3) Leadership Philosophy I believe strongly in service; to our profession, to the organization we work for, and by extent for our community. However, as Heifetz reiterates, “it is a terrible responsibility to be a leader”. As I translate this, it means for me that being a leader inherently means we cannot stay in our comfort zone. It means knowing when and how to take responsible risks, and most importantly being agile and being willing to fail, reflect, apologize and move forward. However, I also believe that when you operate from a combination of your strengths and self-knowledge you can achieve excellence. I work towards that every day through a combination of meditation and self-reflection; as Sam Harris called it, to achieve an examined life. As such I work diligently to be fair, “all in”, a good listener, and participatory. I am open to feedback, willing to course correct and not afraid to apologize. I tend to be serious but also with a sense of humor and am always interested in my staff and faculty’s personal and professional well-being and progress. I care greatly for the organization I serve, the profession and the community. I conscientiously lead by keeping a sense of urgency, managing processes, engaging with people while being a visionary leader, understanding the connectedness of it all.

4) Philosophy of Diversity, Equity and Inclusion I strive to contribute to diversity in a variety of ways. As an immigrant of this country I feel connected to many of the students in my class as well as the staff and faculty who I work with. I understand the struggles of feeling alone or misunderstood because of one’s accent or mannerism and I know I bring a level of compassion to my classroom, fellow faculty and staff that is sincerely welcomed. I am acutely aware of different cultures and how they may play out in my lesson plan. I incorporate diversity into my case studies and learning materials. I make a consistent and concerted effort ensuring that the staff and faculty I am responsible for feel included and valued. I strive to recruit faculty and staff who are cultural diverse. I actively foster a climate of open communication and honest feedback.

Diversity has also become an important focus within my research. One of our latest funded projects explores the occurrence of and disparities in dental clinic adverse events. As such we will explore whether certain groups of people are at higher risk for adverse events (harm) in the dental setting.

5) Leadership Accomplishments

When I arrived at UCSF as Chair of the PRDS department in July 2016 we restructured the Vice Chairs team, focusing on pre-doctoral and post-doctoral education, research and added an "at large" Vice Chair who focused on general administrative, financial as well as quality improvement projects. We spent considerable time building a leadership team with the Vice Chairs and Division Chairs through a leadership assimilation effort, with bi-annual follow up retreats. We documented this process in a manuscript which has been published. We rebuilt the financial capacity of the department, including introducing P&L and cash flow statements as well as exploring the overall value proposition of the current departmental compensation plan; rebuilding the staff management team with the hiring of a new Administrative Director as well as adding a new position of Finance Director; and building a system for PRDS research administration, including solid post award support. In the Preclinical division, we have focused on introducing the axiUm EHR and modern teaching methods into the sim lab. We rapidly move the private faculty Clinic to a more profitable enterprise, as well as a more cohesive group of faculty and staff working together. The Externship program was revitalized. Close to twenty new applications for Volunteer faculty were processed within a few weeks and several new sites have been added. A revived focus on Inter Professional Education (IPE) and Inter Professional Practice (IPP) was implemented through collaboration with the externship director. The Portfolio Exam was reorganized and its curriculum updated. Through a bottom up process, we revised the Mission and Vision of the department and developed defined strategic goals projects which were implemented over a 36 months period. We accomplished:

• Implementation of rapid focused culture survey • Updating the curriculum, teaching strategies and assessments to foster and test

critical thinking of students in preclinical simulation lab • Hiring 7 FT and numerous PT faculty in the Clinical General Dentistry/Prosth

and Endo divisions • Developed a robust post award research financial support system • Developed a support mentoring system for junior research faculty and obtained

funding for enrichment opportunities • Launched extra-curricular evidenced-based study club for 3rd and 4th year

students per their topics • Implementation of a detailed academic annual renewal process with clear

expectations • Revitalized the Dental Public Health residency program from 1 PT to 4 FT

residents and received special accolades during 2019 CODA accreditation visit

Based on the culture survey feedback, we implemented consistent open communication and feedback opportunities for faculty and staff through the implementation of monthly “birthday breakfasts” with the Chair, bi-monthly town halls, and bi-annual departmental retreats. With the leadership team, we developed a Manual of Operations to better define the advancement process for all faculty. Active dissemination through town halls and two retreats provided great opportunity for faculty and staff to understand how to advance their careers at UCSF. My major contributions to academic leadership at Harvard have been as a result of my positions as Assistant Dean of Clinical Affairs (2007-2011) and (interim) Chair of the Oral Health Policy and Epidemiology department (2008-2011). Major accomplishments include:

• As Assistant Dean for Clinical Affairs I first implemented a novel credentialing & privileging approach that was published in JDE and is now used as a model for other dental schools.

