Dear Parents/Guardians:
Welcome to Lynwood Unified School District! In order to ensure that the District has the most
accurate and up-to-date information about your child, we have included the following information
regarding the registration process.
Instructions to Register a Student in the Lynwood Unified School District.
1. Parent/Guardian must print and complete one (1) registration packet per student. Packets can be
obtained from the school website at www.lynwood.k12.ca.us or any school building in the
District.
It is important that packet(s) be completed BEFORE attending registration appointment!
2. Parent/Guardian must bring completed registration packet(s) AND the following documentation
to registration appointment:
Proof of Age: Legal Birth Certificate, Baptismal Certificate, Passport, Bible Record,
Immigration Certificate or affidavit from parent/Guardian
Custody paperwork or proof of guardianship (if applicable)
Immunization records
Proof of physical exam for school entry
Previous school records (grades, attendance, behavior)
Proof of residency: (two of the following is required)
Homeowners – Original Tax Bill, Title, Mortgage Statement, or Piece of Mail
OR
Renters – Original Lease (Parent/Guardian’s name must appear on this lease) or
Piece of Mail
OR
Living with a homeowner or renter of the District – Resident of the District must
provide a letter stating that you and the child(ren) reside at such address, along
with proof of residency listed above.

1. El padre o tutor debe imprimir y llenar un (1) paquete de matrícula por estudiante. Los paquetes

To make sure your child is ready for school, California law, Education Code Section 49452.8, now requires that your child have an oral health assessment (dental check-up) by May 31 in either kindergarten or first grade, whichever is his or her first year in public school. Assessments that have happened within the 12 months before your child enters school also meet this requirement. The law specifies that the assessment must be done by a licensed dentist or other licensed or registered dental health professional.
Take the attached Oral Health Assessment/Waiver Request form to the dental office, as it will be needed for your child’s check-up. If you cannot take your child for this required assessment, please indicate the reason for this in Section 3 of the form. You can get more copies of the necessary form at your child’s school or online from the California Department of Education’s Web site at http://www.cde.ca.gov/ls/he/hn/. California law requires schools to maintain the privacy of students’ health information. Your child’s identity will not be associated with any report produced as a result of this requirement.

Remember, your child is not healthy and ready for school if he or she has poor dental health! Here is important advice to help your child stay healthy:
• Take your child to the dentist twice a year.
• Choose healthy foods for the entire family. Fresh foods are usually thehealthiest foods.
• Brush teeth at least twice a day with toothpaste that contains fluoride.
• Limit candy and sweet drinks, such as punch or soda. Sweet drinks and candycontain a lot of sugar, which causes cavities and replaces important nutrientsin your child’s diet. Sweet drinks and candy also contribute to weightproblems, which may lead to other diseases, such as diabetes. The lesscandy and sweet drinks, the better!
Baby teeth are very important. They are not just teeth that will fall out. Children need their teeth to eat properly, talk, smile, and feel good about themselves. Children with cavities may have difficulty eating, stop smiling, and have problems paying attention and learning at school. Tooth decay is an infection that does not heal and can be painful if left without treatment. If cavities are not treated, children can become sick enough to require emergency room treatment, and their adult teeth may be permanently damaged.
Many things influence a child’s progress and success in school, including health. Children must be healthy to learn, and children with cavities are not healthy. Cavities are preventable, but they affect more children than any other chronic disease.

El deterioro dental es una infección que no sana y que puede ser dolorosa cuando se deja sin tratamiento. Si las caries no son atendidas, los niños pueden enfermarse al grado de requerir atención médica de emergencia y sus dientes adultos pueden presentar un daño permanente.
Hay muchas cosas que influyen en el progreso y éxito de los niños en la escuela, entre ellas está la salud. Los niños deben de estar saludables para aprender y los niños con caries no son niños saludables. ¡Las caries son prevenibles, pero afectan a los niños en mayor medida que cualquier otra enfermedad crónica!

