PA Board of Barber Examiners Adams County Gary Lynn Riley, Sr., license no. BL053245, last known of Hanover, Adams County, was indefinitely

suspended immediately stayed in favor of probation, based upon his felony conviction. (11/30/17)

Allegheny County Wilbert C. Taylor, III, license no. BL053741, last known of Pittsburgh, Allegheny County, was suspended

pursuant to the Order of the Court of Common Pleas of Allegheny County dated December 11, 2017,

which the court issued under section 4355 of the Domestic Relations Code. The suspension is effective

Lancaster County Roberto E. Juarbe, license no. BM093658, of Lancaster, Lancaster County, was suspended for a period of

one (1) year, such suspension shall be immediately stayed in favor of probation, subject to the Board's

terms and conditions; and a public reprimand was placed on his permanent disciplinary record with the

PA Board of Certified Real Estate Appraisers Out of State Robert P. Strell, of Williamsville, New York, was assessed a $5000 civil penalty for having completed a

real estate appraisal of property without holding an appropriate, current and valid certificate from the

PA Board of Cosmetology Philadelphia County Martha Pena t/d/b/a Martha’s Beauty Salon and Boutique Unisex, license no. CB120891, of Philadelphia,

Philadelphia County, was indefinitely suspended and was assessed a $2,250 civil penalty, based upon

employing unlicensed individuals, aiding and abetting in the unlicensed practice of the profession, and

failing to have an owner or designated person in charge of the salon during regular business hours.

Robert L. D'anjolell, II, license no. FD012875L, of Broomall, Delaware County, was ordered to pay a

civil penalty of one thousand two hundred dollars ($1,200.00); and within six (6) months from the

effective date of this Order, attend and successfully complete the missing hours of continuing education to

meet the requirements for the February 2, 2014 - February 1, 2016 biennial reporting period, because he

was six (6) hours short of the minimum six (6) total hours of continuing education required for the

Berks County Melissa Jennifer Szymanski, license no. MSG003733, of Birdsboro, Berks County, was suspended for no

less than three years retroactive to July 15, 2016, based on findings she violated the terms of a previous

Cumberland County Liping Yan, a/k/a Li Ping Yan, L.M.T., d/b/a Li's Asian Spa, license no. MSG009316, of Carlisle,

Cumberland County, was order to pay a civil penalty of ten thousand dollars ($10,000), suspended for one

(1) year to be immediately stayed in favor of probation, and a public reprimand placed on her permanent

disciplinary record with the Board, because she utilized the services of the unlicensed Wang Yi Fan in

April of 2017 to provide massage therapy services for clients of Li's Asian Spa; aided and abetted Wang

Yi Fan in the unlicensed practice of massage therapy by allowing Wang Yi Fan to provide massage

therapy services; did not cooperate with the Board, the Department of State, or the Bureau of Enforcement

and Investigation in the investigation of a complaint; did not provide massage therapy records

immediately upon demand by the Board; did not maintain massage therapy records for at least 3 years

from the last date that services were provided to the client; did not conspicuously display the massage

therapist’s name and the title L.M.T. on an identification badge where massage therapy services were

being provided; and did not include the massage therapist’s license number in all advertisements

Delaware County William Eric Webb, license no. MSG004011, of Wayne, Delaware County, was revoked and assessed a

civil penalty of $20,000 based on his felony and misdemeanor convictions. (12/22/17)

Lackawanna County Chris Clarke, of Scranton, Lackawanna County, was denied a massage therapist license based on his

felony conviction under the Drug Act, a crime of moral turpitude, and inability to practice with

reasonable skill and safety. (10/19/17)

Lancaster County James R. Deiter, license no. MSG002800, of Holtwood, Lancaster County, was revoked and assessed a

civil penalty of $40,000 based on his felony convictions and crimes of moral turpitude. (12/19/17)

PA Board of Medicine

Adams County Rita Marie Boutot a/k/a Rita B. Harrison, license nos. MD067781L and PAK000136, of Biglerville,

Adams County, had her licenses to practice as a Medical Physician and Surgeon and Physician

Acupuncturist temporarily suspended, pending a hearing, on the grounds that her continued practice

within the Commonwealth may be an immediate and clear danger to the public health and safety.

orthotic fitter, because he failed to demonstrate that he could safely practice the profession due to