• During my time as Clinic Dean I improved and opened communication with the faculty through monthly faculty meetings, weekly in person meetings and daily huddles.

• One of my major projects was the implementation of the electronic health record (EHR) during that time. We seamlessly switched from paper records to our EHR in the student clinics and then the faculty clinic.

• As Chair of the OHPE department, this department was rebuilt from 1 advanced graduate education student, 1 staff member and 1 faculty member, to a vibrant department with 12 graduate students, 7 full time faculty, 20+ part time fully involved faculty, one visiting professor, 4-6 post-doctoral fellows and research assistants and 4 full time staff members. The key to this success was a strategic approach that included rebuilding the dental public health (DPH) residency program; a niche strategic approach for the research direction of the department; strengthening ties with Cambridge Health Alliance for the general practice residency (GPR) program; development of the dental geriatric program; and attracting students who would be dedicated to our departmental mission, vision and research direction.

• I introduced HSDM to Meaningful Use (MU) and in fact we were the first dental school to implement MU. We published a manuscript in JDE which allowed many other dental schools to also adopt MU.

• As my research is focused on Quality Improvement, we have implemented many of its results at HSDM. I instituted and chaired the Quality Improvement (QI) Committee, through which we implemented and monitored the MU metrics, as well as implemented and reviewed root cause analysis for all sentinel and many lesser patient incidents. As such we were able to learn from our mistakes and make many system level improvements.

• We implemented the dental diagnostic terminology that was developed by my research team and measured its frequency of use as well as its valid use. Results were shared throughout the organization and consistent improvement

resulted. This terminology has since been harmonized with the American Dental Association’s SNODENT terminology and approved as an ANSI standard.

3) Teaching Philosophy At UCSF, I teach as an ad hoc lecturer in some of the DDS courses. I developed, co-direct and teach in the new dental public health ethics course for our graduate students. Likewise, I developed, co-direct and teach in the new leadership course for our DDS students. In my teaching, I use a hybrid flipped classroom/adult active style teaching approach as I believe this way of active engagement of the students is the best way to learn and retain knowledge. My first exposure to hands-on teaching was as a senior dental student when I was hired into the competitive position as an oral surgery assistant in the extraction clinic. I still remember the satisfaction I felt when novice junior students were able to master extraction techniques after teaching them through chair-site coaching techniques. While at the American Heart Association we put on large-scale adult educational learning conferences for nurses and physicians to share knowledge on our novel quality improvement programs in reducing heart disease and stroke through the implementation of quality metrics. This is where I learned about curriculum development as well as the adult active style teaching approach. While at Harvard, I developed a novel 54-hours clinical leadership course, which was fully incorporated into the curriculum as a compulsory course. We were the only dental school with such an extensive mandatory leadership and management course for its pre-doctoral dental students. Next, I developed an advanced leadership course for the graduate dental students. This course was similarly set as compulsory. Understanding that clinical leadership is an important topic for the established dentist, I developed a CE course with a specific focus on dental leaders from developing countries, as well as CE courses for dentists in Spain. In these courses, I teach several sessions, including sessions on Team Building; Clinical Leadership in Quality Improvement; Communications; and Leadership in Transitioning of students into the professional world. As a novel curriculum developer, innovative teacher and academic leader I have always felt the responsibility to publish the accomplishments of my teaching efforts. Some of my teaching related publications include:

RESEARCH AND INNOVATION

1) Major Research Interests

A. Mentoring Mentoring our students, fellows, residents and faculty is an absolute privilege. And indeed, with some of the mentees the relationship has grown deeper, allowing for wider career coaching and development of these individuals as our next oral health leaders. I was very involved with the mentorship program at the Harvard T.H. Chan School of Public Health while at HSDM. I participated in their annual mentor day and have mentored many students over the years getting them ready for job interviews, providing career advice and being a mentor on specific research projects. At UCSF, I have completed the UCSF Mentor Training Program and mentor junior faculty, dental students and residents, research fellows, pre-dental students, and post baccalaureate students. Additionally, I continue to work with visiting professors. Mentoring postdoctoral fellows and faculty is rewarding as together we have been able to move the agenda of dental diagnostic terminology and patient safety. I feel I am able to not only share the rigors of qualitative research, but more importantly mentor on how to build a collaborative research team.

