PRESENTATION OF THE PROGRAM

Who is covered under International SOS program

International SOS provides CAPGEMINI employees with quality health care and security assistance services worldwide 24 hours a day

Group Assistance program covers

- Business travelers on short-term assignment o Capgemini employees (as well as their dependents traveling with them) while on

business trip and short-term assignment (lt3 consecutive months and lt180 non-consecutive days per year) (i) more than 200 km from their main home or office location OR (ii) when an employee stays away from home for one night (or more) OR (iii) if an employee takes a flight

- Long-term assignees expatriates seconded personnel

o Capgemini employees (and their dependents) on long-term assignment (gt3 consecutive months or gt 180 non-consecutive days per year) in their assignment location (usual workplace and home) during their vacation and during their business trips

Long-term assignees expatriates seconded personnel (and their dependents) in their assignment location (usual workplace and home) and during their vacation are only covered for assistance to persons

Are my spouse and children covered under the International SOS program

All Travelers Insuredrsquos SpousePartner and also hisher dependent Children accompanying himher on the Assignment are covered under the International SOS program

What is the role of International SOS

International SOS Assistance Centers are your unique point of contact for any Assistance request

- Medical amp security information - Medical amp security advice - Referral to medical amp security accredited providers - Or any medical or security assistance services

The global program will also indemnify for medical assistance travel assistance and any assistance services covered under the insurance policy (ACE Europe) in the event of an accident or emergency during a business trip

This program is not intended to replace any primary medical insurance (ie ongoing medical treatments and medical expenses of employees and their dependents when in their home country or host country for expatriate or abroad on private trips)

No your membership is already active Simply carry the card in your wallet at all times while traveling Whenever you need advice or service contact one of the phone numbers listed on the back of the card You do not need to report specific trip dates to International SOS each time you travel

As a Traveler can I use International SOS Business Travel Assistance during my vacation

No International SOS Business Travel Assistance is not designed to cover Capgemini employee and family during their vacation However for all Travelers Insured and their SpousePartner as well as hisher dependent Children accompanying himher on the Business Assignment covers shall continue to attach to Insured who extend their trip on a private basis up to a maximum period of 15 days If you are an expatriatelong-term assigneeseconded personnel you will be covered for assistance to persons during your vacation throughout your assignment period

Does the assistance program and covers operate in case of terrorism acts civil or foreign war

Yes there is no exclusion for terrorism or war in your assistance program

However International SOS shall not be held liable for failures or mishaps in the execution of the obligations resulting from cases of force majeure or events such as civil or foreign war revolution civil commotion riots strikes seizure or distraint by the law-enforcement agencies official prohibition piracy hijacking explosion of devices nuclear or radioactive effects or prevention due to climate

What are Email Alerts

You have the option to sign up for Medical andor Security Email Alerts Medical alerts are issued when there is an unusual health risk that in the opinion of International SOS medical professionals may negatively impact travelers or expatriates visiting a specific country or region Security alerts are issued when the combined team of security professionals from a joint venture of International SOS and Control Risks has identified a security risk in a specific country or region

What if I need a doctor

The International SOS Worldwide Assistance Centers are listed on your card Call the International SOS Assistance Center that is nearest to you for a referral to a doctor who speaks your language

If I face dental care expenses are those covered under the Capgemini Group Assistance Program

Dental care expenses are excluded when the original problem is not due to an accident However the benefit shall be forfeited for dental care expenses falling under the opinion of a doctor of International SOS a serious medical emergency preventing the Insured from carrying out their assignment or requiring a medical intervention a surgery or an urgent curative treatment in order to prevent from rapid changes in hisher health condition

What if I need prescription medication

We can help if you require a prescription that a local physician cannot obtain or you need to replace lost stolen or depleted medication International SOS will when permissible by local law send the medication you need

What if I am hospitalized

Call the nearest International SOS Worldwide Assistance Center International SOS will immediately take steps to evaluate the care you are receiving and determine what actions must be taken to ensure your safe and speedy recovery

