GENERAL INSTRUCTIONS
READ THIS! RETURN ONLY ONE COMPLETED FINANCIAL STATEMENT TO THE GUIDANCE SECRETARY. NO APPLICATION WILL BE CONSIDERED WITHOUT THIS FINANCIAL FORM along with a copy of Student Aid Report (from FAFSA) with Family Estimated Family Contribution (EFC) listed. Only SAR not entire FAFSA. When student’s financial aid package is received from college that needs to be submitted also.
This step cannot be forgotten.
Please read “ELIGIBILITY” section of each scholarship carefully to make sure you qualify. Pay strict attention to directions regarding letters of reference. Be sure to include ALL requested material, including application forms. If these are not included, your application will not be considered. Letters must be typed or completed on a computer. Only include what the committee asks for, do not attach anything else. In the upper right hand corner of each letter being written, please indicate the name of the scholarship for which you are applying. Your name should be under the scholarship name. Letters must be typed or completed on a computer. Don’t use pencil for any part of your application package. Those applications that are to be addressed to the principal should be given to Mrs. Adams in the Guidance office. Please do not put applications in envelopes, but be sure to staple multiple pages together. No paper clips! Regarding applications that must be MAILED to the scholarship committees - IT IS YOUR RESPONSIBILITY to prepare the completed application and all attachments to the appropriate committee. Your transcript will be given to you to copy. Counselor recommendations and teacher recommendations will be kept in the Guidance Office. If one of your scholarships requires a recommendation, submit the completed package to Mrs. Adams with a post-it note attached requesting the recommendations to be enclosed. If it is to be mailed, have the envelope completely addressed with the appropriate stamps affixed. As a general rule, two stamps will be needed if the complete application will be 3-5 pages; add three stamps if 6 pages or more. If you are using a larger manila envelope postage is more. Allow enough time for mailing deadline. Bring in packages to be mailed one week before deadline date of April 1, 2020. Check due dates carefully - applications will not be considered after the date specified. Most applications are due April 1st. That means received not postmarked by that date. You are responsible for attaching a student transcript to all scholarship applications. If you have questions, please consult Dee Smith or Krystal Adams. 2020 Advocates for a Community College Education Scholarship (ACCE)
A component fund of the Cape Cod Foundation About ACCE ACCE was formed in 2011 and is a group of individuals who have a passion for making a community college education available and affordable for graduates from Nauset Regional High School as well as other Cape and Islands’ high schools. The scholarship program is unique because in addition to providing funds to attend the college, each recipient also will be assigned one or two mentors who will meet with the student once or twice a month throughout his or her two years at Cape Cod Community College (CCCC). Information about ACCE and the scholarship program is available at www.advocatesforacommunitycollegeeducation.org.
Committee: ACCE scholarship committee members in collaboration with Ms. Dee Smith and Mr. Brian White.
Award: Recipients can receive up to $4,000.00 in scholarship funds. Up to $1,000 in funds, based on the number of enrolled credits, are paid directly to the college each semester. To earn the full scholarship, students must enroll in 12 credits each fall and spring semesters and adhere to the guidelines listed on the ACCE website.
Eligibility: Graduating NRHS seniors who are applying to or have been accepted by CCCC and plan to enroll in the fall semester following graduation. The scholarships are for students with: a track record of academic success and/or improvement; a demonstrated financial need; a record of extracurricular activities, volunteering or paid employment; and, educational and/or career goals following graduation from the college.
Application: Completed applications are due on March 23, 2020 and must include: A completed ACCE scholarship application form (below). A letter stating why you decided to attend CCCC that includes information about your educational and/or career goals. A writing sample that describes a personal accomplishment that you are particularly proud of. Two letters of recommendation. Your high school transcript that shows your GPA and class rank. Your Student Aid Report (SAR from your FAFSA) that shows your Estimated Family Contribution (EFC) that will be provided by the NRHS Guidance Department. Finalists will be interviewed by two ACCE scholarship committee members.
Payments: Payments will be made directly to CCCC after the recipients submit copies of their semester schedule and invoice verifying enrollment to their mentors before the start of each semester. End of semester grade reports also must be submitted.

