C. DESIGNATED FUND GRANT INSTRUCTIONSPlease make your selection for annual grant distributions from the options below. If you want different distribution options for different charities, please consult with staff. InFaith will assume perpetual grants to charities if no box is checked. To make a one-time grant to a charity upon InFaith’s receipt of your initial gift, please complete the one-time grants section below.
V Perpetual grant support. With this option, your permanent donor advised fund automatically distributes 5% of your fund’s value on an annual basis (divided among your recommended charities.) The percentage distribution is subject to change.
V Term of years grant support. Your donor advised fund automatically distributes 10% of your fund’s value annually for 10-20 years. At the end of the set period, the remaining fund value benefits your requested charity(ies) listed on page 8. V OPTIONAL Immediate one-time grant to specific charity. If your initial gift exceeds $50,000, you can recommend an immediate grant distribution to a specific charity(ies) upon InFaith’s receipt of your initial gift. To help further explain, consider these two hypothetical gift examples:

• A donor making a gift of $60,000 could recommend a one-time grant of up to $10,000. The remainder, at least $50,000 or more, is directed to the donor’s Designated Fund for ongoing grants.

INFAITH GIFT ADVOCATE CODE OF ETHICS
The purpose of this code is to encourage responsible charitable gift planning by all who are active in advocating
gifts to benefit InFaith Community Foundation. Professional advisors, such as gift planners, insurance agents, financial
planners, board members and InFaith staff, are collectively referred to as “Gift Advocates.”
PHILANTHROPIC MOTIVATION First and foremost, charitable gifts should always be made with a desire on the part of the donor to support the humane work of charitable institutions.
TRUSTA Gift Advocate will act with fairness, honesty, integrity and openness, and shall strive at all times to represent and promote the concept of Christian stewardship.
PROFESSIONALISMA Gift Advocate shall advise donors only in areas in which he or she is qualified. Acting in the interest of InFaith, the donor shall be encouraged to discuss proposed gifts with legal and tax advisors of the donor’s choice.
PRIVACYA Gift Advocate will not share, sell or exchange donor names, addresses, email addresses or telephone numbers. Disclosures regarding gifts will be made only to those parties specifically noted by the donor.
EXPLANATION OF GIFTSA Gift Advocate shall make every effort to ensure donors receive a full explanation of how the proposed gift works on their behalf.
FINANCIAL AND TAX IMPLICATIONSCongress has provided tax and financial incentives for charitable giving. To that end, the emphasis in this statement on philanthropic motivation in no way minimizes the necessity and appropriateness of a full and accurate explanation by the Gift Advocate of those incentives and their implications for the donor.
FULL DISCLOSUREThe role and relationship of all parties involved in the making and distribution of gifts shall be fully disclosed to the donor.
FULL COMPLIANCEA Gift Advocate shall comply with all applicable federal and state laws and regulations at all times.
COMPENSATIONPayment of finder’s fees, commissions or other fees by InFaith to a Gift Advocate as a condition for the delivery of a gift is never appropriate.

