SAFETY POLICY: AN INTRODUCTION

City Beautiful Horticultural Services, Inc. (City Beautiful) is committed to providing safe working

conditions for all employees and complying with all current and applicable occupational health, safety,

and environmental laws. City Beautiful has developed the operations, procedures, and policies that we

believe are needed to provide such conditions. These include training employees to work safely on their

jobs at all times and to report unsafe working conditions promptly to their supervisors.

This manual details the components of our safety program that make up our safety policy. Each employee

receives a copy of this manual and must sign a form indicating that he or she has received and read the

manual and understands the contents. Should any employee have questions concerning any of the content

in this manual, he or she may ask a supervisor or the Office Manager.

For clarification, the terms “the company” and “we” may be used interchangeably in this handbook. The

reader should interpret “we” in this context to mean the owner, Rusty Woodall, and his designees (usually

management personnel) who have the authority to make decisions and take actions, as appropriate,

regarding company policy, procedures, or other company business.

OUR SAFETY GOALS

To support a clear and successful safety policy, we have identified the following goals, which are to

establish and maintain a culture of safety at City Beautiful;

maintain informed and enthusiastic leadership to model excellent safety management and

practices for the company;

cultivate safety-conscious employees who understand company safety policies and implement all

safety procedures at all times;

conduct regular safety training;

have and enforce a system of consequences for employees who violate policy or procedure;

plan and provide facilities that are safe and efficient;

reduce costs related to accidents, neglect, and unsafe practices; and

generally be proactive regarding safety on company and customer properties.

The remainder of this manual will address the specific programs and strategies we use to accomplish the

above goals. We present these in the following categories: Compliance, Personal Safety, Training,

Vehicles and Equipment, Company Property, and Chemical Safety.

COMPLIANCE

City Beautiful will comply with all applicable legal requirements regarding safety and occupational

health. Because the rules and regulations are so extensive, each employee is encouraged to consult with

his or her supervisor about any questions, but the following areas provide additional details about

compliance issues.

Safety Director

To ensure that safety remains a priority with City Beautiful, we have appointed a safety director to

develop, review, and administer safety policies and procedures. The director cooperates with managers

and production staff to ensure compliance with all laws and regulations and with all programs and rules

within the company. The director also supports managers in their efforts to hold safety meetings, plan

trainings, and support safety in all respects.

Facility Compliance Reviews & Planning

City Beautiful will conduct periodic safety reviews to ensure that all company facilities meet legally

required standards. The Safety Director or other authorized employee will document any violations or

concerns and implement a plan to promptly address them. We will also make safety a primary

consideration in any planning for facility improvements.

Environmental Considerations

City Beautiful will follow the provisions of any applicable environmental regulations as they relate to

occupational health and safety. In keeping with a proactive approach to safety management, we will make

every effort to prevent development of conditions harmful to the environment. Should it come to our

attention that we are responsible for creating an environmental hazard, we will act immediately to remedy

it and will fully cooperate with federal, state, and local efforts in the process.

Additional efforts to comply with environmental safety may be found in the section on Chemical Safety

in this manual.

Safety Violations and Penalties

City Beautiful provides safety rules to which we expect each employee to adhere. Each rule has a specific

class of penalty attached to it. The following is a brief description of each class of offense, and the next

section on general safety rules will provide examples:

Class A Offense

These are serious violations related to our drug and alcohol policy and procedures. Please refer to that

section in the City Beautiful Horticultural Services, Inc. Staff Handbook.

Class B Offense

Violations that have a lesser degree of punishment and reflect consequences according to the following:

First Offense: Written Warning

Second Offense: Written Warning

Third Offense: Suspension or Dismissal

General Safety Rules

Our company’s ability to comply with safety laws and regulations and to keep all employees safe is

increased by employees’ adherence to the following rules:

Safety Glasses: We issue safety glasses to each production employee upon employment and require their

use when the probability exists of eye injury from flying materials. This includes during the use of all gas-

powered equipment. (Class B Offense)

Ear Protection: We provide ear plugs, but the employee is responsible for obtaining them from

inventory. Employees must wear ear protection when operating loud equipment, such as a backpack

blower, for long periods of time. (Class B Offense)

Note: The employee is responsible to care for and keep track of safety items that the company issues,

such as safety glasses and ear plugs. Should the employee misplace or lose the items initially provided, he

or she is responsible to request replacement and will be charged for any such replacements. Extra items

are available at all times in the tool checkout area in the warehouse.

Clothing: Employees are to wear only company-issued uniforms and T-Shirts. Wearing loose, torn, or

frayed clothing is prohibited.

