Our Mission

The social health practice is KP’s effort to

integrate social health with physical and mental

health across the enterprise by focusing on

systems, tools, interventions, and culture

change required to support KP members. KP will

address social factors that impact the health of

our members by identifying social needs,

connecting members to resources, and

establishing support and follow-up. The social

health practice includes tools like Thrive Local,

and KP funded resources to reach KP members

for food, housing, and other social needs.
