Electronic Health Record (EHR) Use Cases & Scenarios > EHR to create a virtual patient and develop a medical history via a library of electronic forms. EHR enables learners to:

Create a Virtual EHR Patient

1. Click Cases & Scenarios > EHR (Electronic Health Record) > New.

Note: If EHR is not visible, check Settings > System Administration > Task Access to apply the EHR task.

2. Complete the required fields and then click Create.

Field Description

Patient Details

Description Type a complete description of the virtual patient.

Classification 1. Click Add New to add primary and secondary identifier for this patient. This helps you organize your EHR patients and search more easily.

2. Click Show All to view all virtual patient classifications.

3. Click Manage to search for or edit a specific classification by keywords.

4. Click Save Changes to save the Patient Details.

Support Files

Instructions Type instructions for how the learner should use the support files as well as complete the EHR forms.

Support Files Click Support Files to upload files. The Support files will give more information about the patient. Perhaps the support files are X-rays, lab results, etc., that will help give more insight into the patient’s medical history or used to help the learner fill in the information on the patient’s EHR forms. The support file can be a document, audio, video or image file. You can upload files from your computer as well as files from the Multimedia Library. Click Save Changes to save the Support Files.

Patient Forms

Patient Forms The Templates are forms for the learner and educator to fill out as part of the patient’s medical record. The forms can be saved and can be used for future follow-up visits with this virtual patient when an EHR patient has been assigned to a case or scenario, and the case has been defined as a follow-up case.

Lab Reports Click to add existing multimedia files or add files to the multimedia library.

Pre-filled Forms Pre-filled forms can be either:

Forms for which you enter complete, partial, or no information, depending on how you want the learner to use it. For example, you might fill it out completely if the learner is going to review it as part of the pre-encounter. You might leave it blank if the learner will fill it out as part of the post-encounter exercise.

Forms that were already filled out in a prior case/scenario or EHR Exercise. When a Case or Scenario includes a follow-up visit, the forms travel with the virtual patient and the latest information can be added to the pre-filled forms or to new blank templates, all adding to the Patient’s Medical History.

If a learner has previously entered information in a patient’s EHR, you can attach the record the learner last saved to the same patient when the learner, or a different learner sees that patient again.

Click on the column headers to sort the information by ascending or descending order. Click this button to view the list of Pre-filled Forms in the database. Clicking on the Template Name hyperlink will allow you to view the form. To add the form to the patient’s record, check the checkbox next to the form name, and then click the Add button. You can add multiple pre-filled forms to the EHR virtual patient.

Add Blank Templates

Click the checkbox next to the template you wish to add or type a keyword to search for a specific form. Click the Template Name hyperlink to view (read-only) the template. You may select multiple templates by checking the checkboxes next to forms you want to add, and then click Add to associate the templates with the virtual EHR patient. Once the template is associated with a patient, you can edit the template.

Click Save Changes to save the Patient Forms.

Note: Click the drop-down arrow next to the form to change the order in which the learner should complete the

forms.

Define Form Availability

After the templates are added, specify whether the forms are available for edit or are read-only for the learner during the following:

Pre-Sim Study – Preparation for simulation session; scenario only and not graded.

Pre-Encounter – Preparation for simulation session and may be graded.

Post-Encounter – After simulation session, graded and counts in learner’s final score. If read-only is defined for that particular assessment, then they will only be able to view the forms. Edit access, the learner will be able to add responses.

Patient Form Default Settings

These are the edit and read-only default settings. Which means, if changes are not made to this section, this is

what the Pre Sim Study, the Learner Pre-Encounter, and the Learner Post-Encounter will be set to:

Learner Post-Encounter: Edit

For example, you can present the learner with several fully-populated, non-editable assessment forms in the

pre-encounter, for review. Then, have the learner complete a blank or partially completed form as part of the

post-encounter.

1. From the Patient Forms tab, click the Form Name.

2. Scroll to the bottom of the Form Viewer screen to the Display Options section.

3. Select View or Edit to grant access rights to the learner when the learner receives the form to complete during the: Pre-Sim Study, Pre-Encounter, and/or Post-Encounter.

