, blocked drainages were repaired, new walkways were constructed, pumping machines were purchased to supply water into the facility and on an occasion, a building was completed from an existing foundation.

Before equipment were procured, medical equipment vendors were identied, the vendors’outts visited and inspected to verify their capacity to deliver on specications. The medical equipment were supplied, branded, delivered and installed in the various health facilities.

In the project state ofces, a procurement committee was set up to review the scope of work required in each facility, the quantication and quotations submitted by artisans as well as review processes and costs for direct purchases at the state level. Thereafter, the facilities were cleaned, reorganized and equipment were arranged in the family planning waiting area, counselling and procedure rooms. The entire process is supervised by a team consisting of a NURHI staff, health facility staff and community members to ensure quality and completeness of the job.

Health facilities were grouped into batches and a schedule was developed to cover a maximum of ve facilities per weekend within the same LGA or locality. There should be 15 to 20 facilities per batch.The commissioning ceremony was highly participatory with the involvement of dignitaries and key stakeholders from the state and local government levels. The involvement of these policy and opinion leaders was very strategic to promote ownership and identify with NURHI's niche for quality service delivery. In addition, it provides an opportunity to advocate for government and other stakeholders to replicate the process in other facilities which are not supported by NURHI.

ChallengesThe 72- hour clinic makeover as with other laudable projects was not without its own challenges. The concept itself is very new to most health facility staff and community members alike, hence, NURHI staff had to always keep both artisans and facility members on their toes to ensure that the target to deliver the work in 72 hours is met. Although this rigorous process rufed a few feathers and some occasions resulted in mild confrontations, all participants and stakeholders involved in the process were usually amazed at how a rapid turn-around is made to the physical structures of the facility within such a short period and using minimal funding.

Also, it is important to know that the geographical location of each facility signicantly inuences the implementation process of the 72-hour clinic makeover activity. The estimated time for delivery of equipment and building materials are determined by location of facility as bad roads will sometimes slow down the delivery vehicles and prevent access to the facilities. Sometimes, the equipment were off-loaded and individually carried by community members to the facilities. Furthermore, harsh weather conditions may hamper and slow down the pace of work during renovations, for example during heavy down pour of rain, wall painting could be delayed or damaged thus necessitating repainting.

Other challenges of notable mention are poor communication network, inability of artisans to utilize e-banking services, and unavailability of building materials in some host communities which required artisans to travel as far as 300km to source for the materials. Lastly, there is a tendency for artisans to charge exorbitant prices with a false assumption that there is a lot of money because the project is being sponsored by government.

Success StoriesAmid the challenges, success stories were recorded from the 72-hour clinic makeover. During the commissioning, community members trooped out to witness the transformation of the health facilities which was thought to be “unbelievable”. They came with rolled out drums and dances. Other notable outcomes include; clients showing eagerness to use the facilities upon completion of the renovations, pledges by community associations to x other areas in the facility not covered by the makeover and some traditional heads provided lunch for workers throughout the makeover period. In addition, community leaders promised to ensure adequate security of the health facility, there were commitments by government ofcials to earmark a certain percentage of the budget for consumables and relling of the camp gas for sterilization to mention a few.

ConclusionThe overall implementation of the 72-hour clinic makeover has been a huge success that helped to showcase that so much can be done with little resources. This document chronicles the 56 facilities that benetted from the 72-hour clinic makeover under the NURHI 2 project in Oyo State.

It is one of the 11 local governments that make up Ibadan metropolis. It was named after the late Olubadan Isaac Babalola Akinyele with its headquarters at Moniya. It shares boundaries with Ajio local government to the North, Lagelu local government to the East, Ido local government to the West and Ibadan North local government to the South. Akinyele Local Government Area was created in

21976. It occupies a land area of 464.892 km . Using 3.2 percent growth rate from 2006 census gures, the 2010 estimated population for the Local Government is 239,745. The people of the local government are predominantly farmers but there is also a sprinkle of artisans. Education is also a prized industry.

The local government is host to many notable institutions including; Nigeria Institute of Social and Economic Research (NISER), International Institute of Tropical Agriculture (IITA), Federal School of Statistics, Second Mechanised Division a.k.a Odogbo Cantonment and a reasonable part of the University of Ibadan land area falls within the Local Government at Apete. There are veritable tourist attractions such as the Bowen's Tomb and Ope ljaye of the historical Kurunmi War in ljaye, Oke Idan in Ikereku, many festivals cultural and It is subdivided into 12 wards, and due to its rapid economic and political development, a new Local Council Development Area Ajorosun Local Council Development Area was carved out. It has 25 health facilities offering maternal and child health services a General Hospital. The LGA has a MMR of 268.6/100,000 live births, MR of under 5 of 11.2/1000 live births and CPR of 16.9 percent.

MONIYAGENERAL HOSPITAL

The hospital was established as a primary healthcare centre in 1998, under Akinyele Local Government Area but was later converted to a General Hospital in 2000. It consists of Male General Ward, Female General Ward, Emergency Unit, Labour Room, Maternity Ward (for both patients in labour and post delivery patients) and Mini Theatre for minor surgeries. Other units within the facility include; Pharmacy, Laboratory, Physiotherapy, Dental, PMTCT units, Immunization Clinic, Antenatal Clinic and Family Planning Clinic.

