The LPC will negotiate to ensure that all services have suitable training and set up costs built into agreements to enable this to happen. The LPC will also work with contractors to encourage uptake of the Medicines Use Review advanced service by patients and to improve its acceptance by doctors and other healthcare professionals. We will make use of the national communications programme being developed but we will also develop our own local programme. The LPC will also work to encourage training, to support contractors to develop their staff and will actively encourage contractors to maximise the use of medicine counter assistants, dispensing assistants, dispensing technicians and checking technicians wherever possible. This will enable pharmacists to have more time to talk to patients and offer advanced and enhanced services to them, improving the quality   of   patients’   health   (as   encouraged   by   the   recent White Paper.) We will look to support clinical Continuing Professional Development (CPD) via local branch meetings and Centre for Pharmacy Postgraduate Education (CPPE) meetings as well as encouraging pharmacists to undertake training leading to an Independent Prescriber

qualification or Diploma in Medicines Management.

It has been a very busy year for all the officers in Sunderland LPC, dominated throughout by the forthcoming reorganisation of the NHS and abolition of PCTs on 1st April 2013. We have worked very closely with Gateshead & South Tyneside LPC and together we have shared best practice to support all the contractors across the boundary between the two LPCs.

With Kathryn, Ann and Mukarrom I attended the PSNC’s community pharmacy conference in April 2012.

I had numerous meetings with manufacturer and wholesaler representatives to discuss quotas and try to improve the situation for contractors when ordering stock. However there is still a lot of work to be done about shortages of medicines, price concession and NCSO, which is affecting all contractors.

Ann and I have attended meetings with the PCT medicines management team, GP prescribing lead and chief pharmacist from Sunderland City Hospitals to resolve problems with supply of MDS on discharge. This has improved understanding of all parties and improved the system for the benefit of patients.

National & regional work

Kathryn attended the first public meeting on the regional medicines waste service. This started life as the ‘behaviour change project’ across the NE strategic health authority and has taken a social marketing approach, hence it has taken some significant time to develop interventions. The service has now been rebranded as My Medicines, My Health and interventions are being tested in small areas before being rolled out across the area. Kathryn negotiated successfully with the PCT to have the requirement for repeat CRB checks for enhanced services funded by the PCT, not by contractors

The LPC submitted a response to the GPhC consultation about the new standards for premises and the inspection regime for premises. Gillian and I attended the North East and Yorkshire LPCs' meeting in York in May.

Public health

Ann worked closely with the public health team in Sunderland PCT to develop the Healthy Living Pharmacy initiative locally. The first wave of applications resulted in five pharmacies being accredited as level 1 HLPs. A second wave of applications saw another 11 pharmacies achieve level 1 status and two pharmacies achieve level 3 status. These were the first level 2 accreditations in the country.

Through Ann, the LPC continued to be part of the Be Clear on Cancer programme, national campaigns on lung and bowel cancer, a regional campaign on oesophago-gastric cancer and a local pilot campaign on bladder and kidney cancers, which were all supported by community pharmacies.

Along with Gateshead and South Tyneside colleagues we successfully negotiated a pharmacy flu vaccination service again for winter 2012-13. It was disappointing that the service was only available to those aged under 65 years in a clinical at risk group but this year pharmacies would be able to commence their service on 1st October, the same as GP practices. The LPC again worked with Novartis to provide training and the cost of training was subsidised by the LPC.

Discussions started between the LPC and the public health team in preparation for the move of public health to the local authority. The LPC commented upon the public health services contract which was proposed by the public health team. Mukarrom continues to attend the Adult Partnership meetings, which report into the Health and Wellbeing Board.

Services & training

The LPC also hosted a respiratory training evening around COPD guidelines, using the NMS to support patients with their medicines and inhaler technique with the support of Novartis and Teva.

Ann joined colleagues from G&ST LPC to meet with the NHS procurement team following the AQP process for the commissioning of an anti-coagulant service. This process does not provide a level playing field for smaller contractors as the work required for submitting an application is significant

The shadow Sunderland Clinical Commissioning Group invited the LPC to work with the prescribing team to develop a non-prescription supply service for gluten-free foods. After three meetings a paper was submitted to the CCG board and the service was rejected as not being cost neutral, therefore supply will continue via FP10. We are not alone in being unable to secure a gluten-free supply service. LPCs on other parts of the region have had similar experiences.