4. Weight – Add a weight so that the responses can be collected in the scoring process. The default weight is “1”. Forms can be weighted differently, depending on importance of the overall forms the learner fills out per patient or per exercise. Scores are given for each individual form and if there is more than one form assigned to the learner(s), the scores of each form are averaged together for an overall grade.

5. Save Changes at the bottom of the form.

6. Save All Changes at the bottom of the EHR Patient Activity page.

Tool Belt – EHR Patient Activity Page

The EHR Tool Belt appears on the left column of the screen.

Click Cases/Scenarios to find any cases or scenarios that are associated with this EHR patient.

Click Sessions/EHR Exercises to find any sessions or EHR exercises in which this patient was used.

Click Home to return to this patient’s activity page.

Search for an EHR Patient

To view a list of EHR Patients assigned to a case or scenario, click Cases and Scenarios > EHR. A list of EHR patients appears in the grid. Any Classifications that were created in any of the EHR Patient Activity pages appear in the Explore tab. This is a convenient way of finding EHR patients that have been associated with that classification. Advanced Search Options Click Cases and Scenarios. Use the following search features to search for a specific EHR patient:

Click the column headings in the grid to sort the list in ascending or descending order

In the Search field, enter a keyword and then click Search

Click the drop-down box next to the Search field to narrow your search by name, gender, age, or status

Use the following features to manage your EHR patients:

Delete: Click the Delete icon. Note: The EHR patient cannot be deleted if assigned to a case or scenario.

Copy: Click the Copy icon to create a new EHR Patient based on an existing patient.

Print: Click the Print link to print the EHR Patient List. Note: Defining search criteria in either the Explore or the Search tabs, and then adding a search criteria in the Search field at the top of the Search Results Page will further filter your search.

Search Tab

Use the following search features to conduct an advanced search for a specific patient:

1. Click the Cases & Scenarios module.

2. Click the Search tab. The advanced search features appear.

3. Click EHR.

4. Enter the search criteria to search for a specific EHR patient by:

Note: Defining search criteria in either the Explore or the Search tabs, and then adding a search criteria in the

Search field at the top of the Search Results Page will further filter your search.

Map an EHR Patient to a Case or Scenario

Multiple EHR patients can be mapped to a case or scenario, then when you schedule a session, you can

choose which EHR patient you want to associate with that session.

To map an EHR patient to a case or scenario:

1. Click Cases & Scenarios and then click the desired case or scenario.

2. From the Tool Belt on the left, click EHR to assign a virtual patient to this case or scenario. The EHR Patients screen appears. This will list any EHR patients already assigned to the case or scenario.

3. Click Assign EHR Patient.

4. Click the checkbox next to one or more EHR virtual patients and then click Assign to associate the patient(s) with the case or scenario.

View All EHR Patients Assigned to a Case or Scenario

1. Click Cases & Scenarios and then click the desired case or scenario. 2. From the Tool Belt on the left, click EHR to view all EHR Patients assigned to this case or scenario.

Note: Click the EHR Patient Name link to add or edit the Support Files or Patient Forms.

Schedule the Case or Scenario with an EHR Patient

1. Click Session & Courses > Simulation Sessions > Add New. The Add Session screen appears. Note: If you have already scheduled the session, but just want to add the EHR patient to the session, proceed to Step 4.

4. Click the Case/Scenario Details tab.

6. Select the Room Name, the Case/Scenario Name and then select the EHR patient. 7. Complete the Case/Scenario and Room Details and then click Save Changes to create the session

with the EHR patient.

Note: The learners assigned to this session on Participant Details tab will receive the EHR forms to complete

with this session.

Assigning the EHR Forms to a Team of Learners

A team of learners can complete the assigned EHR forms from a Case/Scenario. Create a Team Simulation Session, add the case/scenario, EHR Patient, team of learners and then specify the EHR forms to be a “team form submission.”

