OVERVIEWTHE COMMONWEALTH CENTRE FOR DIGITAL HEALTH (CWCDH) HAS ESTABLISHED A COMMONWEALTH DIGITAL HEALTH FELLOWSHIP PROGRAMME FOR HEALTH INFORMATICS SPECIALIST FROM THE GLOBAL SOUTH TO EXPERIENCE HEALTH SYSTEMS IN THE GLOBAL NORTH. IN ADDITION, IT IS EXPECTED THAT THESE FELLOWS WOULD CONTRIBUTE TO DEVELOPMENT ACTIVITIES IN THE GLOBAL SOUTH THROUGH COLLABORATIVE PARTNERSHIPS THAT THE CWCDH PLANS TO ESTABLISH WITH INTERNATIONAL DEVELOPMENT PARTNERS. THE CWCD WISHES TO FORM COLLABORATIVE PARTNERSHIPS WITH DIGITAL HEALTH HUBS IN ORDER TO IMPLEMENT THIS PROGRAMME.

Southampton University, along with wider partners, are well-positioned to form and co-ordinate the first Hub of the Commonwealth Centre for Digital Health to create a common good for securing a healthy Planet for All with the development of a digital framework and platform for ‘Universal Health Systems for Planet, Place and People’ building upon the sustainable and low cost experience from Sri Lanka and supported by a rotating team of Sri Lankan Commonwealth Digital Health Fellows (CWDHF).

The Origins of the Commonwealth Centre for Digital Health (CWCDH)

Sri Lanka during its Presidency of the Commonwealth Medical Association from 2016 to 2019 conceived and operationalised the Commonwealth Centre for Digital Health and fellowship Initiative. The seeds of this build upon the ambitions of the Commonwealth Health Ministers (CHMM 2016) to enable shared learning and collaborative networks and approaches in order to scale up sustainable health systems across the 53 Commonwealth countries. This was further shaped by the commitments made at the Commonwealth Medical Association Conference on Digital Health in Sri Lanka in 2016, where health officials and partners came together to produce the ‘Colombo Declaration’ and collaborative action plan.

This built upon over a decades experience of the Health Informatics Masters course in Sri Lanka, along with the later development of the Commonwealth Health and Education policy – knowledge hubs. High-level commitment for the formation of the Commonwealth Centre for Digital Health was communicated by an exchange of letters between the Sri Lankan Government and the Commonwealth Secretariat. The Commonwealth Centre for Digital Health was then officially launched at the Commonwealth Heads of Government Meeting in London, UK on 20 April 2018 by the President of Sri Lanka, His Excellency Maithripala Sirisena: https://cwcdh.org/events/ The Vision of the CWCDH

The Vision of the CWCDH is that all Commonwealth nations will have affordable access to the benefits of Digital Health solutions and services in order to scale up sustainable Health Systems for Universal Health Coverage (UHC). This can be achieved by building upon and adapting the components of the ‘Commonwealth Systems Framework for Healthy Policy’ (Nurse J et al, 2016), and establishing a digital health system with a vision of creating a Common Good to Secure a Healthy Planet for All.

In order to take this vision forward, Southampton University is partnering with the Commonwealth Centre for Digital Health to create a Hub for Universal Health Systems for Planet, Place and People. This also builds upon wider partnerships and collaborative networks developed to enable the ambitions outlined in the ‘Charter for One Health’ – an initiative advanced by the InterAction Council – a group of former Heads of Government, in order to enhance the role of health in securing a healthy planet for all and scale up responses to the growing climate and environmental crisis. This in turn will support the delivery of multi-sector approaches for Universal Health Coverage and the Sustainable Development Goals.

A COLLABORATIVE INITIATIVE BETWEEN THE COMMONWEALTH CENTRE FOR DIGITAL HEALTH, UNIVERISTY OF SOUTHAMPTON, UK

AimTo develop an accessible Digital Hub for ‘Universal Health Systems for Planet, Place and People’ in order to Secure a Healthy Planet for All and enable the Sustainable Development Goals, with a focus on delivering Universal Health Coverage (UHC) for All.

Objectives

• Create a Digital Framework for ‘Universal Health Systems for Planet, Place and People- to develop a validated and accessible digital system for delivering Universal Health Systems for Planet, Place and People - building upon the multi-sector, community orientated ‘Commonwealth Systems Framework for Healthy Policy’ in order to secure a healthy Planet for All.

• Establish a Digital Platform for Universal Health Systems for Planet, Place and People: to create an on-line platform that provides free on-line access for the key components for delivering Universal Health Coverage;

• Scale up the Digital Health Workforce: building on the Sri Lankan MSc in Digital Health, extend national capacity for creating digital health systems through a series of regional hubs and the development of on-line courses, including Massive Open On- Line Courses and global health networks.

