For the last 32 years I have been working as the assistant professor of
psychiatry in adult psychiatry in Shahid Beheshti medical University in
Tehran. I have been working with a variety of adult psychiatric disorders
esp. Affective disorders, PTSD and geriatric and C&A as well. I was
responsible for 40 inputs and a psychiatric clinic in the mornings, training
under/postgraduates. In the afternoons i had to visit up to 30pts at the
office. I was also the referral for the combat PTSD pts from the Iranian
Veteran Administration.
During 32 years activities, from 1995 I have been working in the field of
Child & Adolescent Psychiatry.
From 1995, I had been working in Family Therapy Unit, and I had been
working as supervisor up the time of my retirement.
In 2003, I took 16 months scholarship to Australia at Children’s Hospital at
westmead for 16 months at Psychological Medicine Ward, alongside this ,
entering at the New south Wales Institute of Psychiatry on Masters course
of Family Therapy.
In 1982 after 1 year working at Psychiatric unit at Taleghani Hospital, as
Director of Emam Hosein Hospital for the first time in 1983, founded
Psychiatric wards (Adult and Child &Adolescent Psychiatry) with 80 beds,
out and inpatient clinics.

In 1994, after entering in School Mental Health Course in Rawelpendi School
Of Medicine ( WHO Assignment) for 3 months, for the first time I founded
School Mental Health In Iran, started with 2 schools, I was appointed as the
director of School Mental Health Project in Iran.
In Bam Earthquake I was appointed to be supervisor for training of
psychiatrist whom they were volunteered working with Child and
Adolescent.
In 1996 I founded Child & Adolescent Psychiatric Association and I started
working as The Director.
During 32 years I have been working at the university hospital in the
mornings where i was responsible for 5 psych beds and runs OP clinic(usu
visit 15 pts with the residents and undergraduates. I run my private office in
the evenings. There I visit 30-35 pts every other day, for 2 days in every week
I have specified my work on Family Therapy for my clients.

Tehran Psychiatric hospital (where I worked at
during most of my residency program), 200 bed
psychiatric hospital, sees an average of 12000
outpatients and 2400 inpatints annually. It has 4
general adult wards (men's emergency
ward(EW) with 50 beds, women's EW with 50
beds, men's acute ward(AW) with 50 beds and
women's AW with 50 beds), , an occupational
therapy unit and a non-medication therapies
unit. There has been 10-15 positions for
postgraduates each yr and the SHOs would take
15 on-calls/month, senior residents 12, and
specialist residents 9. 5-10 pts would be
admitted each day ,The hospital was rated as
one of the best psychiatric hospitals in Iran. The
hospital was directly connected to an urban PHC
centre in the city, a centre specially designed for
community-based psychiatric education (in Iran
medical universities are directed by Ministry of
Health).

Training experience: it was my first introduction to concept of psychiatric
illness, classification of psychiatric disorders, and how to diagnose and
manage psychiatric disorders. Initially my supervisor advised and
supervised me to interviewing psychiatric patients, psychiatric
emergencies, and major categories of psychiatric disorders and
medications. I immediately took the responsibility of 1:2 rota on-calls
during which I was the only doctor in the hospital. I had the responsibility
of 30-40 inpatients. 40-50% of the patients were usually diagnosed as
suffering bipolar disorder, 10-15% schizophrenia,10-15% schizoaffective
and other psychotic disorders, 10% combat related-PTSD, 10% personality
disorders, others were of somatoform disorders and OCD. In outpatient
clinic were diagnosed as GAD, OCD, addiction, PTSD, mood disorders
moderate type, BPD.....
I was introduced to ECT unit which accepted 5-10 patients
(outpatient/inpatient). I attended an outpatient clinic on wkly basis under
supervision of my attendant.
When I was on-call I had been involved with 6-8 patients. It was necessary
to call senior officer sometime.
Leaning interpretation of EEG was one of our tasks.
During my education I passed 7 days in research Methodology and entered
in research unit.
I took 3 months education in insight awareness, group therapy, and
behaviour therapy.
Medication Therapy had been stressed in our meeting every day.
My first experience for visiting patients without supervision started when I
was at third year.
At the beginning of third year for 6 months we had to enter in the chronic
Mentally ill patients, child, and forensic Psychiatry.
We started Consultation-liaison psychiatry at the begging of the second
year with regard to the requests that our supervisor had received. It has
been done for 10-15 patients in every week from general hospital. I was so
impressed when i understood how many patients with somatoform,
internal like medicine such as gastrointestinal problems and medical
symptom like, suffering from GAD, Depression, and Factitious disorder.
Anorexia Nervosa and bulimia is very rare in Iran but we had sometime as
referral patients.
“ Until I started my work at Westmead in 2004 and understand how these
2 disorders are more common in AU”.
Ground round had been done with Director of the ward in 3 days in week.
Neuropsychiatric assessment, diagnosis and treatment education was one
our program that was formed at the last 3 months at the second years.
This ward was at another hospital so we had to go there for 3 months as
our rotation. EEG interpretation, assessment of neurologic patients and
stress on co-morbidity with Psychiatric illness was impressive eg, Glioma in
frontal lob, shows some of psychiatric symptoms and Epilepsy. In this
ward I took EEG interpretation classes also.
Forensic Psychiatry: latter I interviewed the individuals referred from legal
system and the courts. The next day the history and the diagnoses would
be presented to the hospital attendants who would discuss and further the
investigation. The results would be written back to the referring body
thereafter.