The LPC continued to meet twice a year with colleagues from Gateshead & South Tyneside LPC to continue our close working arrangements. In September, the committee was sorry to lose Kathryn from the role of secretary, due to personal circumstances. I would like to thank Kathryn for her vision and forward thinking and support she gave for all the years she was our Secretary. Professor Jim Smith was appointed as her successor, taking up the role in January 2013. Before Jim took the role of our secretary, Ann paid a pivotal role in managing LPC business and was very supportive to the committee and to me.

I am very thankful to our Treasurer for his management of our finances and to all of the LPC team for the support they have given me through the year.

Looking forward, it is shaping up to be a very challenging year for all of us with more work and increasing pressures on funding locally and nationally. The LPC team are dedicated to supporting and advancing community pharmacy in the city and will continue to work hard on your behalf to get the best agreements we can for contractors.

Pharmabase is a new platform for contract monitoring and invoicing and is very important in helping to collect valuable data from contractors for services like EHC, supervised methadone and other services that will be planned for the future. We have encouraged NHS South of Tyne & Wear to use Pharmabase to reduce admin time for contractors and the PCTs.

The General Pharmaceutical Council has been consulting on the standards required for community pharmacies; our LPC has responded to this premises consultation on behalf of contractors. It is vital that contractors respond to any consultations that could affect them and we have responded to a Flu consultation and the Consultation on Control of Entry on behalf of our contractors.

Later in the year, we were fortunate to have engaged Ann Gunning as our Service Development Lead. She has been working on HLPs, Health checks and other services, to help contractors meet service requirements.

I have always been mindful about working smarter to save LPCs and Contractors money. I am thankful to our secretary, Kathryn Featherstone who has dedicated a lot of her time for LPC work and she has always given me good support to run the day to day business of our LPC.

Our LPC wants to preserve its independence so that we are in touch with our contractors and service providers at local level. However, Sunderland LPC is working jointly with the North East Regional LPC and Gateshead & South Tyneside LPC to implement policies and plans to secure a better future for our contractors.

Our LPC has been making plans to prepare our contractors for the changes that will have huge impact on how contractors will run their day to day community pharmacy business. In spite of all the uncertainties and difficulties that will have huge impact in the coming years to all the community pharmacies, we will help our contractors to make cost effective plans to give them support and to overcome their difficulties and I believe Sunderland LPC has a dedicated and a very experienced team to support all our contractors in this.

There was one change in membership of the committee over the

year: Ms K Crockatt resigned mid-year and was replaced by Ms H

Dodds, also from the CCA.

Miss Kathryn Featherstone also attended all meetings of the

committee, as LPC Secretary.

Mrs Ann Gunning attended meetings from September 2011, as the

Service Development lead for the LPC.

SECRETARY’S REPORT

Kathryn Featherstone (to September 2012)

(from January 2013) It has been a year of great change for the LPC. Kathryn stepped down from the post of secretary in September 2012 after five years’ of excellent work and achievement for the committee. I took up the position in January 2013 and, in the interim period, Ann Gunning acted as secretary and provided invaluable support to the chair and members, in addition to her job as service development lead. The committee met five times in 2012-13. Attendance records of members are shown below. There was one change in membership during the year: Kathryn Brown resigned mid year. Greg Clark was appointed to the committee as nominee of the Company Chemists’ Association in 2013.