1. Click Sessions & Courses > Simulation Sessions > New. The Add Session screen appears.

2. To create a team session, select Team from the Session Type field.

3. Complete the required fields and then click Create. The Session activity page appears.

4. Click the Case/Scenario Details tab. 5. Click the Add Case/Scenario Details button. 6. Select the Room Name, the Case or Scenario Name and then select the EHR patient.

7. Complete the Case/Scenario and Room Details and then click Update to create the session with the EHR patient.

8. Select the Room Name, the Case/Scenario Name and then select the EHR patient. 9. Complete the Case/Scenario and Room Details and then click Save Changes to create the session

with the EHR patient. 10. The team of learners assigned to a session with this case or scenario will receive the EHR forms

associated with this EHR Patient.

2. Search for and select participants by checking the checkboxes next to their names. Note: If this is a Scenario with roles, select the roles the participants will be playing. The roles are used for the Role-based Checklist Performance Assessment.

3. Add a Team Name and then click Save Changes.

4. Click Advanced View to define whether this will be a Team EHR form submission or an individual form submission.

5. Click the EHR patient name link.

If Team Form Submission is selected, one copy of the EHR document will be available for the entire

team. Once a team member accesses the document, it is locked for editing. When the document is

locked, it will be available to other team members as read-only. All team members’ responses will be

collectively on the same form.

It is possible to have a Team Session, and have the team members fill out the EHR forms individually. Define this by choosing the Individual Form Submission radio button in the Patient Summary Details section.

8. You can also add Support Files and Patient Forms applicable only to this session. Note: Any changes made to this Support files and Patient Forms sections will not be reflected on the EHR Patient’s records in the Cases & Scenarios > EHR Patient task module. The changes will only pertain to this session only.

Preview the EHR Patient Forms

See what the learner will receive by clicking on Practice Response in the Session’s Tool Belt.

How the Learner Accesses the EHR Patient Forms – Case/Scenario

When a learner and the EHR Patient are both assigned to the session, the learner will receive the link on his or her dashboard when he or she logs in. The link will be according to which type of checklist was assigned in the EHR Patient Activity Page: Pre Sim Study, Learner Pre-Encounter, Learner Post-Encounter, read or edit access. See Define Form Availability for more information.

If read-only was defined for that particular assessment, then they will only be able to view the forms. Edit

access, the learner will be able to add responses.

Requirements to Receive the EHR Patient Forms Assigned to a Simulation Session

At least one question needs to be added to the respective checklist type: Pre-Sim Study, Pre-Encounter, and Post-Encounter, as defined in the Display Options of the EHR Patient activity page > Patient Forms section in the Learner Assessment area of the Case / Scenario activity page, in order for the EHR patient activity to be presented to the learner at that time.

The “Availability in days” for the respective checklist type needs to be defined in the actual Case or Scenario being scheduled.

Learner(s) need to be assigned as participants in the session.

1. On the learner’s Dashboard, the learner will click on the Action Start link from the To-Do List to start

the assessment.

2. Click on the EHR Patient’s name hyperlink to begin the EHR activity.

4. To navigate to the next form when more than one form is attached, click the Next button; alternatively, navigate back to the previous form by clicking on the Previous button.

5. If this activity is view-only, then close the window by clicking on the close (x) icon when finished with

the EHR Activity, to resume to your checklist questionnaire.

6. If this activity is editable, navigate between the forms using the Next and Previous buttons, when ready to save your responses, click the Save Changes button. Close the window by clicking on the close (x) icon. This will bring the learner back to the assessment checklist questions. While in the questionnaire, the learner can click on the EHR patient name hyperlink to continue working on it and then close the window to return to the questionnaire portion of that assessment.

7. When the learner is finished with the questionnaire (on previous page), they will need to click the

button, which will send it from the learner’s dashboard and submit the entire assessment - questionnaire and EHR forms to the Evaluations task module, under the Learner Encounter Notes.