ALEX B HAAS MEMORIAL SCHOLARSHIP COMMITTEE: Dr. Christopher Ellsasser, Principal, Justin Bohannon (Swim Coach),

AWARD: Multiple awards ranging from $1000-$2500 ELIGIBILITY: A member of the graduating class of Nauset Regional High School
who has been a member of the varsity swim team and/or sailing team.
APPLICATION: Letter to Principal including transcript by April 1ST, 2020 and
submitted to the Guidance Office. Student transcripts must accompany all scholarship applications. Please read the attached biography of Alex Haas and let us know in your narrative if you share any or many of the qualities he possessed.

ALEX B HAAS 1987-2005 “What we do for ourselves, dies with us; what we do for others and the world remains and is immortal.” -Albert Pine Alex Haas graduated from Nauset Regional High School in June of 2005. He was an
honor student as well as an outstanding athlete. During his four years of high school Alex was a member of the swim and sailing teams, co-captain of both in his senior year, a percussionist in the band and a member of the ski team and the Nauset Cares community service organization. He was headed to Massachusetts Maritime Academy to pursue a degree in Marine Transportation and a life on the ocean. Upon the news of Alex’s untimely death, swim coach Katie McCully and sailing coach Warren Silver were instrumental in spearheading the formation of the Alex Haas Memorial Scholarship designed specifically for Nauset swimmers and sailors wishing to attend an institute of higher learning. From an early age, Alex loved regimentation, flags and America and so began his involvement in scouting. At an annual awards dinner on March 6th, 2006, Alex was honored by the Boy Scouts of America and was presented the Spirit of the Eagle Award which celebrated his life as an Eagle Scout and the dedication, determination, motivation and years of hard work it took to achieve that rank. He was also given the BSA National Endowment Award for monies donated posthumously in his name to the Cape Cod and Islands Council. An accomplished sailor and entrepreneur, having created his own boat cleaning and detailing business, Alex spent many hours in Saquatucket Harbor in Harwich Port; quickly becoming his home away from home. On May 21st, 2006 at the Lost Sailor’s Park at Saquatucket Harbor a bench was installed, a cherry tree planted and a stone monument erected commemorating Alex’s love of and commitment to sailing. On that date, the park was also renamed the “Alex B. Haas Memorial Park.” Around the same time, thanks to an underclassman that Alex had befriended the year before, a similar stone was set in a small park on the campus grounds of Nauset Regional High School. On June 7th, 2007 in that small park, a very meaningful remembrance ceremony took place. Alex’s dream career from his first time behind the controls on the Elisar, his Uncle Philip’s boat, was that of a ship’s captain. He loved the water and performed well in it and on it. Although he never made it to Mass Maritime, we’d be remiss if not to mention the support we have received from the administration, staff, parent group and the corps of cadets. A sea-term scholarship has been set up at the Academy in Alex’s memory for freshman cadets demonstrating financial need, majoring in Marine Transportation with a penchant for sailing and swimming. In August of 2011 the Orleans Yacht Club created the Alex B. Haas Memorial Award recognizing the Outstanding Regatta Participant in the Junior Sailing Program who best exemplifies a contagious passion, enthusiasm and love for the sport of sailing. Alex was especially fond of Laser and 420 regattas while at OYC. Alex was extraordinary and did much in his short 18 years on Earth. He was well-liked and respected by both adults and peers. As his parents, we are very proud of the young man he was and admire and respect the places, awards and scholarships in his name, however; we can’t help but wonder what he’d have accomplished had he had more time.

