D. THEORIES/MODEL OF HEALTH PROMOTION

1. Pender’s Health Promotion Theory (HPM)

v proposed by Nola J. Pender (1982; revised, 1996) v designed to be a “complementary counterpart to models of

health protection.”v defines health as a positive dynamic state not merely the

absence of diseasev Health promotion is directed at increasing a client’s level of

wellbeing.v describes the multi-dimensional nature of persons as they

interact within their environment to pursue health.v focuses on following three areas: Individual characteristics and experiences, Behavior-specific

cognitions and affect, and Behavioral outcomes.v notes that each person has unique personal characteristics and experiences that

affect subsequent actionsv Health-promoting behavior is the desired behavioral outcome and is the end point in

the HPM.v Health-promoting behaviors should result in improved health, enhanced functional ability

THE MAJOR CONCEPTS AND DEFINITIONS OF THE HEALTH PROMOTION MODEL

Individual Characteristics and Experiences

PRIOR RELATED BEHAVIOR – Frequency of the similar behavior in the past. Direct and indirect effects on the likelihood of engaging in health-promoting behaviors.

PERSONAL FACTORS – Personal factors categorized as biological, psychological and socio-cultural. These factors are predictive of a given behavior and shaped by the nature of the target behavior being considered.

Personal biological factors – Include variable such as age, gender, body mass index, pubertal status, aerobic capacity, strength, agility, or balance.

Personal psychological factors – Include variables such as self-esteem, self-motivation personal competence, perceived health status and definition of health.

Personal socio-cultural factors - Include variables such as race ethnicity, acculturation, education and socioeconomic status.

Behavior-Specific Cognition and Affect

PERCEIVED BENEFITS OF ACTION – Anticipated positive outcomes that will occur from health behavior.

PERCEIVED BARRIERS TO ACTION – Anticipated, imagined or real blocks and personal costs of understanding a given behavior.

PERCEIVED SELF-EFFICACY – Judgment of personal capability to organize and execute a health-promoting behavior. Perceived self-efficacy influences perceived barriers to action so higher efficacy result in lowered perceptions of barriers to the performance of the behavior. 

ACTIVITY-RELATED AFFECT– Subjective positive or negative feeling that occur before, during and following behavior based on the stimulus properties of the behavior itself. Activity-related affect influences perceived self-efficacy, which means the more positive the subjective feeling, the greater the feeling of efficacy. In turn, increased feelings of efficacy can generate further positive affect.

INTERPERSONAL INFLUENCES – Cognition concerning behaviors, beliefs, or attitudes of the others. Interpersonal influences include: norms (expectations of significant others), social support (instrumental and emotional encouragement) and modeling (vicarious learning through observing others engaged in a particular behavior). Primary sources of interpersonal influences are families, peers, and healthcare providers.

 SITUATIONAL INFLUENCES – Personal perceptions and cognitions of any given situation or context that can facilitate or impede behavior. Include perceptions of options available, demand characteristics and aesthetic features of the environment in which given health promoting is proposed to take place. Situational influences may have direct or indirect influences on health behavior.

Behavioral Outcome  

COMMITMENT TO PLAN OF ACTION – The concept of intention and identification of a planned strategy leads to implementation of health behavior. 

IMMEDIATE COMPETING DEMANDS AND PREFERENCES – Competing demands are those alternative behavior over which individuals have low control because there are environmental contingencies such as work or family care responsibilities. Competing preferences are alternative behavior over which individuals exert relatively high control, such as choice of ice cream or apple for a snack

HEALTH-PROMOTING BEHAVIOR – Endpoint or action outcome directed toward attaining positive health outcome such as optimal well-being, personal fulfillment, and productive living.

2. Bandura’s Self-Efficacy Theoryv A theory of situation-specific self-confidence that proposes that self-efficacy is fundamental

to initiating certain behavior necessary for competent performance.

v According to the theory, self-efficacy is enhanced by four factors: successful performances, vicarious experiences, verbal persuasion, and emotional arousal.

v Successful performance, which can be achieved by participatory modeling, is regarded as the most important factor.

