AN ITEMIZED LISTING OF SERVICE OFFERINGS These prices are effective as of August 15, 2021, but thereafter subject to change without notice.

The goods and services shown below are those we can provide to our customers. You may choose only the

items you desire. However, any funeral arrangements you select will include a charge for our basic services

and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for,

we will explain the reason in writing on the statement we provide describing the goods and services you

selected.

BASIC PROFESSIONAL SERVICES OF STAFF & OVERHEAD:

Our fee for basic services of funeral director and staff includes, but is not limited to:

• Staff to respond to initial request for service

• Arrangement conference with family or responsible party

• Arrangement of funeral or disposition details

• Secretarial and administrative preparation & filing of necessary authorizations and permits

• Recording vital statistics; ordering & filing of death certificates

• Notification to Social Security Administration

• Preparation and placement of obituary notices & obit photograph (includes basic editing of photo)

• Staff assistance prior to, during and following the arrangement conference, including coordinating with

providing other portions of the service, (mausoleum, cemetery, crematory, and/or others)

• Temporary care of Decedent (first 24 hours)

• Assistance for Veterans with obtaining benefits

Also included in this charge are overhead expenses relative to our facilities such as sheltering of remains,

professional licensing, legal and accounting fees, insurance, building and utility expenses, parking lot and

grounds, maintenance, taxes, equipment, furnishings, inventory costs, recordkeeping, and secretarial and

administrative expenses. This fee for our basic services and overhead will be added to the total cost of the

funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate

burials, forwarding or receiving remains and our packages.)

Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select

CHAPEL RENTAL: $ 1100

This fee allows for the rental of the chapel area at Smith-Lund-Mills for events such as celebra-

tions of life, memorials, or other gatherings that are independent from Smith-Lund-Mills Funeral Chapel.

Included in the rental fee: 1) Limited services of staff and assistance with facility equipment. 2) Facilities

charge for use of our chapel. 3) Entry 2 hours prior to event, then 2 hours from start of event for a maxi-

mum of four hours. (Additional time will be billed at $275 per hour). 4) Utilization of facility equipment to in-

clude, PA system, video, tables, and chairs. 5) Standard cleaning. Note: Use of facility does not include third-party

equipment rentals, linens, catering, beverage service, honorarium for officiant, musicians, and flowers. (Weekend, evening, and holiday

charges will apply, all events subject to approval by management.)

FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME: $ 2565

This charge includes a reduced share of basic professional services for funeral director, staff, &

overhead (1495.00), transfer of deceased to our facility (485.00), embalming (585.00), if relevant, securing

necessary authorizations and permits. This charge does not include a casket or other container for the

deceased, transportation to and from the airport, additional visitation; preparation for visitation or ceremo-

nies prior to forwarding of the deceased or other fees related to domestic or international travel such as:

airline expense and consulates, etc.

RECEIVING OF REMAINS FROM ANOTHER FUNERAL HOME: $ 1860

This charge includes a reduced share of basic professional service of funeral director, staff, & and

overhead fee (1495.00), temporary shelter of the remains, and transportation of remains to the cemetery in

Funeral Coach (365.00). This charge does not include a casket or container for the deceased body, trans-

portation from the airport, refrigeration, if relevant, and the use of the facilities & staff for visitation or

ceremonies.

This charge for immediate burial, unscheduled without ceremonies includes the following: A reduced share

of the basic professional services of funeral director, staff, & overhead (1085.00), transfer of decedent to our

facility (485.00), refrigeration (325.00 up to 3 days), necessary services of staff, acquiring permits, authoriza-

tions and transportation to a local cemetery in funeral coach (365.00). $ 2260 to $ 9260

DIRECT CREMATION - WITHOUT A SERVICE:

This charge for direct cremation without ceremony, visitation or embalming includes: A reduced share

of the basic professional services of funeral director, staff, and overhead (1085.00), transfer of decedent to our

facility (485.00), temporary refrigeration, necessary services of staff, acquiring permits and authorizations, and

the cremation (495.00). If you want to arrange a direct cremation, you can use an alternative container.

Alternative containers encase the body and can be made of materials like fiberboard or composition materials

(with or without an outside covering). The containers we provide are fiberboard bottomed with a

corrugated top. $ 2065 to $ 8565