AUSTIN JACOBS MEMORIAL SCHOLARSHIP
COMMITTEE: Jody Craven, Joe Hoyt and member of the Jacobs Family
AWARD: 2 recipients at $300 each
ELIGIBILITY: A graduating senior of Nauset Regional High School planning on post graduate education. Preference will be given to students of metals, wood or video tech programs.
APPLICATION: A letter to the committee turned into the Guidance Office describing yourself and your hopes for the future by April 1, 2020.
PAYMENT: Payment to the student after presentation of a tuition bill.
Austin Robert Jacobs
Austin attended the Nauset District from grade school through high school and graduated in 2010. He exceeded in academics and enjoyed running track and various other sports. People were drawn to Austin's charisma, genuine nature, and his beautiful smile, which could melt your heart. Austin had a natural ability of bringing people together and was always helping others and giving back.
During his years at Nauset High School Austin took a special interest in wood and precious metal work. Austin won the Boston Globe Silver Key Award for
one of the many rings he crafted. He was always there to assist classmates and many times created rather amusing scenes in shop (we are told). He donated many of his finished projects to charity to help their efforts to raise funds.
After high school Austin earned his associates degree from Greenfield Community College and attained state certification as an Emergency Medical Technician. He was passionate in supporting research in finding a cure for a disease known as Fibrodysplasia Ossificans Progressiva which affected a family member. Austin was an organ donor and gave his gift of life after a tragic motorcycle accident in 2013.
Austin always helped others and fought for what he believed in. He had a strong sense of justice and would be the first to stop and help someone in need. Austin embraced life and love in a very giving way. We hope this scholarship will help several students attain their goals, while keeping Austin's memory and legacy alive.

majoring in music. Financial need will be considered. APPLICATION: Letter to Principal including transcript by April 3rd and submitted to
the Guidance Office. Letter should state why you are interested in music and what effect it has had on your life. Include an insight into what you plan to do with a musical career, your goals and objectives and where you expect to be in music within the next six to eight years.

2020 BRESTER VFW POST 9917 SCHOLARSHIP
COMMITTEE: The Scholarship Committee shall be comprised of the principal of
the respective school and/or his designee, the Post Commander and three additional members of the Brewster Post 9917 as assigned by the Post Commander.
AWARD: Three (3) scholarships may be awarded annually in the amounts as
follows, 1st place $1,000.00, 2nd place $500.00, 3rd place $250.00 ELIGIBILITY: Any member if the graduating class, in good standing, continuing on
to at least two years of higher education, from Nauset Regional High School or Cape Cod Technical High School may be eligible for this scholarship award.
APPLICATION: A letter, 250 to 500 words in length, must be submitted by the
applicant clearly stating the reason for requesting said scholarship, the intended program of study and the students career goals. Student transcripts must accompany all applications. Submit the letter application to the school guidance office no later than April 1st, 2020.
CRITERIA FOR Awards are based on the following areas; AWARD: Grade Average, Community Activities, Scholastic Interest, Financial
Need. Preference will be given to a child or grandchild of a military veteran.
PAYMENT: Payment of the scholarship monies awarded shall be provided to
the school of which the recipient is attending upon completion of a successful first semester.

CAPE ASSOCIATES, INC. SCHOLARSHIP In Memory of Lester F. Langhans, Jr.
PURPOSE: To assist in furthering the education of a Nauset Regional High
School student who exemplifies the qualities of integrity and citizenship.
COMMITTEE: One representative from Cape Associates, the Principal, and the
Director of Guidance of Nauset Regional High School AWARD: $8,000.00 ($2,000.00 per year, for four years) ELIGIBILITY: A graduating Nauset senior who has attended the high school for the past two years, is accepted for enrollment at an institute of higher education, and shows financial need. APPLICATION: A letter to the Principal by April 1st , submitted to Guidance Office,
discussing the goals, needs, how the student and parents will be contributing, achievements, and examples of citizenship and integrity. Student transcripts must accompany all scholarship applications.
PAYMENT: To the student upon successful completion of the first semester and enrollment in the second semester, in each of the four years.

