SUBJECT: SUBMISSION OF 2018 ANNUAL DRRM REPORT

1. This office through the School Governance and Operations Division - Disaster Risk Reduction and Management would like to remind all schools regarding the submission of the 2018 DRRM Annual Report for the period of January- December 2018.

2. As a reiteration of our agreement last March 2019 during the School and District DRRM Coordinators' Meeting, the report is due on or before 31 May 2019 and that:

a. Secondary schools shall submit directly to the division office while; b. Elementary schools shall submit their school Annual DRRM report to the district office for

consolidation by the District DRRM Coordinator. The consolidated DRRM Annual report shall then be submitted to the division office while the report of each elementary school shall remain and be filed in the district office.

GUIDELINES in FILLING UP THE TEMPLATE

I. Rationale (in narrative format). This shall indicate the background, reasons and importance for the conduct of any DRRM activity. This can refer to the guideline, directive issued as basis for the conduct.

II. Summary of Activities Conducted (in table format). The table shall indicate ALL activities conducted. This may be based on the plan of activities as detailed in the DRRM Plan prepared by the school but also include activities conducted but not included in the plan.

Activities (first column of the table)- These are the activities conducted for the reporting period. These could be the targeted/planned activities for the quarter or activities which were conducted but not previously included in the Annual DRRM Plan. Also include any training/workshop/seminar attended by the school in relation to DRRM.

Actual dates (second column)- these are the datels of the conduct of activities. It is expected that the activity is done as targeted in the plan. But in cases where the activity is not accomplished as planned or is not included in the plan, the actual date of accomplishment shall be indicated .

Place (third column)- this is the actual area or barangav where the activitv or activities was/were conducted.

No. of Attendees (fourth column)- this is the actual number of participants to the activitv. In cases where the activity is DRRM lesson integration, then this shall refer to the no. of students present during the lesson integration.

Facilitating Factors- These are the reasons or occasions or practices which contributed to the accomplishment of the activities. This also includes number of partners participating in the activity; include names of MIBLGU partners/community volunteers/leaders who helped facilitate the activities or in cases or traininglworl<shop/seminar then indicate the sponsor of said activity.

Hindering Factors- These are the causes or reasons or obstacles why the activities were not conducted or have been delayed or not conducted smoothly as planned.

Ill. Trainings & Workshops Conducted (in table format)- These are trainings and workshops conducted bv the DRRM Coordinator or in partnership with other agencies in relation to DRRM. Fill up the table appropriate depending on the level of training conducted.

Name of Specific Training/Workshop (1 st Column)- this is the specific title/name of the training or workshop conducted

Inclusive Date/s (2nd Column)- indicate the actual date!s of the conduct of the training or workshop

Venue (3rd Column)- indicate the actual place of conduct of the training or workshop

Number of Participants (4 th Column)- indicate the total number of attendees during the conduct of the training . Specific names of participants shall be reflected in the training report to be submitted to HRD - TD

Sponsoring Agency/Organization (5 th Column) - indicate the namels of Resource Speakers/Facilitators and their affiliation or their office; specify the source of funding .

Level (6th Column)- indicate which level is the training conducted where choices are School, District or Municipal, Division or Provincial, Regional or National.

Status of Report submission to DO-HRTD (7th Column)- indicate the status of submission to HRTD; if report was submitted then write "submitted" and indicate the date when the report was submitted; if not vet then write "not yet submitted" or "still for submission" and specify the target date of submission.

IV. Trainings & Workshops Attended (in table format) - These are trainings and workshops attended by the DRRM Coordinator or anybody from the school in relation to DRRM. Name of Specific Training/Workshop (1 st Column) - this is the specific title/name of the training or workshop attended

V. Emerging Good Practices (in narrative format or in bullets only). These are the activities which are not common or ordinary which, in the School Heads' judgment can contribute to the smooth implementation of DRRM and eventually towards the achievement and implementation of the Department's mission, vision and objectives.

VI. Learning Experiences, Lessons Learned, Realizations. (in narrative format or in bullets only) These include school's lessons learned and realizations (whether positive or negative) as the school implements DRRM activities.

VII. Issues & concerns and Recommendations (table). The issues, concerns, or problems (especially those that need DO interventions or responses) shall be written in the first column and the recommendatK:lns for the possible solutions of the issues and concerns shall be written in the second column. Any queries coming from the learners and personnel should be captured here and any answers be captured as well.

VIII. DRRM Plan (in table form). The plan shall form part of the Report. This should include planls to address deficiencv/ies as a result of the observations done by evaluators. This will be the basis of the report for the ensuing quarter or year.

IX. Photo Documentation/Pictorial Reports -Insert photos as evidence of the conduct of activities, put captionls in every picture

X. Attachments/Appendices. List a// documents which are attached in the report as evidence on the conduct of activity. Example of attachments/appendices indude but not limited to the following: short narration of the conduct of drills (earthquake, fire, flood and other relevant drills) and the EQ drill evaluation form, attendance sheeUs, Photos in CD (with caption please), DLLs as a MOV (mode of verification) to the DRRM Lesson Integration, Legal Bases (memorandum or guidelines) and other necessary document which shall serve as proof of conduct. Every 151 quarter of the school year, the following shall be attached. 1) Annual DRRM Report 2) Vicinity/School, Hazard & Evacuation Maps, 3) Annual DRRM Plan, 4) MOU or MOA of the twinning system, 5) SDRRM Composition Team