What if local medical facilities are not adequate

If you are hospitalized in an area where adequate medical facilities are not available International SOS will obtain approval from Capgeminirsquos Health and Medical Services to evacuate you to a medical facility capable of providing the required care A physician supervises evacuations and when necessary a medical specialist or nurse will accompany you during the evacuation An air ambulance will be used when required

What happens when I am released from the hospital and still need help

When your condition is stabilized and International SOS has determined that it is medically advisable to bring you home or to a facility near your permanent residence International SOS will again obtain approval from Capgemini to arrange the repatriation under medical supervision

Will International SOS pay my medical bills

The global program will indemnify for medical assistance and any assistance services covered under this policy (ACE Europe) in the event of an accident or emergency during a business trip Warning This program is not intended to replace any primary medical insurance (ie ongoing medical treatments and medical expenses of employees and their dependents when in their home country or host country for expatriate or abroad on private trips) International SOS will advise Capgemini employees on whether medical expenses are eligible and covered (or not) under the relevant insurance policy If the service andor reimbursement is covered by an insurance the insurance will however have the final authority in confirming the eligibility Referring medical expenses please consider the below procedures to be followed up

- External Outpatient medical expenses (appointment or consultations daily

hospitalization) please call at the earliest stage one of our Assistance Center in order to confirm whether these medical expenses or your medical condition should be covered under the existing program and policy and then you will have to pay for these medical expenses Once eligibility is confirmed you will have to follow the claims procedure below to get reimbursed This is what we call on a ldquopay and claim basisrdquo In some exceptional case International SOS may place the guarantee of payment for outpatient and upon Capgemini approval

- Inpatient Hospitalization Medical evacuation or repatriation medical expenses

please call at the earliest stage one of our Assistance Center in order for our operations to be involved as soon as possible and thus in a position to place guarantee of payment when required and accepted facilitate the admission into the hospital start the medical monitoring and ensure the medical expenses will be covered and recharged to the insurer or to Capgemini

International SOS will assist you in the event of any terrorist attack on a best effort basis Even if the costs incurred are not covered by ACE Europe International SOS will provide you with assistance services with prior approval of Capgemini Authorized Persons

Where possible International SOS will assist notably to obtain clearances and arrange transportation for the return of mortal remains

Referring the claims reimbursements process you need to get in contact with one of our Assistance Center which will proceed for travel assistance expenses

What is the coverage against loss damage theft or destruction of personal baggage and professional computer equipment

1 Insured interest

The Insurer shall cover

- Loss - Damage - Theft committed by forcible entry robbery or unmistakable violence - Total or partial destruction

- Theft and destruction of personal belongings left in the boot of a parked vehicle between 0700 and 2200 Hours following an unmistakable forcible entry

The Insurerrsquos cover shall only apply if

- The loss damage or destruction takes place while the baggage is under the responsibility of a carrier and it has been booked in

- The Insured must issue the carrier with all the necessary reserves in the period and manner laid down by the rules and must produce evidence of this to the Insurer

- The loss damage or destruction is the consequence of a catastrophic event such as a fire flood collapse or a terrorist act

- The theft is the subject of a complaint to the local authorities and the Insured sends the original of the receipt to the Insurer

- The theft of personal belongings left in the boot of a vehicle shall only be covered provided that they were not visible and on presentation to the Insurer of the report of the offence to the prosecuting authorities

These covers shall be granted during the entire period of the assignment

2 Limit of Liability

- Cover in respect of the Insuredrsquos baggage and personal belongings shall apply up to the limit of Five Thousand Euros (euro5 000)

- Cover in respect of valuable items jewelry and furs shall apply up to the limit of Thirty per cent of the indemnity paid in respect of ldquobaggagerdquo cover

- Cover in respect of computer equipment belonging to the Insured or to the Corporate Policyholder shall apply up to the limit of Three Thousand Euros (euro3 000)

3 Exclusions

A Specific to baggage belongings and personal effects

The following are expressly excluded from cover - Dentures artificial eyes and other prostheses spectacles contact lenses - Cash personal papers commercial documents administrative documents travellerrsquos cheques

credit cards air tickets travel tickets and vouchers - Damage caused by normal wear and tear depreciation and inherent defects in the baggage