Alcoholic Beverages and Illegal Drugs: Alcoholic beverages or illegal drugs are prohibited during

working hours and after working hours at the job site or on company premises. (Class A Offense)

Portable Music Players: Portable music players, such as a Walkman radio or i-Pod are not allowed

during the work day. (Class B Offense)

Manufacturer-Equipped Safety Items: Employees are not to remove from the company premises or

tamper with any manufacturer-equipped safety items.

Equipment Malfunctions: The employee is responsible to report any equipment malfunctions and must

never operate equipment that does not appear safe. Employees must report any such equipment problems

to a supervisor. (Class B Offense)

Special Conditions for Turning Off Vehicles or Equipment: Employees are to turn off vehicles or

equipment under the following conditions:

when emptying grass clippings,

if the vehicle or equipment poses a threat to the general public,

when loading or unloading equipment,

when adjusting height of cut, and

during any other unsafe condition.

(Class B Offense)

Running Equipment: Employees must never leave running equipment unattended (Class B Offense)

Behavior: We prohibit horseplay, scuffling, or other acts that may adversely affect the safety or well-

being of other employees or others. (Class B Offense)

Lifting: Employees may not load or unload equipment without assistance, unless the vehicle has an

electric tailgate. If lifting other heavy materials, employees must use proper lifting practices, including

never lifting equipment or material over 50 pounds without assistance from a fellow employee.

Job Site Inspections: Before beginning work at a job site, employees must inspect the site for hazards

such as broken glass, trash, or other objects, including ones that may be hidden from view, to prevent

injury to employees or bystanders.

PERSONAL SAFETY

This section provides details on personal protective equipment and what to do when an injury happens.

Personal Protective Equipment (PPE)

City Beautiful requires employees to wear all appropriate PPE at the proper times and in the proper

environments. Failure to wear required protective equipment may result in a verbal or written reprimand.

Repeated offenses may result in suspension or dismissal. The company is legally bound to make sure that

each employee complies with this policy. All supervisors will strictly enforce it.

The following table provides both specific equipment that represents the various tasks that employees

must perform and the related PPE that employees must wear when completing these tasks. The last two

table entries include required inspections.

First Aid Kits

First aid kits are available on each vehicle and in the office. The assigned driver/crew foreman is

responsible to ensure that the kits are well-maintained. Each kit should contain Band-Aids, gauze pads,

tape, an ice pack, latex gloves, burn cream, antibiotic ointment, and eyewash.

Safety Recommendations

The company is concerned about your safety. While the following precautions are not required, we highly

recommend that you follow them:

Wearing steel-toed boots with slip resistant soles for all phases of work

Wearing caps or hats. Heat influences the productivity of each crew member. By keeping the top

of the head from direct exposure to the sun, the probability of heat stress or heat stroke is reduced.

What to Do When an Injury Happens

In the event of an on-the-job injury, employees are required to abide by the following procedures. Failure

to follow any of these procedures could result in the loss of any benefits for which you might be eligible.

1. In a life-threatening emergency, call 911 as soon as possible. If you do not have access to a

phone, radio the main office, and they will call 911 and notify your supervisor.

2. In a non-life-threatening emergency, contact your supervisor. Your supervisor will determine

if the injury is severe enough to require professional medical attention. You will be direc ted

to the nearest medical facility if the injury requires immediate attention. If the injury is less

severe, you will be directed to an appropriate facility for care. If you are hurt on the weekend

or if the work injury develops while you are off work, you must notify your supervisor

immediately the next business day.

3. After our company doctor notifies your supervisor or the company safety manager regarding

your expected time of recovery or the seriousness of the injury, the company will determine

if you must either report daily to the office for light duty or rest at home. City Beautiful

wants to have each employee back to work as soon as possible after any accident.

4. An employee injury report needs to be filled out as soon after the injury as possible .

Witnesses to the injury must also fill out a statement regarding the injury. Employees may

obtain these forms from their supervisor.

5. Depending on the severity of the accident and the circumstances causing each accident, the

injured employee may be required to submit to a post-accident drug and alcohol test.

6. It is imperative that employees follow all instructions set by the company doctor and return to

work only after providing the company with a doctor's release. We will maintain close

communication with the employee’s doctor at all times.

SAFETY TRAINING PROGRAM

City Beautiful is committed to providing practical and high-quality safety programs. We believe that by

increasing awareness, we will decrease hazards, risks, and accidents. Our safety training program includes

the following:

New-Employee Orientation: In addition to other company policies and procedures to which new

employees are introduced, the employee receives a copy of the City Beautiful Horticultural

Services, Inc. Safety Manual, and is required to read it, ask any questions, and sign off that he or

she has read and understands the information. The employee also receives, in person, an overview

of City Beautiful’s safety programs and requirements.

