is a national vision program administrator that provides benefits to nearly 55 million people in all 50
states, Puerto R ico, Guam and Saipan. Benefits are available through an extensively credentialed
network of vision care providers. Please note that you must use a network provider to receive
In-Network benefits.To use the program,Call the network provider of your choice and schedule an appointment.
Identify yourself as an employee or covered dependent of HVHC INCORPORATED.
Provide the office with the employee’ s Identification Number and the name and date of birth of
any covered dependents needing services.
Additional information regarding your benefits and participating providers may be obtained by calling
Davis V ision, Monday through Friday between the hours of 8:00 AM and 11:00 PM Eastern T ime,
Saturday between 9:00 AM and 4:00 PM Eastern T ime, and Sunday between 12:00 PM and 4:00 PM
