Lower Urinary Tract Symptoms April 2013 Cystitis Cystitis is an inflammation of the bladder, and is typified by a burning sensation when passing water and a frequent need to urinate. Both of these symptoms are due to the presence of bacteria in the urine, which makes it more acidic than normal. Sometimes the urine becomes dark or cloudy and may smell unpleasant. A raised temperature, vaginal discharge or pain in the lower back may also be reported. Potassium and sodium salts are the mainstay OTC treatments. These products make the urine more alkaline and render conditions less favourable for bacteria. Cranberry juice appears to act by stopping bacteria sticking to the walls of the bladder. There is evidence supporting its use in cystitis prevention, but less for cystitis treatment. 1 A man or child reporting symptoms of cystitis. 1 Cystitis that persists despite trying an OTC remedy. 1 Symptoms accompanied by fever, vomiting, unusual vaginal discharge or blood in the urine. I Pregnant women. I Recurrent symptoms accompanied by an increased thirst, weight loss and thrush, as this may indicate undiagnosed or uncontrolled diabetes. 1 Patients with high blood pressure or heart conditions, as they should not take potassium or sodium-containing medicines without their doctor knowing. Assocîàtéii advice < Start taking the remedy as soon as possible after onset of symptoms, and complete the course of treatment even if symptoms appear to resolve. • increase fluid intake to help flush out the infection and encourage urination. Similarly, reduce caffeine and alcohol intake. • Refer women who suffer from frequent bouts of cystitis to their doctor for investigation. • Repeated trauma in the genital area can cause cystitis, and can be eased by using lubricants, avoiding irritants such as spermicides, and cleaning the ano-genital area before and after sex. • Avoid toiletries containing perfumes or colourings, as they can cause irritation. • Observe good toilet hygiene by wiping from front to back. • Avoid tight fitting clothes and underwear, as these can encourage bacterial growth. • Avoid acidic drinks and spicy foods as these can cause bladder irritation.