B. Contribution to Science

I. Developing a Patient Safety System for Dentistry Despite an enormous national financial investment in oral health, there is little understanding of the return in terms of quality. Moreover, with the existing data, we have no way of estimating what proportion of these costs may be attributable to adverse events (AEs). Only the most severe of dental AEs have been documented, and very few disputes that are raised by patients are successfully addressed through the malpractice system. Evidence about patient safety practices and AEs is sorely lacking in dentistry. Therefore, our long-term goal is to understand the occurrence and causes of dental AEs in real practice and to develop empirically-based interventions to minimize their occurrence and to ensure that patients receive the safest oral healthcare possible through the institution of a dental Patient Safety Initiative (PSI). To date we have been able to develop the first element of the PSI, Determining Threats to Patient Safety, by developing tools necessary to document dental AEs, generating a classification scheme and repository that can help organize and link AEs, and enabling five dental organizations in the United States to systematically collect and analyze AEs. We have been able to raise awareness of the importance of patient safety in dentistry, developed a novel categorization and severity classification of dental AEs, and help unearth underlying causes. I serve as one of the two multiple PIs in this study.

II. Building a diagnostic centric dental profession Until recently, a commonly accepted standardized diagnostic terminology (DxTM) did not exist for oral diagnoses. There are many benefits of documenting diagnoses in a

standardized way. This prompted this research team to develop a dental DxTM (formerly called ‘EZCodes’ then called DDS). The DDS terminology has been mapped to other DxTMs: SNOMED, ICD 9, ICD 10 and ICD 10-CM) as well as the treatment terminology CDT. The DDS terminology is developed as an interface terminology (a set of terms designed to be compatible with the natural language of the user, used to mediate between a user’s colloquial conceptualizations of concept descriptions and an underlying reference terminology) to be used in the dental clinic with SNOMED CT as its back-end reference terminology (each term has a computer-usable definition to support retrieval and data aggregation). Through our research, we have been able to identify and reduce cognitive and functional impediments to diagnostic term entry in the existing EHR workflow and interface to the dental diagnostic terms. We have freely disseminated the DDS terminology, thus allowing for the development of quality care metrics, tracking disease as well as patients across providers. In 2017, the DDS was harmonized with SNODENT into SNODDS and in December 2017 voted an ANSI standard. I serve as the principal investigator for this study.

III. Development of a dental data repository. Few oral health databases are available for research and for the advancement of the evidence-base in dentistry. Additionally, there is currently little understanding of the effectiveness of common dental treatments, and we are blind to many potential facets of the oral health-systemic health connection. As is the case in medicine, trials are costly. Secondary uses of data already stored in dental EHRs have great potential to improve the data-driven knowledge base in dentistry. Linking data from dental EHRs to medical EHRs may also help in more efficiently determining inter-relationships between oral health and the health state of the rest of the body. In this research, we developed a centralized data repository (BigMouth) derived from EHRs at four dental schools part of the Consortium of Oral Health Research and Informatics (COHRI). A multi-stakeholder committee developed a data governance framework that encouraged data sharing while allowing control of contributed data. We adopted the i2b2 data-warehousing platform and mapped data from each institution to a common reference terminology. I served as co-investigator. IV. Leadership skills development of the dental student. Leadership competency in a clinical or research setting should comprise not only theoretical and conceptual lessons but also practice in the application of a broad set of skills. These include relationship building, facilitation, negotiations, communication, and development of emotional intelligence. There are a variety of reasons that support the inclusion of a course for dental students on leadership concepts and their implementation. Not only because of potential shortages in qualified leaders within academic dental institutions as noted by Slavkin in 2007, but also because of significant challenges every clinician and researcher will face within his or her practice with colleagues, staff, and patients. The need for cultural competency, demand for patient safety, understanding of team dynamics and ability to understand and address oral health disparities are only a few of the current challenges that need to be

