Renewal Conditions: By joining a plan, you are authorizing Melaleuca to bill your credit cardor checking account for the plan you have selected. This charge shall renew until you notify Melaleuca in writing of its cancellation. By joining you indicate you have read the terms andconditions of the plan. This plan will automatically renew at the end of your membership term, andyour credit card or bank account will be automatically charged or drafted for the appropriateamount.Termination Conditions: Melaleuca and Careington reserve the right to terminate planmembers from its plan for any reason, including non-payment. If Melaleuca terminates theplan or your membership for a reason other than non-payment, you will receive a pro-ratarefund of your membership fees.Cancellation Conditions: You have the right to cancel within the first 30 days after effectivedate or receipt of membership materials (whichever is later) and receive a full refund, less theprocessing fee, if applicable. After completing the complaint resolution process, if you remaindissatisfied you may contact your state insurance department.
HOW TO USE YOUR MEMBERSHIP CARD
1. Read the Membership Agreement and the enclosed description for eachproduct.
2. Keep your membership card with you at all times so it will be easily accessibleto present at provider locations. You must present your membership card at thetime of service to receive a discount. Remember, your entire household can usethis program.
3. To locate and verify participating providers in your area, simply call MemberServices at 1-844-486-5602. You may also visit www.melaleuca.com/healthcare to search for providers. When calling to schedule an appointment, please refer tothe back of your membership card for proper network identification whenspeaking to a participating provider's office.
4. You are responsible for paying the provider the entire discounted fee at thetime service is provided.

Step 2: When scheduling an appointment, please inform the participating provider’s
office you are a member of the Careington Dental Network.
Step 3: Please present your Careington ID card upon arrival to receive savings.
Step 4: The provider will collect payment at the time of service. You are responsible for
paying the total bill, less the applicable savings, when service is provided.
Counseling Program As a member, you and your eligible dependents have access to My PersonalCounselor, a confidential, telephonic counseling and referral service*. AmericanBehavioral is a national network of professional behavioral healthcare providersthat are licensed and credentialed in their area of expertise.
Your counselor will help you identify problems and assist in working through them.The counselor can also determine the best alternatives and, if necessary, makeappropriate referrals to other professionals that specialize in your area of concern.

How to Access the Services:
Step 1: Please call toll free at (800) 925-5327 and an intake coordinator will verifyyour membership. Please identify yourself using your Melaleuca member IDnumber.
Step 2: The coordinator will document your concerns and will call you with atelephonic counseling appointment scheduled at a convenient date and time.
Step 3: The counselor will call you at the appointed time. If the counselor believesyou are in need of additional services, he or she may refer you for more sessions.
Step 4: You will be offered the opportunity to participate in a satisfaction survey totell American Behavioral about your experience with your Personal Counselor.

This is a sign & drive program. Members must call the Toll-Free Roadside Assistance number listed above sothat RP’s agents can locate a service provider to assist them. Services secured independently are notcovered and will not be reimbursed. However, if RP cannot secure dispatch or if service is unavailable, RP’sagent will provide an Authorization Number which gives the member the option of securing their ownservice and submitting for reimbursement consideration up to their program benefit limit. Emergency Roadside services are provided to members to Roadside Protect, Inc. by Signature’sNationwide Auto Club Inc.
Roadside Assistance Service Excludes:1. Service if member is not with the disabled vehicle. 2. Transportation for the member to the vehicle forservice or from the vehicle to another destination after service has been rendered. 3. Service will not berendered in areas not regularly traveled, such as vacant lots, beaches, open fields, or other places whichwould be hazardous for service vehicles to reach. 4. Towing a vehicle off a boat dock or marina. 5. Deliveryor repair of tires. 6. Installation or removal of snow tires and chains nor dismounting, repairing, or rotatingtires. 7. Vehicle’s storage charges. 8. Cost of parts, installation, products, materials, impounding, andadditional labor relating to towing. 9. Service of any kind on vehicles used for commercial purposes or usingdealer tags (Note: All trucks in New York have commercial license plates whether they are used for businessor private use. If the truck is used for private use, we will dispatch.) 10. Service for taxicabs, tractors, boats,trailers, dune buggies, or vehicles used for competition, or stolen vehicles. 11. Service for unlicensedvehicles. 12. Service for vehicles with expired safety inspection sticker, license plate sticker, and/or emissionsticker(s) where required by law. 13. Service for vehicles illegally parked or impounded. 14. Repeatedservice calls for a car in need of routine maintenance. 15. Service when a vehicle is snowbound. We do nothoist, winch, or shovel vehicles from unplowed roads, snow banks, snowbound driveways, or curbsideparking. 16. Accident towing or towing to/from an impound lot. 17. Service for vehicles that have beeninvolved in an accident or have been vandalized.

• Disease risk assessment for six chronic conditions which includes cholesterol panel blood test,
glucose blood test, blood pressure, body mass index and waist circumference.
Total Wellness Package: Savings of $130• Carotid Artery – Stroke is the fourth leading cause of death and the number one cause of
disability in the U.S. Over 80% of strokes are preventable if diagnosed early even though four out
of five strokes do not have symptoms.
• Atrial Fibrillation – AF is the most common type of heart arrhythmia. The heart’s upper chambers
beat irregularly, and blood is not pushed completely out of the heart which can cause a stroke.
15% to 20% of all strokes are caused by AF. An ECG or EKG (electrocardiogram) using electrodes
placed on the wrists and ankles is used to screen for the presence of AF.
• Peripheral arterial disease – PAD, also known as hardening of the arteries, affects about eight
million Americans. PAD occurs when the large and medium-sized arteries supplying blood to the
legs become narrow or clogged, constricting the flow of blood. People with PAD have a two to
six-fold increased risk of death from heart attack and stroke .
• Abdominal aortic aneurysm – AAA occurs when the large blood vessel that supplies blood to the
lower part of the body enlarges or balloons which may cause a rupture. This is the tenth leading
cause of death for men over age 55 with a 90% mortality rate. 30% to 50% of AAAs are fatal before
reaching the hospital. Ultrasound technology is used to measure the size of the abdominal aorta
to detect the early development of this condition .
Ultimate 6 Screening Package: Savings of $39• Disease risk assessments: Biometrics (blood pressure, lipid panel, fasting glucose, BMI and waist
circumference) and medical history questions are run through a proprietary algorithm to calculate
