Please ensure that you get presentation material to Elaine in good time – Elaine will send out template and specify cut off date in advance. Kier have offered the Oxford site for the August meeting which will be held on 15th August 2013. The venue for the 14th November 2013 Forum has yet to be agreed

• Engaged with Dramanon to develop a bespoke dramatisation and to tour our projects: Taking the message to the workplace. 200 personnel briefed including some TW operational staff
– Feedback on impact has been very good

HS&E inspection
� Specialist operatives apparently oblivious to risk working around open 4.0 metre deep valve chamberdeep chamber.
� No form of internal guarding or guardrails

Accidents, Incidents and learning
� Refresh manual handling training session, to all site personnel, including some
practical/ problem solving aspects to engage delegates
� Behavioural safety training now refreshed, to all site personnel, and annually from
now on. (IP had good motive, but chose to do an unsafe task).
� Metatarsal protection boots trialled and now a minimum standard at Riverside

These steps are being procured for a deep excavation at the Outfall Culvert for primary access and egress.

� Trench (Universal)Steps

� 2 RIDDOR Incidents this year.
� Pumping Incident: Selwoods had set up a pump for operations.
� 3 12” flexi hoses were connected from the pump to the 12” steel Pipes.
� At the security gates there were more flexi hoses which had been disconnected and the gate valve shut so that the gate could be closed.
� The hoses were connected by this gate and the pump started
� The gate valve was not opened

� One of the flexi hoses near the pump failed at the joint and the water pushed the operative into the pump

� The IP sustained bruising to his lower legs and a laceration that required sutures above his right eye.
� The Flexi hose failed. It was rated SWP 10 bar. (FOS 15 bar)
� The pump could only deliver at 4 bar
� Subsequent tests which were filmed by Selwood showed the flexi hoses from the same batch failing at 4 bar.
� All hoses from that batch destroyed. Safety Alert sent out by MBV and Selwoods.

� 2nd RIDDOR Incident:
� Locomotive fitter stepped out of manrider in tunnel, slipped and as he grabbed rails dislocate his shoulder.
� Accident happened during stand down for inline slurry pump installation.

� Main Causes: Working area was not cleaned up and ambi-decking laid for the whole working area
� Planning. Concentrated on high risk elements of task and not enough attention paid to low risks.

either side of saturated area. • No hand dig to locate cable
due to saturated ground• Cable curved between points
and thereby within excavated area
• No exclusion zone set up and CAT3-I report indicates no use of CAT in layers
• Some pressure to repair as night shut

to work 17:30 same day of incident.
Erection of rebar and shutters for new building.
Architectural changes – led to requirement to remove
