Members of Nottinghamshire Local Education and Training Council (LETC) are working together to improve care for older people with frailty through an innovative toolkit and supporting training programme that aims to: Enable supported self-care; Promote integrated assessment; Foster multi-professional and multi-agency learning and working; Deliver consistent education and training standards. The programme is funded by Health Education East Midlands (HEEM) and its direction and development have been shaped by professionals and older people with frailty working together. The Steering Group is chaired by HEEM Transformation and LETC Lead, with senior workforce representatives from Local Authorities. Multi-professional, cross-organisational ‘Expert Advisory Panel’ and ‘User Group’ have provided input into the content and design of the toolkit and training events, whilst a small group of older people with frailty, their families and carers, have directed case studies. > > > > The f irst wave o f tr aining e vents and the launch o f the Fr ailt y T oolkit t ook plac e in Oct ober 201 5 . To date, over 150 health and care professionals representing more than 20 organisations from across the NHS, Adult Social Care, Private and Third sector providers have joined the programme. Throughout the training days, participants from many different roles have worked together in multi-disciplinary teams. 92% of participants rated the training effective or very effective in achieving the learning outcomes, whilst networking, including multi-agency working is the most frequently mentioned ‘best thing about the event’ feedback. The training events in early 2016 are filling up fast. See above for details of dates, locations and how you or your staff can join the programme. There are around 20,000 older people living with frailty across Nottinghamshire and Nottingham.Members of Nottinghamshire Local Education and Training Council (LETC) are working together to improve care for older people with frailty through an innovative toolkit and supporting training programme that aims to:

• Enable supported self-care;

• Promote integrated assessment;

• Foster multi-professional and multi-agency learning and working;

FRAILTY TOOLKIT APP

MULTI-AGENCY TRAINING EVENTS

Accessed through your web browser, this easy to navigate Toolkit includes frailty and supported self-care learning materials, planning templates, directory of resources, and case studies demonstrating practical applications.

Available on any smart phone, tablet or computer connected to the internet, you can access the resources at any time for further learning and to help you in your work with people with frailty.

Participants work in multi-agency, multi-professional teams throughout a very interactive day. Supported self-care themes - empowerment, motivation and capability building - run throughout.

Morning sessions focus upon understanding frailty, skills for supported self-care, and identifying ‘helps and hindrances’ to joint-working.

Afternoon sessions cover practical application through interactive case-based exercises.

89% of participants said they were ‘likely’ or ‘very likely’ to recommend training to colleagues

I liked being able to network with other organisations - knowing what is available and how to access their skills.

SUPPORTED SELF-CARE PLANToo often our systems of care can overlook what is really important to individuals, particularly older people living with frailty. The toolkit and training recognises this.

The process of supported self-care planning explores the importance of promoting independence and individual choice, and of accepting and managing risk along the way.

The person-centred, holistic planning template used in the training empowers care professionals to support practical changes in an individual’s life by applying their knowledge and skills to help identify individual goals and to create a ‘plan on a page’.

FRAILTY FULCRUMThis new model articulates the complex nature of frailty. By exploring the balance and interaction of all the different aspects of an older person’s life, care professionals can use this model to identify opportunities to build resilience and thereby improve quality of life for older people with frailty.

The model is introduced through a short animation and underpins the holistic approach to assessment used throughout the training day.

VIRTUAL REALITY SIMULATION

CO-DESIGNED CASE STUDY EXERCISES

How does it feel to be an older person with frailty living at home? Using our newly developed virtual reality simulation, we put you in the shoes of that person.

This part of the training helps develop empathy with those whom we care for, as well as building awareness and observational skills, and introducing the related case study exercise.

The Toolkit and the Training events have been co-designed with a small number of older people living with frailty.

These individuals and their families have kindly shared their stories, creating person-centred case studies that bring to life the principles and the tools. They highlight the practical application of supported self-care at different stages of frailty and settings of care.

Through the training events they provide memorable experiences of how the Frailty Fulcrum and Supported Self-Care Plan work together to optimise an individual’s quality of life.

Very innovative in looking at how impairment affects function and, more to the point, of the feelings associated with impairment.
