Special Notices Food for Thought Public servants are the new ‘labour elite’ "SA’s public sector wage bill now swallows nearly 35% of the budget and 14% of GDP. "Not only is this way out of line with other emerging economies, it is extreme by virtually any measure – including developed economies" – Ciaran Ryan, Moneyweb on Monday 12 February. Treasury’s Medium-Term Budget published late last year has showed public sector wages increased at 10.3% a year since 2009, considerably above the rate of inflation. Up to 70% of some provinces’ health budgets go to wages. One of the key figures being monitored in the upcoming budget on February 21 is the size of allocation to public sector wages. Adrian Saville, CEO of Cannon Asset Managers, argues that the government wage bill is already at an unsustainable level and any hike will only exacerbate the problem. “There is no governance and no accountability in respect of the spend. This perpetuates a system in which spending continues to rise without regard for impact or effectiveness.” According to a study: Demographic, Employment, and Wage Trends in South Africa by UNU-WIDER, the Brookings Institution and the Development Policy Research Unit (DPRU) at the University of Cape Town: "Unionised public sector workers have become the new 'labour elite'." According to the OECD’s 2017 Economic Survey of SA the public sector wage bill is at 35.3% of the budget. Despite a freeze on public sector employment in 2015 and 2016, the OECD says further efforts could be made to limit annual wage increases by reallocating workers to areas where they are more needed, such as health and education. Last year, Mike Schussler, the chief economist at Economists.co.za, warned that SA could no longer afford a bloated civil service, and that the only way out was a massive private sector job creation drive. "But that would require government to abandon policies that stood in the way of investment and job growth, " wrote Ryan. Graphics: Figures from OECD, published on Moneyweb, 12 February 2018 View on Special News SASOP's view on cannabis SEMDSA's warns patients about 'The Secret Fat Burner' In a press release regarding its position on cannabis, SASOP notes its concern as a growing public perception of cannabis as a ‘harmless’ plant. A review article by the WHO (2016) concluded that current evidence points to a modest contributory causal role for cannabis in schizophrenia and that a consistent dose- response relationship exists between cannabis use in adolescence and the risk of developing psychotic symptoms or schizophrenia. SASOP commended the MCC’s decision to limit the use of cannabis for medicinal purposes and supports ongoing research. SEMDSA, The Society for Endocrinology, Metabolism and Diabetes of SA issued a press release warning about “The Secret Fat Burner”. A number of endocrinologists have recently noted patients using “The Secret Fat Burner” presenting with overactive thyroid (thyrotoxicosis). SEMDSA has had the above product tested by an independent accredited laboratory, and the test results found that the capsules contained Hydrochlorothiazide, Sibutramine, Levothyroxine (T4) and Triiodothyronine (T3). None of these substances are mentioned in the product’s information leaflet, which means the product is a classified a “secret remedy”, the use of which is prohibited by the Health Professions Council of South Africa. The product is also not approved by the Medicines Control Council (MCC), but is advertised on social media. Symptoms associated with consumption of this product may include: palpitations, anxiety, heat intolerance, aggravation of high blood pressure and cardiac complications such as atrial fibrillation and heart failure which may be potentially life-threatening. Protecting Patient Data in 2018 The new year is upon us and for many it’s started with a bang. But although there may be a need to get back into routine as fast as possible, there are also elements in the typical day-to-day workings of your practice that you should be reviewing and one such element is the protection of patient data. Ensuring the security of patient data should always be a top priority, but you want to avoid data security solutions becoming a bottleneck to productivity. Here are some tips that you can implement within your practice to give you the most time with your patients, safe in the knowledge that data is securely stored. New Year, new passwords It’s a good idea to regularly update passwords and passcodes across your practice and while it’s tempting to recycle old passwords, it should be avoided. Ensure that every staff member with access to patient and practice data updates their passwords to a minimum of six upper and lower case