natural reaction to experience shock. In many ways, the shock and denial felt in the wake of this emotional trauma is very similar to the reaction to physical trauma. Once again, Noyes Health employees participated in a local holiday charity drive! This year employees reached out to both Pregnancy Resource Center in Bath and Focus on the Children in Caledonia.
The Noyes Health employees donated gladly, with over 1500 items, including diapers, clothing, toys, personal items and cash donations, which were collected at the hospital by Gail Wing and the Human Resources Department. The two charities had to make numerous pickups throughout the season and were amazed by the outpouring of support for the less fortunate in our region.

Diabetic Foot Ulcers Deservedly Feared DEAR DR. ROACH: My father is an 84-year-old diabetic. He has had a wound on his foot (on the inner side of his right foot, near his big toe) for about five months. He has been treated by a podiatrist, who has debrided the wound every week. He prescribed antibiotics when the wound has looked infected. A visiting nurse and my mother have changed and dressed the wound regularly during these months.
This week, the podiatrist said he could see the bone in my dad’s foot, and recommended hyperbaric treatment. Could you tell me what your opinion is of this treatment in my father’s case? Do you know if it has a high percentage of success? -- C.O.
ANSWER: Foot ulcers in diabetes are common and feared. Diabetes damages blood vessels, both large and small. Also, longstanding diabetes decreases the effectiveness of the inflammatory response and immune system. Finally, poor nerve function in people with diabetes combines with these other factors to predispose one to developing breakdown of the skin and development of ulcers. These ulcers can get large and deep, and when the bone is visible (and sometimes even when it’s not), the bone is infected. In this case, there is a high risk of the need for amputation.
By far, the best way to deal with diabetic foot ulcers is to not get them in the first place. Good control of diabetes, proper footwear, regular checks by a professional and daily self-checks of the feet for people at high risk can reduce the likelihood of developing ulcers. Early and aggressive treatment of precursor lesions, even mild redness of the skin, ingrown toenails or fungal infection of the feet, should prompt urgent evaluation by a podiatrist.
Once the ulcer has progressed to the point where your dad’s is now, aggressive measures are called for. In addition to care by a podiatrist or orthopedic surgeon, evaluation by a vascular surgeon and a wound-management specialist may be limb- and even life- saving.
While hyperbaric oxygen has indeed been shown to be beneficial in several studies (reducing amputation rate from 33 percent to 9 percent in one study), only an expert with detailed knowledge of your dad’s case can decide whether this is appropriate treatment. If you trust your podiatrist, I think it may well be a useful treatment. However, I would be sure that he, or a vascular surgeon, has looked at the quality of blood flow to the foot.

ANSWER: I’ve written about the vaccine lots of times. I am a strong believer in it and think anyone over 60 should get it unless they have a medical reason not to. The older you are, the more important it is to get the shot, since the complication of pain (postherpetic neuralgia) is so devastating and tends to lasts longer the older you are.

Renovations for senior safety As adults approach their golden years, the homes they once
thought of as sanctuaries can become unsafe. Families wrestle with the decision to keep parents and grandparents in their homes or move them into assisted living facilities or other senior residences.
Modifying seniors’ homes to make them safer is another option. The National Institute on Aging says that families may be able to have a senior stay at home by helping older relatives remain independent but safe. The following are a few ways to do just that.
• Remove fall hazards. The NIA reports that six out of 10 falls happen at home, where people spend the most time and fail to think about their safety. Seniors who want to be independent may overestimate their physical abilities. Because falls can be so dangerous, leading to cuts, abrasions, broken bones, and more, seniors and their families must take steps to prevent falls in homes. Improve lighting, especially at night when sight may become diminished. Install hand rails and grab bars where possible. Pick up clutter and remove tripping hazards, such as slippery rugs or electric cords. Install an electric stair climbing seat to make traversing stairs less risky.
• Improve visibility and ability to communicate. Vision loss may accompany aging, but technology can help mitigate such losses. Use big-button phones, remote controls and even keyboards so seniors can keep in touch. Voice-activated thermostats or smart home technology also can make it easier for seniors to voice their needs.
• Modify fixtures and other features. Dexterity may wane with age, and arthritis can make grasping or turning doorknobs and faucets more challenging. Take inventory of areas of the home that present the biggest obstacles to seniors.
Replace knobs in the shower or on faucets with lever handles, which are easier to maneuver. Install new cabinets and doors that freely glide open and self-close. Replace toggle light switches with easier paddle-type switches that can be pushed with a hand or even arm. Motion-sensor lights also can be handy. Push-button oven controls may make cooking easier.
• Prepare for medical emergencies. Invest in medical alert systems, such as necklaces or bracelets, that can be used to contact police or emergency medical personnel directly. Make phones available in commonly used rooms in the home, such as bedrooms, the living room, bathrooms, and the kitchen.
• Install ramps and nonslip flooring. Ramps can make it easier to reach the front door or cross over elevated doorway thresholds. Nonslip flooring also can prevent falls around the house, offering more traction for feet, walkers or canes.
• Repair cracks in walkways and driveways. Safety should also extend to the outdoors. Be sure to repair cracks or uneven pavement. Replace loose patio blocks or bricks with a more stable design or with concrete or asphalt. While outdoors, trim back bushes and make sure there are no tripping hazards outside as well.
A few modifications can make it possible for seniors to live comfortably in their homes for many years.
The Most Treatable Serious Senior Health Issue: Loneliness
Here are some tactics to help seniors combat loneliness: • Get moving. Physical exercise increases the body’s level of “feel good” hormones, and can contribute to improved sleep, as well as fewer aches and pains. Seniors should get 30 minutes of moderately intense exercise per day, and if you exercise with companions, you’re also reaping the social benefits of the activity. • Sit down with a friend or family member and watch a funny movie or TV show together. Laughter’s positive effects on the mind and body are well documented. Laughing together improves mood and can make people feel more connected. • Make new connections. Sit with someone at lunch whom you don’t know well. Ask a neighbor to go for a walk with you. Join a club or social organization. • Help yourself feel better by helping others. Volunteering has many positive effects on mental health. When you volunteer your time to help others, you’ll feel good about your actions and also expand your opportunities to meet new people. • Learn something new - it’s not only good for cognitive health, it gives you the chance to make new connections with other students. Take a craft class, go to a personal finance seminar or take a cooking class.
