Healthwatch Enfield (EHW) is run by Enfield Consumers of Care and Health

Organisation Community Interest Company (ECCHO).

MINUTES AND UPDATES

1. Minutes of Last Meeting

Agreed as an accurate record.

2. Action Points from last meeting

Item 1 The Board noted the success in getting agreement from the Royal Free hospital

that their Board papers would be published in advance of the meetings from February.

Item 4 Lorna reported that the bank account with Unity Trust was now open. The Board

reported that they were happy with the presentation of the finance papers and thanked

Michelle for her work on these.

Item 16 Noelle will liaise with Emma on data presentation.

All other Action Points had been completed, were not yet due or were on the agenda for

this meeting.

3. Questions from the Public

There were no questions from the public.

ITEMS FOR DECISION

4. Finance report and Budget for 2016-17

John, as treasurer, introduced the Finance report and reported that all was going as

planned. There were no surprises or concerns. The projected spend had been updated to

take account of recruitment costs and revised NI and salary payments.

2016-17 Budget: A report with revised figures was tabled (and will be uploaded onto the

website). Deborah reported that LBE had requested a more significant reduction in our

budget and a meeting to discuss this was being sought. The Board was mindful of the

need for the organisation to keep expenditure as low as possible. As the organisation had

been successful in attracting some external funding this year it was agreed to reflect this

in the income projection for the budget for next year.

In the light of Lorna’s retirement the Board agreed that Audrey Lucas be appointed as a

signatory to the Unity Trust bank accounts. The meeting resolved that:

i. The amendments to the Mandate for the operation of the bank account(s),

payment instructions and banking services be approved and be provided to the

Bank by persons authorised to do so in accordance with the Bank’s procedures and

the Mandate;

ii. The Bank is entitled to rely upon the amended Mandate until it receives a later

Mandate amending it, and the Bank is entitled to rely on instructions given by any

person named in Section 3 and may disclose any information relating to the

account to any such person;

iii. The Bank is under no duty to make enquiries before acting on the instructions of

any person named in Section 3;

iv. The Bank will be notified in writing of any changes to the organisation as per the

terms and conditions of the account.

Accommodation: Lorna advised the Board that work was to be done to the office floor

which would necessitate moving everything out of the office. Hopefully this would be

done over a weekend but there was likely to be disruption on a Friday and Monday. She

would keep the Board apprised of the situation. The Board agreed to any additional costs

that might be involved in this.

Action: Lorna to arrange for Audrey to be a signatory to the account. Lorna to advise

the Board of dates for the floor repairs.

5. Proposed Priorities for the coming year 2016-17 (this item was moved up the

agenda so it was taken before the item on the draft Strategic Plan)

The Board thanked staff for the papers. There was a discussion around the long list of

topics and the Board noted that, although the following would not be major work areas,

there was an ongoing need for us to keep them under review: children & young people;

maternity services; integrated care; and carers.

Following discussion the Board agreed that the priorities for 2016-17 would be:

a) Primary Care: access to GP services.

b) Mental Health

c) Adult social care

d) Dementia

e) People with communication impairments

f) Communication and information provision for patients

Further details for items a) to e) are in the attached appendix. Item f) was new and

details will be developed and shared with the Board. It was noted that the time frame for

some of the areas extended to 2017-18.

Lorna reported on her recent meeting with the Carers Centre and it was agreed that we

should maintain our close working relationship and ensure that carers were included

within the above priorities, especially b), c) and d).

It was agreed not to proceed with the proposed work on Assessing the Involvement and

Engagement work of Providers/Commissioners.

It was agreed that we should write to Providers, CCG and LBE setting out our priorities

and asking for feedback about any work they had planned that might impact on our

proposals or areas where we might work together.

Action: Lorna to amend the individual plans, develop a plan for item f) and circulate

to the Board. Lorna to write to providers, CCG and LBE.

6. Proposed 3 year Strategic Plan 2016-19

The Board thanked Emma for her work on the Enfield context section of the plan. The

Plan was agreed with some minor amendments and with the addition of the priorities

agreed in 5 above. Deborah will sign off the final version. A copy will be uploaded onto

the website and copies sent to LBE, CCG, the Reference Group and be covered in our e-

newsletter.

Action: Lorna to make amendments. Deborah to sign off the final version. Lorna to

circulate as agreed.

7. Review of Policies and Procedures

The Board formally approved the following policies and procedures: Safeguarding policy;

Customer Service Standard; Capability policy; Grievance procedure; Supervision policy.

8. Review of Health and Safety policy

The revised policy which Parin and Lorna had worked on was agreed subject to amending

‘role’ to ‘activities’ in Section 8. Risk assessments for key activities undertaken by staff

and volunteers remained to be carried out. Parin will work with Lorna on this.

Action: Lorna to revise the policy as agreed. Lorna and Parin to carry out the risk

assessments.