characters, including at least one numeric and one symbol character and free of consecutive identical characters. If the thought of changing passwords makes you want to hit the eggnog, investigate password manager programs that will safely store your passwords and you only have to remember one master password. Go paper-light If you’re still using paper in your practice, ensure that letter trays aren’t placed on the front desk in full view of other patients in the consulting rooms. Keep them behind the desk, out of sight and file paperwork daily in a control-access room. And be sure to dispose of paper records appropriately and in accordance with POPIA. Advocate Adila Hassim, appearing on behalf of Section 27, representing the families of 74 people’s families said the State had agreed to a R20 000 claim per family for funeral expenses, and R180 000 for emotional shock and psychological injury, and counselling services. The number of people who died after being moved from the care of Life Esidimeni could reach 156. The hearings was finalised on Friday, 9th February and former Chief Justice Dikgang Moseneke, will later release his findings, including on compensation. "I feel people have been betrayed and I also felt betrayed as the Minister of Health. This has tarnished the health system of the country," said Health Minister Aaron Motsoaledi at the hearing. The Trade and Industry Department expects to submit a revised version of its draft intellectual property policy to the Cabinet by the end of March. The department is facing intense lobbying from health activists and pharmaceutical companies as the draft policy proposes closer scrutiny of patent applications for medicines, a simpler system for issuing compulsory licences and mechanisms for tightening the criteria for granting patents. The Innovative Pharmaceutical Association of SA (IPASA) criticised the draft policy when it was released, saying it placed too much emphasis on drug firms’ “ever-greening” patents on medicines. The draft policy covered only the first phase of the government’s overhaul of its intellectual property regime, which targeted public health and intergovernmental coordination. Phase two will include intellectual property rights in the informal sector, agriculture and genetic resources, as well as the branding of SA goods and services. View on healthcare in the Provinces Limpopo Healthcare in dire straits Protesting healthcare workers in Limpopo warned that the health department’s money problems could be keeping pay checks out of doctors’ hands and health workers out of clinics. Doctors complain that the state can no longer pay for overtime while nursing unions say the province is reneging on commitments to employ medical bursary holders. About 80% of 21 Limpopo clinics surveyed by TAC were short staffed. The opposition party claims that the department has overspent its budget by R1.7-bn since the start of the financial year. As of 2014/2015, salaries ate up nearly 65% of provincial health budgets, according to a July 2016 paper by the non-profit organisation the Rural Health Advocacy Project (RHAP). Gauteng: R1-bn for medical negligence KwaZulu-Natal: Oncologists from Cuba and India Northern Cape Mental hospital for Kimberley The Gauteng DoH forked out over R1-bn in settling 185 medical negligence cases from January 2015 to May last year, according to MEC Gwen Ramokgopa. 76% of the cases were for 50 cases of brain- damaged babies. Chris Hani Baragwanath Academic Hospital in Soweto had the most cases against it, with R514-m paid to 44 claimants. KZN eyes oncologists from Cuba and India as no one will work in state hospitals. Durban still does not have a single permanent oncologist at its two state hospitals designated to address cancer treatment. The KZN health department is now working on plans to import oncologists either from Cuba or India to mitigate the cancer treatment crisis. It is a decade behind schedule and cost more than six times more (R1.86-bn) than originally planned, but the Kimberly Mental Health Hospital will finally be completed next month. The hospital will replace the Free State Psychiatric Complex (Oranje Hospital), which will be decommissioned. However, there is a question mark over whether the provincial health department has the money to run the facility. Financial Views Drug makers want another price hike Pharmaceutical manufacturers are lobbying the DoH to give them an extra price increase for private sector sales, in an attempt to soften the blow of the modest raise (1,26%) permitted for 2018. The 2018 SEP increase failed to cover the rise in input costs faced by manufacturers in the year, said Stavros Nicolaou, chairman of Pharmaceuticals Made in