It serves as referral centre for all the Primary Healthcare facilities in all the 12 wards in Akinyele Local Government and also serves as secondary facility to the communities in the LGA. The facility has two Consultants, ve Doctors, four Youth Corp Doctors, twenty Nurses/ Midwives, four Nurses, one Dentist, three Pharmacists, three Pharmacy Technicians, six Laboratory Scientists/ Technicians, one SCHEW, seven Cleaners/Attendants.

The average number of patients/clients seen per month include; ANC (155), Deliveries (35), Immunization (700), Family Planning (250). Following the 72-hour clinic makeover by NURHI 2, the facility was commissioned by Dr. Jonathan Osamor on the 20th day of September, 2016 to promote family planning services in the community.

OJOO PRIMARYHEALTHCARE CENTRE

The land on which the hospital complex stands was donated by Ogundipe Family of Oke Offa Babasale, Ibadan. Ojoo Primary Healthcare Centre was built and donated to Ojoo Community in Akinyele Local Government, Ibadan, Oyo State by Bodija Lions Club during the 1991/1992 Lionistic year and commissioned by Lion Alex Irotumhe MJF District Governor District 404.The building was handed over to Akinyele Local Government and was commissioned by His Excellency Chief Kolapo Olawuyi Ishola, (Former Governor of Oyo State) on 12th May, 1993. The Communities being served by Ojoo PHC are: Aba-Eja, Tinuoye, Trinity, Aworawo, Kajorepo, Kajola, Gospel, Oguntule, Goodness, Aba-Opa, Abatakan, Ojoo, Elemu, Onile-Aro, Olororo, Gbanda, Crown height, Isokun, Olore and Alabede. The staff strength of the facility include; one Nurse/Midwife,one CHO, three SCHEW, one JCHEW, two Health Assistants, one Medical Record,one Lab Technician and one Pharmacy Technician.

The services rendered are: Immunization, Antenatal care for pregnant women and adequate care during and after delivery , Family Planning Services, Laboratory Services, HIV/AIDs counselling and Testing, Prevention of mother to child transmission of HIV/AIDs, Treatment of tuberculosis (DOT centre) Preventive and curative treatment for patients, Referral of Patients/Clients to Secondary and Tertiary Health Institutions. The average numbers of Clients/Patients seen in the facility per month are as follows: Deliveries (30), Immunization (300), Growth monitoring promotion (400), Family Planning (90), Ante-Natal Clinic (180), Out Patients (650).The facility was renovated under NURHI 2 72-hour family planning makeover where family planning equipments and instruments which were donated and later commissioned by The Caretaker Chairman, Akinyele South LCDA, Igbooloyin, Hon. Olumide Yakub Alli on 17th July, 2017.

RENOVATIONS/EQUIPMENT SUPPLIED

Painting of the facility exterior and fence, facility extension, FP room, OPD, consulting room, corridors, two toilets, two bathrooms, procedure room and pharmacy.

Tiling of the procedure room, two toilets, two bathrooms and the pharmacy.Plumbing works including purchase ofwater closet and two wash hand basins.

Bricklaying- The pharmacy was moved and converted to the FP counselling room hencetwo doors leading into the FP room from the new pharmacy had to be blocked and anotherentrance created for the pharmacy. Also, therewas need to plaster part of the fence.Welding- New lintel had to be made where blocks were broken to create a door space.

Electricals- The facility meter was relocated from its original space as a door space was created there. There was need to purchase lamp holder and energy saving bulbs for the FP room.

Carpentry works- including netting of windows,xing of doors, door nets, door frames and asbestos ceiling.

Aluminium demarcation was done to separatethe counselling and procedure rooms.

OLORISAOKO PRIMARYHEALTHCARE CENTRE

The Primary Healthcare Centre was built with mud by the Community in the late 60s, as a dispensary. It was rebuilt under Millennium Development Goal (MDG) and handed over to the community on 2nd October, 2014. In July, 2017 the facility was renovated under the NURHI 2 72-hour clinic makeover with family planning equipment and instruments donated. On the 16th September, 2017, the Society for Family Health (SFH) equipped an ofce for Youth Friendly Programme called A360 9ja girls. The communities being served by Olorisaoko PHC are: Olorisaoko, Falade, Akuro, Akinrinola, Abeowu, Idi-oke, Abaigbagbo, Omolarere, Idiroko, Odo-Oba, Mogaji, Gbaku, Oke-oba, Omolarere New Settlement, Olosaoko Baale, Seriki, Folarin, Ejitolu, Gbesura, Ajuwon, Oyigun, Elewedu, Ifelodun, Ifeloju, Olorunsogo, Akinboro, Akonko, Agedungbe, Adegboro and Ayetoro.

The staff strength of the facility include: one CHO, one SCHEW, one Lab Technician, one Health Assistant and three ad-hoc staff. The services rendered are: Antenatal Clinic, SFH – SFH for Youth Friendly Clinic (Provision of contraceptives and skill acquisition for adolescent girls (15 to 19 years), Prevention of mother to child transmission of HIV/AIDS, Delivery, Treatment of minor ailments, Family Planning Services, Referral of Patients/Clients to Secondary and Tertiary Health Institutions. The average number of clients seen in the facility per month are as follows: Delivery (15), Immunization (57), Family Planning (37), Ante-Natal Clinic (84). The newly renovated facility was commissioned on 17th July, 2017 by Akinyele Local Government Caretaker Chairman, Hon. Michael Fadoyin.