At the annual general meeting in November 2012 the committee re-elected officers as follows: Chair: Umesh Patel Vice-chair: Gillian Cucchi Treasurer: Tim Dobbin Ann Gunning was reconfirmed as service development lead. The post of secretary was filled in January 2013. The year was inevitably dominated by the impending abolition of Sunderland PCT and the Strategic Health Authority, the vesting of commissioning responsibilities variously in Sunderland Clinical Commissioning Group (SCCG), Sunderland City Council (SCC) and the new NHS Commissioning Board, later renamed as NHS England. There was considerable uncertainty over roles and responsibilities and, although much was clarified during the year, some uncertainties still remain. NHS England’s Cumbria, Northumbria, Tyne and Wear (CNTW) area team emerged as the local body responsible for most community pharmacy services after 1st April, with many public health services being commissioned by the City Council. We therefore invited Stuart Cuthbertson from the City Council, to the LPC’s November meeting to discuss the draft Health and Wellbeing Strategy, which is aimed at reducing health inequalities in the City. There is a continuing need to keep public health officials in the City Council aware of pharmacy services and the scope for development and contributing further to improving health in the Sunderland. An important step was securing a place for the LPC on the City’s Adult Partnership Board which is a sub-group of the Health and Wellbeing Board. Mukarrom now attends on our behalf. The LPC worked hard through the year to keep track of all these changes, build relationships with key people in the new organisations, and ensure that contractors’ interests were taken account of in the transition and services protected. The relationship with Cumbria LPC will be important, as NHS England CNTW will in future commission services such as flu immunisation across the geographical area. To this end, Cumbria representatives now attend the regional LPC. We continued to work closely with Gateshead and South Tyneside LPC throughout the year. This collaboration is very important in achieving efficiency and avoiding duplication, especially on training events, communications, and negotiations with commissioners. Current views are against a formal merger – both committees wish to retain maximum responsiveness to contractors and issues in their area – but strongly in favour of close collaboration where appropriate. This approach appears to be working well and helps us to keep down the costs of the LPC. The two committees continue to meet jointly twice a year.

Much work was done in the LPC’s working groups, often jointly with colleagues in Gateshead and South Tyneside. These include:

• Sunderland prescribing group – Robbie Burns with Nigel Herdman as deputy

• Electronic prescription services group – Nigel Herdman

• Joint LPC events group – Gillian Cucchi and Katherine Featherstone, with three members from G&ST LPC

• Joint LPC communications group – Gillian Cucchi, Kathryn Featherstone and

Clare Richardson, with four members from G&ST LPC

• Local representative Committees group – Gillian Cucchi and Katherine Featherstone, with three members from G&ST LPC

We responded to a number of consultations on behalf of contractors, including the GPhC’s proposals on premises, which when implemented are likely to have a big impact standards and the inspection regime. We also worked throughout the year with suppliers, PSNC and DH to try and ease the persistent and damaging problem of medicines shortages.

Numerous alerts and other communications were also issued during the year, keeping contactors up to date with training events and opportunities for new services. Work is ongoing to improve and upgrade the LPC website. Gillian also represents us on the local representative committees group, which brings together the LPCs and also the local medical, dental and optical committees in the area to discuss common concerns. The chairs of clinical commissioning groups are invited to the meetings, which are therefore an opportunity for the primary care professions jointly to raise issues of common interest. Concerns were raised during the year about the new NHS 111 non-emergency telephone service; GPs in particular were worried about how effective this service would be (clearly justified by early experience of implementation. There were issues for pharmacy too; while NHS Direct has a well-established process for referring callers to pharmacies when appropriate, NHS 111 has no such system.

The Category M situation continues to cause financial hardship for contractors and

the situation around supply chain shortages has been a major headache for all

contractors. The LPC has worked with a number of manufacturers to try to resolve

local difficulties with contractors and to get quotas reset, as well as informing PSNC,

the Department of Health and local MPs of our ongoing problems.

We have also been able to get new services commissioned and were delighted to be

able to launch the Healthy Living Pharmacy scheme and the Community Pharmacy

Flu Vaccination service this year.

South of Tyne & Wear Local Representative Committee The LRC was set up as a vehicle by which each of the community

heath care professions could be kept abreast of developments within

the others. Four meetings have taken place and a diverse range of

issues discussed, any significant information or feedback was

published in the LPC newsletters. Recently the emphasis has been on the upcoming

implementation of the new Clinical Commissioning Groups and the impact on each

of the professions. Although the overall direction is becoming clearer further

clarification will come as the implementation process develops.

Joint Communications Committee – Gateshead & South Tyne and Sunderland LPCs Slow start to the year due to the loss of several committee members, however the

committee is now back to full strength. The main focus is to provide contractors with

information that is easily accessible and timely, to this end all news items will be

published   on   the   G&ST   and   Sunderland   LPC’s   websites.   Information   can   also   be  

accessed   on   both   the   LPC’s   Facebook   pages   (when available) and their individual

Twitter feeds.