Damage caused by moths or vermin or methods of cleaning repairing or restoring or poor baggage handling through the action of the Insured

- Damage resulting from confiscation seizure or destruction by order of an administrative authority

- Baggage belongings and personal effects left in a parked vehicle between 2200 and 0700 hours

- Valuable items jewellery and furs left in a parked vehicle and this irrespective of the time - Valuable items jewellery and furs entrusted to carriers - Keys and any other similar objects (eg magnetic cards or badges) - Any baggage or personal belongings left unattended by the Insured - Mobile telephones

- Audio-visual equipment cameras video or hi-fi equipment entrusted to carriers

B Specific to Computer Equipment

Besides the policy exclusions the following shall not be indemnified - The costs of reinstating media - The increased costs of working - Damage covered under the manufacturerrsquos warranty - Laptop computers and all their accessories when they are left in baggage entrusted to carriers

or when they are carried in a baggage compartment or when they are left in a parked vehicle and this regardless of the time

C Common to personal baggage and professional computer equipment

- The costs that may be indemnified by another insurance policy or those that are the subject of an indemnity awarded either to the Insured or to the Corporate Policyholder

When do I need to report to get reimbursement of losses

Cover shall only apply to any losses reported to International SOS within 5 days after their occurrence unless you can prove that a case of force majeure prevented the loss from being reported within that period

What documents and receipts do I need to provide International SOS with

Before sending any documents please contact one of our Assistance Centers to obtain detailed information about claims reimbursement process and claim form

1 For travel Incidents

You must provide all the supporting evidence of costs generated by the aforesaid incidents

2 For loss damage theft or destruction of baggage

- You must file a complaint for loss damage theft or destruction of baggage with the competent local authorities within a period of Twenty-Four (24) Hours following the date of the loss

- The complaint receipt and also a detailed statement must be forwarded to International SOS within a maximum period of Ten (10) Days

- You must submit a claim for loss damage theft or destruction of baggage to the carrier within a period of Twenty-Four (24) Hours following the date of loss The complaint receipt and also a detailed statement must be forwarded to International SOS within a maximum period of (10) Ten Days

- In the event of the theft of baggage from the boot of hisher vehicle the Insured shall be obliged to produce proof of the break-in (photograph of the damage repair bill for the lock)

- You shall be obliged to present to International SOS all the supporting proof enabling it to verify or assess the damage (photograph of the damaged baggage bill) and also any document that the Insurer reserves the right to request

Who is covered under International SOS program

International SOS provides CAPGEMINI employees with quality health care and security assistance services worldwide 24 hours a day

Group Assistance program covers

- Business travelers on short-term assignment o Capgemini employees (as well as their dependents traveling with them) while on

business trip and short-term assignment (lt3 consecutive months and lt180 non-consecutive days per year) (i) more than 200 km from their main home or office location OR (ii) when an employee stays away from home for one night (or more) OR (iii) if an employee takes a flight

What if local medical facilities are not adequate

If you are hospitalized in an area where adequate medical facilities are not available International SOS will obtain approval from Capgeminirsquos Health and Medical Services to evacuate you to a medical facility capable of providing the required care A physician supervises evacuations and when necessary a medical specialist or nurse will accompany you during the evacuation An air ambulance will be used when required

What happens when I am released from the hospital and still need help

When your condition is stabilized and International SOS has determined that it is medically advisable to bring you home or to a facility near your permanent residence International SOS will again obtain approval from Capgemini to arrange the repatriation under medical supervision

Will International SOS pay my medical bills

The global program will indemnify for medical assistance and any assistance services covered under this policy (ACE Europe) in the event of an accident or emergency during a business trip Warning This program is not intended to replace any primary medical insurance (ie ongoing medical treatments and medical expenses of employees and their dependents when in their home country or host country for expatriate or abroad on private trips) International SOS will advise Capgemini employees on whether medical expenses are eligible and covered (or not) under the relevant insurance policy If the service andor reimbursement is covered by an insurance the insurance will however have the final authority in confirming the eligibility Referring medical expenses please consider the below procedures to be followed up