SASA PRIMARYHEALTHCARE CENTRE

The facility was built by IITA and handed over to Sasa Community, but was transferred to the Akinyele Local Government to be used as Primary Healthcare Centre and as an ofce for Environmental Health services. In 2014, a Non Governmental Organization called “SOS” from Netherlands renovated the PHC building. The Communities being served by Sasa PHC are: Sasa, Alfonso, Alapata, Ebedi, North-Gate, Ajibade, Ashawale, Omowole, Idita, Ofce Area, Okomalu, Kusu, Idi-ori and Seriki. The facility has a staff strength of one Nurse/Midwife, two CHOs, one SCHEW, one JCHEW and one Health Assistant. The services rendered include; Treatment of minor ailments, Immunization against childhood diseases, Growth monitoring for children (0 to 23months), Antenatal care for pregnant women and adequate care during (Perinatal) and after delivery (Postnatal), Family planning services, Good environmental sanitation, Treatment of tuberculosis (DOT centre), Prevention of mother - child transmission of HIV/AIDS, Referral of Patients/Clients to Secondary and Tertiary Health Institutions.

The average numbers of clients seen in the facility per month are as follows; Delivery (8), Immunization (80), Family Planning (40), Ante-Natal Clinic (70), Out Patients (180). The facility was selected as a Type 2 facility and beneted from the NURHI 2 modied 72-hour clinic makeover and was commissioned by Caretaker Chairman, Akinyele South LCDA, Igbooloyin, Hon. Olumide Yakub Alli on 17th July, 2017.

IKEREKU PRIMARYHEALTHCARE CENTRE

The land was donated to the Local Government by Adegbaike Ogunsiji. The facility was built and commissioned by Honourable Abiola and the Caretaker Chairman, Honourable Sangogade of Akinyele Local Government in 1958. The Communities being served by this PHC are:Ikereku, Alakeji, Babamogba, Akande, Onikange, Ago, Beyioku, Ese-oke, Osanyindeyi, Iya-ofa Ekefa, Olokuta, Abintin, Oyada, Olanla, Agbabiobi, Agbagi, Bantu, Faleye and Olulonsin.

The facility has a staff strength of one Nurse Midwife, two CHOs and one Health assistant. Services provided in the facility are as follows: Family Planning, Ante-natal clinic, Immunization, Delivery, Treatment of minor ailment, HIV counselling and testing, Child Welfare, Referral of Patients/Clients to Secondary and Tertiary Health Institution. The average number of clients seen in the facility per month are as follows: Delivery (3), Immunization (42), Family Planning (28), Ante-Natal Clinic (25), Out Patients (90). The facility was renovated under the 72-hour clinic makeover by NURHI 2 and was commissioned on the 17th July, 2017 by Hon. Akin Akinpade, the Caretaker Chairman of Akinyele East LCDA, Ikereku.

Egbeda Local Government Area was carved out of Lagelu Local Government in 1989 and has its headquarters in Egbeda town. The local government shares boundaries with Osun State to the East, Lagelu local government to the North, Ibadan North East to the West and Ona Ara local government to the South. It has an area of 191 km² and a population of 281,573 at the 2006 census.

Oremeji PHC was built during the administration of Hon. Olasunbo Oyedijo and ofcially commenced operations on the 19th April 2006. The facility is located a few blocks away from the market serving 53 settlements under Olubadan ward and includes Oremeji, Moradeyo, Alarere etc. The facility has a staff strength of eight comprising of one CHO, three CHEW and four Health Assistants. Services rendered are immunization, ANC, Family Planning and treatment of minor ailments.

The average number of clients seen per month during immunization is 170, ANC-60 and Family planning-53. The facility was commissioned following the 72- hour clinic makeover on the 5th of September 2016 by Hon Ademola Adewuyi, the Caretaker Chairman of Egbeda LGA.

KAJOREPO PRIMARYHEALTHCARE CENTRE

Kajorepo PHC is a NURHI HVS situated in Egbeda LGA. The PHC serves the following communities; Odo Ogberi, Oluwo, Ogungbade, Kukumada, Celica, Adegbayi Area. Clients are seen daily even during the Routine Immunization and ANC for different FP services with the staff actively involved.

Family planning methods offered in the facility include; Implanon, Depo-provera, Jadelle, Excluton, Noristerat, IUCD, Microgynon, Female and Male condoms. An average of 30 clients are seen monthly for family planning services. Following the 72-hour clinic makeover, the facility was commissioned on the 24th of July 2017 by Hon. Sadiq Akintunde , the Caretaker Chairman of Egbeda LGA.

KUMAPAYI PRIMARYHEALTHCARE CENTRE

The hospital was established based on a request made by the community members to Chief Adelakun, a teacher at Kumapayi Primary School who was made a commissioner between the year 1979-1981. The hospital was demolished and the land sold some few years but was regained and rebuilt by the community members.

The catchment areas are as follows: Oki, Kumapayi, Ojoyeye, Ori Bakery, Olojo, Idi Osan, Ogundipe, Areke, Omilewonde, Ahoyaya, Omolewa, Opeyemi, Opa Musa, Ori-Ile, Gbaga, Oke-Alaa and Majue lt was renovated by NURHI 2 and commissioned on 24th July 2017 by Hon. Adeyinka Adeyemi, the Caretaker Chairman of Aj'orosun.

Olode PHC was established over 30 years ago and is located in Olode community, Egbeda. The catchment communities include; Iyere, Aba titi, Ibugbe tutu, Olode, Atofarati, Onilu, Amuro, Adekola and Ogooluwa.

The staff strength include: ve CHEWs, two Health Assistants and three Adhoc staff. The average number of clients seen per month are; ANC- 10, Immunization-100 and Family Planning-50. The facility was commissioned on the 24th of July 2017 by Hon. Sadiq Akintunde, the Caretaker Chairman of Egbeda LGA.

