Dear Parent/Guardian(s),

For 6 weeks this summer, memories will come alive as we host the 7th Annual City of

Miami Camp Shriver, which is a collaborative venture between City of Miami’s Sandra

DeLucca Developmental Disabilities Center (SDDC), Dade Amateur Golf Association

(DAGA), The National Inclusion Project (NIP) and Special Olympics Miami-Dade County.

From June 25 through August 03, 2012, our inclusive camp will provide young adults the

experiences of participating in an inclusive sports camp program. City of Miami Camp

Shriver permits these campers to realize their potential, develop physical fitness and

experience the joys and friendships that come from the camp.

The primary focus of this year's City of Miami Camp Shriver is to provide a new generation

of young adults with intellectual disabilities and without disabilities an opportunity to enjoy

the experiences of participating in a social and sports camp. The camp provides sports

training in swimming, soccer, softball and basketball to boys and girls with intellectual

disabilities, registered with Special Olympics Miami Dade County, ranging in age from 12-

22 and teens 14-18 without disabilities. In addition, sports clinics and learning activities,

arts and crafts, golf, tennis, bocce, sailing and field trips will be part of the program.

Camp will take place at SDDC which is a Division of the Department of Parks and

Recreation in the City of Miami located at 4560 NW 4th Terr. Miami, FL 33126. City of

Miami Camp Shriver is free to participants, with only a small registration fee per camper.

Campers will need to provide their own transportation. REGISTRATION FORMS, LUNCH

APPLICATION AND POLICIES AND PROCEDURES CONTRACT, NEED TO BE

RETURNED NO LATER THAN FRIDAY, May 04, 2012.

2012 CITY OF MIAMI CAMP SHRIVER

POLICIES AND PROCEDURES CONTRACT Please adhere to the following rules and regulations for our FREE summer camp. We are very happy to have your child participate. In order to ensure the safety of all campers, we must ensure the following rules are followed as indicated. Be aware we will not tolerate behaviors that are not considered appropriate for our camp. Therefore, it is imperative that you carefully read our contract and indicate your agreement by signing below along with your child, and camp director. We will follow these rules to ensure the safety of our campers and staff. We have the right to dismiss any camper at any time for breaking any of the rules specified below. 1. In order for a child to participate in our camp, all required paperwork must be submitted and signed by

parents/guardians. (i.e., City of Miami Registration Form, Special Olympics Medical Release form, DOE Summer food program form).

2. All campers must be Special Olympics Miami-Dade County REGISTERED athletes. 3. Regular camp hours are from 8:30AM to 3:30PM. There will be no special arrangements to those hours.

Campers must be dropped off no later than 9 AM everyday, since we leave at 9 AM for our field trips. If you arrive after 9AM, you will have to drive your child to wherever we are that day. Pick up time is NO LATER THAN 3:30PM. PLEASE PICK UP YOUR CHILD ON TIME. You will be charged $10 for each additional 15 minutes you are late!!!!!

4. Please let camp staff know of any absence in advance. 5. Please label all the campers belongings. (i.e., lunch bag, water bottle, change of clothes, etc.) 6. No electronics (i.e., cell phones, iPods, etc.) will be permitted during sports activities. 7. Athletic attire is mandatory. (NO JEANS, LONG PANTS, SKIRTS, OPEN TOE SHOES.) On days we go

swimming, proper pool attire is mandatory. Wear your swimsuit from home, since we have limited time to change. Swimming shorts only! No street shorts or basketball shorts allowed in pool, ONE PIECE bathing suits for girls is also required. Remember, this is a SPORTS CAMP.

8. Your child must ACTIVELY engage in Sports Activities (i.e., SOCCER, BASKETBALL, SOFTBALL, SWIMMING, KICKBALL and BOCEE) if your child refuses to participate we will contact the parents to pick them up for the day. WE ARE NOT A BABY SITTING SERVICE; therefore we DO NOT PROVIDE ONE-TO-ONE SUPERVISION/ASSISTANCE.

9. IF YOUR CHILD HAS A ONE-TO-ONE PARAPROFESSIONAL DURING THE SCHOOL DAY, WE WILL NOT PROVIDE THAT SERVICE DURING CAMP. OUR RATIO IS 15 TO 1.

10. If your child shows symptoms of illness please keep them at home for the day. We will not administer any medications, hold any medication, or assist your child to take any medications.

11. Should injury occur during any activity, it is the parent/guardian’s responsibility to provide medical care for the participant. We will contact you immediately and the proper authorities if the needed.

12. Any behaviors that camp staff considers to be disruptive will result in the camper’s expulsion from the camp. Some examples of disruptive behavior include:

• failure to follow rules • rough conduct • disrespect to staff and others

• damage to property or equipment

• vandalism

• fighting/hitting • use of profanity

However, we will make every effort to de-escalate the situation, to the best degree possible. 13. GROUP SUPERVISION AND ASSISTANCE IS PROVIDED FOR YOUR CHILD. WE ARE NOT STAFFED TO PROVIDE ONE-TO-ONE SUPERVISION OR ASSISTANCE. 14. All campers must be able to feed, dress and toilet independently.

15. FAILURE TO ADHERE TO ANY OF THESE RULES WILL RESULT IN THE FOLLOWING: 1ST OFFENCE: VERBAL WARNING 2ND OFFENSE: WRITTEN NOTICE 3RD OFFENSE: SUSPENSION 4TH OFFENSE: EXPULSION FROM CAMP