CAPE COD ASSOCIATION SCHOLARSHIP COMMITTEE: Cape Cod Association Scholarship Committee AWARD: Cape Wide Scholarships at various amounts, dependent on income
and number of acceptable applicants ELIGIBILITY: A senior who was born on Cape Cod and resides here and will
attend either a two year or four year college or technical school. Also available to past graduates but you must reapply each year.
APPLICATION: Completion of the application form (available on line at
www.capcodassoc.org) to be done on line with supporting data, including financial aid form, by April 1, 2020 to Cape Cod Association, 259 Willow Street, Yarmouthport, MA 02675. Student transcripts must accompany all scholarship applications.
PAYMENT: The full amount of the scholarship will be paid to the school of the
student’s choice in August 2020 with proof of college acceptance. Student will be contacted by Cape Cod Association for payment information. Scholarship is renewable each year with yearly application with a deadline of March 15, 2020.

CAPTAINS COURSE MEN’S GOLF ASSOCIATION SCHOLARSHIP
In Memory of Mike Austin COMMITTEE: Appointed by the President of the CCMGA with the concurrence of
the Board of Directors. AWARD: $1,000.00 ELIGIBILITY: The recipient must be a male member of the graduating class of
Nauset Regional High School who is planning to attend a degree-granting institute of higher learning. The applicant should be a regular playing member of the High School Men’s Golf Team including senior year. Good sportsmanship and quality of completed application shall be important factors in awarding the scholarship.
APPLICATION: Please write a letter to the Principal by April 1st including a
transcript and submit to Guidance Office stating the reason you are requesting this scholarship, your intended program of study and career goals. Candidate will be selected based on academic preparation, leadership skills, contributions to school and community, and written recommendation of golf coach. Be sure to include an additional recommendation from a faculty/staff member.
PAYMENT: Payable to student upon successful completion of the first
semester.

CARL HEVERT MEMORIAL SCHOLARSHIP
Carl and his brother Herb, sons of German immigrants, were educated at Stevens Institute of Technology Engineering School in Hoboken, N.J. A guidance system aerospace engineer throughout his career, Carl retired as Vice President of Manufacturing of Kearfott Corporation in Little Falls, NJ. Among other accomplishments, Carl held a patent for the Vertical Gyroscope. In retirement, Carl was a dedicated public servant, working tirelessly to make Orleans an even better place to live now and for future generations. He deeply loved the Cape, enjoying Nauset Beach with family and friends year round. Carl instilled in his children and grandchildren respect and appreciation for nature, and taught them the importance of hard work, dedication and perseverance. Education, in all its forms, was paramount to Carl throughout his life. Carl and his wife Nora were dedicated to providing financial assistance to their children so that they could focus on academics during their tenure at college, and be better prepared to begin their careers. The family therefore established this memorial scholarship to honor Carl, as they know it would mean a great deal to him to assist accomplished and motivated students in their pursuit of higher education. He appreciated and supported every person he met at the arena. Tim’s ability to identify, value and nurture each member’s talents and interests helped to foster the growth of our community. CHRISTOPHER DUFFY MEAD
Nauset Regional High School Class of 1984
Chris Mead was an active participant in the Nauset community and made many connections over the course of his life. During his elementary and middle school years he enjoyed playing on recreation teams for the towns of Eastham, Wellfleet and Orleans. In high school Chris played soccer, basketball and baseball. He was an enthusiastic teammate and team leader. Chris went on to play basketball at Johnson State College in Vermont. As a high school and college student Chris worked summers for the Orleans Recreation Department. He frequently shared stories with friends and family about the personal growth, maturity and adventures of his young charges. Chris was a loyal New England sports fan, having attended a Red Sox game the night he was tragically killed. Chris is survived by the Mead, McEnaney and Walters family, including 10 nieces and nephews and 1 great nephew. The entire family invites any NRHS student-athletes to apply for the Christopher Duffy Mead Scholarship.

AWARD: $4,000.00 Scholarship
ELIGIBILITY: Student must be a member of the 2020 graduating class of Nauset Regional High School, and shall be pursuing a Bachelor of Science Degree in Civil Engineering or Land Surveying.
APPLICATION: Applicant shall submit an essay of approximately 300 to 500 words that describes his/her reasons for pursuing a career in civil engineering or land surveying. Student transcripts must accompany all scholarship applications.