Sean Morgan, license no. MA058414, of Bridgeville, Allegheny County, was suspended for no less than

three years, such suspension to be immediately stayed in favor of no less than three years of probation to

allow his participation in the Disciplinary Monitoring Unit, because he could not practice as a medical

physician assistant with reasonable skill and safety to patients by reason of illness, addiction to drugs or

alcohol, or mental incompetence. (2/07/18)

Cambria County Glenn Bryan Davis, license no. MD072541L, last known of Johnstown, Cambria County, was

automatically suspended based on his numerous felony convictions under the Drug Act. (12/27/17)

Fayette County Donald Kenneth Brooks, license no. YM007255L, of Vanderbilt, Fayette County, was indefinitely and

actively suspended until such time as the Board grants the reinstatement of his license, because he is

unable to practice the profession with reasonable skill and safety to patients. (2/06/18)

Lawrence County Brandi Nichole Haswell, license no. YM014264, of Ellwood City, Lawrence County, was indefinitely

suspended for no less than three years, such suspension to be immediately stayed in favor of no less than

three years of probation, because she is unable to practice the profession with reasonable skill and safety

to patients by reason of illness, addiction to drugs or alcohol, or mental incompetence. (2/06/18)

Thomas Ranieri, M.D., license no. MD037844E, of New Castle, Lawrence County, was suspended,

pending a hearing, on the grounds that his continued practice within the Commonwealth may be an

Lehigh County Marcus Alexander Saunders, license no. MD439206, of Center Valley, Lehigh County, was ordered to

pay a civil penalty of $2500, because he was convicted of a felony or a misdemeanor relating to a health

profession or received probation without verdict, disposition in lieu of trial or an Accelerated

Rehabilitative Disposition in the disposition of felony charges, in the courts of this Commonwealth, a

Federal court or a court of any other state, territory or country; was convicted of any felony; has been

convicted of any misdemeanor related to the practice of the profession and made a misleading, deceptive,

untrue or fraudulent representation in the practice of the profession or practicing fraud or deceit, either

alone or as a conspirator, in obtaining a license, certification or registration or in obtaining admission to a

John Francis Mitchell, license no. MD020824E, of Allentown, Lehigh County, was ordered to pay costs

of investigation of $3500 and successfully complete 30 hours of remedial education, because he failed to

comply with the Board's regulations when prescribing controlled substances to his patients and was guilty

of immoral and unprofessional conduct with respect to his prescribing of Vyvanse at dosages greater than

manufacturer's recommendations and prescribing Vyvanse together with simultaneous prescriptions for

other similar medications without adequate documentation in the medical record to justify the use of said

Joel Bennett Glass, license no. MD011184E, of Philadelphia, Philadelphia County, permanently and

immediately placed on probation for no less than five years, based upon disciplinary action taken by

Mark David Schreiber, PA license no. MD039206E, last known of Tamarac, Florida, was automatically

suspended for no less than ten years from date of conviction based on a conviction which would be the

equivalent of a felony under the Drug Act in this Commonwealth. (01/09/18)

Navigation Commission for the Delaware River and its Navigable Tributaries No disciplinary actions taken.

Secretary of the Commonwealth – Notaries Public

Philadelphia County Ashley M. Aviles, notary commission no. 1272365, of Philadelphia, Philadelphia County, agreed to the

suspension of her notary commission for six months, such suspension immediately stayed in favor of

probation, to successfully complete a six-hour notary practice and procedure course, and to pay a civil

chronological notary register, by sharing a notary register with another notary and failing to record all

notarizations in chronological order; failed to be familiar with the duties and responsibilities of a notary

public, by failing to maintain custody and control of her notary seal, sharing a notary register with another

notary, and failing to record all notarization in chronological order; and failed to be of good character,

integrity and ability, by sharing a notary register with another notary and failing to record all notarizations

in chronological order. (02/20/18)

Deirdre Kane, notary commission no. 1264459, of Philadelphia, Philadelphia County, permanently and

voluntarily surrendered her notary commission because she failed to maintain custody and control of an

accurate chronological register; failed to be familiar with the duties and responsibilities of a notary public,

by failing to maintain custody and control of an accurate chronological register, notarizing a copy of the

naturalization certificate, and failing to include all of the essential notarization language on the

naturalization certificate; and failed to be of good character, integrity and ability, by failing to maintain

custody and control of an accurate chronological register, notarizing a copy of the naturalization