• Sustainable Development Goals enabled: with a central focus on Securing a Healthy Planet for All – that accelerates progress towards the Sustainable Development with a focus on SDG 3, enabled by multi-sector, community orientated Digital Systems for One Health for People and Planet.

Core Principles

2. That applies Quality principles of ensuring No Harm and based upon the following components: Safety, Effectiveness, Efficiency, and Equity; Appropriate and Accessible whilst enabling Resilience, Health and Well-Being in Populations, communities, families and individuals.

3. To place the Health of the Planet and the well-being of current and future generations at the heart of decision making and promotes digital solutions and innovations that enhance co-benefits for health, the environment, society and the economy.

AIMS AND OBJECTIVES OF ESTABLISHING THE HUB The below outlines the aim and objectives of establishing this collaborative hub, along with an overview of the draft Digital Framework for Universal Health Systems for Planet, Place and People, followed by the origins of the Commonwealth Centre for Digital Health CWCDH and the Commonwealth Digital Health Fellowship Program. Further information and references are provided at the end of this document.

A DIGITAL FRAMEWORK FOR ‘UNIVERSAL HEALTH SYSTEMS FOR PLANET, PLACE AND PEOPLE’

A FRAMEWORK FOR ‘UNIVERSAL HEALTH SYSTEMS FOR PLANET, PLACE AND PEOPLE’1. Governance: planetary health guardianship

and legislation; science based policy and strategy; cyclical investment; planetary health index.

2. Knowledge: surveillance, monitoring and evaluation; research and evidence; risk and innovation; digital systems and dissemination.

3. Protection: Planetary Health risks and response; IHR and Sendai Framework; communicable disease control; emergency preparedness; environmental health; climate change and sustainability.

4. Promotion: intergenerational inequalities; AIR – WATER – FOOD systems; co-benefits and sustainable development; ecological health literacy; life-course resilience; healthy eco-settings.

5. Prevention: primary prevention: vaccination; secondary prevention: screening; efficient healthcare management, waste and recycling.

6. People: primary health care (ecological community officers; maternal & child health); secondary health care; tertiary health care & recovery.

7. Advocacy: leadership and ethics; planetary health community guardians; green connected communities; multi-media networks.

8. Capacity: workforce development for planetary health professionals; workforce planning: numbers, resources, infrastructure; standards, curriculum, accreditation; capabilities, teaching and training.

Building upon the Commonwealth Systems Framework for Healthy Policy, (Nurse J et al, 2016), and Health Protection Toolkit, (Sandifer Q et al, 2017), develop a Digital Framework for Universal Health Systems for Planet, Place and People, to strengthen systems to respond to the growing climate and environmental crisis and their impact upon human and environmental health.

The below outlines an initial structure, which builds upon the Global Charter for the Public’s Health, (WFPHA, 2016) and Commonwealth Systems Framework for Healthy Policy:

THE COMMONWEALTH DIGITAL HEALTH FELLOWSHIP PROGRAMME: IN KEEPING WITH THE VISION OF CREATING A COMMON GOOD TO SECURE A HEALTHY PLANET FOR ALL., THE COMMONWEALTH DIGITAL HEALTH FELLOWSHIP PROGRAMME OF THE CWCDH IS AIMED AT PROMOTING SOUTH-NORTH AS WELL AS SOUTH-SOUTH COLLABORATION TO DIGITALISE THE HEALTH SYSTEMS.

The Commonwealth Digital Health Fellows are specialists in Biomedical and Health Informatics from the global south who have experience project implementation and workforce capacity development in Digital Health in their home countries. They would be co-funded by their home country as their government contribution to their further capacity development in the global north and their contribution to health systems digitalization in the global south. The first country that has come aboard this fellowship programme is Sri Lanka.

The Commonwealth Digital Health Fellows from Sri Lanka are Biomedical and Health Informaticians with a first degree in medicine or dental sciences who have specialised in Health Informatics. They hold a MSc in Biomedical Informatics and an MD in Health Informatics. They have worked in the National Health Service in Sri Lanka as Health Informaticians for 5 years or more. They have experience in project implementation and workforce capacity development in Digital Health. Some of them have experience working with international development partners in projects outside Sri Lanka.

Sri Lanka will provide up to 20 Fellows per year starting in in 2019. Sri Lanka will also make a contribution of GBP 2000 per month as Sri Lanka’s contribution up to a year for each Fellow (GBP £24,000/ year/ fellow). This works out to a contribution of approximately GBP 480,000 annually – (US $612,416/ year). This is a continuing commitment that the Sri Lankan government has made and we have assurance that this programme would continue into the future as long as the Biomedical and Health Informatics specialist training programme run by the Specialty Board in Biomedical Informatics of the Postgraduate Institute of Medicine of the University of Colombo, which was initiated in 2008, is in operation.

