Community Outreach strategies
Platinum Email to CBOs with explanation of survey, website survey link, a request to post the survey link on their website, promote the survey in their organizational publications, and a request to send out to their e-list.
Silver Platinum outreach strategies + Personal contact from committee rep to discuss the intent of the survey, specifics on how best to reach their constituencies, development of individual outreach plan to include possible paper surveys, banner, fliers, etc. for distribution and display at community site. Distribution of flier at community events or community meetings.

• 83% of those identifying a Fair or Poor overall health status had incomes <$50,000/year and 58% were either NHBlack, NHAsian or Hispanic.
• 69% of those identifying a Fair or Poor mental health status had incomes <$50,000/year and 57% were either NHBlack, NHAsian or Hispanic.
