Dorset Supported Living About us Our specialist supported living and outreach services in Poole, Bournemouth and Dorset, provide support for individuals with Acquired Brain Injuries, Autism, Learning Disabilities and Mental Health Needs. We understand the importance of providing person-centred support which enables independence and meets individual’s needs. Through our outcome focussed approach, we support people to maintain choice and control of their own lives. We meet with each person referred and their family so that we can develop a strong understanding of their life story, any support or communication needs and their personal aspirations. We work in partnership with Local Health Professionals such as social workers, community nurses, and specialists such as Speech and Language Therapists, Physiotherapists and Occupational Therapists. Person Centred Planning Our staff support each person to create a person-centred plan that tells us how they want to live their life now and in the future. Through a range of communication tools, we ensure that each person is closely involved in the development of their own plan. Some examples of how we have supported people include: Working towards independence with personal care tasks Skills building to manage money Travel training to gain confidence and independence Learning to stay healthy and exercising Support with medication and health appointments Discovering new hobbies, getting out and having fun! Accessing vocational training and work experience Managing correspondence and bills Building social skills and expanding social Circles Access to independent advocacy Our supported living service provides bespoke packages of up to 24-hour support, including overnight support when needed. This can be provided within the family home or a persons own supported tenancy. Quality We are passionate about quality and we specialise in supporting people to achieve their full potential. Peter’s Supported Living JourneyIn 2014 Peter* suffered a road accident in Thailand that resulted in brain injury, diffuse axonal injury and severe cognitive decline. Peter spent more than 2 months in a coma whilst receiving treatment overseas.

After transferring him home to England Peter embarked on a 2 year rehabilitation programme in Glenside Hospital, Salisbury. Staff supported Peter by travelling to Salisbury with him and undertaking mentoring and supervision from Mental Health nursing and Multidisciplinary teams.

He has since been supported by the accomplish Supported Living team in Dorset to achieve his biggest goal, which was to move home to Dorset where his family and friends live.

After a failed first transition, it was felt by professionals that this move may not work. Staff at accomplish continued to support Peter to work towards his goal and in July 2017 he moved into his new home.

Peter has a consistent and professional team to support him with his everyday life and care, developing his skills and managing his challenging behaviour. By setting up a person centred support plan involving a number of external professionals, this has maintained Peter’s development and progression. It has also reduced his challenging behaviour, enabled him to participate in activities and promote his independence.

Today we are proud to see Peter as a different person who enjoys accessing the community, carrying out his food shopping and participating in activities of daily living. Peter is now able to see his family anytime as they live in the same town. This was all possible thanks to the dedication shown by the accomplish Supported Living team in Dorset and their refusal to give up during times of difficulty.