Edit Responses from the Case/Scenario Assessments

When an EHR patient has been assigned to a session and the learner has filled out the forms, there are two areas to edit submitted responses: the Sessions and Courses > Simulation Sessions > click on the scheduled session; and in the Evaluations task module as described in the Score an EHR Submission section of this document. Sessions and Courses

1. Click Sessions and Courses and then click the scheduled session.

Schedule a Standalone EHR Exercise

You can schedule a standalone EHR Exercise, one that is not associated with a session. Learners can review support files and even prior medical records to complete the attached forms. Individual learners can be assigned to fill out the forms. More than one learner can be assigned to the exercise, however the forms are filled out individually by each learner. This is not a team exercise.

1. Click Sessions and Courses > EHR Exercise > Add New.

2. Complete the required fields and then click Create. Availability dates and times will indicate when the forms are available to the learner(s).

3. The EHR Exercise activity page appears. Changes can be made to the Name, and available dates. Continue through the page to add the Patient and Participant information Save Changes at every section that information has been added or edited.

5. Click the checkbox(es) next to one or more EHR patients and then click Assign to assign the patient(s) to the EHR exercise.

6. Click the following Options as needed:

Define start and end date for each EHR patient exercise. The Start and End Dates appear. Click on each link to define when the learner can start the Patient Exercise and when it will no longer be on the learner’s dashboard to access.

Complete or review each EHR patient form before displaying the next EHR Patient in the assigned order. This is valid when assigning multiple patients to a learner. This ensures that the learner completes each form first before continuing to the next.

8. Click the checkboxes next to the participants you wish to assign to the EHR exercise and then click Add.

9. In the Schedule Participant grid, click the drop-down arrow under the Order Column, to change the participant order.

10. Click Save Changes..

How The Learner Accesses the EHR Patient Forms – EHR Case or Scenario

The EHR appears as a Pre- and/or Post-encounter item on the Learner’s Dashboard in the To-Do list. When

the learner clicks Start, the Pre- or Post-encounter screen opens. If the case or scenario has other pre- or

post-encounter activities (Note, Support Files, etc.) the student will have to complete those also.

1. On the learner’s dashboard, the Pre- and/or Post-encounter will be listed under their To-Do List. The learner will click on the Action Start link to start the Exercise.

2. The learner will click on the EHR Patient’s name to begin the EHR activity.

3. The Patient’s forms will display for the learner to complete.

4. Click the X icon in the top right corner to close the EHR forms.

How The Learner Accesses the EHR Patient Forms – EHR Exercise

The EHR Exercise appears on the Learner’s Dashboard in the To-Do list. When the learner clicks Submit, the

EHR forms will display. The exercise appears on the learner’s Dashboard until the Due Date defined when the

exercise was created.

1. On the learner’s dashboard, the EHR Exercise, as well as the EHR Patient’s name will be listed under their To-Do List. The learner will click on the Action Submit link to start the EHR Exercise.

2. The Patient’s forms will display for the learner to complete.

Edit Responses of the EHR Exercise When the EHR Patient forms have been filled out by the learner, the submitted responses can be viewed and edited. There are two areas to edit submitted responses: from Sessions and Courses > EHR Exercise > locate the scheduled EHR Exercise, and the Evaluations task modules. For instructions to access them in Evaluations, see Score an EHR Submission section of this document. For instructions on accessing the submitted responses in Sessions and Courses, please consult the Sessions and Courses section of the SIMULATIONiQ Enterprise Online Guide.

2. From the Tool Belt, click Edit Responses. The Learner EHR Submission grid appears. The Educator can review and edit all responses that have been submitted.

3. Click on the Learner’s name to view and edit their submitted EHR Exercise.

4. When finished with editing, Save Changes (click the checkbox and press Print to generate a .pdf of responses).

Score EHR Patient Forms or EHR Exercise Submission

When the Learner completes the form(s) and clicks Saves Changes, the form(s) appear in the Evaluation module so it can be scored by the educator.

3. The Educator clicks the Score link in the Score column to view and score the responses.

Scoring the Exercise

EHR Exercises require manual scoring:

1. Enter a Percentage score for the form and then click Save Changes.

2. Click Next to proceed to the Next form.

3. Calculate overall score for all forms in the exercise.