Immediate & ongoing support to charitiesWhile all charitable gifts contribute to a common good, gifts to a Designated Donor Advised Fund offer the added value of ongoing, permanent support to the charities you recommend.You may also request grants for a term of years and/or immediate, one-time grants. See page 7 for more information.
If you have established a Dynamic Donor Advised Fund, you may recommend grants to any charities at any time.
Donor communicationsAt least annually InFaith sends reports to donors with funds that are actively making grants. These reports outline grants to charities, administrative fees and investment gains or losses. Statements for active funds are also available online at InFaithFound.org. Please contact us whenever you have a question about any aspect of your gift or InFaith policies.
Once your donor advised fund is established, you can request electronic delivery of statements at InFaithFound.org/fundlogin.
IrrevocabilityWhile you take an active role in recommending how your gifts benefit charities, gifts cannot be revoked after the transfer of gift assets has been completed. With gifts made upon death, such as bequests and beneficiary proceeds, you retain flexibility and control of the gift assets during your lifetime.
Administrative feesThere are no fees charged to set up your donor advised fund. Once your gift has been made, an annual fee equal to 1% of the fund’s assets is assessed on a quarterly basis to cover program and administrative expenses. Fund assets in excess of $1,000,000 are assessed a reduced fee of .5%. Fees are assessed on a per fund basis.
Dynamic Donor Advised Funds with a value of less than $10,000 are assessed a minimum $25 quarterly administrative fee. There may be additional fees for gifts of illiquid assets (e.g., real estate, closely-held stock, life insurance). Contact InFaith for additional information.
InFaith’s annual administrative fee for Advisor Managed Funds is 1% or $2,500, whichever is greater. Investment advisory fees assessed by a financial representative are not to exceed 1% annually. Total fees, which also include custody and investment management and embedded management fees within mutual funds, commingled funds, etc., are not to exceed 3% of a Dynamic Donor Advised Fund’s value. Please consult InFaith Advisor Managed Fund Policies for additional information.
Acceptance of your giftIn accordance with procedures adopted by the InFaith Board, a duly-authorized officer or other designated person must accept your gift on behalf of InFaith. Although in many cases the acceptance of a gift is largely a formality, this policy is in place to protect the interests of all parties involved. Once your gift is accepted, a receipt will be sent to you indicating the date of the acceptance. In some cases where multiple gifts are made in one year, a final comprehensive receipt is sent following year end.
Qualifying charities & grantmaking due diligenceAs part of our mission to serve you, the donor, InFaith works to ensure that grants from your donor advised fund achieve your giving intentions. In its role as steward, InFaith verifies the charitable status of all nonprofits receiving grants and each grantee’s financial and program information is reviewed.
While the vast majority of grants recommended by donors are honored by InFaith, the following types of grant requests are not permissible:• Grants that provide a more than incidental benefit to the
donor or other third party. This includes: all or a portion of the cost to attend a charitable event, goods bought at charitable auctions, raffle tickets, grants to satisfy a financial obligation or any individual or entity, grants that fulfill fundraising or other commitments or pledges, or that are directed to or for the benefit of specific individuals (e.g., school tuition, scholarships earmarked for individuals);
• Grants to organizations whose purpose or work is not solely charitable or when the grant will be used for a non-charitable purpose (e.g., cemeteries, VFW, fraternal societies);
• Grants to private non-operating foundations; and• Grants to supporting organizations (501(c)(3)s identified
in 509(a)(3)) that do not have an IRS determination letter stating they are “Type I” or “Type II”.

Fundraising policyInFaith cannot accept checks from individual supporters in response to fundraising events (e.g. golf tournaments, banquets, etc.) promoting donor funds at InFaith. Gifts received at a fundraising event for a specific fund should be made payable to the individual organizing the event, and are not eligible for a charitable tax deduction through InFaith.
Investment recommendationsAs per IRS regulations, investment recommendations are advisory only and InFaith may, at its sole and absolute discretion, follow or decline to follow your recommendation and/or change the investment of all or any portion of the assets in the fund. Investments are administered in accordance with the financial policies of InFaith and are subject to normal market and interest rate fluctuation risks. Any gain or loss generated by the above investments will be reflected accordingly in the fund’s value. You may change your investment allocation quarterly.
Capital asset sale policyGifts of capital assets, including securities and real estate, are sold upon receipt by InFaith. Proceeds from the sale are directed to your donor advised fund and invested according to InFaith’s investment policy.
Advice of counselCharitable giving can result in significant tax, legal and financial benefits and consequences. Because InFaith cannot give you legal or tax advice, you are strongly encouraged to consult your personal attorney and tax advisor before making any gifts.
Your privacy Protecting the privacy of your information is a responsibility InFaith takes seriously, and physical, electronic and procedural safeguards are in place to protect all data.
InFaith respects your wishes for privacy and will not share, sell or exchange your name or contact information with any person or entity other than those acting on behalf of InFaith. Disclosures regarding your gift will be made only to relevant parties specifically authorized by you in the Fund Workbook. In order to provide you with the best customer service possible, InFaith also provides online view-only access to the financial advisor(s) identified in the Fund Workbook. You have the option to authorize online view-only access to others if you wish. This online access includes your contact information, your gift(s) and grants from your donor advised fund.
