APPENDIX A.
Office of the Municipal and Charitable Institutions' Commission, Parliament Houses, 19th September, 1862.

issue a Commission to enquire into and report upon the finanmal pOSItIon of the vanous MUlllmpalitIes in the Colony of Victoria; the proportion whi?h thei,r sources 9f revenue bear to th.eir necessary expenditure; the extent of the. powers and duties whICh should be entru~ted to ~n.d lmpos~d 1?-pon them· and generally to enquire mto and report upon all matters connected With Mummpal Instlmtlons, I hav~ the honor to announce to you that the Commissioners have commenced their sittmgs.
The Commissioners propose to follow in their mode of procedure the example which has b~en set in similar enquiries in England. As a preliminary step, therefore, they have prepared the followmg statement of the points on which they deSIre to be informed; and I ani directed to request that you will he so good as to furnish, at your earliest convenience, whatever infonnation, documentary or otherwise, the corporation can suppl'y upon these points, in the order in which they arc severally stated :--
1. The name and style of the corporation.
2. The date when the corporation was crea.ted.

6. The financial position of the corporation- Financial posi!iQn (1.) A statement of the revenue during the past municipal year, giving the amount received
under each head or source; of the expenditure for the same year, giving the amount paid under each head or class; . and of the balance in hand at the commencement and close of the year. .
(2.) A statement in like form of the estimated revenue and expenditure for the present municipal year.
(3.) A statement of the property, whether real or personal, of the corporation, showing the nature and valne of such property.
(4.) A statement of the liabilities on contracts, and of the bond and other debts (if any) due by the corporation. .
(5.) Copies of the half yearly audited abstracts of accounts of the corporation from the municipal year 1857, inclusive; or from the establishment of the corporation, if created since that year.
7. The revenue of the corpo,ration(1.) How and by whom the assessments are arrived at; what exemptions (if any) are allowed;
by whom the rates are paid; by whom collected or received; and to what officers of the corporation paid over.
(2.) The gross value, and the rateable value of the property assessed each year in the municipal district, from the date of the establishment of the corporation, or from the year 1857 if previously established; the rates which have been struck in each year; the amount which they have severally produced; and the amount of arrears (if any) remaining uncollected.
(8.) The receipts for each year arising from licenses, tolls, and dues, giving the infonnation separat!lly under each head; the mapner in which the amolIDts to be charged are respectlvely 'fixed; by whom the amounts are collected; and to whom paid over. Any portion of the municipal revenue which may be leased or farmed out should be separately specified, and full particulars given.
(4.) The fees (if any) paid on election or appointment to any office in the corporation, and to whom paid. Also the nature and amount of any other fees, or of 'fines, or other revenue derived by the corporation, and to whom paid. .

8. The expendimre of the corporation- Expenditure. (1.) To what purposes and by whom the corporate funds are applied; and the means taken to
ensure the due application of the moneys. (2.) The several officers of the corporation-how and by whom elected, appointed or removed.
the time for which they hold their respective offices; their functions a~d privileges: their salaries and incidental emoluments, from the year 1857 to the present time. '
(3.) The remuneration or allowance (if any) paid to members of the cOlIDcil, or a.nyof them and tho amolIDt thereof, for a.like period.. .

9. Public Works-(1.) The Public Works completed or commenced by the corporation,. and the present condition
of such works rcspectively. (2.) The co.st of for~ing and metalling streets, per chain; also of kerbing, channelling, and
formmg or pavmg footpaths. . . , (3.) The annual cost of maintaining the same when completed.
(4.) The position of the {::~~r~~~} district, as to site and facilities for drainage and other works. .
(5.) The area of Crown lands unsold in the {:!~f~~~l} district, and the approximate value of the same.
10. The powers and duties of the .corporation-, (1.) The nature of the local revenue at present collected by the central government (if any),
the collection of and propcrty in which might with advantage to the colony be transferred to the corporation.
(2.) Thc local expenditure of the central government (if any) in public works or otherwise, the . charge and execution of which might in like manner be·transferred to the corporation.
(3.) As to the markets, water, supply, baths, parks, gardens, squares, free libraries, cemeteries. abattoirs, sewers, roads, and·(in port towns) the wharves and jetties, whether under th~ present management and eontrol of the corporation 01' not, with uny suggestions which may appear desirable. .
(4.) The' amount and 'eharacter of the relief (if any) afforded from corporate funds to the I destitute poor and sick (see 18 Vic., No. 15, Sec. 27), with any suggestions which the
corporation may be in a position to make. (5'). As to the question· of municipal endowment; the prel\ent method or any proposed improve

