As is commonly known, Ulcus cru-ris indicates an ulcer in the lowerleg due to local disturbances in thevenous and arterial circulation. Forthe therapy of these wide spreaddiseases, the SANUM preparationsMUCOKEHL 5X for injection andMUCOKEHL 3X ointment wereexamined in patients regardingeffectiveness and compatibility.Both SANUM remedies wereapplied simultaneously in a com-bination therapy.

This study was performed at theInstitute for Pharmacology andToxicology (Pomeranian MedicalAcademy) in Szcecin, Poland. Thedirector of this study was Profes-sor Dr. med. Jerzy Wojcicki, Headof the Department for ClinicalPharmacology. Dr. med. LidiaSzyroki (Director of the Dermato-logical Department) and threespecialists carried out the study.The study had been approvedbeforehand by the Ethics Com-mission of the hospital. Likewise,the patients had been instructed bythe examiners regarding the nature,significance, and effect of theirparticipation, and they gave theiragreement to participate.

Demographic Data an theExaminationIn total, 30 male patients sufferingfrom Ulcus cruris of various stageshad been admitted to the study.They were treated exclusively withthe above SANUM preparations,the contents and effectivesubstances whereof are known toconsist of low growth-forms ofMucor racemosus. The averageage of the patients was 58 years(the youngest patient was 37 yearsold; the oldest patient was 83 years

old). The average body weight ofthe patients was 78 kg (minimum68 kg, maximum 88 kg), with anaverage body size of 172 cm (mi-nimum 164 cm, maximum 179 cm).In most cases, the UIcus cruris hadits pathogenesis in a thrombo-phlebitis or in varicose veins (Ulcusvaricosum).

Tests and Tr eatmentsAccording to Their Type andDurationAt the beginning of the study, allpatients underwent tests. There-after, tests were performed everyother week. These tests comprised:medical routine examinations, he-matological exams, urine analysis,examination of the spread and

depth of lesions, examination ofthe condition in which the margin ofthe ulcer is, along with its surround-ing tissue, and comparison of thegirth of the extremities. Questioningthe patients concerning subjectivesymptoms (pains, pruritis, skintension, burning sensation) was alsopart of these examinations.

The study was characterized by“prospective, without compari-sons, controlled, and monocentric.“Twice a week (on Tuesdays andFridays), the patients received aninjection of 1 ml ampule ofMUCOKEHL 5X i.m.. MUCO-KEHL 3X ointment was appliedtwice daily over the entire surfaceof the ulceration. The length of the

treatment was eight weeks, on theaverage.

The Results of the ClinicalExaminationsBefore the treatment, every ulcer-ation was covered with fibrinouspussy coverings or necrotic crusts.Several ulcerations were painfuland peripherally strongly inflamed.Already after 5-10 days of thecombination treatment with theabove preparations, a reduction ofthe painful symptoms could benoted. There was also a reductionof erythema and secretion indica-ting the effectiveness of thesepreparations. The change ofbandages occurred with consider-ably less pain. Moreover, therewas a distinct tendency toward theformation of granulations tissue.After 14-28 days of treatment,there was already the beginningof epithelizatio noticeable.

Concerning the compatibility of theapplied SANUM preparations,there was no undesirable systemicside-effect observable in any of thetreated patients. The local tolerancealso was evaluated as good. Thei.m. injections with MUCOKEHL5X were not felt to be painful, andthey caused no inflammationswhatever at the point of application.In only one patient was there atemporary erythema at the start oftreatment with MUCOKEHL 3Xointment, which disappeared as thetherapy progressed.

Evaluation and Discussion ofResultsThe effectiveness of the prepa-rations was evaluated according tothe following 5-step scale: “totalhealing“, “excellent“, “very good“,

“good“, and “no improvement“.Both the objective and subjectivesymptoms were the basis for theevaluations. In result, the followingevaluations of the effectivenesswith the patients were determined(see also Picture 3):

- in 14.28% the effect was„excellent“ (see Pictures 1 and2);

- in 57.14% the effect was „verygood“;

- in 28.5% the effect was „good“.

This means that in appr. 71% of thepatients, there was noted anexcellent or very good therapeuticaleffect of the SANUM prepara-tions. In no case was there noticeda change to the worse or an absenceof changes in the ulceration.

Concerning compatibility, neitherany systemic nor local signs ofincompatibility could be observedfor both forms of application. Thus,the cutaneous ointment applica-tion, likewise, showed a goodcompatibility. The submitted resultsof the study prove that the applied

combinations therapy for Ulcuscruris deeply and permanentlypromotes wound-healing, withoutany relevant side-effects. In view ofthe relatively small number ofpatients examined, it appearsappropriate to undertake a larger-scale study for additional con-firmation.

Apart from the reported study,during its course there were twopatients with severe burns treatedwith MUCOKEHL 5X injectionsand MUCOKEHL 3X ointment,according to the identical scheme.

As confirmed by Pictures 4 and 5,here too, a definitely good healingwas achieved with these prepa-rations.
