CASES 1. Patient MS is known hypertensive. He was admitted because of severe abdominal pain and series of vomiting. The patient has the following maintenance medications: Felodipine 50mg PO OD Furosemide 20mg PO OD Aspirin 100mg PO OD Clopidrogel (Plavix)75mg PO OD 2. Patient RS weighs 156 lbs, s/p appendectomy and to be discharged tomorrow. Her attending physician ordered the following home medications: Continue Ceftriaxone (Rocephin) 500mg PO Q12 Mefenamic acid (Ponstan) 500mg PO as needed for pain 3. Patient MY 16y/o, female, diagnosed to have a nosocomial pneumonia caused by Pseudomonas aeuruginosa. She has the following medications to manage her disease. Paracetamol (Biogesic) 250mg/5ml (15ml) PO q6 RTC for fever Carbocisteine (Solmux) 500mg/5ml (10ml) PO TID for cough 4. Patient MM is 45 y/o and weighs 180 lbs. He has history of allergic reaction with Ibuprofen, Mefenamic Acid and particularly Aspirin which is characterized by swelling of the eyelids. He had been using Tramadol 50 mg for pain. He was prescribed with the following after operation: Parecoxib (Dynastat) 40mg IV OD x 3days Claforan 1g IV Q8 x 7 days Recommended Dosing for Claforan: Clcr 10-50mL/minute: Administer every 8-12hours Clcr <10ml/minute: Administer every 24 hours
