PROFESSIONAL SERVICES

MINIMUM SERVICES OF THE FUNERAL DIRECTOR AND STAFF: $780In order to provide the finest service possible, we maintain a staff of trained, experienced and appropriately licensed personnel, which are available 24 hours every day to respond to your call. This fee includes our arrangement interview with your family, coordination of your plans, securing and recording the death certificate and disposition permit. Coordination of plans with cemetery, crematory and/or other parties involved in the final disposition. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.)

EMBALMING: $860Except in certain special cases, embalming is not required by law. Embalming may be necessary however if you select certain funeral arrangements such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.

OTHER PREP OF THE BODY: $245This includes any dressing, casketing, cosmetology, refrigeration, bathing of unemblamed remains, or handling of the body other than embalming.

USE OF FACILITIES OR EQUIPMENT

VISITATION: $485FUNERAL SERVICE: $385Use of facilities and the services of the staff necessary to coordinate the funeral, church or memorial service, provide the necessary personnel to answer telephone enquiries, staff public functions, handle floral/memorial tributes, other required duties of the staff which become necessary beyond the basic minimum services of the staff. (In the case of funeral conducted at your church, a donation to the church is not included in this fee.)

COMMITTAL SERVICE: $325Use of equipment and the services of the staff necessary to coordinate the graveside/committal services within 30 miles of the church or funeral home, handle floral/memorial tributes, other required duties of the staff which become necessary beyond the basic minimum services of the staff.

AUTOMOTIVE EQUIPMENT

TRANSFER OF REMAINS TO FUNERAL HOME: $345The care and transportation of the body to our funeral home from a local residence, health care facility or airport.* Human remains excess of 300 lbs: +$275 Human remains excess of 500 lbs: +$550

TRANSPORTATION TO CEMETERY OR CHURCH FOR SERVICE: $335Hearse or staff vehicle and staff member to any cemetery within 30 miles. ($4.00 per mile beyond 30 miles).

Limousine, 3 hour maximum: +$450*For transportation of family members to graveside, chapel, church or other location.

The goods and services shown within this booklet are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

MISCELLANEOUS PACKAGED SERVICES

IMMEDIATE BURIAL: $1460Our charge for immediate burial (without ceremony) includes: basic services of funeral director and staff, transportation of remains from place of death, care and shelter of remains and local transportation to cemetery. Does not include embalming, viewing, services or casket. Immediate burial with casket provided by purchaser: $1460 Immediate burial with minimum casket: $1875 Immediate burial with casket purchased from Funeral Home (casket is additional): $1875 to $10,000

FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME: $2320This charge includes the minimum services of staff, transportation from place of death*, embalming or sanitary care and transportation to airport. Does not include air-cargo charges, merchandise or use of facilities for visitation or ceremonies prior to forwarding of remains. Air tray or combination unit purchase required for air transportation.*

RECEIVING OF REMAINS FROM ANOTHER FUNERAL HOME: $1460This charge includes minimum services of staff, transportation from airport, temporary protective shelter of the remains at our facility and transportation to the cemetery. It does not include any merchandise or other services.

CREMATION PACKAGES

DIRECT CREMATION: $1125 to $1190Transportation of the body*, preparation of required documents and authorizations, “Worry Free” cremation in our crematory, crematory fee, temporary urn.

If you want to arrange a direct cremation, you must use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are unfinished wood boxes and fiberboard boxes. Direct Cremation with container provided by purchaser: $1125 Direct Cremation with container purchased from funeral home: $1190 Direct Cremation with "Final Goodbye": $1490

BASIC MEMORIAL SERVICE FOLLOWING A CREMATION: $1890Transportation of the body*, preparation of required documents and authorizations, cremation in our crematory, an alternative container and temporary urn, guest register book, 100 memorial folders and 25 thank you notes, visitation to be held prior to memorial service (up to two hours*), coordination and supervision of memorial service at our chapel or your church, and an online memorial tribute.

MODERN SERVICE WITH CREMATION TO FOLLOW: $4390 to $4590Transportation of body to our facility*, preparation of required documents and authorizations, embalming and cosmetic preparation for “open casket” viewing, use of ceremony casket, guest register book, 100 memorial folders, 25 thank you cards, and casket spray of flowers visitation to be held prior to service, coordination and supervision of service at our chapel or your church, cremation in our private crematory, an alternative container, temporary urn, and an online memorial tribute.

MERCHANDISE

CASKETS are available ranging in price from $415 to $10,000. A complete price list will be provided at the funeral home.

OUTER BURIAL CONTAINERS are required by most cemeteries and are designed to house the casket. Outer burial containers are available ranging in price from $1,200 to $6,000. A complete price list will be provided at the funeral home.

URNS are available ranging in price from $50 to $900. A complete price list will be provided at the funeral home.

MEMORIAL PRINTED MATERIALS are available for purchase. We offer custom printed memorial folders, thank you notes, prayer cards, and bookmarks.

PAYMENT OPTIONS

Payment is expected prior to services being rendered unless other arrangements have been made prior to need. For your convenience we accept cash, check, credit card for services and merchandise only.

We also accept assignable insurance policies. For your convenience we will assign your insurance policy to a third-party assignment company for immediate payment of incurred charges. For this service they charge

a reasonable 5% rate on the assigned amount — not the total policy.
