State Kindergarten Entrance Law A student must be five (5) years of age on or before Au-gust 31, 2014, to enroll in school. Copy of birth certifi-cate and immunizations as required by the state are nec-essary for enrollment. You will find a copy of the USD #312 policy in regard to this matter in your child's student handbook.
Physical Forms Before any student in Haven grades 7-12 (other centers 6-8) practices or participates in any extracurricular athlet-ics (including cheerleading), the student must have a physical examination form on file with the school. This examination must have been given after April 30, 2014, in order to be valid for this school year. The appropriate physical forms are available at the school offices. This requirement is a part of our compliance with the Kansas State High School Activities Association and the Kansas State Department of Education.
Student Directory Notice to parents or guardians of students enrolled or to be enrolled in the schools of Haven Unified School Dis-trict #312 during the 2014-2015 school year and all other persons concerned: You are hereby notified that the Board of Education of USD #312 designates the following student infor-mation categories as "Directory Information" a.) the student's name: b.) the student's participation in offi-cially recognized activities and sports: c.) roster information for a sport or activity which may include the following: name, grade, height, weight, uniform number, and picture: d.) dates of attendance: e.) degrees and awards received: f.) the most recent previous school district attended by the student. This information is considered public information which may be released by the school district without your prior consent. Your have 10 days from the day of this notice to inform the school district that any or all of the directory information should not be release without your prior consent.
The custodian of said records will maintain a record of those persons, agencies or organizations who have access to said records. This record will indi-cate the legitimate educational or other interest that each such person, agency or organization had in seeking the student's records. The record will be available only to the parents of said student and the custodian of the records. When your child becomes eighteen years of age, all rights formerly accorded to you as parents of said student become the sole rights of the student, and you will not longer have the right of access of the records. The above notice is required to be published in newsprint or newsletter by the Family Educational Rights and Pri-vacy Act of 1974. Under this law, the parents of students enrolled in any educational institutions receiving federal funds are given certain rights concerning the educational records of their children. This notice is advising that the district will publish such things as honor roll, team roster etc., as the district has done in the past. If you do not wish your child's name, picture, etc. to be published you need to notify Superin-tendent Rick White in writing on or before September 13, 2014.
Yearly Asbestos Notification. August 1, 2015
This is to inform you that the Asbestos Hazard emergen-cy Response Act (AHERA) (40 Code of Federal Regula-tions 76.3) requires Unified School District #312 to in-spect its buildings for asbestos, and to develop a man-agement plan for those asbestos-containing materials that were located within its buildings. This initial inspec-tion was completed in March of 1988. The law also re-quires re-inspecting of these materials every three years. Each of our school buildings has been inspected for as-bestos-containing materials by EPA accredited inspec-tors. The results of these inspections/reinstallations are included in the management plans that are on file in each
school's administrative office and at the district's main office. Evidence suggests that the mere presence of asbestos in a building does not necessarily pose a hazard. Re-search indicates that unless asbestos is disturbed, allow-ing fibers to become entrained into the air, the potential health consequences appear to be negligible. Our goal is to control the disturbance of asbestos-contained mate-rials and the release of fibers into the air in our schools. During the upcoming year Unified School District #312 plans to do two 6 month periodic surveillances. Bus Rules 1. The driver is in charge of the students and the bus. Students must obey the driver promptly and cheerfully. 2. The driver will assign seats. (Students in less desirable seats may move to another seat for added comfort after the passenger load is lightened and permission is first obtained from the driver and only if the bus is STOPPED.) 3. Students must be on time. The bus will not wait for those who are tardy. 4. Students must never stand in roadway while wait-ing for the bus. Students must walk on far left side of road facing traffic when going to the bus stop. Students must wait in an orderly manner and never push a fellow student. Board the bus only after it has come to a complete stop. 5. Unnecessary conversation with the driver is prohibited. Do not talk loud or distract the driver's attention. Remember safety is in his/her hands. 6. CLASSROOM CONDUCT IS TO BE OBSERVED. 7. Students must stay seated while bus is in motion. 8. Aisle must be kept clear.
9. Keep bus clean and sanitary at all times. Any trash must be put into provided trash cans. 10. Students must not extend any part of body or place any objects out of window. Windows must not be lowered below halfway.
11. Any damage/vandalism must be reported immedi-ately to driver. (Student will be responsible for dam-ages.) 12. Use or transport of tobacco, alcohol or illegal drugs is prohibited. 13. Animals will not be transported on a bus. 14. Electronic devices like video games and music players may be used, only if it does not cause disrup-tion on the bus. 15. When leaving the bus, students must observe di-rections of driver. If crossing the road, cross in front of bus after making sure road is clear. 16. During stormy seasons, students are advised to listen to their home radio for storm warnings. Do not try to reach school when forewarned. 17. District may install and use video cameras to assist with disciplinary action and to monitor bus.
Consequences For Misbehavior 1st Time - Warning 2nd Time - Referral to principal, parent conference, probation, possible Friday night detention. 3rd Time - Referral to principal, suspension from bus for 3 days. 4th Time - Referral to principal, suspension from bus for 5 days. 5th Time - Referral to principal, suspension from bus for the remainder of semester.
Severe - Immediate suspension from the bus. Penalty determined by school policy and administration.
Inclement Weather Policy
1. School Closing: In the event of inclement weather or mechanical breakdown, school may be closed or starting time delayed. The same conditions may also necessitate early dismissal. These events will be an-nounced over radio stations KWBW 1450 AM, KSKU 106 FM, KHUT 102.9 FM, and KWCH television chan-nel 12, KAKE television channel 10 and KSNW televi-sion channel 3. Reports in the morning will be be-tween 6:00 and 7:30 a.m. You will also receive infor-mation from our Schoolreach system. 2. It is recommended that each family formulate a plan of action in the event school is dismissed early. Young children must know what they are to do when they arrive home earlier than usual if parents are not there to receive them. A copy of this plan should be filed with the child's teacher. 3. Bus drivers will not travel roads which appear to them dangerous or impassable. 4. In all cases the final decision about whether a child may safely attend school rests with the parent or guardians.
Bus Route Information Bus routes cannot be completely finalized until enroll-ment is completed but it is anticipated that the bus schedule will be similar to last year. Regular bus stops at baby-sitters will be permitted during the 2014-2015 school terms.

U.S.D.#312 Vision Statement U.S.D.#312 will provide a comprehensive curriculum, which emphasizes communication, problem solving and critical thinking skills, and knowledge that will be used beyond the student’s school years. The curriculum will be flexible and will focus on our students’ needs in the 21st century. Our instructional strategies will include techniques which actively involve students and which will result in improving student achievement. Special needs will be addressed which will assure an opportunity for success for all students. Students will be expected to be mentally and physically prepared to participate in a respectful manner. Staff will set a positive example and have high expectations for students and themselves. The U.S.D.#312 teaching staff will be responsible for ongoing professional growth by keeping current with curricular trends and the processes of teaching and learning. Interaction among students, parents, staff and community will be encouraged and will be based on mutual respect. Students will be tolerant of other students’ differences and needs. Teacher to teacher interaction will focus on a collegial sharing with each oth-er in an atmosphere of open communication, mutual respect and cooperation. We will have a positive school climate, which gives everyone a sense of belonging. The physical appearance of the buildings will be safe, inviting, clean and well maintained. Each day when students and staff leave their USD buildings, they will do so with a feeling of success and accomplishment.
