INTRODUCTION

Welcome to the FrontPath Health Coalition Provider Network! Your employer, together with otheremployers located within northwest Ohio, northeast Indiana and Southeastern Michigan, havedeveloped a network of hospitals, medical facilities and professionals to provide your health care needs.

Your employer’s participation in the FrontPath Health Coalition Provider Network (FrontPath) enablesyou to take advantage of FrontPath’s contractual arrangements when you utilize the FrontPathproviders who are listed in this Directory. By obtaining medical services from a FrontPath provider, yougain several advantages. For example:

FrontPath Providers have agreed not to “balance bill” employees or their dependents foramounts exceeding their plan’s deductible or co-pay requirements. When using a FrontPathprovider, you need no longer be concerned with charges which might exceed your plan’sreasonable and customary fee limits.

FrontPath Providers will file your claim forms. You no longer have to worry about filing claimforms if you use a FrontPath Network Provider.

Your out of pocket expenses will typically be lower if you obtain treatment from a FrontPathNetwork Provider, due to FrontPath’s negotiated fee arrangements.

Your employer will provide you with specific information as to how your Plan of Benefits coordinateswith the FrontPath network. Remember, the choice of health care provider is always yours to make.