9. Review of Volunteers policy

The policy had been revised so that it was consistent with the Health and Safety Policy.

The Board approved the revised version.

11. Chief Executive’s Report

Board members expressed their appreciation for the information and presentation of the

report. It was noted that Healthwatch had not been informed about the involvement of

Enfield in the Health Devolution pilot around assets and estates across NCL.

12. Update on Community Engagement work, Volunteers and Enter & View visits

The Board welcomed the report and expressed thanks to Esen for her engagement work

with the Turkish community. Lorna drew attention to the project with West African

organisations which Christiana was undertaking for us. The Board noted the change of

emphasis with pop-up stalls from libraries to supermarkets.

13. Update on Key Issues from Engagement and Signposting Work

The report was welcomed. A correction would be made to the totals on page 2 and a

revised version circulated to the Board. Noelle is to liaise with Emma around alternative

ways of presenting the data.

Action: Lorna to ensure table is corrected and circulated to the Board. Noelle to liaise

with Emma on data presentation.

14. Update on Communications activity

The Board liked the amended format. Lorna reported that our Twitter following had

reached 822. It was noted that the timeframe for the Most Popular News items table

should have read November/December (not August –October). Targets for 2016-17 will

need to be revised in the light of performance – we had been over-optimistic with

Facebook but our media coverage is well over target.

15. CQC reports affecting Enfield

There was nothing to report.

16. Meetings Attended on behalf of Healthwatch Enfield

(December, January and February)

Board members were reminded to send any changes to Michelle.

17. AOB.

Annual Report: Lorna reported that Healthwatch England were producing a new

template. It was agreed that we seek 3rd party endorsement of our work and some

suggestions were made.

Lorna’s retirement: The Board expressed its appreciation of Lorna’s work in

successfully establishing Healthwatch Enfield and ensuring it had become a credible

and influential organisation.

Dates of future Meetings.

Board meetings will take place on the following dates, at Community House. The usual

formal start time is 11.00am. The meetings are open to the public.

Communications Strategy Introduction Healthwatch Enfield formed in Spring 2013 in response to the Health and Social Care Act 2012 requiring each local authority area in the country to set up a new organisation called Healthwatch. Healthwatch Enfield formally launched in October 2013.

Vision

Our vision is to be an independent champion for local people who use health and social care services in Enfield. Achieving this vision will mean that all those who plan and provide services in Enfield will listen to the voice of local people and work with them to shape good quality provision that meets people’s needs. The intention is to ensure that local people are able to

Key Messages Our messages clearly reflect our objectives, they are key to placing us in the minds of local people as independent, trustworthy, credible, consumer champions who will amplify the voice of local people at a decision making level. We believe them to be clear and succinct and that they describe what we want people to hear and understand about Healthwatch Enfield.

• Healthwatch Enfield is the independent consumer champion for health and social care in the borough. We’re here to give you a voice and to help you find the health and social care services you need. We will use opportunities to highlight our role in relation to social care as we are aware that our name may lead people to see us as solely a health watchdog.

• Healthwatch Enfield has a place on decision-making bodies like the Health & Wellbeing Board and the local Clinical Commissioning Group enabling us to directly influence the way services are planned and delivered.

• Healthwatch Enfield is about local voices being able to influence the delivery and design of local services. We want to hear from all sections of the community especially those who face difficulties being heard.

Communications Tools Healthwatch Enfield will use a broad mix of communications tools to reach its target audiences encompassing traditional and digital media, publications, community events and stakeholder engagement. We will segment our audiences choosing the most appropriate tools and methods of dissemination.

Objectives tools and evaluation

The objectives of the communication plan will be different for the three audience segments. Therefore we will use different, though sometimes overlapping, tools to communicate with the different segments and evaluate the success of the communications work differently.

Primary Audience (General Public)

Aims

• To build awareness of Healthwatch Enfield amongst Health and Social care users.

• To demonstrate Healthwatch Enfield is an effective consumer champion, with influence at a decision making level, in order that service users see the benefit of contacting Healthwatch if they have an issue.

• To be seen as an effective signposting service to local people on information about health and social care services, providing good quality information and good quality, evidence-based reports.

• To enable and encourage participation in local commissioning and service delivery.

• We will produce printed materials including leaflets/brochures, posters and newsletters in accessible formats (large text, easy read and where possible in alternative languages), disseminated through the following distribution channels: GP surgeries, hospitals, dentists, pharmacies, places of worship, day centres, children’s centres, care and nursing homes, schools, colleges, universities, youth centres, libraries and community centres.

• We will ensure a steady stream of press and news releases, letters to opinion pages, posts and comments to local, minority and young people’s press, hyperlocal websites, blogs and social media pages relevant to our target audiences as well as support and user group journals and newsletters.

• We will seek opportunities to be included in local, community and hospital radio – providing press releases, information and offering interview opportunities.

