summer is finally arriving in the MeNA region! For so many, the summer months represent the coming to an endof an era, schools close, students graduate, businesses wind down and employees take some rest. During the pastfew months we have been spending lots of time with soon-to-be graduating dental students from across theregion at a variety of events and meetings. We recognise that whilst graduating is an enormous achievement andexciting time, for many students, graduation represents a huge change which can bring some discomfort. Wehave always felt passionate about supporting students in their studies with the various grant and educationalprograms that we have on offer and we feel it’s important to extend our support to graduating students that aremaking the transition to becoming fully fledged dentists. At the end of June, we held a special graduation eventfor dental students from Lebanon in Beirut, giving them the chance to not only get know our products better, butmost importantly build relationships with our local team (page 19). in addition, we recently hosted the regionalheat of the Global Restorative Clinical Case Contest (GCCC) for dental students. i had the pleasure of co-judgingthe winners from each of the 10 participating universities and selecting the three MeNA finalists that went ontothe global final at our Restorative headquarters in Germany. i was impressed with the clinical quality, confidenceand presentation skills of the finalists and congratulate the winners – go to pages 20-21 to see the results.

in addition to articles on our university and student support, the fourth edition is full of new information includingan introduction to our Clinic solutions department (page 4), a commentary from Professor Mannocci on how 3Dimaging is streamlining the endodontic workflow (page 14) and a practice management article on the perceptionsvs. realities of advanced dental technology (page 10).

single tooth immediate placementusing the Astra Tech implantsystem® eV and Atlantis® AbutmentThe patient presented with a fractured maxillary right lateralincisor (#12) with a dislodged endodontic post. Due tocrown-to-root ratio and short remaining root, extraction andimmediate implant placement was elected. Care wasprovided with an Osseospeed® eV 3.6 diameter implantplaced towards the palate following the 3x2 rule. Followingeight weeks of healing, stage-ii was performed and the finalrestoration completed within six weeks using an Atlantis®Abutment in gold-shaded titanium with concave emergenceshape selected. The final crown was an all-ceramic zirconiacrown.

What percentage of your patients take advantage of your investment in the following technologies?

imaging technology turns out to be the real workhorse ofdental practices. seven out of ten survey respondents reportincorporating 2D digital imaging into the care plan ofbetween 76% and 100% of their practice’s patients. intraoralcameras are the next most often used technology. 49% ofrespondents say that an intraoral camera figures into thetreatment protocol of between 76% and 100% of patients.

Patients and TechnologyAdvanced technologies, including single Visit Dentistry withCeReC (which saw a 96% brand recall among therespondents), are clearly gaining awareness and significanttraction among patients. it is important to know that in therecent “What Dental Patients Want”* survey fromFuturdontics®, many patients between the ages of 25 and44 said that they would switch from their current dentist ifthey did not offer advanced technology treatment options.Dentists who are holding onto many traditional methodsand not integrating new technologies waiting for “patientdemand” to catch-up before they invest may findthemselves also playing catch-up with competing practices.

in recent years, a team at King’sCollege London has completed anumber of clinical trialshighlighting the importance ofCBCT (Cone Beam ComputedTomography) in diagnosis andoutcome assessment inendodontics. As endodontists,we are all now familiar with thebenefits of using CBCT scans toidentify where the problems are

within the tooth. We use this technology to help us viewtrauma such as tooth fracture or examine where a root canaltreatment has failed.

it is well known that the presence of radiolucencies at theapex of a root is symptomatic of endodontic infection suchas granulomas or cysts. in the majority of endodontic cases,the assessment of outcomes is reliant on the detection ofthese apical radiolucencies or exposing any change in theirsize.

