    INSTRUCTIONS andPROCEDURE to

    Submit Online Application for Admission

    The Prospectus can be obtained from The Director, Directorate of Distance Education,

    Kuvempu University, Shankaraghatta - 577 451, either by post or in person or at the

    Approved Student Counseling Centres on payment of the required fees. Candidates are

    informed to read all the instructions before submitting application online. Note that this

    Prospectus shall be valid only for the current year.

    Online form to be submitted on or before the last date specified in the admission

    notification. (Refer Calendar of Events in the Prospectus).

    www.kuvempuuniversitydistanceeducation.in is the website for submission of online

    application. Follow the additional instructions displayed onscreen. Candidates can also visit

    website: www.kuvempuuniversitydde.org and click Online Application Form link, which will

    connect to the website for online application submission.

    Note: DDE accepts admission only through online. Therefore, no printed application

    form is issued by the DDE.

    I. INSTRUCTIONS TO FILL ONLINE APPLICATION FORMThe items/fields that are to be entered, typed or selected while submitting online

    application are listed below. Candidate should take utmost care while filling the

    online form.

    Part 1: Candidates Personal Information

    Item 01 Name of the Candidate: The name should be entered exactly as it is printed in

    candidates SSLC Marks Card/ Degree Certificate. Leave one space between two

    words. Maximum character that can be entered here is 30. In case your

    name not fitting in 30 characters, shorten appropriately.

    Item 02 Name of the Father/Guardianshould be entered as it is printed in Candidates

    SSLC Marks Card.

    Upload Photograph and Signature files. Candidates should keep ready scanned/soft

    copies of their (1) passport size photograph and (2) signature in two separate soft copy

    files, preferably in jpeg format. They should be scanned to their exact sizes and in

    appropriate scanning (~300 dpi) resolution. Candidates should use his/her recent,

    good quality and colour photograph. Signature should be proper and to be same as that

    candidate is using currently in all official purposes, and also, should match the signature

    affixed on the printout of the application form.

    Item 03 Select the Gender by clicking the appropriate Circle.

    Item 04 If the candidate is Physically Handicapped click PHC, if Visually Handicapped

    click VHC. Otherwise leave marking at 'None'.

    Item 05 Date of Birth: Candidate should select the Day, Month and Year of birth from

    the pull down menu. Selected date of birth should be same as printed in the

    SSLC Marks Card.

    Item 06 Category: Selection to be made from the pull down menu.

    Item 07 Nationality: Default marking is Indian. If the candidate is other than Indian

    national, it must be indicated.

    Item 08 Postal Address: Correct and complete postal address of the candidate should

    be typed here. Do not enter the candidates name again here. Select the

    State, and type the District name and pin code. (Note that the address given

    here will be used in all future communications.)

    Item 09 Enter candidate's Mobile Number, if not, of a person close to the candidate.

    Also, the land phone number, if any.

    Item 10 Enter candidates E-mail IDwithout any mistakes. Some of the information will

    be sent through E-mail ID to the candidates for quick communication.

    Item 11 & 12 Candidates Occupationand Annual Incomeare to be given here.

    Part 2: Selection of the Course

    Item 13 Candidates should select the Degree, i.e., PG/UG/Diplomathat he/she is

    going to register.

    Item 14 Select the Courseamong PG/UG/Diploma.

    Item 15 Select the Subject opting, i.e., Kannada, Economics, Sociology, Botany,

    Chemistry, etc., which you wish to Study. (For some of the courses it is

    automatically displayed.) B.A. and B.Sc. candidates to select the Combination

    here.

    Item 16 Course Codeis automatically displayed based on the selection in Items 13, 14 &

    15.

    Item 17 Choose the Mediumof Instruction, if option is available for the course.

    Item 18 Select theOptional (available in some PG Courses). In case of UG select the

    Language of choice, i.e., Kannada/Sanskrit/Hindi/Urdu.

    Item 19 Give the details of the qualifying examination, i.e., PUC/BA/B.Sc. etc. based on

    passing which you are seeking admission at KUDDE.

    a. Enter Name the Qualifying/Eligibility examinationpassed.b. Enter the Combination/Subjectsstudied. Also the electives, if any.c. Register Numberat the qualifying examination.

    d. Select the Monthand Yearof passing.e. Enter the name of the College/Institution where you studied, and also

    place name where it is located.

    Part 3: Selection of the Student Counseling Centre and Fee Payment

    Item 20 Select theStudent Counseling Centre through which you are going to take

    admission to the KUDDE Course, starting from State District and theCentre name. Centre Codeis automatically displayed.

    Item 21 Fee Payment Details: Select option of Fee Payment, i.e., if you are paying full

    fee, select Full Payment or if in two installments select Installments. Note

    that based on your selection the relevant amount with breakup will be

    displayed.

    Select the Mode of Payment, i.e., either through SBM Bank Challan or NEFT

    Form. Give the Bank Name, Branch Name and Place.

    For those candidates who are unable to pay the fees in a single installment the

    facility for two installments is also provided. Please note that there shall be no

    reminder from the DDE to pay the second installment. If the second installment is not

    paid in time, such students shall not be permitted to take the exam and the first

    installment fee will be forfeited.