• We will consider local and specialist or minority press advertising and other avenues such as bus or billboard advertising where these are deemed cost effective.

• We will develop our public facing website and social media content to target our priority audiences using accessible language and formats.

• We will continue to release e-newsletters and ensure that sign up is available both on and offline at events, conferences and at pop-ups.

• We will develop a diverse and accessible portfolio of events such as conferences, workshops, public meetings, pop-ups at public events or in high-footfall public spaces (for example Freshers Fairs, college events, community events etc).

• We will use our knowledge of, and contacts with, third sector organisations such as community and service user support groups to disseminate communications to their members and audience using press and media releases and our e-newsletter.

Evaluation methods and targets We will:

• Engage with at least 800 people per year at events.

• Produce 4 print newsletters per year, distributing at least 500 copies.

• Produce 10 e-newsletters per year with a minimum reach of 500 per edition

• Maintain at least a 50% new visitor rate.

Secondary Audience (Influencers and Decision Makers)

• To establish Healthwatch as a credible and trustworthy voice for patients, in order that our recommendations are listened to.

Our Information, Signposting and Advice service

Background

The Health and Social Care Act 2012

Under the Health & Social Care Act local Healthwatch organisations are to provide

advice and information about access to local health and care services and about

choices that may be made with respect to aspects of those services. This function

is now described as information, signposting and advice.

The Government’s purpose in creating this function is “to enable people to take

more control of their own health, treatment and care, and understand and use the

increased choices available to them”.

A local Healthwatch is intended to provide a single point of contact to help people

find information about the choices they have or to put them in touch with the right

advocacy organisation.

Underlying principles

The principles underpinning Healthwatch Enfield’s information, signposting and

advice service are:

1. People are entitled to a good quality, accessible and responsive service

2. Statutory agencies should themselves provide good quality accessible

information about their services

3. Using limited resources sensibly and avoiding duplication

1. People are entitled to a good quality, accessible and responsive service

Healthwatch Enfield’s information, signposting and advice service will:

• provide, where possible, a “tell us once” approach to service provision

• provide signposting and information services relating to health and social

care to all sections of the local population

• be visible and known as an information and advice resource

• develop and maintain a database of existing local networks and support

systems to direct people to the services they require

• offer a redirection service to existing health and social care services

• ensure that information is provided in plain English avoiding the use of

jargon and in alternative and appropriate formats where appropriate

other partners.

• develop relationships with commissioners and providers

• ensure that appropriate IT support systems are in place to enable

Healthwatch Enfield to analyse and develop insight from the information

and data that it will gather.

Our Service Standards

Individuals should experience a service which:

• is easy to find and access

• is aware of the communication barriers faced by some individuals and makes

efforts to address these

• informs them how they can get more involved in their own health and social

care as part of the wider Healthwatch remit and informs them about how

they can get involved in Healthwatch to bring about change to health and

social care services

• provides information through a range of mediums and formats e.g. leaflets,

electronic, telephone

• communicates general health and social care information through local

networks

• supports and promotes local public health information and awareness raising

activities

• provides accurate and accessible information to patients, carers and

families, about local health and social care services and can help introduce

people to local networks and support groups.

Organisations (Voluntary & Community, Statutory, Regulatory, Health and

Wellbeing Board and Healthwatch England) should experience:

• a service which links with other local and some national information sources

for health and social care

• an organisation with extensive local networks

• an organisation which actively communicates information from local health

and social care organisations to the local community through its networks

• an organisation which provides an early warning system for local health and

social care commissioning and provider bodies by identifying issues or gaps

in services raised through the signposting service

• an organisation which understands the limits of the local Healthwatch

information and advice giving function

• a process for consulting people who will use the information and signposting

services about what their needs are and how they can best be met – this is

likely to mean providing information by different means to different groups.

2. Statutory agencies should themselves provide good quality accessible

information about their services

A key role for Healthwatch Enfield will be to ensure that statutory agencies are

good information providers and are sensitive to the needs of local communities and

those who face additional barriers in accessing information.

Healthwatch Enfield will identify where there are unmet needs for information, or

difficulties for people in accessing information, and will encourage statutory

agencies to improve provision.

3. Using limited resources sensibly and avoiding duplication

In developing an information and signposting system it will be important to

understand what information and information networks already exist locally,

through libraries, voluntary sector organisations and community groups (VCS), as

well as statutory organisations.

The service provided by Healthwatch Enfield should not replicate or duplicate

functions that the local authority, CCGs or local VCS provide.

Healthwatch Enfield has a role in co-ordinating current activities to ensure the

best possible range of sources of high quality and accurate advice and information.

Healthwatch Enfield has a role in identifying gaps in provision and ensuring that

these are met either by an already existing organisation(s) or by direct provision.

Definitions

Signposting

Signposting takes place when you give someone basic information about other

knowledge of the organisations. There is no attempt to determine the outcome of

the signposting.