Egbeda PHC is located in the heart of Egbeda LGA with a target population is 4,971. The facility serves the entire people of Egbeda community including Akita, Koroboto, Aba Edun, Olorunsogo, Oluwo, Fashade, Agbaaka, Iyana Ajia, Mosafejo, Obaloriegun, Bolunpelu, Ajoda, Idi Omo, Ayegun, Pakistan, Abidolu and Ajule.

The staff strength for the facility is: two Nurses, one CHO, four CHEWs and two Health Assistants. The FP client ow usually increases during market and immunization days. The facility beneted from the 72- hour clinic makeover and was commissioned on the 24th of July 2017 by Hon. Sadiq Akintunde, the Caretaker Chairman of Egbeda LGA.

Ibarapa East was founded in 1989 and has its headquarters in the town of Eruwa. It has an area of 838 km² and a population of 118,226 at the 2006 census. This facility is located in Eruwa. It is very spacious and strategically located in a busy area within the community. The catchment areas are:- Idiope, Osunrinde, Adeeka, Oluwo, Ile oga, Ayinde ode, Akande, Ikolaba, Town Hall, Tewure, Dabiri, Oke aro, Ile akan, Ile Awo, Asanso, Ajisafe, Olukotun, Abede, Ile agiri, Iyalode, Baale sunmola, Elerin, Arowolo, Star house, Odo baba ode, Ogundoyin. The population size of Anko community is 19,407.

The facility has a staff strength of 12 comprising of SCHEW, JCHEW and Health Assistants. The facility attends to an average of 120 clients per month during ANC, 93 clients per month in family planning clinic and 83 clients per month on immunization days. The facility was commissioned following the 72-hour clinic makeover on the 17th of October 2016 by Hon. Atanda Adelore, the Caretaker Chairman of Ibarapa East.

The facility is strategically located within a major market with an advantage of client inux on specic market days. The catchment areas are; Ipa, Oniyo, Siiki, Ariya, Oke-cele, Jobi, Akeete, Ogudu 1&2, Surulere, Ojonamo, Gbolaja.

The facility has a staff strength of seven comprising of the following: one SCHEW, two JCHEW, two Health Assistant and two Health Attendants. The facility was commissioned following the implementation of the 72 -hour clinic Makeover on the 17th of October 2016 by Hon. Atanda Adelore, the Caretaker Chairman of Ibarapa East.

The 72- hour clinic makeover has greatly improved the client ow in the facility with low turn out prior to the renovation. The average number of clients attending various clinics per month include: Family planning (50), ANC (10) and immunization (52) .

Oke Imale Primary Healthcare Centre is stituated at Lanlate township in Oke lmale, Ibarapa East Local Goverment Oyo state. The primary healthcare Centre was established in 1960 with a total population of 13,571. The services rendered include; ANC, Infant welfare clinic, familly planning, normal delivery, treatment of minor ailment, health education, nutrition, immunization and HIV counselling/testing.

The staff strength include; a CHO, a SCHEW, ve Health Assistant, a Record Ofcer, a Cleaner, a Scientist (N-power), a Gardener, three Night guard. The facility was commissioned following the 72-hour clinic makeover on the 17th of October, 2016 by Hon. Atanda Adelore , the Caretaker Chairman of Ibarapa East.

ERUWA GENERAL HOSPITAL

General Hospital Eruwa was established on 21st March, 1970. It was commissioned by His Excellency, the then military Governor of the Western State Of Nigeria, late Robert Adeyinka Adebayo. Family Planning unit is one of the units in General Hospital Eruwa, since the time of its establishment in 2016 to respond to family issues of the community family issues of the community.

The average number of clients seen per month include; ANC- Eighty (80), FP- Ninety-three (93) and immunization-seventy-eight ANC average percentage is eighty(80). The monthly average percentage on Family Planning is ninety three(93) while the monthly average percentage on immunization is seventy eight (78). The General Hospital staff strength is forty eight (48) while the family planning unit strength is six (6).

On the 10th of April 2017 following the completion of the 72-hour clinic makeover, the facility was commissioned by Chief Olaoye, the Alanko Of Eruwa on behalf of The Honourable Commissioner for Health, Dr Azeez Adeduntan . He was accompanied by Dr Mrs Mubo Olaoye and other chiefs.

The average number of clients seen per month include: ANC-25, Immunization-40 and Family Planning-40. The facility was renovated and commissioned on the 10th of April 2017 by the Honourable Commissioner for Health, Dr. Abdulazeez Adeduntan.

Iseyin Local Government Area was created in 1976 and its headquarters is in Iseyin town. It has a land 2mass of 988.54km and a population of 260,000 at the 2006 census. There are 11 wards in the Local

Ode Oba PHC is the Central PHC of Iseyin LGA established over 4 decades ago and it is located at Ode Oba Ekunle 1 Ward 3. The catchment areas include: Ode oba, Kuji, Ekunle,Bashorun, Itaepo, Agbede, Ebu, Yerokun, Elegede, Ojusango, Odo- Omu, Idi iya, Idi Ori, Ebedi, barracks, Odoogun Oke/Isale, Niko Village, Sina Village.

The facility has a total of twelve staff members comprising of one Doctor, two Nurses, three CHEWs, three Health Assistants, one Medical Laboratory Scientist and two records Ofcers.