A radiographic technique (CBCT) demonstrates far bettersensitivity and specificity at detecting radiolucencies thantraditional periapical radiographs. With periapicalradiographs, it has been demonstrated that the number ofroots cannot be seen clearly, so we are not just missingradiolucencies at the apex of the root, but missing entireroot canals. it is important to remember that the radiationdose delivered to the patient must also be considered, whenassessing treatment modalities. A periapical radiographdelivers 0.14% of annual background radiation, rising to 0.2%with panoramic, whilst a conventional CT scan delivers 39%.A small field of view CBCT scan delivers barely 1%, whichalthough around 7 times higher than a traditional scan, is infact, much less than taking a long-haul flight, say from Paristo Tokyo, that delivers 4 times this radiation dose.

We can now also use CBCT in the actual design of root canaltreatment, effectively planning access to the pulp chamber,and 2017 sees the launch of 3D endo, a new software byDentsply sirona, which will improve individual treatmentplanning using CBCT. This software will help us to visualisethe direction and position of the canal and the ideal shapeof the access cavity. 3D endo enables the user to isolate thetooth being treated and locates the orifice and apex of thecanals. This makes it possible to add more points to thecomputer image, resulting in more precise tracking of eachindividual canal, no matter how curved they may be. The final3D representation can be rotated 360° and allows us todetermine the working length, analyse the natural shape ofthe canal and select the appropriate files using the integratedfile database.

in root canal treatment, there is always a need for strictinfection control to prevent bacteria getting into lesions andcreating infection that can lead to failures. A tight coronalseal is especially important to prevent bacteria penetratingthe tooth at a later stage. if the tooth is particularly damagedit is more difficult to achieve an adequate coronal seal andmakes the tooth more prone to failure through bacterialinfection. CBCT imaging plays a vital role in such cases, asthese teeth are likely to be more prone to small cracks andfractures, which are difficult to detect using traditionalscanning methods.

Dr. Ahmed soliman idris – Welldent Clinic, Cairo, egyptDr. Ahmed soliman idris is the founder of Welldent Clinic which was established six years ago in Cairo. Two branches arenow open, one in Dokki and one in Fifth settlement. Dr. Ahmed soliman idris graduated with a Msc & PHD in FixedProsthodontics. He is a lecturer at the Faculty of Dentistry Cairo university since 2001 and a lecturer at the Faculty ofDentistry British university in egypt since 2013.

A. i think the most challenging part of a Class ii restorationis the creation of a proper contact. The main difficulty isto make a proper contouring of the contact area in a shorttime and with great precision to avoid food accumulationand subsequent periodontal problems later on.

Global Clinical CaseContest 2018

MeNA Regional FinalDubai, uAeOur regional finals took place on 12th

April 2018 and were judged by Dr. AmroAdel and Dr. Peet van D. Vyver in ourDentsply sirona Academy. Theshortlisted students were from 11different universities but there couldonly be 3 winners - Ghida Lawand(Beirut Arab university, Lebanon),Monika Ayoub (Ain shams university,egypt) and Martin Moubarak (Lebaneseuniversity, Lebanon).

Dentsply sirona’s Global Clinical Case Contest (GCCC) is a globaldental school talent competition which honours outstanding clinicalresults submitted by dental students.since its inception in 2004-2005, more than 2300 dental students have participated, with the 2016-2017 competitiondrawing a total of 592 entries from 169 universities. Dental undergraduate and graduate students, with less than 2 yearsof clinical practice, are invited to participate by documenting a patient case in a standardised format with photographsand text of clinical anterior and posterior direct restorations using the entire Dentsply sirona restorative product line. Forthe MeNA region, over 150 students from 10 universities took part. They selected their best student case to representthem at our regional final in Dubai, uAe. The 3 winners then went forward to participate in the global final in Konstanz,Germany.