Information

A basic information service relies on individuals finding and using information

themselves. Assistance in locating the information can be provided within this

definition but not the identification of specific answers to a client’s problem. For

example, someone asking for information on registering with a GP could be helped

to find the appropriate part of the CCG website or be given a leaflet. Much basic

information provision will be via material on our website and direct links to other

websites (eg NHS choices).

We would aim to provide a service which went beyond this. Using standard

material we would help the client to locate the information they need and give an

explanation about what they need to do next. For example, a family with a

disabled child who had moved into Enfield asking about services could be given a

range of information and links to statutory provision and also told about local and

national voluntary organisations which offer further information and support

relevant to their child’s condition.

Generalist advice

This includes understanding the client’s problems, giving information and

explaining options, helping the client decide between options and giving assistance

such as identifying next steps. It does not involve taking on a case. In some

instances we might be able to offer this level of advice.

Draft outline work plans for 2016-18

At the February 2016 Board meeting the following priorities were agreed for

Healthwatch Enfield’s work over 2016-17 and into 2017-18.

a) Primary Care: access to GP services.

b) Mental Health

c) Adult social care

d) Dementia

e) People with communication impairments

f) Communication and information provision for patients

Preliminary Work Plans for these Priorities

a) Primary Care access to GP services and information 2016-18

1. Continue to monitor the situation in relation to people who are

unregistered. Assess the impact of the Public Health information drive on

registration. Engage with homeless people.

2. Depending on the outcome of the additional appointments pilot consider

undertaking work on the availability of appointments, possibly adopting the

methodology used by Healthwatch Haringey.

3. Carry out follow-up visits to GP practices to test whether there have been

improvements since the publication of our report. Publish a report.

4. Carry out an audit of online GP services, potentially including work on NHS

Choices (cf HW Newcastle).

5. Consider how best to publish accessible data around Enfield primary care

provision. This to include a ‘local data hub’ section on the website where

people can go to look at a range of data where we have extracted the

information relating to Enfield residents. For example; GP FFT

performance, national patient survey results and any smaller surveys we

carry out ourselves (cf HW Camden).

b) Mental Health 2016-18

1. Develop and follow up the recommendations from our forthcoming thematic

report.

2. Consider how best to promote good practice examples from other areas.

3. Working at a strategic level (across 3 boroughs and 5 (NCL) boroughs)

promote service user involvement and service improvement. Participate as

the HW representative for the 3 HWs on the Barnet, Enfield and Haringey

Mental Health Strategic Partnership.

4. Dual Diagnosis: to investigate further the services available to people with

dual diagnoses (severe mental health and substance misuse) including IAPT

with a view to undertaking some work in year two. The Mental Health Trust

will be producing a report on Dual Diagnosis in September 2016 as part of

6. Continue to participate in, and monitor, the patient involvement work of

the Mental Health Trust.

C) Adult Social Care 2016-18

1. Complete the programme of Enter and View visits to care of the elderly

wards and the rehabilitation ward. Reports with recommendations will have

been produced and responded to and this element of our work will be

completed early in 2016-17.

2. Continue to seek out opportunities to engage with older people who may be

recipients of care services or in need of information about them (for

example, visits to day centres, sheltered housing schemes, Greek and Greek

Cypriot Community group service users and collaboration with Winchmore

Hill Community Care). In particular to seek out harder to reach groups who

may not be aware of services.

3. Continue to investigate availability of data.

4. Continue to learn from work being done on social care by other

Healthwatch.

5. Continue to collect feedback via survey responses.

6. Continue with a programme of Enter and View visits, some of which will

include care/nursing homes.

NB. This area of work should be considered in conjunction with work on

Dementia and will also involve work with carers.

d) Dementia 2016-18

1. Building on engagement work already undertaken, continue to identify any specific areas of concern and examples of good practice. Carry out dementia-themed focus groups with other partners including Age UK Enfield, Over 50s Forum, Dementia Care (Edmonton), and specifically groups supporting BAME families affected by dementia.

2. Find out more about clinical care, including meeting colleagues at the Memory Service at Chase Farm.

3. Use our participation in partnership boards and working parties including carers’ partnership board, mental health partnership board, and integrated care for older people working group to raise the profile of dementia and seek service improvements.

4. Develop a programme of Enter & View visits to the dementia specialist wards and day services at Chase Farm.

5. Work with the Carers Centre to obtain improvements in services for carers of people with dementia.

6. Consider production of an initial report.

7. The second phase of the project would focus on specific areas which had come up as of concern.

e) People with communication impairments 2016-17

1. Follow up with providers and commissioners their responses to the

recommendations made in our report on Deaf patients. Consider writing this up

as a short summary report.

2. Using volunteers with a range of communication impairments work with the 12

other local HW in the NCEL area to complete the programme of Enter and View

visits to Emergency Departments, produce a report and recommendations.

Complete the Mystery Shopping programme and consider how we might use

this locally.