The services rendered in the facility include: ANC, Infant Welfare Clinic, Family Planning, Labour and Delivery, PNC, Treatment of minor ailments, Aged and Widows Clinic. Isia Primary Healthcare Centre was established in 2004 in a rented building being paid for by the Isia Community. In 2006 the permanent structure was constructed during the regime of Hon. Isiaka Olalekan Salami, the then Executive Chairman of Kajola Local Government Area.

Okeho General Hospital was founded in 1987 and is situated opposite Oke Igana Grammar School, Misi Ole, Okeho. The facility serves the entire Misi Ole area. Lagelu Local Government Area was created in 1976 with its administrative headquarters located at Iyana Offa. It has an area of 338 km² and a population of 147,957 at the 2006 census. The facility was established on the 16th of March 2007 and is located in Kajola area of Lagelu LGA. It is a 5 bedded maternity centre with an inow of patients from within and outside Monatan community. The services rendered in the centre are: ANC, Infant Welfare clinic, Immunization, Family Planning, HIV Counselling and Testing (HCT) and Prevention of Mother to Child Transmission (PMTCT) of HIV/AIDS, Health Education, and Treatment of minor ailments. It is located in Alegongo Ward one Olorunda, Agelu LGA. It serves several communities like Otunolode, Aliwuri, Idiape, Olukotunolopomeji and about 15 other communities. The facility has a staff strength of six and includes; two Nurse/ Midwives, one Adhoc CHO, one Laboratory Scientist, one Health Attendant and one Record Ofcer.

The centre was built by the collective efforts of the Abidiodan community and the Lagelu Local Government Authority in 2013. The centre renders services to clients both within and outside the community. The services rendered include; Family Planning, ANC, Infant Welfare Clinic, Immunization, HIV Counselling and Testing, Health Education and Treatment of mino ailment.

The facility has a staff strength of ten comprising of; one Nurse/ Midwife, one CHO, one SCHEW, two Health Attendants, three Adhoc staff, one Laboratory scientist and one Night Guard. Following the implementation of the 72-hour clinic makeover, the facility was commissioned by Hon. Mohammed Oyolola on the 1st of July 2017. The facility was one of the facilities visited by the representative of T.J Mather in the person of Ms. Rodio Diallo, on the 8th of August 2017.

LALUPON PRIMARY HEALTHCARE CENTRE

The facility was established in 1958 and is located in ward seven of Lagelu LGA. The services rendered in the centre include; ANC, Infant Welfare Clinic, Immunization, Family planning, HIV counselling and Testing, Health Education and Treatment of minor ailments. The facility has a staff strength of eleven comprising of one Nurse/Midwife, one CHO, one SCHEW, one JCHEW, one Laboratory scientist, four Health Attendants, one Night Guard and one Adhoc Staff. The facility received an infrastructural upgrade with provision of equipment to offer quality family planning services by NURHI 2.

The newly renovated facility was commissioned on the 1st of July 2017 by the Chairman Lagelu LGA, Hon. Mohammed Oyolola. Following the 72 hour Clinic Makeover, the community members were impressed with the transformation of the facility and so took it upon themselves to construct a shed outside of the facility to serve as an extension of the waiting area for Maternal and Child Health services. The facility and entire community was also priviledged to host the representative of T.J Mather on the 8th of August 2017 in the person of Rodio Diallo.

LAGUNGENERAL HOSPITAL

The General Hospital, Lagun was commissioned to begin operations on the 4th of May, 2000 by the then Executive Governor of Oyo state, Alhaji Lamidi Onaolapo Adeshina. It is a 20-bedded Hospital with Outpatient Department, Female ward, Male ward, PNC ward, Infant welfare clinic , Pharmacy Department, Laboratory and Family planning Clinic. It is a multipurpose hospital with the inow of patient/client from within and outside Lagun community. The facility has 33 staff members including two doctors, nine nurses and other staff.

Up until July 2017 there was no standard family planning unit though services were being provided. The clinic was commissioned by the Hon. Commisioner for Health, Dr. Azeez Adeduntan on the 1st of July, 2017 following the 72-hour clinic makeover by NURHI 2.

The present Ogbomoso North local government area with its administrative headquarters at Kinnira was carved out of the Old Ogbomoso Local Council on the 27th September 1991, the Old Ogbomoso Local Council was however established on 1st of April 1973. It is bounded by Ogbomoso South, Orire and Surulere local government areas to the West and East respectively. It covers a total land mass of

BAAKI PRIMARYHEALTHCARE CENTRE

The present Baaki PHC centre was opened in the year 2009 after operating in a rented apartment at Olorunla's compound for some years. The centre was built by the joint communities of Abogunde, Alasa and Saja/Isale Ora wards but in year 2010 it was renovated and modernized by the Federal Government's MDGs NHIS/MCH project 2009. Later in 2015, the centre was renovated by AIDS Prevention Initiatives of Nigeria (APIN). Finally in March 2017 it was made over by NURHI 2 and an attractive family planning unit known as 'Glass Room' was built and equipped with modern family planning equipment. The newly renovated facility was commissioned on the 7th of March 2017 by the Honourable Commissioner for Health, Dr. Abdulazeez Adeduntan. The centre has since been enjoying good patronage of patients and clients from different communities both within and outside Ogbomoso North Local Government. Catchment communities for the centre include: Baaki, Atala, Ojude Keji, Ode Arubiewe, Patematawin, Abede, Ojude Imam, At-Taoheed, Eeku, Sarumi, Omosin, Akoleta, Ita Alasa, Oke Ora among others. The staff strength of the centre varies from time to time according to government policies and transfers/posting of staff members. The categories of staff have been Community Health Ofcers, Community Health Extension Workers, Junior Community Health Extension Workers, Registered Nurses & Registered Midwives, Health Assistants and other supportive staffs (Health Attendants, Records Ofcers and N power beneciaries). Presently there are 9 staff members in the centre.