addressed thoughtfully and skillfully. The qualitative and quantitative data gathered in the form of two end-of-course evaluations suggested that we created a successful approach to introducing dental students to concepts of leadership in the oral health arena. Qualitative feedback was overwhelmingly positive and indicated that the students found the course “life-altering” and highly valued its breadth of topics. I served as the PI for this effort. V. Humanistic characteristics of students. Humanistic medicine can be taught and learned. The curriculum at HSDM combines training in conducting the medical interview with human values such as compassion, caring, sincerity, understanding, and integrity. It places a high value on strengthening relationships with patients, emphasizing empowerment, respect and strong communication skills. In a humanistic approach to dentistry, active listening is used to emphasize the shared humanity of patient and provider, enabling the dentist to practice with empathy, reflection, professionalism, and trustworthiness. We developed an innovative rating system for qualitative narrative assessments of first-year student interviewing skills. This study demonstrated the feasibility of measuring dental students’ humanistic qualities during the first year and that humanistic qualities during the first year are associated with clinical performance in the third year of dental school. I served as the PI for this study.

(PI) NIN/NIDCR 09/2015 08/2017 Evaluating Standardized Preventive Care to Reduce $360,187 $781,432 total Dental Disparities in Children direct/yr1 This project aims to determine whether the use of a caries-prevention program developed by the Willamette Dental Group (WDG) in Oregon reduces dental caries and decreases costs in high-risk children enrolled in Medicaid (Oregon Health Plan-OHP), compared to a group of commercially insured children.

3) Other Innovative Activities 1. Clinical Innovations (Shared Nursing Governance Model at CECC; 1990-1995) - At

Children's In this Pediatric Nursing Home I developed and implemented a shared governance model for the nursing department, as well as an advanced medical model, decreasing hospitalizations by 20%. I established one of the first well-functioning Ethics Committees in long-term care and obtained federal grants, effectively integrating these pediatric patients into the public-school system. Administratively, I managed a financial turn-around, realizing positive cash flow and net income, expansion of capital projects and acquisition of abutting real estate while 98% of patients were publicly funded.

2. Clinical Innovations (Physician manpower plan at North Shore Medical Center; 1995-1997) - I developed a physician manpower documentation plan to assure compliance with the Attorney General's safe harbor guidelines. This model was subsequently used for all community hospitals that merged into the Partners Healthcare System. Administratively, I developed and successfully implemented a tracking system to assure complete implementation of a $13 million restructuring effort.

3. Clinical Innovations (Clinical Improvements at North Shore Medical Center; 1997-1999) - I developed and implemented systems to effectively manage decision-making regarding observation admissions, realizing $1+ million/year savings and regarding bed site registration, realizing perceptual wins as well as $360,000 annual financial gains. I developed and implemented in collaboration with the Surgical Leadership, a Surgical Step-Down unit, reducing length of stay by 1 day for 25% of patients in the ICU. I facilitated guideline development with the ICU leadership for intubation to tracheostomy timeframes. I developed and implemented a HF (heart failure) management model for in/outpatients, reducing length of stay by 1 day and readmission rate by 40%. Administratively, I successfully developed and implemented the business plans for the merger with Union Hospital. I developed and implemented a physician productivity model for a 60-member Primary Care Physician group, realizing $1+ million improved revenue.

4. Clinical Innovations (Balanced Score Card, Error Reduction, Hospice at North Shore Medical Center; 1995-2001) - I realized the implementation of a balanced score card system (fiscal, customer service, employee satisfaction and quality) as a tool to attain best business results. I coordinated the medication error reduction effort, resulting in improved data collection/ analysis and significant cultural and operational system changes. I developed and received approval for the "Application for Open Heart Surgery Service" and developed the implementation plan. I developed the strategy and implemented a 4-bed hospice unit, realizing net gains and true patient and family satisfaction. Administratively, I developed and successfully executed a $10 million restructuring effort.

5. Clinical Innovations (Housing for Domestic Violence survivors; 2003-2004) - At Transition House, I expanded the short-term (90-day) shelter component with 55 permanent and

transitional housing units through a $490,000 HUD grant, providing permanent and meaningful solutions for domestic violence survivors.

