June 28, 2007 Mr. Doyle Meadors 4003 Hwy 71B South Springdale, Arkansas 72764 RE: Meadors Truck & Van Salvage NPDES Permit Tracking No.: ARR00B717 Dear Mr. Meadors: On June 13, 2007, I performed an industrial storm water discharge permit compliance inspection of the Meadors Truck & Van Salvage facility in accordance with the provisions of the federal Clean Water Act, the Arkansas Water and Air Pollution Control Act, and the regulations promulgated there under. This inspection revealed the following violations:
1. There was no evidence that a storm water pollution prevention plan (SWPPP) has been developed in accordance with the requirements of the general permit. There was no SWPPP available for review at the site, nor was one made available for review during the period of the inspection. This is in violation of Part III.A of the permit. Only annual sampling data was available for review at the time of the inspection.
2. There were no records or evidence at the facility to indicate that any requirements of the SWPPP
have been met or implemented other than the performance of annual sampling. This is in violation of Part III.A.4 of the permit. These requirements include, but are not limited to, the identification of a pollution prevention team, a description of potential pollutant sources, the development and implementation of appropriate storm water management controls, and performance of an annual comprehensive site compliance evaluation. Refer to this section for all the necessary elements and requirements of the plan.
3. Presence of petroleum-based spills and leaks on the exposed ground surface in several areas
throughout the salvage yard. This is in violation of Part III.B.16 of the permit. These impacted areas are located in the vicinity of exposed driveline parts, outside of various storage areas where driveline parts are stored (ex: some parts are stored in buses and vans, however, fluids from the stored parts are leaking from some of these storage areas), and particularly in the area surrounding and downgradient from the vehicle crusher. Another area of concern is near the aboveground fuel tank. Any hazardous substance or hazardous waste tank or storage vessel at the facility should be covered, have appropriate secondary containment, and/or employ other measures to effectively prevent any possible releases from these tanks or vessels from coming into contact with the ground surface or Waters of the State. Each of the various contaminated areas currently on your site must be remediated, the soils must be disposed of properly, and confirmation samples from each area must be taken to ensure that cleanup is complete. An assessment report including all sample analytical results and records of the final disposition (including copies of all receipts for any contractors hired and/or disposal fees) of the contaminated soils must be made available for review by the ADEQ.

4. The Department has no records that Discharge Monitoring Reports have been submitted to the Water Division since issuance of your permit in March, 1996. This is in violation of Part II.C.1 of the permit.
The above items require your immediate attention. Please submit a written response to these findings to the Water Division Enforcement Branch when the violations have been corrected. This response should contain documentation describing the course of action taken to correct the items noted.