3. Later in the year follow up on the recommendations in the E&V reports for our

two local Trusts (NMUH and Royal Free).

4. Consider how we might best use the trained volunteers who are local Enfield

residents.

5. Continue to liaise with the Pocklington Trust around services for visually

impaired residents.

NB. Any additional work undertaken with the 13 borough HW group would be

dependent on external funding.

f) Communication and information provision for patients

1. Encourage providers, regulators and commissioners to review their

communications (letters, patient information materials and website

materials) with patients/service-users.

2. Seek opportunities for patients and service users to be involved in checking

Over the year from 1 April 2015 to 31 March 2016, we estimate that we have engaged with nearly 1500 local people through our programme of pop up stalls and community engagement events. The attached spreadsheets show the range of our outreach and engagement with different local communities in all parts of the borough. Through these activities we have reached out to many people who had not previously heard of Healthwatch Enfield, explained what we do, and invited them to share their experiences of and comments about local health and care services. We have used the information they have given us as a basis for enquiries to local commissioners and providers, pointing out where services have not fully met the needs of local people and making suggestions for how services could be improved. In our pop up stalls, we try to strike a balance between (a) reaching out to the general public to let them know about Healthwatch, for example through our stalls at supermarkets, libraries and the various summer fairs which take place in Enfield each year, and (b) directly reaching people who are using health and care services, by holding stalls at local hospitals and at events targeted at particular groups of service users such as the Over 50s forum events and the Beyond HIV event in December. We try to keep our engagement meetings as interactive as possible, allowing plenty of time for questions, comments and group discussion, and often focusing on areas of particular relevance to the group in question. We have established good relationships with many of the CVS groups which operate in the borough supporting different sections of the community. Some of our meetings are with service user groups, such as the Diabetes support group, the Positive Enfield group, Patient Participation Groups at local practices, etc. A programme of visits to hear from elderly people who receive care in their own home is currently in progress. Other meetings are targeted at particular BAME communities, such as the Chinese community, Turkish community, Caribbean community and so on. We have also noticed that in those meetings or events which are not targeted at a particular ethnic group, such as Parent Engagement Panel meetings, there is almost invariably good representation from many different ethnic groups. Our volunteers play a critical role in our outreach and engagement activities. We now have a diverse and committed team of 17 volunteers who come from different ethnic backgrounds and between them speak 10 community languages in addition to English, and they have proved invaluable in helping us to communicate with and start to build relationships with some of the harder to reach communities. For example, a Turkish volunteer has helped to organise several meetings with the Turkish community, as well as a slot of Turkish local radio and coverage in Turkish newspapers. Our volunteers support us in all our engagement work, taking it in turns to run our pop up stalls, accompanying us on our visits to

different community and service user groups, and playing a full part in our Enter & View work. We have gradually increased the amount of information which we provide on our stalls and at engagement meetings. In addition to information about Healthwatch Enfield and the Voiceability advocacy service, we also display information from a variety of sources including Public Health, including information about how to live healthily with certain conditions such as diabetes (available in English and Turkish), information about different mental health conditions, information about adult social care services etc. We tailor the information to the expected audience so that, for example, at our meetings with the Ghanaian church group and with the African Women’s Welfare Association, we provided information about health conditions such as TB, high blood pressure, prostate cancer etc which are known to affect people from African communities to a greater extent than their white counterparts. Our self-seal Freepost Tell us your story cards which we hand out at pop up stalls and engagement meetings have proved very popular and people have sent in many comments about local services on these cards. We have made an effort to offer some materials in different languages and have had our Tell us your story cards translated into Turkish and Somali. At our request, the Voiceability standard leaflets have also been translated in Turkish and Somali. (They were already available in other community languages.) We always display and promote any relevant materials published in different languages by other organisations such as Public Health. One particular strand of our community engagement work is our programme of Enter & View visits to care homes, nursing homes, hospital wards etc. Enter & View visits provide an opportunity for patients, residents and family members to comment on the quality of the care in these services. Reports of our visits are sent to providers and commissioners, who are expected to respond to our recommendations. Our annual conference, which is always oversubscribed, is another opportunity for local people to share their views on local services and ask questions of our guest speakers who usually include representatives of local commissioners and providers. The issues and concerns which are raised in our engagement work are all recorded on our database, are regularly analysed for emerging themes, and inform much of the ongoing work of Healthwatch Enfield, including feeding into the decisions which are made about what aspects of health and social care to prioritise. Staff and Board members raise specific concerns in their meetings and correspondence with commissioners and providers, thus ensuring that these issues are considered at the highest level.