COMMUNICATIONS Gillian Cucchi Our vice-chair, Gillian Cucchi, leads on communications for the LPC, supported by Kathryn and Clare. They met with colleagues from G&ST to plan jointly our communications strategy. The aim is to maximise the efficiency of communications with contractors across the area, avoid duplication and keep costs down. Six issues of LPC News were published jointly during the year.

She worked with colleagues in Gateshead and South Tyneside to secure recommissioning of the pharmacy flu immunisation service for those patients aged under 65 years who have a clinical risk, as defined in the Green Book. This is the group of patients for which the PCT traditionally failed to meet the vaccination target, having achieved only 54% in 2011-12. This year’s target is 70%. These patients may well be working and therefore be less likely to attend their GP practice for vaccination. Novartis were commissioned jointly by LPCs in Sunderland, G&ST and North of Tyne to provide training for those pharmacists who wished to provide the service. Feedback from participants in these training events was excellent. Sexual health continued to be an expanding role for pharmacy. A new Chlamydia screening service was commissioned, again following joint discussions with Gateshead and South Tyneside LPC and the PCT. One in 14 Chlamydia screening tests are positive for the infection. The community pharmacy service involves offering screening kits to those aged 15 to 24 years of age who are asymptomatic. The Health Protection Agency also has evidence that there may be some 24,000 people living in the UK with HIV who are unaware of their positive status. Pharmacy-based HIV screening might therefore be commissioned in the future. Respiratory training supported by Teva and Novartis was provided in June. It comprised an update on local COPD guidelines, how medicines use reviews and the new medicines service can contribute to improving care, and a practical inhaler workshop. Again, the training was very well received. Finally, Ann wrote and circulated an invaluable guide to the new NHS organisations – “Sunderland LPC’s guide to the new local healthcare landscape”.

SERVICE DEVELOPMENT LEAD’S REPORT Ann Gunning Ann continued to lead for the LPC on development of new services and worked closely with Sunderland City Council in the run up to its taking responsibility for local public health functions in April 2013.

It has been another trying year for pharmacy with shortages due to manufacturing and distribution issues showing no sign of easing. The Net Drug cost is still falling £6.00 (£6.47 in 2012) although the discount clawback is steady at 8.5% (8.51%). The total drug cost in Sunderland was down 4%, which would have a small, but not significant, effect on the LPC’s income. The six months of this year so far indicate that this reduction has been reversed slightly, although prescription numbers were up 5% on last year and currently up 4% this year. The number of contractors has also increased from 59 to 62 in that period. The LPC accounts (see below) are showing a very healthy balance. There are outstanding liabilities to the University of Sunderland for room hire and refreshments, and also to the secretary, which will reduce the balance. The PSNC recommends that we keep a balance of 6 months expected cash requirement in the accounts and, with this in mind, I recommend to the AGM that we institute a three months’ levy holiday for contractors, which will bring the cash in hand to around this figure.

The Category M situation continues to cause financial hardship for contractors and

the situation around supply chain shortages has been a major headache for all

contractors. The LPC has worked with a number of manufacturers to try to resolve

local difficulties with contractors and to get quotas reset, as well as informing PSNC,

the Department of Health and local MPs of our ongoing problems.

We have also been able to get new services commissioned and were delighted to be

able to launch the Healthy Living Pharmacy scheme and the Community Pharmacy

Flu Vaccination service this year.

South of Tyne & Wear Local Representative Committee The LRC was set up as a vehicle by which each of the community

heath care professions could be kept abreast of developments within

the others. Four meetings have taken place and a diverse range of

issues discussed, any significant information or feedback was

published in the LPC newsletters. Recently the emphasis has been on the upcoming

implementation of the new Clinical Commissioning Groups and the impact on each

of the professions. Although the overall direction is becoming clearer further

clarification will come as the implementation process develops.

Joint Communications Committee – Gateshead & South Tyne and Sunderland LPCs Slow start to the year due to the loss of several committee members, however the

committee is now back to full strength. The main focus is to provide contractors with

information that is easily accessible and timely, to this end all news items will be

published   on   the   G&ST   and   Sunderland   LPC’s   websites.   Information   can   also   be  

accessed   on   both   the   LPC’s   Facebook   pages   (when available) and their individual

Twitter feeds.