AWARD: Scholarships are awarded each year to graduating high school seniors, students currently enrolled in a college undergraduate or graduate program, community college or vocational-technical school.
ELIGIBILITY: Applicants must be graduates of, or graduating from, Nauset Regional High School. Applicants must have regularly attended a church, or church services.
Students/applicants participating in their church worship services and/or their church activities will be given priority

The scholarship is named after former NRHS swim coach (Katie McCully’s) father who was a long time Northampton High School swim coach and teacher.
The recipient will also be named on a perpetual plaque in memory of this award.

AWARD: $500.00
ELIGIBILITY: A NRHS senior currently on the NRHS swim team and pursuing further education and collegiate level swimming.

John Randall Mayo Jr 1961-2016
John Mayo Jr graduated from Nauset Regional High School in 1979. While at Nauset he was a member of the tennis and cross country teams as well as well as a member of the National Honor Society. In addition to his achievements during high school John also attained the rank of Eagle Scout.
After graduating From Nauset he went on to obtain a Bachelor’s degree in aerospace engineering from the University of Southern California before completing his Masters in electrical engineering from Yale.
John began his work career at Sikorsky Aircraft in Stratford, CT, where he
received his first paten. He served as the Assistant Program Manager on the UH-60L BLACK HAWK Helicopter.
In 2005 John moved to Savannah, GA to work for Gulfstream Aerospace and became the Director of Advanced Technology Programs focusing on the G650 and G500/G600 aircraft. He was part of the team that won the 2014 Collier Trophy for innovative development of the G650 Aircraft.
John was an avid world traveler and sports enthusiast. He completed nine marathons and enjoyed biking, skiing and playing tennis.
He will be remembered for this enthusiasm for life, his dedication to his family and career as well as his devotion to his Cape Cod heritage.

NMBC Senior Scholarship Application Nauset Music Booster Club (NMBC) Scholarship Information
Committee: NMBC executive board members; NRHS instrumental and vocal music teachers.
Senior Scholarship: Up to two (2) senior scholarships of $1,500 each.
Eligibility Requirements: The NMBC Senior Scholarship is open to graduating seniors who have been active in Nauset Regional High School music programs and wish to pursue music at an accredited school, college or university. They may pursue music by studying music education, music performance, music therapy, etc., OR by continuing lessons and performance while studying another field of their choice.
Each student will have demonstrated a strong commitment to the Nauset Regional High School music program for 4 years (unless a transfer student) or have shown commitment to musical performance in other important ways. Music should be a significant part of the student’s high school career, but they do not have to be first chairs or lead players. Students will have shown leadership in the music community, participated in music events inside and outside the classroom, supported Nauset Music and NMBC activities, and have an appreciation for the arts. Students will be selected jointly by the board of NMBC and the music teachers based on their overall contribution to the music program.
Procedures: Students must complete the NMBC Senior Scholarship Application (see below), including a short essay (one page) and details of their participation in music programs and their plans to continue in the future.
Payment: The scholarship award will be contingent upon the successful completion of the first semester of the recipient’s continued education, which must occur within one year of the award. NICHOLAS ‘NICK’ DESIMONE
Our son Nick graduated from Nauset High School in 1998. He loved sports; all sports. Nauset Ball was probably his favorite activity in PE class. He played Baseball, Hockey and Football while in high school. He went on to become Junior and Senior Captain of the Football program which was an unfunded program originated in his sophomore year. He helped fund raise for the program by waiting tables and washing dishes for team dinners, selling ads for the program book and setting up for the Pops in the Park concerts. He loved the camaraderie of the football team and made many close friends. After graduating from high school, Nick had the opportunity to coach alongside his father, Lenny, the Nauset Freshmen Football coach. This was by far one of his fondest memories. Nick’s favorite sport though was Hockey which he played from the time he was four years old throughout high school and into college. He had a passion for the sport both as a player and a spirit raiser, often loving to get the crowd into the game. Upon graduation from Nauset High, Nick went on to Bridgewater State University where he majored in Physical Education. He also attended the University of Massachusetts in Amherst. While in college he continued his love of sports by playing Football and Hockey. Nick continued to have aspirations to one day get back into coaching. As an adult, Nick worked at a number of local restaurants including The Brewster Fish House and Fin in Dennis, where he became a talented cook. We hope this gives you an insight into what kind of person Nick was. He loved to make people laugh and many of his friends thought he was the funniest guy they knew. ORLEANS EASTHAM AMBULANCE ASSOCIATION (Administered by the Orleans Rotary Club)
ELIGIBILITY Only applicants in their senior year at Nauset Regional High School or Cape Cod Regional Technical High School are eligible for selection to receive an Orleans-Eastham Ambulance Association 4-year scholarship, which is awarded by the Orleans Rotary Club. An applicant must be a permanent resident of the Nauset Regional School District, generally inclusive of Brewster, Eastham, Orleans, Provincetown, Truro, and Wellfleet. A permanent resident is considered to be a person who declares any of these towns as his or her legal residence, and has actually lived in one or more of these towns for at least six months prior to the date of the application. Applicants pursuing a medically-related course of study will be given primary considerations. A medically-related course of study is not limited to programs leading to a medical degree, but rather may be liberally interpreted to include those such as paramedical training, licensed practical nursing, dental hygiene or any medically-related training considered to be qualifying by the Orleans Rotary Club Ambulance Scholarship Committee. Criteria used in the selection of scholarship recipients include: Declared interest is a specific medically-related course of study, demonstrated academic diligence and achievement, good citizenship and financial need.
APPLICATIONS Scholarship applications must include: (1) home address and telephone number of the applicant; (2) high school transcript and appropriate teacher recommendations; (3) statement by the applicant regarding college and career aspirations; (4) summary of high school and other activities of relevance; and (5) a detailed statement of financial need. Applications must be submitted before April 1st to the Guidance Secretary at the appropriate school. Each year no more than five scholarship applications selected by the Guidance Department of Nauset High School and as many as five selected by the Guidance Department of Cape Cod Tech may be submitted to the Chairman of the Orleans Rotary Club Ambulance Scholarship Committee for recipient selection by the Committee.
PAYMENT PROCEDURE After the first semester in each of four years in college, scholarship recipients must submit a timely report (including photocopy of the college’s grades summary) to the guidance director of the high school from which the student graduated. Subject to satisfactory review of the relevant guidance department and the Rotary Ambulance Scholarship Committee, arrangements will be made for prompt funding to be sent directly to the student.