SA. He said the formula applied by the department does not consider global market dynamics or material changes to input costs such as utilities and labour. The Board of Healthcare Funders of Southern Africa (BHF) approved of the 1,26% increase. View on New Products Test can detect 8 types of cancer in a drop of blood Novo Nordisk aims to break mould with new pill Researchers in the US have reported that a test, dubbed CancerSEEK, was developed by Johns Hopkins University. It looks for 16 DNA mutations as well as 8 proteins (biomarkers) associated with cancer. CancerSEEK was tested on 1 000 patients with cancers in the ovary, liver, stomach, pancreas, oesophagus, colon, lung or breast. The test was 70% accurate at spotting people with cancer. Denmark’s Novo Nordisk - the world’s biggest insulin maker - wants to transform the diabetes market with a once-daily pill that belongs to a blockbuster class of treatments known as GLP-1s that stimulate insulin production. If Novo gets over the pricing hurdle, the new pill is set to reshuffle treatment regimens, with oral semaglutide being used as a second or third treatment option after cheap, generic metformin. View on General News Global push to track ‘superbugs’ 'SA Health system was better under apartheid' The Medical Research Council (MRC) and the Foundation for Innovative New Diagnostics (FIND) signed a memorandum of understanding on antibiotic resistance, in an effort to boost collaboration in combating “superbugs”. Globally, an estimated 700 000 people die from drug-resistant strains of common bacterial infections each year. Scientists fear if available antibiotics are not used more judiciously, and new ones developed more rapidly, the world may return to the pre-antibiotic era when an infected scratch or insect bite could prove deadly. Listeriosis outbreak in SA The death count in SA’s listeriosis outbreak - the worst documented listeriosis outbreak in global history - has topped 100. According to the National Institute of Communicable Diseases (NICD) the number of confirmed listeriosis cases was 852‚ and 107 people had died. The death rate was 30%. Roughly two-thirds of cases have been confirmed in state hospitals. Economic Freedom Front (EFF) leader Julius Malema has claimed that public healthcare services were better during apartheid than they are now under the ANC government. He referred to what he called a “national health crisis”, which the EFF is planning to expose. He explained how a functioning clinic in Limpopo, built during apartheid, was closed by the current government. The EFF declared 2018 the year of public healthcare. Abortion: legal service performed mostly illegally Health workers who refuse to help women who want abortions are driving them into the arms of illegal abortionists who perform more than half of the country’s procedures. Shenilla Mohamed, executive director of Amnesty International SA, said it was of major concern that fewer than 7% of health institutions offered safe abortions, and that more than 50% of abortions were carried out by back street abortionists. Teen pregnancies down; TB cure up; NCD's on the increase Pregnancy and condoms Less than 7% of all hospital births were to teen mothers in 2016/17, with the lowest in Gauteng (less than 5%). A decade ago almost 15% of teenage girls were likely to fall pregnant, according to the District Health Barometer 2016/17 (DHB). While HIV and TB rates are dropping slightly, a collection of non-communicable diseases (NCD) - particularly cerebrovascular diseases, ischaemic heart disease and diabetes – are on the increase. The cure rate for TB now stands at 81% - the highest since 2005 - according to the DHB. In March 2013 SA introduced bedaquiline, used for patients with drug-resistant TB, and to date 9 650 drug resistant TB patients have been put on to a bedaquiline- based regimen. Meanwhile SA’s first survey to assess the incidence of tuberculosis in the country has been commissioned by the National Health Department and will enrol an estimated 55 000 participants. Hardly any pregnant women are having safe sex, leaving them at high risk of HIV infection, according a team led by Simnikiwe Mayaphi, a medical virologist at the University of Pretoria. Of the 7 886 pregnant HIV-negative women questioned 99.5% had had recent unprotected sex. Of the 1 661 HIV-negative men and women recruited at counselling and testing clinics, 88.5% admitted having unprotected sex in the last three months. released,sayingitplacedtoomuchemphasison

drugfirms’“ever-greening”patentsonmedicines.

Thedraftpolicycoveredonlythefirstphaseof

thegovernment’soverhaulofitsintellectual

propertyregime,whichtargetedpublichealth

andintergovernmentalcoordination.
