Why do we collect your information?

Sefton CVS collects information about you to provide you with the best possible service. In order to do this your personal details may be held in a manual written record or on a computer system.Records may contain information such as: Your name, address, date of birth Details of the services we have provided you with Details of visits or interviews Notes or reports about your circumstances Relevant information from people who care for

you or work on your behalf.

How do we use your information?

To contact you if you need further help or support To plan services to meet your needs To investigate your concerns if you make a

complaint To provide up to date, accurate information for

professionals who look after you or represent your interests in the community

To help plan your ongoing support with other agencies, for example social services

For research - you would be asked for your consent

To assess the type and quality of the service you’ve received from us

To provide evidence of advice or support given, so that we receive payment from service commissioners

To audit Sefton CVS accounts and services To prepare statistics on our performance.

Who has access to your information?

Sefton CVS staff and other professionals may need access to your information to help or work with you - however this is on a strict need to know basis.

There may also be times when administrative staff will need some limited access to your information to carry out their tasks.

Staff employed by or working with Sefton CVS have a duty to keep your information secure and confidential.

We have an Information Governance Handbook (including a Confidentiality Code of Conduct) and a Data Transfer Procedure which all staff must follow - you can see these on our website www.seftoncvs.org.uk

Who do we share your information with?

We may need to share your information with other organisations so that you can receive further help or advice, but this will only be with your consent.

Your information may be shared, where necessary and subject to strict agreements, with one or more of our local partners for instance: Voluntary, Community & Faith sector

organisations Sefton Council Health Service providers Commissioning organisations Criminal Justice agencies

We will not disclose your personal information unless there is a genuine need, there are exceptional circumstances or the law requires us to do so, for instance under a court order.

What should I do if I do not want to share my information?

If you wish to restrict the sharing of your information please inform the person who is dealing with you at the time.

They will be able to discuss this with you and explain why this may not always be in your best interest.

Following that discussion, if you still do not wish to share your information, a note will be placed on your records and on any computer records.

What shall I do if my details change?

It is important that you tell the person advising or working with you if any of your details such as your name or address have changed or if any information that we have for you is incorrect.