1 Insured interest

The Insurer shall cover

- Loss - Damage - Theft committed by forcible entry robbery or unmistakable violence - Total or partial destruction

- Theft and destruction of personal belongings left in the boot of a parked vehicle between 0700 and 2200 Hours following an unmistakable forcible entry

The Insurerrsquos cover shall only apply if

- The loss damage or destruction takes place while the baggage is under the responsibility of a carrier and it has been booked in

- The Insured must issue the carrier with all the necessary reserves in the period and manner laid down by the rules and must produce evidence of this to the Insurer

- The loss damage or destruction is the consequence of a catastrophic event such as a fire flood collapse or a terrorist act

- The theft is the subject of a complaint to the local authorities and the Insured sends the original of the receipt to the Insurer

- The theft of personal belongings left in the boot of a vehicle shall only be covered provided that they were not visible and on presentation to the Insurer of the report of the offence to the prosecuting authorities

These covers shall be granted during the entire period of the assignment

2 Limit of Liability

- Cover in respect of the Insuredrsquos baggage and personal belongings shall apply up to the limit of Five Thousand Euros (euro5 000)

- Cover in respect of valuable items jewelry and furs shall apply up to the limit of Thirty per cent of the indemnity paid in respect of ldquobaggagerdquo cover

- Cover in respect of computer equipment belonging to the Insured or to the Corporate Policyholder shall apply up to the limit of Three Thousand Euros (euro3 000)

3 Exclusions

A Specific to baggage belongings and personal effects

The following are expressly excluded from cover - Dentures artificial eyes and other prostheses spectacles contact lenses - Cash personal papers commercial documents administrative documents travellerrsquos cheques

credit cards air tickets travel tickets and vouchers - Damage caused by normal wear and tear depreciation and inherent defects in the baggage

Damage caused by moths or vermin or methods of cleaning repairing or restoring or poor baggage handling through the action of the Insured

- Damage resulting from confiscation seizure or destruction by order of an administrative authority

- Baggage belongings and personal effects left in a parked vehicle between 2200 and 0700 hours

- Valuable items jewellery and furs left in a parked vehicle and this irrespective of the time - Valuable items jewellery and furs entrusted to carriers - Keys and any other similar objects (eg magnetic cards or badges) - Any baggage or personal belongings left unattended by the Insured - Mobile telephones

Besides the policy exclusions the following shall not be indemnified - The costs of reinstating media - The increased costs of working - Damage covered under the manufacturerrsquos warranty - Laptop computers and all their accessories when they are left in baggage entrusted to carriers

or when they are carried in a baggage compartment or when they are left in a parked vehicle and this regardless of the time

C Common to personal baggage and professional computer equipment

- The costs that may be indemnified by another insurance policy or those that are the subject of an indemnity awarded either to the Insured or to the Corporate Policyholder

When do I need to report to get reimbursement of losses

Cover shall only apply to any losses reported to International SOS within 5 days after their occurrence unless you can prove that a case of force majeure prevented the loss from being reported within that period

- You must file a complaint for loss damage theft or destruction of baggage with the competent local authorities within a period of Twenty-Four (24) Hours following the date of the loss

- The complaint receipt and also a detailed statement must be forwarded to International SOS within a maximum period of Ten (10) Days

- You must submit a claim for loss damage theft or destruction of baggage to the carrier within a period of Twenty-Four (24) Hours following the date of loss The complaint receipt and also a detailed statement must be forwarded to International SOS within a maximum period of (10) Ten Days

- In the event of the theft of baggage from the boot of hisher vehicle the Insured shall be obliged to produce proof of the break-in (photograph of the damage repair bill for the lock)

Who is covered under International SOS program

International SOS provides CAPGEMINI employees with quality health care and security assistance services worldwide 24 hours a day

