SUBMISSION OF APPLICATION

l All applications must be received at CPSP Head office, Karachi or its Regional Offices on or beforethe dates mentioned above. Delays in postal deliveries shall not be considered a valid reason forthe late receipt of application.

l For further correspondence please mention your name, subject and Examination centre fromwhere you are appearing in the examination.

REFUND OF FEESeventy five percent of the examination fee shall be refunded to the candidate after deduction of 25% fee forprocessing, subject to:l Receipt of application for withdrawal from the examination if received on or before the specified date.l Rejection of the application by CPSP on account of ineligibility.l While applying for REFUND, candidate should write his / her NAME, CENTRE, SUBJECT and BANK

RECEIPT NUMBER.

In all other cases the fee paid for a particular examination will neither be refunded nor carried over tonext examination in case of absence / exclusion from the examination for any reason, whatsoever.

VENUE OF EXAMINATION

l PLEASE NOTE THAT SEPARATE APPLICATION FORMS ARE TO BE USED BY FRESH APPLICANTS &REPEATERS.

THEORY: The theory examinations are held simultaneously in thirteen different cities of Pakistan. Thecandidates have an option to choose one of these venues for appearing in the theory examination accordingto their convenience. The choice of the city should be clearly written in the appropriate box in theapplication form. The CPSP reserves the right to delete a particular centre, or shift the candidates from oneCentre to another on account of logistic reasons.

ORAL/CLINICAL: The College will hold clinical examination ONLY in one or more cities of PAKISTANdepending upon the number of candidates eligible to take this examination and logistic facilities available inthat city irrespective of the venue at which a candidate appeared in theory examination. Exact Venue ofClinical examination will be communicated after the assessment of theory examination papers.

NOTE: Training must have been completed one month before the date of theory examination.