    After verifying the above details the candidate can submit the online application by

    clicking Finish button. Candidate should follow the instructions/procedures displayed in

    the Onscreen Response.

    An SMS can be obtained by the candidate wherein the Application Numberis informed.

    Preserve this 10 digit alphanumeric Number safely. Candidates must quote this

    application number in all the future correspondences until a regular Registration

    Number/Card is issued.

    II. PROCEDURE TO SUBMIT APPLICATION FORM The PDF copy of the filled application is to be downloaded / printed from the website

    link, and the candidate should duly affix his signature on both sides/pages of the

    application form.

    The printout Application Form, alongwith the necessary enclosures such as, marks cards,certificates regarding fulfilling the eligibility - one set photocopies duly attested by

    gazette officer and the originals for verification, and the fee paid Challan Form DDE part

    are to be sent to theDirector, Directorate of Distance Education, Kuvempu University,

    Jnana Sahyadri, Shankaraghatta -577 451.

    All your original documents will be sent back to your address directly or through StudentCounseling Centres immediately after the approval of your application.

    Candidates selecting KUDDE Shankaraghatta Centre (Centre Code-01, Direct Admissions)should send their applications to the Directorate with necessary enclosures by Register

    Post with Acknowledgement Due or by Speed Post on or before the last dates specified.

    For these candidates Admission confirmation, Registration Card Course Materials will be

    issued in person or sent by post by the Directorate Office.

    Candidates who register through the approved Student Counseling Centres shouldforward their application, with all the enclosures stated above, through respective

    Centres. The candidates should obtain a receipt from the Centre for having received the

    application along with enclosures and original Marks Cards and Certificates. They should

    collect back the original marks cards/certificates from the Centre after admission process

    is completed.

    Once the candidate selects particular Student Counseling Centre, there shall be noprovision for change of Student Counseling Centres in any circumstances. No request for

    the same will be entertained by the DDE in this regard.

    Admission confirmation, Registration card duly signed by the Director, DDE, the coursematerials and IA booklets will be issued by the Directorate through Student Counseling

    Centre only.

    Important Notes:

    (i) Application received through unrecognized sponsors will be summarily rejected,and DDE takes no responsibility for their actions.

    (ii) Neither the Directorate nor the Student Counseling Centres have agents orsubleasing offices except approved Student Counseling Centres that are listed in

    this Prospectus and Website.

    (iii)Fee once paid by the candidates will not be refunded under any circumstances.(iv)Application not accompanied by the necessary fees and certificates (enclosures)

    will not be considered and no requests for later production of the necessary

    certificates will be entertained.

    III.LIST OF ENCLOSURES(1)Printout of the Application form (PDF) duly affixing signature at indicated places on

    both sides/pages of the form.

    (2)Fee Paid SBM Bank Challan / NEFT Form / Post Office Receipt, DDE part.(3)Original marks cards SSLC/ PUC/ Degree or equivalent for verification.(4)One set photocopies of the above mention marks cards/certificates duly attested by

    a gazette officer.

    IV.PAYMENTOFFEE

    Payment of fee can be made by any of the following methods.

    1. SBM Power Jyothy ChallanFee can be paid at any of the State Bank of Mysore (SBM) Branches using the print out of

    the Challan Form (PDF), which is a triplet, generated after online application submission.

    Note that the details to be filled in the form are already incorporated in computer

    generated (PDF) form. Candidates need to affix signature, and along with the fee

    amount can submit at the Bank counter.

    Power Jyothi A/c. No. is 64100999567 and the IFSC Code is SBMY0040759.

    The Reference Number given by the Bank should be noted down and to be mentioned

    in the Application Form.

    2. NEFT FormCandidates who have no access to branches of SBM in their area can pay their fee using

    National Electronic Fund Transfer (NEFT) Form at any of the Nationalized Bank Branches.

    NEFT Form (printed as triplet) generated after online application submission must be

    used.

    For NEFT Form, the Account No. is 64101000341 and the IFSC Code is SBMY0040759.

    The UTR Numbergiven by the bank branch is important and this should be noted down

    and also to be mentioned in the printout Application Form along with the date of the

    payment, Bank and Branch names.

    Note that one counterfoil is retained by the Bank; DDE copy should be attached to the

    printout application to be submitted to KUDDE. The student copy should be retained by

    the student safely. Candidates should note that they NEED NOT fill in either the account

    number or bank code which is already printed in the Challan/NEFT Form itself.

    3. Payment at Post OfficesFee payment can also be made at all the Post Offices within Karnataka wherever online

    facility is available. Enquire at the nearest Post Office. Submit Bank Challan / NEFT Form

    at the post office alongwith payment. Post office will retain one portion of the Challan,

    DDE Copy and Student Copy portions will be returned along with a receipt of the Post

    Office. Enclose the DDE copy of challan plus the photocopy of the Receipt issued by the

    Post Office to the Application. Keep original copy of the post office receipt safely.

    Course fee payment in any other form, including Demand Drafts, will not be accepted bythe DDE. No refund of fees will be made after admission is confirmed.