People with weak belief in their self efficacy

3. Health Belief Model (HBM)vdeveloped by Hochbaum, Rosenstock and Kegels

Core Assumptions and Statements

The HBM is based on the understanding that a person will take a health-related action (e.g.., use condoms) if that person:

1. feels that a negative health condition (e.g., HIV) can be avoided,2. has a positive expectation that by taking a recommended action, he/she will

avoid a negative health condition (e.g., using condoms will be effective at preventing HIV), and

3. believes that he/she can successfully take a recommended health action (e.g., he/she can use condoms comfortably and with confidence).

The HBM was spelled out in terms of four constructs representing the perceived threat and net benefits: perceived susceptibility, perceived severity, perceived benefits, and perceived barriers. These concepts were proposed as accounting for people's "readiness to act." An added concept, cues to action, would activate that readiness and stimulate overt behavior. A recent addition to the HBM is the concept of self-efficacy, or one's confidence in the ability to successfully perform an action. This concept was added by Rosenstock and others in 1988 to help the HBM better fit the challenges of changing habitual unhealthy behaviors, such as being sedentary, smoking, or overeating.

Public Health Nursing

v a term coined by Lillian Wald to denote a service available to all people.v refers to the practice of nursing in the national and local government health departments.

It is community health nursing practiced in the public sector.v WHO Expert Committee on Nursing: “a special field of nursing that combines the skills

of nursing, public health and some phases of social assistance and functions as part of the total public health program for the promotion of health, improvement of the conditions in the social and physical environment, rehab of illness and disability.”

National League of Philippine Government Nurses came up with the Standards of Public Health Nursing in the Philippines 2005

A. RESTRUCTURED HEALTH CARE DELIVERY SYSTEM

Philippine Health Care Delivery Systemv composed of two sectors:

1. public sector – largely financed through a tax-based budgeting system at both national and local levels and where health care is generally given free at the point of service (although socialized user fees have been introduced in recent years for certain types of services)

2. private sector – largely market-oriented and where health care is paid through user fees at the point of service

Rationale for Health Sector ReformAlthough there has been a significant improvement in the health status of Filipinos for the last 50 years, the following conditions are still seen among the population:

slowing down in the reduction in the Infant Mortality Rate (IMR) and the Maternal Mortality Rate (MMR)

persistence of large variations in health status across population groups and geographic areas

high burden from infectious diseases rising burden from chronic and degenerative diseases unattended emerging health risks from environmental and work-

related factors burden of disease is heaviest on the poor

The reasons why the above conditions are still seen among the population can be explained by the following factors:

1. inappropriate health delivery system2. inadequate regulatory mechanisms for health services3. poor health care financing and inefficient sourcing or generation of

funds

Global and National Health Imperatives

Public health systems are operating within a context of ongoing changes, which exert a number of pressures on the public health system. These changes include:

1. Shifts in demographic and epidemiological trends in diseases, including the emergence and re-emergence of new diseases and in the prevalence of risk and protective factors;

2. New technologies for health care, communication and information;3. Existing and emerging environmental hazards some associated with

globalization;4. Health reforms.

In response to above trends, the global community, represented by the United Nations General Assembly, decided to adopt a common vision of poverty reduction and sustainable development in September 2000.

Except for goals 2 and 3, all the MDGs are health or health-related.

In order to achieve these goals, the participation of all members of the society from both developing and developed countries is required.

Levels of Health Care and Referral System

1. PRIMARY LEVEL OF CAREv devolved to the cities and the municipalitiesv health care provided by center physicians, public health nurses, rural health

midwives, barangay health workers, traditional healers, and others at the barangay health stations and rural health units.

2. SECONDARY LEVEL OF CAREv given by physicians with basic health trainingv usually given in health facilities either privately owned or government-

operated such as infirmaries, municipal and district hospitals, and out-patient departments of provincial hospitals.

3. TERTIARY LEVEL OF CAREv rendered by specialists in health facilities including medical centers as well as

regional and provincial hospitals, and specialized hospitals such as the Philippine Heart Center.

C. DEVOLUTION OF HEALTH SERVICES (RA 7160)

Republic Act 7160v took place in 1991v all structures, personnel and budgetary allocations from the provincial health

level down to the barangays were devolved to the local government units to facilitate health service delivery.