Group Assistance program covers

What if local medical facilities are not adequate

If you are hospitalized in an area where adequate medical facilities are not available International SOS will obtain approval from Capgeminirsquos Health and Medical Services to evacuate you to a medical facility capable of providing the required care A physician supervises evacuations and when necessary a medical specialist or nurse will accompany you during the evacuation An air ambulance will be used when required

What happens when I am released from the hospital and still need help

When your condition is stabilized and International SOS has determined that it is medically advisable to bring you home or to a facility near your permanent residence International SOS will again obtain approval from Capgemini to arrange the repatriation under medical supervision

Will International SOS pay my medical bills

- Theft and destruction of personal belongings left in the boot of a parked vehicle between 0700 and 2200 Hours following an unmistakable forcible entry

The Insurerrsquos cover shall only apply if

- The loss damage or destruction takes place while the baggage is under the responsibility of a carrier and it has been booked in

- The Insured must issue the carrier with all the necessary reserves in the period and manner laid down by the rules and must produce evidence of this to the Insurer

- The loss damage or destruction is the consequence of a catastrophic event such as a fire flood collapse or a terrorist act

- The theft is the subject of a complaint to the local authorities and the Insured sends the original of the receipt to the Insurer

- The theft of personal belongings left in the boot of a vehicle shall only be covered provided that they were not visible and on presentation to the Insurer of the report of the offence to the prosecuting authorities

These covers shall be granted during the entire period of the assignment

2 Limit of Liability

- Cover in respect of the Insuredrsquos baggage and personal belongings shall apply up to the limit of Five Thousand Euros (euro5 000)

- Cover in respect of valuable items jewelry and furs shall apply up to the limit of Thirty per cent of the indemnity paid in respect of ldquobaggagerdquo cover

- Cover in respect of computer equipment belonging to the Insured or to the Corporate Policyholder shall apply up to the limit of Three Thousand Euros (euro3 000)

3 Exclusions

A Specific to baggage belongings and personal effects

The following are expressly excluded from cover - Dentures artificial eyes and other prostheses spectacles contact lenses - Cash personal papers commercial documents administrative documents travellerrsquos cheques

credit cards air tickets travel tickets and vouchers - Damage caused by normal wear and tear depreciation and inherent defects in the baggage

Damage caused by moths or vermin or methods of cleaning repairing or restoring or poor baggage handling through the action of the Insured

If you are hospitalized in an area where adequate medical facilities are not available International SOS will obtain approval from Capgeminirsquos Health and Medical Services to evacuate you to a medical facility capable of providing the required care A physician supervises evacuations and when necessary a medical specialist or nurse will accompany you during the evacuation An air ambulance will be used when required

What happens when I am released from the hospital and still need help

- Theft and destruction of personal belongings left in the boot of a parked vehicle between 0700 and 2200 Hours following an unmistakable forcible entry

The Insurerrsquos cover shall only apply if

- The loss damage or destruction takes place while the baggage is under the responsibility of a carrier and it has been booked in

- The Insured must issue the carrier with all the necessary reserves in the period and manner laid down by the rules and must produce evidence of this to the Insurer

- The loss damage or destruction is the consequence of a catastrophic event such as a fire flood collapse or a terrorist act

- The theft is the subject of a complaint to the local authorities and the Insured sends the original of the receipt to the Insurer

- The theft of personal belongings left in the boot of a vehicle shall only be covered provided that they were not visible and on presentation to the Insurer of the report of the offence to the prosecuting authorities

These covers shall be granted during the entire period of the assignment

2 Limit of Liability

- Cover in respect of the Insuredrsquos baggage and personal belongings shall apply up to the limit of Five Thousand Euros (euro5 000)

- Cover in respect of valuable items jewelry and furs shall apply up to the limit of Thirty per cent of the indemnity paid in respect of ldquobaggagerdquo cover

- Cover in respect of computer equipment belonging to the Insured or to the Corporate Policyholder shall apply up to the limit of Three Thousand Euros (euro3 000)

