• TEACHING ACTIVITIES (Teaching evaluations and other evidence of quality teaching must be attached; this represents the teacher’s portfolio and should be accurately summarize ALL of your teaching activities.)

Brief statement of areas of teaching interest:

My main clinical and teaching interests include Head and Neck Oncology, microvascular reconstruction, wound healing, and thyroid/parathyroid disease. There are usually five Otolaryngology residents, one Head and Neck Oncology fellow and 1-3 rotating medical students on our Head and Neck (Blue) Service at any time. My involvement in teaching on the wards, in the clinic, and certainly in the OR, is considerable, since our Head and Neck service is usually high volume; surgeries are long, and patients are complex. Therefore, I am committed to teaching our trainees a multidisciplinary approach to treating head and neck cancer patients. I teach them how to approach diagnosis, treatment and follow up of head and neck patients according to the constantly updated National Comprehensive Cancer Network guidelines. In the operating room, my approach to teaching residents and fellows is to provide them with a combination of close supervision as well as gradual independence in making surgical decisions. My goal is to have them be able to work up a patient, develop a surgical strategy, operate on and follow the patient independently by the time they are ready to graduate from the training program.

In early 2008, I became an Associate Director of the Head and Neck Oncology/Microvascular Fellowship, which was approved by the Advanced Training Council (ATC) of the American Head and Neck Society in September 2008, for one Head and Neck Oncology Fellow position a year. My responsibilities include:

• Preparation and successful completion of an application and site visit to the program to become approved by the Advanced Training Council of the American Head and Neck Society.

• Development and implementation of Fellowship Educational Curriculum

• Fellow teaching in the wards and in the OR, and semi-annual evaluations

• Yearly Fellowship candidate interviews: 3 interview dates, 10-12 applicants each year

• My primary goal within the next few years is to expand and improve our fellowship’s curriculum (including formalized robotic training) as well as increase visibility and national reputation of the training program.

• Prepared an application for an ATC re-accreditation site visit in December 2012, and executed a two-day site visit, awaiting accreditation decision in April 2013

• Became a Fellowship Director in December 2012

In December 2010, I accepted the position of Associate Residency Program Director. My responsibilities include:

• Preparation for the Otolaryngology Residency Review Committee of the Accreditation Council for Graduate Medical Education (ACGME) accreditation site visit on February 15, 2011, including writing and editing of several sections of a 260-page Program Information Form (PIF): more than 50 hours of preparation.

• Based on the PIF preparation and the results of this RRC site visit, in August 2011 our Otolaryngology Residency program was granted a full 5-year accreditation, which is the maximum a residency program can receive.

• Membership of the Residency Education and Research Committee, redesigning Residency Educational Curriculum

• Redesigning Residency rotations and clinic schedule: 3-4 hours weekly

• Ongoing evaluations of residents: semi-annual and annual individual review meetings with Otolaryngology residents, currently switching to quarterly review meetings

• Reviewing applications for residency, candidate interviews: 3 interview dates, 36-40 candidates interviewed each year

• Annual Surgical Anatomy Lab

SERVICE ACTIVITIESSee guidelines and instructions to applicants for definitions and suggested documentation of professional and academic service.

Professional Service:The diverse area of professional service includes patient care. Applicants should select measures that most clearly and concisely document their accomplishments and the value of these activities to the Medical School and University. Measures of both quantity and quality of activities are required and if necessary, applicants should provide brief descriptions to assist reviewers. Measures of patient care activities include numbers of patients, time allocation in clinical activity, procedures completed, Relative Value Units (RVUs), and value to the School of the clinical service. If the primary quality evaluation is the subjective assessment of peers, this should be available in letters from departmental colleagues, chair, or referees.

The significance of professional service in the forms of task forces, committees and similar groups should be explained and the specific role of the applicant clarified.

Professional consulting services must have academic credibility and clear service intent and not be performed primarily for personal profit.

KU Clinical Service

Head and neck oncologic and microvascular surgery remain my main passion, and I maintain a high volume head and neck and reconstructive practice, with two full clinic days and two long OR days a week. Over the last year, I have trained in Transoral Robotic Surgery (TORS), and together with my partner, Terance Tsue, MD, have developed the first TORS program in the region at KU. This surgical approach utilizes Da Vinci Robotic technology for treating benign and early stage malignant tumors of oropharynx and larynx. We are in the process of establishing a

referral pattern specifically for transoral robotic surgery, and if it was not available here, the patients would have to travel more than four hours to receive such treatment.

The following table demonstrates Total Relative Value Units (Total RVU’s) for my clinical services at the University of Kansas Medical Center (FY’07-present) and Saint Luke’s Hospital (FY’07-FY’09). For the first three years (FY’07-09), 40% of my effort involved clinical activities at KUMC, 40% at St. Luke’s Hospital, and 20% included research/administrative time at KUMC.

Starting in FY’10, 100% of my time is spent at KUMC, divided between 80% for clinical activities and 20% for research/administrative time.

Data for this chart was provided by the PivotHealth Individual Provider Report Card from October 2010 to December 2010 (outpatient survey).

Among the questions that patients are asked, three are specifically related to physician provider (rate on a scale from 1 to 5):

Question 5: How would you rate the personal manner (courtesy, respect, sensitivity, friendliness) of the doctor? Question 6: How would you rate the skills (thoroughness, carefulness, competence) of the doctor? Question 7: What is the likelihood of you recommending the doctor to your friends or family?

• RESEARCH and SCHOLARLY ACTIVITIES:Brief statement of areas of research and scholarly interest, including current projects:

My research interests include the search for novel biological and molecular prognostic markers in head and neck cancer, novel drug therapies, factors promoting neoangiogenesis and wound healing in microvascular reconstruction, and the role of patient compliance and socioeconomic factors in long term outcomes of head and neck cancer patients. I am a Co-Chair for Head and Neck Cancer Disease Working Group, which meets monthly to discuss and update ongoing clinical trials in head and neck and thyroid cancer. My immediate goals within the next 2-3 years include developing a busy Clinical Trials Program within our Department as well as Translational Research Program. I feel that I am prepared to accomplish these goals and have adequate resources and clinical volume to do so. We have recently completed recruitment of the head and neck cancer patients for a multi-institutional study sponsored by the American Academy of Otolaryngology – Head and Neck Surgery, and our site enrolled the highest number of patients. I have the support of Kevin Sykes, MPH, our Director of

Clinical Research, with opening and running multiple current clinical trials as well as biostatistical analysis. I have recently secured a Young Investigator Centralized Otolaryngology Research Efforts (CORE) grant from the American Academy of Otolaryngology Foundation and American Head and Neck Society (AAO-HNS/AHNS), for a translational research project in studying a novel chemotherapy drug delivery system in animal model of head and neck cancer. I am collaborating with a great multidisciplinary team of scientists from the Department of Neuroscience (Dr. Dianne Durham), General Surgery and Pharmacology (Dr. Mark Cohen), and Pharmacology (Dr. Laird Forrest) on this project.