Services rendered in the centre include the following: Antenatal care, Labour, delivery and postnatal care, immunization, curative services, family planning, nutrition services, HIV counselling and testing, prevention of mother to child transmission of HIV and other components of primary health care as occasion demands. On the average, number of patients/clients recorded per month are as follows: General Out Patient (359), ANC (57), Family planning (88), Immunization (41).

KATANGUA PRIMARYHEALTHCARE CENTRE

Katangua Primary Healthcare Centre was established in the year 2012 as a practise site of the LAUTECH Teaching Hospital. It is located in a suburban area close to Soun High School, Ogbomoso Ibadan and serves several rural communities. The staff strength includes: three doctors, a public health nurse, a CHO, fteen CHEWs, two cleaners and a laboratory attendant. The average number of clients seen on monthly basis include; ANC-30, Immunization-100 and Family Planning-40. Following the 72-hour clinic makeover, the facility was commissioned by the Honourable Commissioner for Health, Dr. Abdulazeez Adeduntan on the 7th of March 2017.

OSUPA PRIMARYHEALTHCARE CENTRE

Osupa Primary Healthcare Center was established in the year 1973. The ancient building was formerly known as Buka Eko (a place where hot and cold pap were sold for food). It was the oldest and smallest of all the health facilities in Ogbomoso-North Local Government Area, surrounded by old buildings and was then called Osupa Dispensary. The catchment communities include; Osupa-Elemu, Osupa-Eleko, Fedegbo, Abepe, Bosunla, Aaje Ogunbado and Takie.

The facility had very low client patronage with constant changes in health personnel who were not willing to work at the facility due to the dirty environment and poor patronage. In 2008, the health facility was renovated by the Federal Government in collaboration with the Millennium Development Goal with health care providers posted to the facility. It was then referred to as “Osupa Primary Health Care Center”.

The following services are rendered in the facility: Family Planning Services, Children and Adult Immunization Services, Out Patent Services, ANC Services, Health Education Services, HIV/AIDs Counselling and Testing, Growth Monitoring Services, Nutrition Services, Distribution of LLIN, Labour and Delivery Services. There are ve staff members in the facility comprising of two CHOs, one SCHEW and two Health Assistants. The facility usually records between 25 to 30 clients for family planning services (Oral pills, Injectables and Condoms) monthly. The effort of NURHI 2 marked another development in the history of the facility which was renovated within 72-hour to enhance and promote family planning services in the community. The facility was commissioned on 7th March 2017 by Honorable Commissioner for Health Dr. Abdulazeez Adeduntan.

TAARA PRIMARYHEALTHCARE CENTRE

The facility is a NHIS supported facility since 2009 located in a densely populated part of Ogbomoso township. It serves the following communities; Taara, Olojo, Apake, Foyanmu, Ojatuntun, Agboin, Sabo, Oke Anu and New Sawmill. The facility has a staff strength of six comprising of two CHEWs, one JCHEW, a Medical Record ofcers and 2 Health assistants.

The services rendered include; family planning, Delivery, ANC, Immunization, HIV Counselling & testing, treatment of minor ailment and health education. The facility sees on monthly basis an average of 25 clients during ANC, 60 clients during family planning, and 80 clients during immunization services. The facility beneted from the 72-hour clinic makeover and was commissioned on the 7th of March 2017 by the Hon. Commissioner for Health, Dr. Abdulazeez Adeduntan.

Oluyole Local Government is one of the oldest local government councils in Oyo State. It shares boundaries with four local government areas; Ibadan South West, Ibadan South East, Ona Ara and Ido all in Ibadan metropolis. The Local Government has its headquarters at Idi-Ayunre, Old Lagos/Ibadan road, while it shares borders with Ogun State through Egbeda-Obafemi, Odeda and Ijebu North local government areas. Oluyole Local Government was established in 1976 and the

2Council occupies about 4,000km . Based on 2006 population census, its population is 202,725.

There were 10 wards in Oluyole Local Government and these wards were divided into two sectors ; Idi-Ayunre sector and Olode sector. However, the Olode sector has been carved out to form Akorede Local Council Development Area in January 2017. The urban section of the local government comprises areas such as: New Garage, Odo-ona Elewe and Orita challenge where many big companies are sited. It also hosts a privately owned University-the Lead City University. It has 27 health facilities offering maternal and child health services. Under-5 MR stands at 0.42/1000 live births and CPR of 13 percent.

Ayetoro PHC was established in the year 2012 and is located off Iwo road expressway, Muslim Bus stop. Mosfala Primary Healthcare Centre was founded in 1989 and is located before Muslim Bridge, Mosfala area of Ibadan. The catchment communities include: Omiyale General, Opeyemi Ogo Oluwa, Binukonu Sanusi, Ogundare Laoye, Kongi, Ife Lajulo, Mosfala 1&2, Odunlade 1 &2, Apete Awowo, Ogundepo, Bolorunduro. The facility has a CHO, one Medical Record Ofcer and two Health Attendants. The average number of clients seen monthly include; ANC-40, Family Planning-93 and Immunization-51. The facility was commissioned following the implementation of the 72-hour clinic makeover on the 5th September, 2016 by the LGA Chairman, Ogbeni Orelope Kola Kazeem.