It is a regular programme of the Postgraduate Institute of Medicine of the University of Colombo funded by the government of Sri Lanka through the Ministry of Health. Since it is the Biomedical and Health Informatics specialist training programme for the Ministry of Health Sri Lanka there is no risk of the programme being terminated. The Sri Lankan component of the fellowship programme would be administered by the Specialty Board in Biomedical Informatics of the Postgraduate Institute of Medicine, University of Colombo, Sri Lanka. The motivation for providing this funding is that foreign exposure/ training is seen as an essential part of the Board Certification process in Sri Lanka and after they return from this fellowship, the fellows would become eligible for formal board certification as specialists in Sri Lanka.

The CWCDH plans to work with International Development Partners to raise funds for the administration of this programme and to identify the projects that would be supported by the Fellows in the Global South. The CWCDH plans to establish similar programmes with the support of governments of other Commonwealth countries.

ESTABLISHING THE COMMONWEALTH CENTRE FOR DIGITAL HEALTH HUB IN SOUTHAMPTON FOR UNIVERSAL HEALTH SYSTEMS FOR PLANET, PLACE AND PEOPLE

1. A Steering Group and Governance structure – an initial International Advisory Board of the Commonwealth Centre for Digital Health has been established, with the first meeting held at the Digital Health Week, with key partners in the health and digital world, between the 7-12th October 2018 in Colombo, Sri Lanka, in the margins of the Commonwealth Digital Health Conference: ‘Transforming Healthcare through innovation in Digital Health’: http://www.dhw2018.org

• Initiate a co-ordinating committee for the Southampton Digital Hub for Universal Health Systems for Planet, Place and People as part of the Commonwealth Centre for Digital Health

2. Establish a rotating team – based upon the rotation of up to 10 Commonwealth Digital Health Fellows for the Southampton Hub, to undertake the following objectives:

• A Framework: create a digital framework for ‘Universal Heath Systems for Planet, Place and People’ for mapping and analysis

• A Digital Platform - To establish a ‘Hub for Universal Health Systems for All’ (HUHSA).

• Capacity - to scale up the Digital Health Workforce through on-line courses and supporting global health networks – supported by Global Health educational courses.

• Climate Crisis and Sustainable Development: strengthen links with cross- sector approaches to enable the Sustainable Development Goals, with a central focus on securing a healthy planet for all – including emergency responses.

• Health Protection and Violence Prevention – mapping and collating digital resources for Health protection and Violence Prevention, based upon the Commonwealth Toolkits co-produced by Public Health Wales.

Purpose: Establish a Southampton digital platform for Universal Health Systems for Planet, Place and People, as a Hub of the Commonwealth Digital Health Centre, supported by Commonwealth Digital Health Fellows:

IDENTIFY PLACEMENTS FOR THE ROTATING COMMONWEALTH DIGITAL HEALTH FELLOWS:

Small groups of 2-3 fellows could rotate between a range of placements, depending upon Fellows skill sets/ interests and the requirements for the establishment of the Hub for Universal Health Systems for All, including:

• Southampton University – Digital Health Department

• Southampton University – Public Health and Primary Health Care Department

• Wessex - Global Health Network, including Global Health at Southampton and Winchester Universities

• Public Health Wales – Health Protection• Southampton NHS Trust – Digital Unit (TBC)• European Connected Health Alliance –

• Map out existing digital tools, based upon the structure of the Commonwealth Systems Framework for Healthy Policy

• Create a validation matrix, to ensure the quality of final tools selected, that are effective, accessible, free to use and safe

• Validate existing digital tools found through the mapping with the matrix

• Identify relative gaps in digital tools across the health system, to promote innovation through the ‘Commonwealth Digital Innovations Awards’

• Create an on-line digital framework for the ‘Universal Health Systems for Planet, Place and People’

Scale up Capacity with a Digital Health Workforce:

• Develop and improve On-Line Training materials on ‘Digital Health’, building upon the established Sri Lankan MSc course and Global Health courses, adapting material for a range of shorter courses to scale up workforce capacity

• Pilot and improve upon the on-line courses, and develop into Massive Open On-line Courses - MOOCs and webinars

• Build workforce capacity for delivering Universal Health Systems for Planet, Place and People to support the achievement of Universal Health Coverage, through global health networks and as resources permit, through the development of a series of regional hubs (Asia, Africa, the Pacific, the Americas and Europe) specialising in different areas

Establish a Digital Platform for Universal Health Systems for Planet, Place and People’

• Based upon the mapping of the Framework for Universal Health Systems for Planet, Place and People, establish an easy to use and accessible on-line platform for the Commonwealth Centre for Digital Health Hub

• Transfer validated digital tools from the mapping onto the on-line platform, including for Smart phone apps

• Develop a communications plan to support wider dissemination, expansion and scaling up

• Establish and maintain links with key partners with the support of an online global health network

• Establish and maintain on-line forums and facilitate discussion groups for regional and topic specific area