3 Exclusions

A Specific to baggage belongings and personal effects

The following are expressly excluded from cover - Dentures artificial eyes and other prostheses spectacles contact lenses - Cash personal papers commercial documents administrative documents travellerrsquos cheques

credit cards air tickets travel tickets and vouchers - Damage caused by normal wear and tear depreciation and inherent defects in the baggage

Damage caused by moths or vermin or methods of cleaning repairing or restoring or poor baggage handling through the action of the Insured

- Damage resulting from confiscation seizure or destruction by order of an administrative authority

Besides the policy exclusions the following shall not be indemnified - The costs of reinstating media - The increased costs of working - Damage covered under the manufacturerrsquos warranty - Laptop computers and all their accessories when they are left in baggage entrusted to carriers

as according to the nature of the event the death certificate proof of the family relationship linking you to the victim the record of residence in the care establishment a copy of the court summons the complaint receipt in the event of theft of papers or a copy of the loss advice in the event of severe damage at your home

The Insurerrsquos cover shall only apply if

- The loss damage or destruction takes place while the baggage is under the responsibility of a carrier and it has been booked in

- The Insured must issue the carrier with all the necessary reserves in the period and manner laid down by the rules and must produce evidence of this to the Insurer

- The loss damage or destruction is the consequence of a catastrophic event such as a fire flood collapse or a terrorist act

- The theft is the subject of a complaint to the local authorities and the Insured sends the original of the receipt to the Insurer

- The theft of personal belongings left in the boot of a vehicle shall only be covered provided that they were not visible and on presentation to the Insurer of the report of the offence to the prosecuting authorities

2 Limit of Liability

If you are hospitalized in an area where adequate medical facilities are not available International SOS will obtain approval from Capgeminirsquos Health and Medical Services to evacuate you to a medical facility capable of providing the required care A physician supervises evacuations and when necessary a medical specialist or nurse will accompany you during the evacuation An air ambulance will be used when required

What happens when I am released from the hospital and still need help

When your condition is stabilized and International SOS has determined that it is medically advisable to bring you home or to a facility near your permanent residence International SOS will again obtain approval from Capgemini to arrange the repatriation under medical supervision

Will International SOS pay my medical bills

The Insurerrsquos cover shall only apply if

- The loss damage or destruction takes place while the baggage is under the responsibility of a carrier and it has been booked in

- The Insured must issue the carrier with all the necessary reserves in the period and manner laid down by the rules and must produce evidence of this to the Insurer

- The loss damage or destruction is the consequence of a catastrophic event such as a fire flood collapse or a terrorist act

- The theft is the subject of a complaint to the local authorities and the Insured sends the original of the receipt to the Insurer

- The theft of personal belongings left in the boot of a vehicle shall only be covered provided that they were not visible and on presentation to the Insurer of the report of the offence to the prosecuting authorities

What is the coverage against loss damage theft or destruction of personal baggage and professional computer equipment

1 Insured interest

The Insurer shall cover

- Loss - Damage - Theft committed by forcible entry robbery or unmistakable violence - Total or partial destruction

- Theft and destruction of personal belongings left in the boot of a parked vehicle between 0700 and 2200 Hours following an unmistakable forcible entry

The Insurerrsquos cover shall only apply if

- The loss damage or destruction takes place while the baggage is under the responsibility of a carrier and it has been booked in

- The Insured must issue the carrier with all the necessary reserves in the period and manner laid down by the rules and must produce evidence of this to the Insurer

- The loss damage or destruction is the consequence of a catastrophic event such as a fire flood collapse or a terrorist act

- The theft is the subject of a complaint to the local authorities and the Insured sends the original of the receipt to the Insurer

- The theft of personal belongings left in the boot of a vehicle shall only be covered provided that they were not visible and on presentation to the Insurer of the report of the offence to the prosecuting authorities

These covers shall be granted during the entire period of the assignment

2 Limit of Liability

- Cover in respect of the Insuredrsquos baggage and personal belongings shall apply up to the limit of Five Thousand Euros (euro5 000)

- Cover in respect of valuable items jewelry and furs shall apply up to the limit of Thirty per cent of the indemnity paid in respect of ldquobaggagerdquo cover