" Exeellency the Goveruor, with ·the advice' of the Executive Council, had been pleased to issue a " Commission to .investigate the amount and:character of the relief provided for the sick and destitute " in the various district.s of the colony by the establishment of charitable institutions, and generally to " enquire into and rep9rt upon all matters ,connected with charitable institutions," I am directed to inform you that the Commissioners have commenced their sittings. .
The Commissioners propose to follow~ in their mode of procedure, the example whieh has been set in similar enquiries in England. As l1J!reliminary step, therefore, they have prepared the following staremeht of the points on which they de/nrc to be Informed; and I am directed to request that you will be so good as to furnish, at. your earliest convenience, whatever information, documentary or otheI'"lv'se, the Committee of Management can supply upon these points, in the order in which they are severallr stated. 6. The income of the Institution from thelear 1857 to 1861 inclusrve, or from the date of the foundation of the charitv, if establishe since 1857 :-
(1.) Amount of public ~oney received annually for building and for maintenanec: any receiptil during the present year on account of 1861 to be'separately stated.
(2.) Annual amount of all boncl fide subscriptions received; also of all other receipts, specifying municipal grants, bequests, rents, interest, sales of property, and extraordinary sources, viz., proceeds of sermons, lectures, bazaars, benefits. &c.
(3.) Estimated receipts for 1862, under the heads of public grants, subscriptions, and all other receipts.
6. The expenditure of the Institution for the same period :-(1.) The expenditure each year on building, furniture, and repairs; for salaries and wages; for
maintenance and support; for medical comforts; for medicines and surgical instruments; for rent, interest, and sundry expenses including a.1I other disbursements. .
(2.) Total expenditure on building, repairs, and furniture respectively. from the foundation of the charity to the close of the past year.
(3.) Proposed expenditure for 1862, under the heads of building, furniture, and repairs; maintenance and support; salaries and wages; and all other disbursements.
7. The property of the Institution :-(1.) A statement of the property of the Institution, glvmg a description of the buildings and
land, and setting forth the nature and value of endowments from private sources (if any), and of each other item of property; stating also th€' amount of t.he cash balance on 31st Deeember, 1861.
(2). In whom is the property vested, whether in trustees or otherwise. 8. The liabilities of the Institution:"""on contracts; for cash adva.nces (if any); for building and
furniture; for maintenance and support; or on ot1Ier outstanding claims of whatever description at the close of the past year.
9. The objects purposed to be attai.ned by the establishment of the Institution:-(1.) The area and popuiation of the district (proper) to which the Institution is a source of relief. (2.) The number of patients, so far as can be ascertained, relieved each year belonging t.n
other districts than that in which the Institution is situated, specifying as far as may be possible the districts from which the cases are received.
(3.) Nature of cases treated in tbe Institution. (4.) Estimated cost of providing accommodation for persons committed in the district on Lhe
charge of lunacy, whether in transit!, or as permanent inmates; probable number of lunatics so to be provided for; number of lunatics (if any) at prescnt on the books of the Institution.
10. The accommodation afforded by the Institution. (1.) The number of patients which tbe Institution can accommodate. (2.) Proposed additions to the building now in progress or to be executed during the present
year; the further number of patients that can be accommodated when such additions are completed.
11. The number of persons in the Institution from the year 1857 to 1861 inclusive, or from the date of the foundation of the charity if established since 1857, (1.) The number of patients who have been cured or relieved each year, or who have died, or
who were under treatment by the Hospital on 31st December, 1861, classified as follows :Indoor patients, Outdoor patients, Casualties.
(2.) Applications for relief, by whom made, received, and decided upon. (3.) Are orders for admission by tbe governors or subscribers conclusive; if not, with whom
rests the final decision as to admission or rejection of the bearers of such orders. (4.) In cases of accident or emergency who is authorised to judge of the necessity for admission
to tbe Institution. (5.) Generally, what tests are applied to secure the admission of proper objects of the charity
only. (6.) Annual cost per head of each class of patients (as above) for the same period. (7,) Out of the wbole number of deaths. state the proportion which have resulted from chronic
diseases; with any other facts from which the success or otherwise of the Institution may be ascertained.
(8.) What provision is made for destitute patients who have been under treatment and have beeome convalescent, or who ha1'e sustained the loss of some member or organ of senS<' disqualifying them from following their previous pursuits?
12. General:-(1.) Claims to public, private, district, or municipal support. (2.) The method of obtaining subscriptions or donations, whetber entirely ~oluntary or ill
consequence of applications made for them. (3.) As to payments by patients, their friends, or otherwise, on their behalf. Are such
payments considered to be SUbscriptions or donations entitling the Institution to claim a rateable proportion of the Government grant thereon?
(4.) Number of applications for admission refused for want of room during the same period. (5.) Remarks-respecting the present system of grants from the Consolidated Revenue; SUll!,
voted by municipal councils ill the district; on the question of endowment; or any other remarks or suggestions on subjects connected with public charities.
The Commissioners confidently rely on the earnest co-operation of your Committee in furtherance of the objects of the Commission, and a~e desirous of ascertaining the views of any individual member of Committee, or offieer of your Institution, whose suggestions may assist them in treating of the matters to which this Oircular refers. As it is necessary that the labors of the Commissioners should be brought to a close at an early date, and as the Commissioners would wish to include in their report evidence respecting the whole of the charitable institutions in the colony, I am directed to request that the information may be furnished by Monday, the 27th October, at the latest.