Primary language support may be provided by the teacher, and/or through the use of para-‐educators, peer tutors, parent volunteers, instructional materials, etc. My child may be grouped with students from other grade levels and other primary languages who have the same English proficiency levels to develop English. Whenever possible, my child is grouped with other students of the same grade level, in order to give the student access to grade-‐level core curriculum. Instruction in the core curriculum utilizes Specially Designed Academic Instruction in English (SDAIE) strategies and techniques are self-‐image and cross-‐cultural instruction is integrated throughout the subject areas.
❒ Mainstream English Program Option In this option my child develops second language literacy in English, and is taught the core curriculum in English, in addition to daily, formal lessons in English Language Development (ELD). He/she is grouped with English Only students, Fluent English Proficient students, and other English Learners. Instruction in the core curriculum utilizes SDAIE strategies and techniques that make the instruction understandable for the students. Self-‐image and cross-‐cultural instruction is integrated throughout the subject areas.
❒ Alternative Program (Bilingual Program Option) In this option, my child develops, primary language literacy, and is taught the core curriculum (Math, Science, Social Studies, etc.) in his/her primary language, all the while acquiring English through daily, formal lessons in English Language Development (ELD). My child will formally transition into English literacy core and curriculum instruction in English when he/she exhibits cognitive proficiency in his her primary language, as evidence by mastery of LUSD’s Transition Criteria, and when he/she has reached the Intermediate Fluency Level in English Acquisition. Each English Learner receives a daily program of English Language Development, along with instruction in the core curriculum delivered either through primary language or Specially Designed Academic Instruction in English (SDAIE), depending on my child’s proficiency in English and his/her primary language.

OPCIONES PARA LA ASIGNACIÓN DE PROGRAMAS FORMA DE DOCUMENTACIÓN

Me han explicado las tres opciones de los programas que son ofrecidos para mi hijo/a quien es un estudiante aprendiz de inglés. Yo escojo la siguiente opción:
❒ La opción del Programa Inmersión Estructurada en Inglés (SEI) En esta opción, mi hijo/a desarrolla la alfabetización en inglés, mientras recibe instrucción del currículo básico en inglés, además de las lecciones diarias, formales, en el Desarrollo del Idioma Inglés (ELD). Apoyo en el idioma materno puede ser proporcionado por el profesor y/ o mediante el uso de ayudantes, tutores, padres voluntarios, materiales didácticos, etc. Mihijo/a puede ser colocado/a con estudiantes de diferentes grado y otros idiomas maternos que tienen los mismos niveles de competencia en Inglés para desarrollar inglés. Siempre que sea posible, mi hijo se agrupara con otros estudiantes del mismo grado escolar, con el fin de dar a los estudiantes el acceso al currículo básico a nivel del grado. La instrucción en el currículo básico utiliza estrategias y técnicas de instrucción académica especialmente diseñada en inglés. (SDAIE) La instrucción integra el aprecio propio y un entendimiento a través de las culturas en todas las materias.
❒ La opción del Programa regular de Inglés (Mainstream) En esta opción, mi hijo/a desarrolla la alfabetización en el segundo idioma, Inglés, mientras aprende el currículo básico en Inglés, además de tener lecciones diarias, formales en el Desarrollo del Idioma Inglés (ELD). Él / ella es colocada con estudiantes que solo hablan Inglés, estudiantes que tienen habilidad en Inglés, y con otros estudiantes aprendices de Inglés. La instrucción del currículo básico utiliza estrategias y técnicas SDAIE (instrucción académica especialmente diseñada en inglés) la cual hace la instrucción comprensible para los estudiantes. La instrucción integra el aprecio propio y un entendimiento a través de las culturas en todas las materias.
❒ La opción del Programa Alternativo (Bilingüe) En esta opción, mi hijo/a desarrolla, la alfabetización en el idioma materno, y recibe currículo básico (matemáticas, ciencias, estudios sociales, etc.) en su idioma materna, al tiempo que la adquiere Inglés a través de las lecciones diarias, formales en Desarrollo del Idioma Inglés (ELD). Mi hijo/a empezara la transición al programa de Inglés e instrucción en Inglés cuando demuestre habilidad cognitiva en su primer idioma, según la evidencia del criterio de transición del distrito LUSD, y cuando él / ella logre la fluidez Intermedia en el nivel de adquisición de inglés. Cada estudiante aprendices de Inglés recibe instrucción diario de Desarrollo del Idioma Inglés (ELD), junto con la instrucción del currículo básico a través del idioma materno o Instrucción Académica Diseñada Especialmente en Inglés (SDAIE), dependiendo de la habilidad del alumno en Inglés y su / su idioma materno.
