Major Areas of Interest:
Necrotizing Enterocolitis: The central hypothesis is that immaturity of the neonatal gut
barrier, coupled with mucosal damage caused by hypoxia, formula feeding, GI tract
colonization with opportunistic pathogens, and other perinatal insults, predispose the
premature infant to bacterial translocation. Bacterial interactions with immunocytes in
the lamina propria result in local release of inflammatory mediators, most notably IL-1,
which leads to sustained upregulation of inducible nitric oxide (NO) synthase (iNOS or
NOS-2), overproduction of NO and formation of peroxynitrite (ONOO-) in the intestinal
epithelium. High levels of ONOO- lead to further tissue injury (increased enterocyte
apoptosis) and inhibition of tissue repair mechanisms (decreased enterocyte proliferation
and enterocyte migration). The resulting imbalance between tissue injury and repair
mechanisms leads to a vicious cycle of injury and uncontrolled inflammatory response
with release of other mediators. The net effect is progressive tissue destruction, intestinal
perforation and systemic sepsis, as seen in advanced NEC. We are testing our hypothesis
using in vitro approach in cultured enterocytes, ex vivo experiments in temporary
culture of intestinal epithelium, and in vivo studies in knockout and transgenic mice.
Gut Barrier Failure During Inflammation: Derangement in the intestinal mucosal barrier
may play an important role in the pathogenesis of systemic infection in critically ill
patients. One of the consequences of systemic inflammation is increased tissue
expression of cyclooxygenase-2 (COX-2), the key enzyme in the biosynthesis of
inflammatory prostanoids, leading to local production of prostaglandin E2 (PGE2).
Based on our in vitro studies with tight monolayers of intestinal epithelial cells, we
hypothesize that PGE2 causes profound and long-lasting derangement of the epithelial
barrier by acting via the PGE2 receptor EP1, phospholipase C, Ca2+ mobilization from
intracellular stores, activation of myosin light chain kinase (MLCK), phosphorylation of
myosin light chain, and alteration of tight junctions. Various aspects of this hypothesis
are being tested using model enterocyte monolayers, ex vivo permeability tests in
Ussing chamber, and in vivo permeability experiments in genetically altered mice. For
the latter approach, we have developed a transgenic model that allows tetracycline-
regulated intestine-specific expression of COX-2. In this model, local expression of
COX-2 and local production of PGE2 can be achieved in the absence of systemic
inflammation. A translational component of this project is testing the efficacy of COX-
2- and EP1-targeting therapies in experimental peritonitis.