been relieved each year, who have been discharged, who have died, and the num"\ler remaining on the books of the Institution on the 31st December. lS61.
(2.) AppJications for relief, by whom made, received, and decided upon. (3.) Are orders for admission by the govern'ors or' subscribers conclusive; if not, with whom
rest the final decision as to admission or rejection of the bearers of such orders. .. (4,) In cases of emergency, who is authorised to judge of the necessity for admission to the
Institution. (5.) Genei'ally, what tests arc applied to secure the admission of proper objects of the charity
only. (6.) Annnal cost per head of each class of patients (liS above) for the same period. (7.) What are the conditions nnder which relief is afforded? Is bodily infirmity only con-
. sidered, or have there been cases in which poverty has been deemed a sufficient ground for relief? In case of the recovery of' infirm persons what means are taken for providing them with employment on being discharged from the Institution? In the case also of persons (if any) relieved on the ground of poverty, are means taken for providing them with employment, so that they may remain no longer on the books of the Institution than the cases actually require.

7. The property of the Institution . (I.) A statement of the property of the Institution, giving a descriptioll of the buildings and
land, and setting forth the nature and value of endowments from private sources (if any) and of each other item of property; stating also the amount of the cash balance on 31st December, 1861.
(2.) In whom is thc property vested, whether in trustees or otherwise. 8. The liabilities of the Instituti~n-on contracts; for cash advances (if any); for building and
furniture; for maintenance and support; or on other outstanding claims of whatever desCli.ption at the close of the past year.
9. The objects purposed to be attained by the establishment of the Institution :-(1.) The area and population of the district (proper) to which the Institution is a sourco of
relief. (2.) The number of patients, so far as can be ascertained, relieved each year belonging to other
districts than that in which the Institution is situated, specifying as far as may be possible the districts from which the cases are received.
(3.) Nature of cases treated in the Institution. (4.) Estimated cost of providing aeeommodation for persons committed in the district on the
charge of lunacy, whether in transitu or as permanent inmates; probable number Of lunatics so to be provided for; numBer of lunatics (if any) at present on the books of the Insti tution.
10. The accommodation afforded by the Institution. (1.) The number of patients which the Institution can aceommodate. (2.) Proposed additions to the building now in progress or to be executed during the present
year; the further number of patients that can be accommodated when such additions are completed.
11. The number of persons in the Institution from the year 1857 to 1861 inclusivc, or from the date of the foundation of the charity, if established since 1857.
(1.) 'fhe number of inmates and of persons receiving outdoor relief (of each sex) who have been reliev(:'d each year. who have been discharged, who have died, and the number remaining on the books of the Institution on the 31st December, 1861.
(2.) The number of patients who have been cured or relieved each year, or who have died, or who were under treatment by the Hospital on 31st December, 1861, classified as follows:-

(3.) Applications for relief, by whom made, received, and decided upon. . (4.) Are orders for admission by the governors or subscribers conclusive; if not, with whom
rests the final decision as to admission 01' rejection of the b(:'arers of such orders. (5.) In cases of accident or emergency who is authorised to. judge of -the necessity for admission
to the Institution. (6.) Generally, what tests are applied to secure the admission of proper objects of the charity
only. (7.) Annual cost per head of each class of patients (as above) for the same period. (8.) Out of the whole number of deaths, state the proportion which have resulted from chronie
diseases; with any other facts from which the suceess or otherwise of the Institution may be ascertained.
(9.) What provision is made for destitute patients who have been. under treatment and have become convalescent, or who have sustained the loss of some member or organ of sense disqualifying them from following their previous pursuits? . .
(10.) W11at are the conditions under which relief is afforded? Is boldly infirmity only conslde!ed, or have there been cases in which poverty has been deemed a sufficient ground for rehef? In case of the recovery of infirm persons what meaus are taken for providing them wit~ employment on being discharged from the Institution? In the case also of persons (If

The Commissioners propose to follow, in their mode of procedure, the example which has been sct in similar enquiries in En~land. .As a preliminary step, therefore, they have prepared the following statement of the points on whICh they desire to be informed; and I am directed to request that you will be so good as furnish, at your earliest convenience, whatever information, documentary or otherwise, the Committee of Management can supply upon these points, in the order in which they are severally stated.
