Emergency Response Checklist Emergency Wardens In case of an emergency an Emergency Warden or designee will be responsible for coordinating the following responses; duties may be delegated: Evacuation (i.e. Fire Alarm) ____ Report to Assistant Lead Warden indicating you are present and that your floor is responding to alarm. ____ Report to Assistant Lead Warden any possible source of alarm, and call 911 if the Lead or Assistant Lead is unable to perform this function. ____ Remind staff to lock areas containing evidence upon evacuation and close doors upon exiting if this can be done safely. Do not lock areas believed to be a source of the emergency. ____ Advise Assistant Lead Warden when your floor has been cleared. ____ Gather at the UT-Southweste rn parking lot or other designated location. Assemble in the following area by floor: 4 th floor toward the closed end of the parking lot, followed by the 3 rd , 2nd, 1st,and morgue. ____ Perform a Roster Check using Employee Phone List and Visitor Log and immediately report any missing staff or visitors to Assistant Lead Warden including their last known location. ____ Be ready to perform a Roster Check for another floor if a Warden is not present. 1 st Floor Wardens: Lock Parkland door. 1 st Floor Wardens: Place emergency floor sign in front of elevator. Records: Take Visitors Log to evacuation point and give to Emergency Warden. Emergency Wardens

Relocation (i.e. Severe Weather)

____ Each floor should relocate to the stairwells or as directed by the Lead Warden orAssistant Warden. Relocation to the stairwell will be conducted as follows:

1st

Floor - Stairwell nearest Parkland2nd Floor- Stairwell nearest Loading Dock 

3rd Floor- Stairwell nearest Parkland

4th Floor- Stairwell nearest Loading Dock 

____ Report in to Assistant Lead Warden indicating you are present and that your floor is

responding to relocation.

____ Report in to Assistant Lead Warden when your floor is clear.

____ Perform a Roster Check using Employee Phone List and Visitor Log and immediatelyreport any missing staff or visitors to Assistant Lead Warden including their last known

location.

____ Be ready to perform a Roster Check for another floor if a Warden is not present.

____ Advise your assigned staff of “all clear” status and assist facility in returning to normal

operation

Lock Down (i.e. Violent Situation) 

Note: Records Area – Contact phone number for telephone room is 214-920-5899.

____ Report in to Assistant Lead Warden indicating you are present and that your floor is

responding to lock down.

____ Assist all staff and visitors in immediately moving to a locked area. Do not let anyone go

to other floors using the stairs or elevators.

____ Report in to Assistant Lead Warden when your floor is locked down.

____ As possible, perform roster check of areas on your floor by telephone from a secure

location.

____ Advise Assistant Warden of emergency related activities occurring in your area.

____ Advise your assigned staff of “all clear” status and assist facility in returning to normal

operation

Switchboard Operator and Field Agent Roles 

The Field Agent Office is the primary point of contact for facility emergencies.

At all times, the Switchboard Operator and Field Agents shall be prepared to receive notificationof a facility emergency and immediately report details to the Lead Warden or senior Field Agenton duty who will direct response.

Upon request of the Lead Warden, Switchboard Operator and Field Agents will be prepared to

immediately contact Emergency Wardens, advise them of a facility emergency, and providedirection for immediate action.

Each Field Agent is responsible for taking a County radio and cell phone, if available, to anyrelocation point.

Warden Roles 

Lead Warden -

  Oversee facility emergency response

  Contact Assistant Wardens and designate replacements as necessary  Advise and update IFS Administration

  Make emergency notifications to DSS, DSO, Parkland, etc.

  Communicate with emergency responders if applicable  Provide direction to Assistant Lead Wardens

  Evaluate whether 911 should be called and make the call  Determine need to deploy a Survey Team and outline their goals and objectives

  Determine “all clear” and communicate to Assistant Wardens

Assistant Wardens –

  Contact Floor Wardens and communicate emergency response actions

  Designate acting Floor Wardens if necessary

  Oversee staff response

  Oversee accountability of staff and visitors

  Organize and implement a Survey Team as directed by the Lead Warden

  Assist Lead Warden as directed  Advise Floor Wardens of “all clear” status and assist facility in returning to normal

operation

Floor Wardens –   Oversee staff and visitor emergency response within designated areas as directed by

Assistant Warden

  Provide backup for other Floor Wardens

  Communicate with Assistant Warden

  Advise Assistant Warden that you are present and available in an emergency

  Advise Assistant Warden that your floor is responding as directed

reported to the Assistant Warden, for example signs of fire, etc.

  Perform a roster check   Advise Assistant Warden of staff or visitors who are unaccounted for

 Communicate information from the Assistant Warden to your assigned staff 

  Advise your assigned staff of “all clear” status and assist facility in returning to normaloperation
