YOUR DOCTOR IS IN WITH PROVIDER FINDERProvider Finder is a fast, easy-to-use online tool that helps you see which doctors and hospitals are in network. Just go to www.bcbstx.com/trscarestandard and click on the Doctors and Hospitals tab. Next, click on Provider Finder, where you’ll be able to:

• search by specialty, ZIP code, language, gender and more

• see certifications and recognitions• compare quality awards• read reviews

If you can’t find the provider you are looking for using Provider Finder, search for the provider’s name instead of the hospital or clinic. You can also call a Personal Health Guide at 1-866-355-5999 for assistance.

TRANSITIONING YOUR CAREThe provider you’re currently seeing is most likely in-network.

You or a covered dependent who’s receiving care from a provider that isn’t in the BCBSTX network can apply to continue to see the same provider temporarily with transition of care. If approved, BCBSTX will pay benefits, up to the allowable amounts, at in-network rates for a certain time period. Certain eligibility guidelines need to be met.

IMPROVE YOUR HEALTH & WELLBEINGKeep up a fitness routine and consider joining the Fitness Program if gyms are open in your area and operating safely.1 You’ll have unlimited access to a nationwide network of fitness facilities with no long-term contract.

Talk to a nurse with the 24/7 Nurseline when an unexpected problem such as dizziness, severe headaches, high fever or other health issues occur late in the day or in the middle of the night.2 Call the 24/7 Nurseline at 1-833-968-1770.

Take care of your mental health with your health benefits. You can get care for depression, stress, alcohol or drug misuse and other conditions.

Live well with Well onTarget® and reach your wellness goals by managing chronic health conditions, like diabetes and hypertension. The engaging Well onTarget portal, available at www.wellontarget.com, connects you with several innovative programs and tools.3 You can also download the AlwaysOn Wellness mobile app to access Well onTarget on the go.

Check your health status by taking a Health Assessment and find out how your health measures up by answering a few questions about your health and lifestyle.

Work with a coach to get one-on-one support by phone or secure online messaging. Your coach will help you set and reach goals like losing weight, improving your blood pressure and quitting smoking.

Get extra support for complex health issues from nurses or other health care professionals who act as health advisors. They can help during certain health situations, such as a serious illness, injury, surgery or hospitalization. You can also connect with a cancer specialist.