Safety Meetings: The company holds safety meetings on a regular basis. Each department

manager conducts the meeting that includes one safety topic, a review of any accidents or near-

accidents, methods to prevent accidents, and suggestions for future safety topics. This program

may include occasional guest speakers.

Employee Input: City Beautiful recognizes that employees are a valuable resource regarding

safety on job sites, on the road, or on company premises. Consequently, company supervisors and

managers will seek input from employees for safety topics that might be helpful in trainings or

safety meeting discussions.

Employee Recognition: Our safety program includes recognizing employees who work in a safe

manner.

Chemical Safety: As applicable, employees receive training on hazardous materials. More on

this training may be found in this manual’s section on Chemical Safety.

VEHICLES AND EQUIPMENT

Only authorized employees may operate company vehicles, including over-the-road vehicles and other

pieces of power equipment. Unauthorized operation of power equipment may lead to dismissal of both the

employee and his or her supervisor.

Drivers of all vehicles must observe federal, state, and local laws in the operation of vehicles. If a vehicle

is subject to Department of Transportation regulations, the driver must be certified or qualified under any

applicable DOT regulations. City Beautiful complies with any testing requirements mandated by the

Department of Transportation.

Employees must immediately report to a supervisor all accidents involving company equipment and/or

vehicles. Likewise, employees must promptly report any moving traffic violations that occur, whether on

or off duty. Failure to do so may lead to termination of employment.

We provide the following additional information regarding the care and use of vehicles and equipment

and rules and procedures regarding them.

Care of Vehicles, Equipment, and Furnishings

The assigned driver/crew foreman is responsible to see that vehicles and equipment assigned to the

crew receive regular, standard maintenance. The assigned driver/crew foreman is also responsible for

small tools issued to the crew. The shop manager will randomly inspect all vehicles and farm equipment

monthly to determine if they are being properly maintained and kept clean.

Employee Use of Company Vehicles

Company vehicles are to be used for company business only. Personal use of company vehicles is

prohibited unless permission is granted by a company officer in writing.

Vehicle Safety Rules

City Beautiful believes that transportation of its employees is a critical issue that demands effective

management, sound operating procedures, and trained people. Disruptions in this system can lower

company efficiency and cause accidents. Therefore, any manager who observes an employee in violation

of one of the vehicle safety rules is required to fill out a safety violation report that will be signed by both

the employee in question and the manager. Copies of the report will be put in the employee's personnel

file and the safety violation file. All employees must adhere to the following policies:

1. All employees who are selected to drive company vehicles must provide a valid Florida

driver’s license. Never drive a vehicle without a valid driver's license. All drivers must

possess any applicable certifications and licenses.

2. Seat belts must be used at all times.

3. All employees are required to ride in the cab and never in the truck bed.

4. Crew supervisors or designated crew members must check daily maintenance items before

leaving company premises and report to management immediately any defects or problems.

Employees must never drive a vehicle that appears unsafe. Daily maintenance items include

but are not limited to

headlights

brake lights

back-up lights

turn signals

tire wear

brakes and horn

emergency brake

mirror adjustment

5. Drivers must always use a spotter, if available, when backing company vehicles.

6. When windshield wipers are on, headlights must be on.

7. Employees must report malfunctions of the vehicle to the company maintenance department

by submitting a Pre-Trip Vehicle Inspection Report.

What to Do When You Have an Accident

While we make every effort to avoid accidents, should one occur, any employee involved will be much

better equipped to handle the situation if he or she knows ahead of time what to do. The following steps

will help in that endeavor:

1. Stop at once and investigate.

2. Protect the scene. Use warning devices. Get help from bystanders. Turn off all engines. Do not

smoke. Guard against fire.

3. Assist injured persons. Don’t move them unless absolutely necessary. Summon an ambulance if

needed.

4. Get help. Use your company, personal, or nearby phone to notify the police and supervisor. Give

the location and nature of the accident. Notify the company.

5. Identify yourself and the company. Show your license and registration on request.

6. Obtain facts about other injured persons, other vehicles involved, and any damaged property.

7. Be courteous. Make no statement about the accident except to police or our insurance company

representative.

8. Fill out and check off all applicable information on a vehicle accident report form located inside

the glove box. This accident report form shall be kept in every vehicle at all times.

9. Take pictures of the accident scene with the disposable camera in your glove box. Turn the

camera in to your supervisor or the office.

