1.Go to drive.google.com2.Sign in to your google account (you
can create one using your school email if needed)
3.Create a new form by hovering your cursor over "More" to find the Google a forms icon
How To Create a Question
Types of Questions Available
Choose the type of question that fits the data you want to collect
For assessment purposes, be cautious of including extended response questions
Adding new questions
Continue to click“add item” until you have all of the questions you need!
Inserting videos/pictures into a FormInsert images from your computer, take a screenshot, or do a Google image search.
You can insert links to videos saved on Google Drive.

Getting Forms to Students
Easiest option: post a direct link on your website: Edline, Edmodo, etc.
URL shorteners: bitly.com or similar (warning: many of these are blocked on the school internet)
