NURSING HEALTH FORM

All new students to the School of Nursing are required to complete the Louise Herrington School of Nursing (LHSON) Health Form.

This includes students coming from the Baylor Waco Campus.

Deadline for Health Form and Immunization Records to be submitted to Certified Profile via Certified Background. com : Summer Admits – April 1st

Exceptions DUE: - May 16th: TB test, CPR Certification & Personal Health Coverage

Students will not be registered for classes until completed LHSON Health Form and immunization records have been submitted and student has been cleared by Certified Profile.

Form must be completed in its entirety. Incomplete forms will not be accepted.

NOTE: Please transfer all vaccination dates from health records to the LHSON Health Form. Forms with “see attached” on date lines will be considered incomplete and will be returned to student for completion.

IMPORTANT: Begin collecting immunization documents and complete LHSON Health Form now. This will help in being ready to upload required documents via Certified Background once Package Code is received.

Meningococcal (Meningitis) – State law students new to Baylor or a student who had a lapse in attendance

during a long semester (fall or spring) and have not had the vaccine in the past 5 years and are under the age of 22 on the first day of class.

VERY IMPORTANTPlease read prior to starting immunizations!!!

Prior to administration of vaccines, please discuss with physician and/or nurse all vaccinations needed in order to begin clinical. The following items need to be taken into consideration when receiving vaccinations:

Live vaccines can be administered on the same day, if you were to have an MMR one day and then attempt to have the Varicella a day or two later the physician/nurse would not be allowed to administer the later vaccine. Student would need to then wait 45 days before the additional vaccines could be administered.

Although TB test can be placed on the same day as receiving live vaccines, TB test can not be placed a day or two after receiving a live vaccine. If this occurs student would need to wait 30 days before having the TB test placed and later read.

IMMUNIZATION EXPIRATIONDATES

Students must be current on all immunizations and CPR certification for the duration of the semester. Month/day/year are required for all immunizations.

Immunizations with expiration dates must be good through the upcoming semester. Fall admits – good through early December Spring admits – good through early May FastBacc Summer admits – good through mid August

Immunizations cannot expire during a semester.

Immunizations must be updated prior to the beginning of each semester if expiration falls during the semester.

The above specifications are set forth by contracts between Baylor University School of Nursing and clinical agencies.

TB TESTS

TB test are good for one year. Documentation to include:

Date placed Date read Results – Negative/Positive Induration (i.e. 0mm, 2mm, …)

If you have had a positive TB test, the following documentation is required along with documentation from the positive reading. Chest X-Ray report

HEPATITIS B

Series of 3 shots. 1st Shot 2nd Shot – 1 month following 1st

3rd Shot – 6 months following 1st

Series must be completed prior to June 1st

MMR(Measles/Mumps/Rubella)

All nursing students are required to have 2 MMR boosters. In order to attend clinical student must have one of the following: Completed MMR series (by June 1st) or Titer confirming immunity of all three – Measles, Mumps, Rubella

If student is not sure whether they have had the immunization or disease, a blood titer can be drawn to prove whether or not they are immune.

If student had disease, a date on which they had the disease will need to be recorded in lieu of the immunization.

TDAP(Tetanus/Diphtheria/Pertussis)

Good for 10 years.

Tdap As long as it has been at least 45 days since an individual received

a Td, the Tdap can be administered.

MENINGOCOCCAL(Meningitis)

“A Texas State law states ALL new students under the age of 22 must show proof of meningococcal vaccination within the last 5 years and at least 10 days prior to the first class day.”

Students MAY NOT register or attend class without providing proof of the Bacterial Meningitis Vaccination or an appropriate exemption form to the Health Services Office. Documentation sent to the School of Nursing will be forwarded to Health Services in Waco for record keeping.

Therefore, any student, under the age of 22 on the first day of class, who is new to Baylor University or has had a lapse in attendance during a long semester (fall or spring) will be required to show proof of the Meningococcal Vaccine within the last 5 years.

FLU

Flu Vaccine is required for all students.

Must be current Flu Vaccine being administered as of August of the current year.

Documentation submitted must include: Date of vaccination Lot # for vaccine administered Manufacturer name of vaccine

VARICELLA(Chicken Pox)

Even though you may have had chicken pox, if you were born after 1979, students are required to provide documentation of one of the following:Varicella Titer showing immunityVaricella vaccine (completed 2 part series by June 1st)

If born before 1980 and have not had chicken pox you will need to provide one of the following:Varicella Titer showing immunityVaricella vaccine (completed 2 part series by June 1st)

CPR CERTIFICATION

AHA (American Heart Association) Health Care Provider

Certification must be current through each semester and is best if it runs through the entire time at School of Nursing.

Students are required to sign and submit a copy of the front/back of card to our office once course is completed.

PERSONALHEALTH COVERAGE

All students are required to have personal health coverage. Copy of insurance card/documentation is to be submitted along with

immunization records. Exception – those purchasing student insurance, please include a note with your

immunization records stating your plans to purchase student insurance at the beginning of January. Once you receive an email from the agency confirming acceptance, please submit documentation of acceptance and follow up with copy of card once received.