COMPANY PROPERTY

In addition to other policies and procedures in this manual that reference care of vehicles, equipment, and

other company property, it bears repeating that the employee is responsible for the safe operation of all

company tools, equipment, machinery, vehicles, or other company property in his or her charge.

Additionally, each employee will respect the company grounds and facilities, exercising care, properly

securing tools, supplies, or other items, and performing clean-up as appropriate.

The company will provide for proper care and maintenance of company property, but each employee

should report any malfunction to his or her immediate supervisor. The supervisor shall investigate and

take the necessary steps to correct the malfunction as soon as possible. The shop manager has the

authority to prohibit use of any vehicle or piece of equipment if it poses a danger to employees.

CHEMICAL SAFETY

Hazard Communication Program

City Beautiful is firmly committed to providing all of its employees with a safe and healthy work

environment. Our company policy is to provide our employees with essential information and training

regarding hazardous materials on the work site or company premises. Our hazard communication

program includes training on container labeling, material safety data sheets, hazardous materials that we

typically use, how to contain spills, first aid, and other important topics. The rest of this section provides

details of our program.

List of Hazardous Materials

The company maintains a list of all hazardous materials that we use on the work site. (A copy of this list

appears in this section of the handbook.) Employees obtain the details of these materials by reviewing

container labels and material safety data sheets (MSDS). Copies of MSDS for all hazardous materials

to which employees may be exposed are readily accessible to employees in the work area during each

work shift.

Employee Training

Employees are to attend a training session on hazardous materials when applicable at the time of

employment, and then as new hazardous materials are introduced. The training session will cover the

following:

an overview of the hazard communication requirements;

a review of the chemicals present in the workplace operations;

the location and availability of our written hazard communication program, a list of hazardous

materials, and MSDS;

methods and observation techniques that may be used to detect the presence or release of

hazardous materials in the work area;

physical and health hazards of the materials in the work areas;

how to reduce or prevent exposure to hazardous workplace materials by using good work

practices, personal protective equipment, etc.;

emergency procedures to follow if employees are exposed to hazardous materials; and

an explanation of our hazard communication program, including how to read labels and MSDS to

obtain appropriate hazard information.

When a new type of product is introduced into a work area or the chemical composition of a product

changes, a supervisor will review the above items as they are related to the new materials.

Non-Routine Tasks

Periodically, employees are required to perform non-routine tasks. Prior to starting work on such projects,

a supervisor will inform such employees about hazards to which they may be exposed and appropriate

protective and safety measures they should take.

Chemical Container Disposal Policy

Hazardous material is any substance that requires a material safety data sheet and has a label, which

contains such words as “danger,” “caution,” and “warning.” When disposing of any container that held

hazardous material, the employee must triple-rinse each container and recycle the rinse water. If the

container is not to be reused (e.g., post-emergent herbicide) then a hole is to be put in the bottom so

the container cannot be retrieved and used by an unsuspecting person. An employee with any

questions about whether a container or material needs special attention is to ask a supervisor.

Exposure to Chemicals

Chemicals may enter the body in three ways:

through the mouth (orally),

through the skin and eyes (dermatologically), or

through the lungs (by inhalation).

Employees who become exposed to chemicals should do the following:

In case of exposure to the skin or eyes, wash immediately with large quantities of water. The

company provides an eye wash station within the warehouse. If you are at a job site, use slowly

running water out of a hose.

In case of exposure through inhalation, get the victim to fresh air immediately. Apply artificial

respiration if necessary.

In all cases of suspected chemical exposure, refer to the product label and the material safety

data sheets for decontamination procedures along with the above first aid procedures.

In all cases of exposure to chemicals, notify your supervisor and get professional medical help

as soon as possible.

First Aid Kits

As stated in the Personal Safety section of this manual, first aid kits are available on each vehicle and in

the office. The assigned driver/crew foreman is responsible to ensure that the kits are well-maintained.

Each kit should contain Band-Aids, gauze pads, tape, an ice pack, latex gloves, burn cream, antibiotic

ointment, and eyewash.

Emergency Spill Response Instructions

1. Make sure to wear all appropriate personal protective items, such as gloves.

2. Stop the leak if possible. Close valves, tape leaky hoses, etc.

3. Contain any spilled material if possible. Use hazard pillows to stop flow of material to sewers,

storm drains, etc.

4. Call your supervisor and office immediately to report the accident.

Additional Chemical Safety Rules

A non-licensed employee may not handle chemicals without the direct supervision of a certified

applicator or technician and will be in violation of company policy if found doing so. When mixing and

handling pesticides or other materials, always consult the product label for required protective

equipment.