THE OUTER CAPE CHORALE SCHOLARSHIP SCHOLARSHIP GUIDELINES PURPOSE: The Outer Cape Chorale’s mission is to promote excellence in the

BLISKA Brad Liska Scholarship $1,000 Brad was kind and compassionate, wiser than his years and made an impression wherever he went. He was curious with an infectious spirit and a wide-range of interests. He was a nationally ranked speed rope-jumper and loved soccer and ski racing. Brad embraced life and touched so many hearts along the way. Brad passed after a very courageous fight with cancer at the age of 13. His spirit is seen in all of his friends and in our fishing community that believes in BLISKA. B-Brave, B-Strong, B-Courageous. Alison Barabe W.O.F.F. Scholarship $500 Allison Barabe was one of the women who helped grow WOFF from the day it began. Alison cared deeply about her family, friends and her fishing community. She was artistic and creative in every aspect of her life, including her role in WOFF. Her laughter was infectious and her smile was even brighter! She was dedicated to helping her neighbors and community in times of need and she worked hard at making tough times just a little easier for the fishing families that WOFF serves. Alison died suddenly in July of 2016, leaving a big hole in our WOFF-sister hearts, in her family and in our fishing community. We continue our important work in Alison’s name. David Murdoch Scholarship $1,000 David Murdoch, III, was an experienced mariner, a skilled craftsman, an athlete, and a loving husband, father, and grandfather. His children, David and Kate, attended Chatham Public Schools (as his granddaughter does now) and, over the years, he coached many of their athletic teams. David was also a longtime volunteer at Chatham Elementary School, helping the younger students by reading with them. David died suddenly last June but lives on in his family’s heart and in the children whom he enjoyed reading with - and supporting - over many, many years.