. Roles and achievements:
A. Clinical:
Having one group meeting every week with all members.
Visiting 10 inpatients (5 for girls and 5 for boys) with different psychopathologies twice a week, my team was working more on mood disorders, conduct and psychosis.
Visiting 5 out patients, in different psychopathologies, from 9 am to 2 pm, one day in every week with fellows and residency staff.
One day working with family by one way mirror, although that I am Psychiatrist, but with regard to my education and experiences in the field of family therapy, I also personally evaluate and assess the patient’s families.

One day as a lecturer, I attend for educating the issues of Family and Marital therapy for fellows and residency staff, by role playing, panel discussion, brain storm, and Problem Base Learning,( PBL).
Process for inpatients:
1. We start with social events for every member of the team.
2. Writing the programs for every member during the week for their rosters and responsibilities and on call.
3. Listing the names of the patients. 4. Getting a brief history, clinical
manifestation, reasons for admission, and therapeutic plan of action.
5. Brain storming around the case, and select the best strategy with regard to evidence-based practice.
6. One hour for case presentation, (rare, difficult and resistance to therapy).
Process for outpatients:
1. Every staff specialist has one day out patient clinic, after preparation for the record, the patient be invited to enter the assessment session by their referral letter, the assessment will be done with Child and Adolescent Psychiatrist, Clinical Psychologist and social worker, with participation of Residency staff, and fellow.
2. As usual, the emergency case immediately has been admitted in emergency psychiatric ward for urgent treatment.
3. The non-emergent patient after gathering all information and Psychological assessment, such as: Assessment by some Tests, they refer to the team leader.
4. This process for 5 patients in every clinic will be done during the week.
5. After this initial assessment, the patient will have a specific time to follow his/her therapeutic programs in team.
B. Administrative:
1. Writing, controlling and settling of the programs of Education for Medical Students, Residency Students and fellows in my team.
2. Evaluation of the processes of quality of education in all sectors.
3. Receiving all reports from programs, such as seminars, conferences and congress that were going on through the year.
4. Reporting the quality and quantity of educational programs to the Deputy of Education to the University.
5. Participating in all sessions, related to quality of Education that have been sponsored by the Faculty of Medicine.
6. Evaluation of the quality of candidates for employment such as; Social workers, Psychiatric Nursing, Occupational Therapist, Psychologist, Psychiatrist, and participating on the interview board
7. Participation in staff meetings, group meetings, team meetings. 8. The group have 10 Post graduate General Psychiatrists in every
year. 9. The group have 10 undergraduate General practitioners, 10
medical students for every month on their rotation. 10. Opening the Sub-specialty in the Field of Child and Adolescent
Psychiatry, 2 fellows for every year.

4. Providing sub-groups for ICAA, such as: Economic, Research, Educational and Administrative.

8. Working continually at my office from 9am-5pm for 3 days in every week. As usual I have 25-30 patients in everyday.