Global Final Konstanz, GermanyOn 27th-28th June the MeNA regional winners travelled tothe Dentsply sirona Restorative Headquarters in Konstanz,Germany and presented their cases to an international panelof judges. We were lucky enough to have Martin Moubarakfrom Lebanese university (tutor: Dr. Paul Nahas), place inthe top 3 of the global winners - his case is on the next page.The 1st place was given to Matteo salvato from italy(universita degli studi di Verona, tutor Dr.ssa ZottiFrancesca) and 3rd place to Anna Braeckmans from Belgium(Ku Leuven, tutor: Prof. Dr. Marleen Peumans). All threefinalists will have their cases presented at the Dentsplysirona booth during the iADR Congress in London, on 25th

introduction to the CaseWhen facing vital anterior teeth that have been previously restored with old composite on multiple surfaces, using directcomposite restorations are still considered to be the least invasive technique with durability comparable to indirectrestorations. This is true due to the advancements made in modern composite components that shifted towards fillers,and gives high wear resistance, glossy surfaces, and increased longevity, especially when compared to older compositematerials.

Material and MethodA deep analysis of the case started from clinical observation,radiological examination and model analysis. Then a wax-up wasprepared on the models with respect to restoring the aestheticparameters: Midline, teeth ratio, position of the incisal edges in relationto the lip line, and the teeth axes. A silicon key was taken in order tocopy the wax-up into the patient's mouth in order to gain her approval.After confirming pulp vitality, deep scaling was performed. Then, aftereliminating the old restorations, teeth were cleaned from all types ofdecay and a minimal preparation was done to the rest of the dentaltissues. isolation, etching (Detrey® Conditioner 36 gel), bonding(Prime&Bond® One) and then building up the teeth with the latest high-end composite (ceram.x® sphereTeC™). Finally, after checking theocclusion and anatomical patterns, finishing was completed with theenhance® Finishing system and polishing was then done with theenhance® POGO Polishing system.

Discussion and ConclusionWhen surfaces and margins of old restorations are infiltrated withdecay, tarnished with stains, have fractured edges, and when theinvolved teeth maintain their vitality, re-building teeth anatomy andfunctionality with conventional composite is still considered aminimally invasive good choice, even if large restorations are involved.in this case, pre-operative analysis was essential, and the choice ofbonding a nano-filled composite should play a major added value tothe longevity and aesthetics of the new restorations.

The patient is a 48 year old female. There are compositerestorations on each tooth (13 to 23) showing a lot of stains withsurface roughness and recurrent caries. Also, neither the midline(between 11 and 21) nor the teeth size ratio is respected. Moreover,axes of the teeth are not parallel.

One week after scaling and hygiene motivation, old restorations andcaries were removed. since all the teeth were still vital, new fullcomposite restorations were done on the 6 anterior maxillary teethusing only ceram.x® sphereTeC™ composite.

step 1 - smile AnalysisPatient‘s upper lip line clearly hid the gum.Therefore, a gum surgery wasn‘t necessary.Teeth shade was selected: A2 to build themiddle and cervical margins of the crownand A1 for the incisal edges.

step 2 - smile DesignMajor aesthetic parameters were evaluateddigitally.

step 3 – Central incisors Preparation,isolation and etchingAfter the preparation of the teeth, a properisolation was achieved with a rubber dam.etching the residual dental structures withDeTrey® Conditioner 36 was done followedby a thorough water spray rinse.

step 4 – Bonding and Composite Build-upPrime&Bond® One etch&Rinse was appliedand light cured from all sides. A silicon keywas used to build the palatal surfaces andthe incisal edges.

step 5 - Restoring the Central incisors using a wooden wedge supported celluloidmatrix, contact points were established,followed by a veneering of buccal surfaces.After making sure that teeth dimensionsand midline position were respected, therest of the preparations could be achieved.

step 6 - Restoring the Rest of the Teethisolation of centrals with teflon tape duringetching and bonding procedures. Then therest of the teeth were restored following thesame methodology.

step 7 – PolishingAfter restoring the lateral and canines, theocclusion was checked. Then finishing ofthe composite restoration was done with theenhance® Finishing and POGO Polishingsystem.

step 8 – Final Result Respecting AestheticParametersPictures taken 1 week after polishingshowing the achievement of the intendedaesthetic parameters and the outstandingmaterial quality.

The endo, CAD/CAM and Laser event took place on 25th April inRiyadh. it was one of the first events in saudi that combined 3topics from consumables to equipment.