External Factors National There is a continuing media focus on the NHS with A&E and financial pressures being the main issues highlighted. Continued action by junior doctors is affecting local Trusts. Budget statement A ‘sugar tax’ was included in the Budget statement though not coming into effect till 2020 to allow time for the industry to reduce sugar content in food and drinks. From an Enfield perspective this has to be a good thing. GP sufficiency The Public Accounts Committee has accused the DH and NHS England of being complacent about the increased demand on GP services and rising public expectations, and the ability of general practice to cope with this. It warns that access to general practice varies considerably across the country, with greater availability in older, white and affluent areas. The report also identifies extreme workload pressures and low morale leading to clinicians leaving general practice. It concludes that neither the DH or NHS England have sufficiently good data to make informed decisions about general practice. It calls for improved data, a policy to encourage GPs to remain in the profession, and a strategy to improve access for ethnic minorities. The DH has responded that improvements will come from the new GP contract, and that GP recruitment has increased. Care Act Revised statutory guidance includes the following updated measures:

• More detail on the role of principal social workers to be lead practitioners and challenge management decisions on care. This includes quality assuring social work practice, supporting effective supervision, advising management on complex or controversial cases, and developing the safety, quality and consistency of social work practice.

• Clarification on how practitioners can comply with safeguarding legislation, including considering the need for a criminal investigation, and considering whether safeguarding enquiries should be made when people are self-neglecting.

• Additional information on financial abuse.

• More detailed explanation on potential conflict of interest with advocacy providers • Clearer guidance on cross-border arrangements.

Community Care magazine reports that the latest stocktake of Local Authorities around Care Act implementation found that the number of adult safeguarding cases doubled in the first six months, between April and October. This strongly suggests that safeguarding caseloads have increased substantially since the Act introduced a national statutory threshold for investigation. There are concerns that this may not just reflect changes due to the Act, but to quality problems in adult social care – in March the CQC identified 30 per cent of services as requiring improvement. The stocktake, which is used by the Local Govt Association and DH to monitor progress on the Act also found:

• an expected additional 56,000 social care assessments, up from 1.65 million the previous year

• a predicted 37 per cent increase in carers assessed as eligible for services and a 48 per cent increase in carers receiving council-funded services

• a 2 per cent increase in independent advocacy (the impact assessment estimated 7 per cent leading to concerns that not all eligible people are being offered advocacy

• only 29 per cent believed they had sufficient resources to implement the Act in 2016-17 and beyond – down from 81 per cent confidence for 2015-16.

Mortality From April all deaths in England and Wales will be investigated by independent medical examiners in order to improve patient safety, tackle inaccurate death reporting and tackle worrying trends. Around 300 senior doctors will be appointed to review cases, and can refer deaths to a coroner for investigation. Early pilots found that one in four hospital death certificates were inaccurate and one in five cause of death were wrong. Given concerns at NMUH this is welcome.

Local and Regional Developments NHSE: I attended a workshop on the NHSE Accessible Information Standards (AIS), which we’ve had input into as they have been developed. Compliance with the AIS is incumbent on all NHS and all adult social care organisations (with public funding) as follows: 1.4.16 – to have identified and recorded the information and communication needs of patients. 31.7.16 – Full implementation of all the standards (SCCC11605) and all Trusts should have Accessible Information policies. We will be looking for evidence that our local NHS providers (including GP practices) are prepared for implementation. CQC: BEH-MHT was inspected between 30 November- 4 December and their report has just been

published. The outcome was mixed with five services rated as ‘good’, with one ‘outstanding’.

Others require improvement and are:

• Child and adolescent mental health wards

• Specialist community mental health services for children and young people

• Community based mental health services for adults of working age

• Acute wards for adults of working age and psychiatric intensive care units

• Mental health crisis services and health-based places of safety

A Quality Summit is scheduled for April 27th. All sites run by the Royal Free were inspected in early February. We referred one care home and one care agency to the CQC because of concerns around quality of service provision.