- Cover in respect of computer equipment belonging to the Insured or to the Corporate Policyholder shall apply up to the limit of Three Thousand Euros (euro3 000)

3 Exclusions

A Specific to baggage belongings and personal effects

The following are expressly excluded from cover - Dentures artificial eyes and other prostheses spectacles contact lenses - Cash personal papers commercial documents administrative documents travellerrsquos cheques

credit cards air tickets travel tickets and vouchers - Damage caused by normal wear and tear depreciation and inherent defects in the baggage

Damage caused by moths or vermin or methods of cleaning repairing or restoring or poor baggage handling through the action of the Insured

- Damage resulting from confiscation seizure or destruction by order of an administrative authority

- Baggage belongings and personal effects left in a parked vehicle between 2200 and 0700 hours

- Valuable items jewellery and furs left in a parked vehicle and this irrespective of the time - Valuable items jewellery and furs entrusted to carriers - Keys and any other similar objects (eg magnetic cards or badges) - Any baggage or personal belongings left unattended by the Insured - Mobile telephones

B Specific to Computer Equipment

Besides the policy exclusions the following shall not be indemnified - The costs of reinstating media - The increased costs of working - Damage covered under the manufacturerrsquos warranty - Laptop computers and all their accessories when they are left in baggage entrusted to carriers

or when they are carried in a baggage compartment or when they are left in a parked vehicle and this regardless of the time

C Common to personal baggage and professional computer equipment

- The costs that may be indemnified by another insurance policy or those that are the subject of an indemnity awarded either to the Insured or to the Corporate Policyholder

When do I need to report to get reimbursement of losses

Cover shall only apply to any losses reported to International SOS within 5 days after their occurrence unless you can prove that a case of force majeure prevented the loss from being reported within that period

You must provide all the supporting evidence of costs generated by the aforesaid incidents

2 For loss damage theft or destruction of baggage

- You must file a complaint for loss damage theft or destruction of baggage with the competent local authorities within a period of Twenty-Four (24) Hours following the date of the loss

- The complaint receipt and also a detailed statement must be forwarded to International SOS within a maximum period of Ten (10) Days

- You must submit a claim for loss damage theft or destruction of baggage to the carrier within a period of Twenty-Four (24) Hours following the date of loss The complaint receipt and also a detailed statement must be forwarded to International SOS within a maximum period of (10) Ten Days

- In the event of the theft of baggage from the boot of hisher vehicle the Insured shall be obliged to produce proof of the break-in (photograph of the damage repair bill for the lock)

- You shall be obliged to present to International SOS all the supporting proof enabling it to verify or assess the damage (photograph of the damaged baggage bill) and also any document that the Insurer reserves the right to request

- In all cases a letter specifying the date and place of purchase as well as the invoice

3 For Cancellation or modification of a journey you must forward to International SOS

the following documents

- The contact details of the travel agent - A copy of the contract signed with the travel agent and also all the documents necessary for

assessing the loss - The precise reason for the cancellation and also all the necessary supporting documents such

as according to the nature of the event the death certificate proof of the family relationship linking you to the victim the record of residence in the care establishment a copy of the court summons the complaint receipt in the event of theft of papers or a copy of the loss advice in the event of severe damage at your home

If I face additional expenses related to flight delay or cancellation are those covered under the Capgemini Group assistance program

For the flight delay flight cancellation or non-admission on board Capgemini insurer may indemnify for restaurant hotel or transfer expenses related to the mentioned reasons and only based upon receiptds provision Franchise of delay 4h Max amount of indemnification 300 euro

If I face additional expenses related to luggage delivery delayed are those covered under the Capgemini Group Assistance Program

For luggage delays if the Memberrsquos baggage booked in and placed under the responsibility of the airline is not delivered to himher twenty-four hours after hisher arrival at the destination of hisher scheduled flight the Insurer shall indemnify the Member up to the amount of six hundred euros (EUR 600) for the costs incurred in making emergency and essential purchases This cover shall not attach to the Insuredrsquos return to hisher home country