6. Clinical Innovations (Get with the Guidelines (GWTG) at AHA; 2005-2006) - I designed and implemented novel strategies to facilitate employment of secondary cardiovascular prevention initiatives throughout American Heart Association's (AHA) markets, resulting in 19% market increase in hospitals participating with the AHA's quality improvement program "Get With The Guidelines". I was invited to discuss these results at various AHA meetings.

7. Clinical Innovations (Stroke Systems at ASA; 2007) - I effectively implemented the American Stroke Association's (ASA) "Recommendations for the Establishment of Stroke Systems of Care", including writing the prospectus that raised $1 million for a public awareness campaign; I facilitated several State taskforces improving the policies for EMS stroke care, the quality of urgent and acute stroke care at hospitals, and the transfer process to rehabilitation options. I was invited to discuss these results at various ASA meetings.

8. Clinical Innovations (Care Coordination at HDC; 2007-2008) - I developed and implemented a clinical committee structure at the Harvard Dental Center, consisting of 9 committees with 100% faculty participation. This resulted in better collaboration between program directors and coordination of patient care between graduate dental students.

9. Clinical Innovations (Credentialing Systems at HSDM; 2008) - I developed and implemented an effective credentialing/re-credentialing and privileging system for all part time and full time Faculty and Advanced Graduate Students at the Harvard School of Dental Medicine practicing at the Harvard Dental Center. This model has been reported in a publication in the JDE.

10. Clinical Innovations (Dental Trigger Tool; 2011-present) - I developed a novel trigger tool for detecting adverse event in patient electronic records, based on the IHI's global trigger tool. I implemented the first patient safety culture survey in the dental arena, based on the AHRQ medical culture survey MSOPS, measuring the readiness of dentists and dental staff for a blame free culture. These two efforts provided pilot data for an R01 submission that was awarded and explores, for the first time, patient safety as a potential issue in the dental profession. Dr. Lucien Leape serves as Chair of the Advisory Board of this effort. Our results have been reported in various journal articles.

11. Clinical Innovations (Meaningful Use - dental at HSDM and consulted with implementation for pediatric dentistry at UCSF; 2012-present) - I developed Meaningful Use for dental school application. I translated the government mandated Objectives and Quality Measures for the dental arena and worked close with the EHR vendor (Exan) to implement a novel dashboard that will be used across all dental schools using the same EHR (84% of all U.S. dental school). HSDM and UCSF pediatrics were among the first school to implement Meaningful Use and as such recognize the importance of integrating care between dentistry and medicine. This has been documented in the only Meaningful Use for dentistry publication in JDE.

12. Clinical Innovations (Dental AEs Classification and Severity; 2014-present) - I developed a novel classification system for the causes of dental adverse events and a novel severity rating for these dental adverse events (AEs), based on the original IHI severity rating scale. This has been documented in a published manuscript.

13. Innovations in Dental Diagnostics (DDS - Dental Diagnostic System; 2010-present) - I developed the first practical dental diagnostic terminology (DDS, formally called EZCodes)

that currently is being used by 20+ dental institutions. Dental diagnoses are now structurally entered in an EHR for the first time in over 2 million patient visits annually. These results have been extensively reported in various journal articles.

14. Innovations in Dental Diagnostics (DDS - ICD, SNOMED, CDT Mapping; 2013) - By structuring the DDS as an interface terminology, entering diagnoses in the EHR is user friendly, while at the same time they are linked (mapped) to ICD for billing purpose, to SNOMED for Meaningful Use mandates and to CDT (treatment codes) in order to develop quality metrics.

15. Innovations in Dental Diagnostics (DDS - Assertional Knowledge; 2015) - By structuring the DDS as an interface terminology, adding assertional knowledge is a terminology best practice. The DDS assertional knowledge shows up as a pop-up box when hovering over the diagnostic term in the EHR providing the clinician with information for each diagnostic term on: prevalence, differential diagnoses, most frequent treatment, colloquial language, synonyms, full description of the term, equivalent ICD term and additional relevant information. This provides invaluable information at chair-site in the form of a mini guideline for each diagnosis. A manuscript describing the process and value of assertional knowledge is in preparation. The DDS' assertional knowledge is copyright protected (HU 5986).