Local Relationships Royal Free London NHS Foundation Trust Lucy has attended the Trust’s EDS2 grading workshops and has been able to take the learning from this to her work on EDS2 with the Mental Health Trust and CCG. Barnet, Enfield and Haringey-Mental Health Trust Rajinder is now attending as our representative on the newly re-established Patient Experience Committee. We arranged (sub-contracted to Barnet HW) two successful NHS Equality Delivery System 2 (EDS2) Grading workshops, one for mental health service users and one focused on Enfield Community Services. The Trust was very happy with the turnout and level of discussion. Along with Healthwatch Haringey, Deborah and I attended a meeting with Maria Kane (CEO), Mary Sexton (Director of nursing) and Andrew Wright (Director of Strategic Development) where we discussed finance, staffing, delays with the re-development of the St Ann’s site and the CQC inspection. North Middlesex University Hospital (NMUH) We’ve had several Enter and View visits (including the HENCEL funded joint HWs one to A&E) and have been using the opportunity presented by the Patient Experience Committee to raise issues of concern. A&E is the main focus of concern locally with ongoing difficulties in recruiting senior staff. CCG Deborah and I met with the CCG at the end of February. We discussed the ongoing pressure on NMUH A&E and raised our concerns about the need for a consistent communications message. We learnt of the new See and Direct initiative –with GPs placed in A&E and doing early triage – which seemed to be working well though it had only just started so too soon to make a proper assessment. Extra appointments initiative: this pilot which finished at the end of March has had mixed results. Despite the background tensions between the two GP networks, some 94% of practices used the service, demonstrating that practices were putting patients first. The surgery in the East of the borough proved more popular. We await a full review of the usage and impact. We asked specifically about the review of the Paediatric Assessment Unit at Chase Farm which had started last summer and were told it was at the clinical assessment stage. (This has now become a negative news story though we’d not been provided with any further update). We asked about Personal Health budgets as we’ve had enquiries from service users about this but things still seem to be at a very early stage of development. We raised our concerns about the poor consultation carried out by the CCG around the proposed withdrawal of gluten-free food on prescription. We had been contacted by a number of individual patients and also by Coeliac UK. The CCG have now undertaken to review the consultation process. We have written to the lead for Primary Care about the readiness of local GPs to implement online services fully from the beginning of April. PPGs I spoke at a meeting of the Forest Road practice PPG in mid –February and at a Borough-wide PPG meeting on 1st March at the Dugdale Centre and was able to highlight our work and promote the idea of joint working.

London Borough of Enfield Having submitted a budget with a 5% reduction (on top of the 2% reduction we had last year) we were surprised to be informed by Bindi Nagra, Assistant Director, Health, Housing and Adult Social Care, that a much larger reduction was expected. We revised the budget and submitted one based on 12% (plus the 2% from last year) and are due to meet Bindi and Michael Sprosson, our commissioners, on 11th April. I attended a meeting of the Quality in Care group which the CQC also attended and this provided an opportunity to discuss providers we share concerns about. We also got a useful update on projects being undertaken by the Quality Checkers. Local Healthwatch Co-operation We continue to work closely with our neighbouring HWs and attend London-wide HW meetings. I am representing Barnet, Haringey and ourselves on the recently established Mental Health Strategic Partnership Board. HW Islington attend the NCL Primary Care Joint Committee on behalf of NCL HWs, Deborah attends the NCL Urgent Care Network. Training on deaf awareness was offered via the HENCEL funded project.

Use of Our Statutory Powers Enter and View visits: see separate agenda item. As part of our HENCEL (Health Education North, Central and East London) funded work with other local HW we visited NMUH A&E on 18th Feb together with Haringey HW and trained volunteers who have communication barriers. The focus of the E&V was to explore provision for patients who face communication barriers – due to visual impairment, deafness, learning difficulties or aphasia. The visit was written up, recommendations agreed and sent to the Trust and their response has just been received.

Workplan Highlights and Exception Report:

GP access: We have produced a report on how local GP practices are using the GP Friends and Family Test. This has been sent to all practices and PPGs. Deaf patients report: Following the publication of the report last year we recently sent a request to all providers, CCG, NHSE and Public Health to ask them to report back on the implementation of the recommendations. We have now received all but one of the update responses and will publish on our website. Mental Health interim report: Board members re-drafted the thematic report following the consultation event held in late January. CRM: the bugs which were limiting what we could do have now been sorted out. Website in a Box: the review of our website had been on hold awaiting the Healthwatch England review and updating of Website in a Box but we are now aware that this may not happen due to cutbacks do Joe will start implementing some interim changes. Annual Report: Healthwatch England have made some changes to the template and this has just been received. Joe is working on it.

Volunteer recruitment and training update Four new community outreach volunteers have been recruited in the last two months and their induction is about to take place. All the new volunteers have personal experience as service users and/or carers, or have relevant professional experience. A volunteer who had previously left when she found paid employment has also returned, bringing our current total to 17 volunteers in all. Several of the new volunteers are interested in joining the Enter & View team in due course. Three of our volunteers recently took part in the PLACE audits at NMUH and BEH MHT Chase Farm site, and quite a few of them have signed up to take part in the next GP audit. We encourage our volunteers to report to us any instances of good or poor care which they or their families have experienced, and they have proved to be a valuable source of information about local services. Two of our volunteers have additional access/communication needs and we are particularly interested to hear whether their needs are being fully met when they use health and care services. Our volunteers continue to support us in our outreach work at pop up stalls and engagement events, and in our Enter & View programme. Their hard work and commitment is greatly appreciated.