Odo Ona Elewe Primary Healthcare Centre is located along Akala Expressway, New Garage, Ibadan. It was established on the 4th of November, 2010. The facility serves the following catchment communities; Olosan, Idi Oya, Samdot Oloparun, Idi Eru, Onipepeye, Koso Estate, Adebomi, Ayegun Oleyo, Agara odo Eja, Agara Omotosho, AgraBaba Pupa, Ashipa, Ologede Estate, Podo, Kasumu, Dalute, Cele Rainbow, Onipon, Abbey tech, Liberty Academy, Ile Aanu, Ayegoro, Aba Igbira, Oyalamin Orita, Ajila Estate, Aba Bota, Ojo Ekun, Maganan, low cost area, Babalola, Oke Abidun.

The staff strength comprises of one Nurse, two CHO, three CHEWs one Medical Records Ofcer one Laboratory Technologist and ve Health Assistants. OPE AGBE PRIMARYHEALTHCARE CENTRE

Ope Agbe PHC is a community based Healthcare Centre located in Omiyale area off Olomi Academy, Oluyole LGA, Oyo State. The PHC was constructed by the community people with support from Local Employment Environmental Management Project (LEEMP) in 2005 in order to improve the health condition of the people and reduce maternal mortality in the community. The catchment areas are; Omiyale, Temidire, Ope-Agbe, Ajota, Transformer, Olomi, Ajisefunmi, Adejare, Ogundepo, Mobolorunduro, Papa- Eleye, Aladi, Onibuke. There are only two permanent staff in the facility i.e a CHO and CHEW and other adhoc staff. The facility render the following services ANC, deliveries, immunization and family planning. Ona Ara local government was created in 1989 with the administrative headquarters located at Akanran. The history of the local government could be traced to the agitation of its founding fathers after the creation of Oluyole local government considered to be extremely large for efcient and effective administration. It is bounded partly in the North by Egbeda local government, Oluyole local government to the South, Ogun/Osun States to the East and in the Western part by Lagos-Ibadan expressway with Ibadan South East local government on the other side of the expressway. It has an area of 290 km² and a population of 265,059 at the 2006 census.

SARAT ADESINA PRIMARYHEALTHCARE CENTRE

Sarat Adesina PHC is located at Atolu Bioke area of Ona-Ara local government. It was established in the year 2000 when the land on which the facility was built formerly belonging to Pa Oguntayo was released to the World Bank for the construction of a primary health centre in the community. Following the completion of the project, the facility was handed over to the Ward Development Committee (WDC) to oversee the day to day running of the facility. Sarat Adesina PHC was named after the wife of the then state executive governor who commissioned the facility.

The facility has a staff strength of ve with an average of 110 clients seen monthly for family planning services, 40 clients at ANC and 50 clients at immunization clinic. The facility is one of the health centres that beneted from the 72-hour clinic makeover which was commissioned on the 29th of August 2016 by Hon. Adesina Adeagbo, Ona Ara LGA Chairman.

ABA-EMU PRIMARYHEALTHCARE CENTRE

The present site of Aba Emu PHC was formerly used as a Garri processing industry but due to the pressing health care needs of members of the community it was converted to a primary healthcare centre in 1997. It is situated along the only major road from Olorunsogo to Akanran and being the major hub for primary health services in the entire community, it is often referred to as the UCH of Ona Ara Local Government. It not only serves Aba Emu community but also extends to adjoining communities like Thywill, Jegede, New Town, Arowona, Amuloko, Idi Ose, Odeyale, Idi Iroko, Palupo, Larodo, Ayetoro, Asolo, Olorunsogo, Alawaye, Tai, Bioku and Idi Osan.

The facility has a staff strength of seven comprising one Nurse/ Midwife, one CHO, two SCHEW, one JCHEW and two Health Assistants. The average number of clients seen per month include; Family planning-94, ANC-98 and Immunization-713. AMULOKO PRIMARYHEALTHCARE CENTRE

Amuloko PHC was commissioned to commence operations in 2010 by the deputy governor of Oyo state, Hon Taoffeek Oladejo Arapaja. It is located in Seminbio area of Amuloko with the following catchment communities; Lato, Fakayode, Kure, Ifaosin, Arawona and Geregere. The facility has a staff strength of two CHOs, two CHEWs and one Health Assistant. The average clients seen on monthly basis include; ANC-150, immunization-560 and Family Planning-100. Following the implementation of the 72-hour clinic makeover the facility was commissioned by Hon. Biliaminu Aderemi Ogundele on, Ona Ara LGA Chairman the 10th of July, 2017.

GANGANSI PRIMARYHEALTHCARE CENTRE

The health centre was established in 2015 through the efforts of the community members led by Alhaji Sheu Taofeek Lawal. The catchment communities include; Itamerin, Aba Egba, Ifesowapo, Ara, Elebolo, Foworanmu, Tioya and Bioku. The facility has a staff strength of three comprising of a CHEW and two Health Assistants. OLOSUNDE PRIMARYHEALTHCARE CENTRE

Olosunde PHC is located at Olorunsogo area of Ona-ara Local Government and was commissioned to commence operation in 1991. The facility was situated in a rented apartment at the ground oor of a storey building which was later bought over and renovated by the Local Government in 2001. AIDS prevention Initiative in Nigeria (APIN) bought some chairs for the OPD and tiled the delivery room and OPD in 2014/15. The catchment communities include Olosunde, Olorunsogo, Ayekale, Olopometa, Adeyalo, Falowo, Obalende, Odoapo, Animashaun, Sawia, Adepoju, Agric, Ajao, Dalemo and Seriki.