Enter & View update We have completed a series of visits to local hospital wards where the majority of the patients are elderly. The report of our visit to Capetown Ward at Chase Farm Hospital has now been published on our website and sent to the relevant authorities. We have also visited Pymmes Zero Ward at NMUH and Magnolia Unit at St Michael’s, and reports on those visits are in preparation. When all reports are completed we will be able to draw some general conclusions about local inpatient care and discharge arrangements for elderly patients. We also took part in a joint visit (as part of the HENCEL funded project) with representatives from Healthwatch Haringey to A & E at NMUH to find out about access for patients with sensory disabilities

and communication difficulties. We have received a response from the Trust to our draft report about this visit, and the final report will be published imminently. In the coming months we intend to revisit some of the premises which we have visited in the past, to find out what improvements have been made in response to our recommendations. We

are also planning to visit a number of different care settings where people with dementia are receiving care. Finally, we note that many of the findings which we reported following our Enter & View visits to local mental health services last year have now been corroborated by the CQC in their recent inspection report on Barnet Enfield and Haringey Mental Health Trust. Similarly, at the recent stakeholder meeting to discuss the BEH-MHT Quality Accounts, mention was made of the “excellent” and “supportive” Enter & View reports from Healthwatch, “which highlight areas of best practice and areas that can be focused on”. This gives us confidence that we are on the right track with our Enter & View visits and reports; it validates the thoroughness of our approach and shows that, with our committed team of volunteers, we have been able to pinpoint the key areas where improvements are needed in local mental health services and to offer constructive support.

Community Engagement update Since the last Board meeting, we estimate that we engaged with about 75 members of the public through our pop-up stalls at Chase Farm and NMUH Hospitals, and at the Moving On event held at the Dugdale Centre in February. The Moving On event – a Transitions event for young people with learning disabilities, which has proved a good way to engage with people in previous years - was rather disappointing this year, as it was organised in a different way and far fewer people attended. We are currently holding a series of focus groups with elderly residents of sheltered accommodation and people who attend day centres. These meetings are helping us to find out more about local people’s experiences of care in their own home. We have attended a session run by Enfield Carers at Community House and spoken individually with a number of carers about their experience of services. This proved to be very useful and we plan to return. We also held a very successful series of meetings which were specifically targeted at African diaspora communities in Enfield. We attended a meeting of Positive Enfield, also known as Umoja, a peer-support group for people living with HIV, predominantly those from a Black African background; we gave a presentation and handed out a questionnaire about GP services at a meeting of the Ghanaian community group at St Edmund’s church in Edmonton; and we co-hosted an event for International Women’s Day with the African Women’s Welfare Association. All three engagement meetings helped to promote the work of Healthwatch Enfield to members of communities who may not previously have heard of Healthwatch; good relationships were established with members of these communities, which we hope to build on in future; and rich feedback was received at each of the meetings about local people’s experiences of health and care services. As a result of these meetings, a total of 35 new people signed up to receive regular updates from Healthwatch Enfield. We estimate that through our engagement events we have reached a further 220 people, bringing the total for the period to approximately 290. We held a productive consultation meeting at the end of January to discuss our draft mental health report, based on the findings of previous community engagement and Enter & View work, where we received valuable feedback on our recommendations. Further work has now been done on this report which we are planning to present to commissioners and providers. This is another example of how we involve local people in our efforts to secure improvements in health and care service design and delivery.

We have released two, and are working on the third. We only began in the summer Produce 10 e-newsletters per year with a minimum reach of 500 per edition We have released 9 so far this year Achieve a Twitter reach of 1 million per year (retweets and mentions) – Twitter now measures impressions instead of reach. I have proposed a target of 5,000 a month in the revised comms strategy. Achieve a Twitter and Facebook follower/like growth of 15% per quarter Though not a full quarter, twitter followers only grew 4% and Facebook 5% in November-December. We will implement further paid advertising on social media in order to improve on this. New follower/like acquisition seem to occur in a linear rather than exponential fashion. Attract new subscribers to our e-newsletter at a growth rate of 30 per quarter We added 59 new subscribers in the 4th quarter 2015/16 Achieve a website sessions per month growth of 10% per quarter This has not been achieved. Page views steadily climbed to a high point in July, but have fallen off since. All time website data is below.

Maintain at least a 50% new visitor rate. We are on 74.7% for the year Maintain an average session length of at least 3 minutes, and a bounce rate of under 60% This is at 1:54 and 65.38% for the year. REPORT: Barnet, Enfield and Haringey Mental Health Trust The report for Barnet, Enfield and Haringey Mental Health Trust was published on 29th March 2016. The trust came out overall as ‘requires improvement’. The ‘Safe’, ‘Effective’, ‘Responsive’ and ‘Well-led’ domains ‘require improvement’ whereas ‘Caring’ was rated as ‘Good’. Overall there were six requirement notices for the trust. The full report can be found on the CQC website http://www.cqc.org.uk/provider/RRP GP inspections This quarter there has been a total of five published reports for GP inspections in Enfield. Two of these reports were rated as ‘good’ and three were rated as ‘requires improvement’. The total number of Enfield GP practices with a rating from the ‘new’ CQC inspection method is now seven (14%). Other CQC reports to note We have been made aware of a couple of care homes that have been recently inspected by the CQC and have been rated as ‘inadequate’. We will disseminate these reports once they have been published, but the CQC did immediately notify Enfield Council.