The facility has a staff strength of ve comprising of one CHO, one PCHEW, one Health Assistant and two SCHEW. The average number of clients seen monthly are as follows; ANC-25, immunization-45 and family planning-38. Olosunde PHC, as one of NURHI 2 High Volume Sites was renovated with equipment supplied during the 72-hour clinic makeover and was commissioned by Hon. Biliaminu Aderemi Ogundele, Ona Ara LGA Chairman on the 10th of July 2017.

Welding- a burglary gate was constructed for the nurses’ entrance door as the room houses valuable materials. The welder also xed the railings at the front verandah.

Oyo West LGA was created in 1996 with its headquarters in the town of Ojongbodu. It has an area of 526 km² and a population of 136,236 at the 2006 census. The local government area is subdivided into 10 wards and has 15 PHCs, with only 13 offering maternal and child health services.

It is bounded in the south by Akinyele LGA, in the north by Oyo East and shares a common boundary with Ejigbo and Iwo LGAs of Osun state in the East. It is the home of one of the most revered traditional ruler among the Yorubas- the Alaan of Oyo. The LGA has a MMR of 63.6/100,000 live births, under 5 MR stands at 2.5/1000 live births and CPR of 10.1 percent.

Iyaji PHC was established in 2001 and is located at Ile Are.The catchment communities include; Iyaji, Ile Are, Ile Sipe, Ile Akinu, Ogodo, Sanga, Alausa, Isale Agbala, Baba Iyaji and Ile Mokayi. The staff strength include; a CHO, two CHEWs, two Adhoc staff. The average number of clients seen monthly include; Immunization-60 and Family Planning-30. Following the 72 hour clinic makeover, the facility was commissioned by Hon. Oladeji Bisi, Oyo West LGA Chairman on the 19th of September 2016.

KOLOBO PRIMARYHEALTHCARE CENTRE

Kolobo Primary Healthcare Centre was established in January 2007 as a drug revolving fund centre located within the community and with easy accessibility. The catchment communities include: Ogbeyo, Kolobo, Fatuke, Aseke1, Aseke2, Elega, Agbolon, Sangote, Opapa, Oroki layout, Osan, Moja, baba Ibadan, Apate and Ifedapo. It has a staff strength comprising a Nurse, ve CHEWs and ve Health Assistants. The services rendered are Immunization, family planning, ANC and HIV Counselling and Testing. The average number of clients seen on monthly basis are as follows: ANC-140, family planning-60 and immunization-300. The facility was commissioned on the 19th of September 2016 by Hon. Oladeji Bisi, Oyo West LGA Chairman following the 72-hour clinic makeover.

AKEETAN BAALE PRIMARYHEALTHCARE CENTRE

Akeetan Baale PHC constructed by the Akeetan community in the year 2001 under the chairmanship of Alhaji Mudasiru Bolanle AJeigbe. The catchment communities are Akeetan Baale, Oke Ose nla, Oke Ose kekere, Kaa Adesokan, Odon, Methodist, Ajiwumi, Ilogbo, Onsa, Ijaogun, Sagunna, Idiope, Ekarun. The facility staff strength include: two SCHEW, three Health Assistants, one Adhoc and three N-Power staff. The average number of clients seen monthly are ANC- 78, Family Planning-54, OPD-96 and Immunization- 306. Akeetan Baale PHC beneted from the 72-hour clinic makeover and was commissioned on the 20th of February, 2017 by the Honourable Commissioner for Health, Dr. Abdulazeez Adeduntan.

ISEKE PRIMARYHEALTHCARE CENTRE

Iseke PHC is located in Iseke Titun area, along Ilora road in Oyo West Local Government. The centre was commissioned in May 2000 by former governor Alhaji Lamidi Onaolapo Adesina. The names of the catchment communities are: Iseke titun, Ago, Gogo, Baasi, Baba, Kanmule, Atatalo, Apena, Ejemu, Elepe Isale, Elepe Oke, Apogidan, Oburo, Funbi, Jekunmo, Olukosi, Akintonde, Alausa, Eleye and Balogun. The services rendered in the facility are: ANC, FP, immunization, delivery, treatment of minor ailments and HCT services. There are twelve permanent staff two Nurses, ve CHEWs, four Health assistants, one laboratory technologist, two adhoc, four N-power and six voluntary workers. NURHI started supporting the centre in 2015 with the implementation of the 72-hour clinic makeover. The facility was then commissioned on the 20th of February 2017 by the Hon. Commissioner for Health, Dr. Abdulazeez Adeduntan.

ISOKUN PRIMARYHEALTHCARE CENTRE

The facility is located at Osi Isokun compound Oyo, a densely populated residential area with total population of 12,830. The community has the following catchment communities; Awumoro, Lapiti, Maradesa, Gudugbo, Ekerin, Isokun, Pastors' School and Farade. The facility sees an average of 41 clients for FP services, 220 clients during immunization clinic days and 5 clients at ANC on monthly basis. Isokun PHC has a staff strength of eight comprising of one CHEW, one Ward Orderly, three Adoc staff and three N-power staff.

Following the implementation of the 72-hour clinic makeover, the facility was commissioned on the 26th of July 2017 by the Hon. Commissioner for Health, Dr. Abdulazeez Adeduntan. .
