Previous Action - Jo O’Connell will talk to NadineEvans about the Transitions Sub Group and what theyshould be doing.

- Jo has spoken with Nadine.

Previous Action - Jo to meet with Emily, Anna, andAmanda to talk about the best way of making sure thatall the points in the Big Plan are being looked at by theSub Groups.

- Jannie said that a meeting with the Co-chairsof the Sub Groups had been arranged for 3rd December.It has had to be postponed till the New Year because Jois on Paternity Leave for the next 2 weeks.

Where We Live Sub Group Up-date - Amanda Fsaid that there will be a new Housing Options Booklet,and they are close to finishing the draft for it.Bournemouth People First will then do a quote for makingan Easy Read version.

- Anna Harris said there was a question last timeabout how the Integrated Team are told about anyproblems that the Citizen Checkers find. She said thereis now a formal link to make sure all information ispassed on.

- Project for Parents with a Learning Disability.Anthony said that Bournemouth People First have sent outnew Membership Forms which include a question askingif people have any children.

- Bournemouth People First to go online and signup to the National Charter for Inclusion, on behalf of theBoard.

Anna Harris said that Anna Brazcek has beeninvited to the meeting with the Co-chairs so it can betalked about then.

- Previous Action - Learning Disability PartnershipBoard Self Assessment - Liz Elwick to check the Protocoland speak to Bournemouth People First about the needfor a Website for the Learning Disability Partnership Board.

- Anna said that there are Minutes of theBoard’s meetings on the Bournemouth People First’sWebsite. This Website is only up-dated every 3 months.

- Previous Action - Agenda for Novembermeeting - postpone the Employment Sub Group up-dateand add in the Transitions up-date.

Steve is the Senior Practitioner for Transitions which coverschildren with all kinds of disability (not just learningdisabilities) as they move from Children's Services to AdultServices.

The Transitions Sub Group was set up following the BigPlan. It meets every 2 months and has people fromHealth, Education, Social Services, Housing etc.

The Government has written a Green Paper about youngdisabled people having just 1 plan, which includes anassessment, for Education, Health, and Care.

By 2014 all young people have to stay in education, ortraining with employment, until they are 18. The mainidea is for people to have support to get skills to helpthem find work.

Steve said 1 problem is that there are not enough workexperience places for people when they are still at school.

Steve said the biggest problem is transport. Studentsfrom 4 local areas go to Bournemouth and Poole College.Each area (authority) has a different policy about transportwhich causes confusion.

The Sub Group feel that the Council could save a lot ofmoney over future years if young people were giventraining in using public transport.

Steve said that information for young people needs to beeasily available in a variety of ways. The Source Directoryis useful because 1 leaflet would not cover the needs ofall the young people in Transition.

In Bournemouth there is a Transitions Operational Groupwhich works very well. This includes Social Workers, andConnexion workers who know the young people andtheir families.

In Poole they have a Transitions Strategy Group which hasmore power to get things done than in Bournemouth.

Young people and their families say they need oneperson they know and trust to guide them through thetransition process. Managers need to agree on how tomake this happen.

Linda Reider said that the Physiotherapists in theIntegrated Team are making links with those in Children’sServices.

It would help if there was 1 lead worker to talk to assome young people have a lot of physical needs as wellas a learning disability.

Steve said some of the difficulty in agreeing who will takethe lead is because the lead worker would then have todo things outside their normal work.

Eileen asked what the timescale is for the Green Paper tobe put into practice as there are many points to getagreement on.

Steve said it is a bit confusing. The Government hasasked some authorities (Pathway Authorities) to have ago at doing these plans, then let people know the bestway of doing them.

The Pathway Authorities need more time, so it seems thatthe laws will be made before their results will be finished.

There will also be a new Special Educational NeedsCode of Practice, up to the age of 25.This should be ready for consultation by September 2013.

Brian Langridge said that the Council have got a newIntegrated Transport Unit. They are making a new TravelPolicy which will be ready for consultation soon.It will be for all people in Bournemouth. This will changetransport arrangements to make sure people do practicetheir travel training.

Anthony asked about Bus Passes. Brian said if they go toschool, their bus pass can be used before 9.30 a.m.There will be information about getting to College in thenew Policy.

Anna Harris said that there will be items about howpeople get around, and travel training in item 8, theTransport Sub Group Up-date. It shows we should bedoing more to get the 2 groups to work together.

Amanda Frost said that Bournemouth People First havenow got a young person to join their ManagementCommittee. Amanda has given her the dates for theTransitions Sub Group meetings.

Jo Findlay and Jenny Phillips gave the up-date.

Jo said that there is no date for the Health ActionValidation meeting.

�More people in Dorset, Bournemouth, and Poolehave been offered a Health Check this year.Everyone with a learning disability, over the ageof 15, can now have one.

� The Community Dental Service in Dorchester hasasked People First Dorset to make some easy readinformation leaflets for them.

� East Dorset Community Dental Service had a openday in their new clinic in Boscombe.

� Bournemouth Hospital had a feedback day forpeople with a learning disability in November.

� Dorset County Hospital had an open day inNovember for people with a learning disability inthe Day Surgery Unit.

- there are now 2 people who areHealth Champions. Chris Mellor from People First Dorset,and John Cotton from Bournemouth People First.

They will be helping to train health staff, and showingpeople how to use their Yellow Books.

Their work will go on until March 2013.

The plan looks at what needs tohappen from the Health Self Assessment. It also hasactions from work already started, and for the workinggroups.

6 actions are ‘Red’ as they have not started.

29 actions are ‘Amber’ as they have started.

2 are green as they are finished, but still need checking.

The ‘Red’ actions are:-

Yellow Books - make sure people know how to use thebooks. Make sure the health action plan is reviewed athealth appointments as well as at person centredreviews.

Make sure health services have easy to read information,and that it is easy to get hold of.

Health and care staff need to know about the MentalCapacity Act - everyone should get the right support formaking decisions.

Make services better for people with more complexneeds - there should be clear pathways for people with alearning disability and dementia. People should use thesame services as everyone else, but their special needsshould be taken into account.

Keeping people safe in health and care services - goodinformation is needed about the number of people whohave behaviour that is ‘challenging’.

Make sure people know how to complain or commentabout services - accessible information should be easilyavailable to show people how they can speak up abouttheir care.

� Checking that health staff know how to change theirservice to support people with a learning disability.

� Checking that care and support staff know how tosupport people when they are using health services.

� Asking services to make sure that their staff trainingis developed and delivered by people with alearning disability.

�Make sure that the NHS involves people in planningfor future health services. NHS and Health CareProviders should invite people with a learningdisability and family carers to meetings.

� Health and Wellbeing Boards should know aboutthe health needs of people with a learning disability.

�Make sure that the Clinical Commissioning Groupknows about the health needs of people with alearning disability.

The Health Action Group was told about a big change tothe community dental service in Poole. It has moved.

Some people were very unhappy that they didn’t knowabout this before it happened, and were not asked whatthey thought.

Some people from Poole said the new service was too faraway and difficult to get to.

The Health Action Group has written a letter about thisand is waiting for a reply

The service has moved from Canford Heath to Boscombe,so is much further away for anyone living in the Poolearea, but closer for people in Bournemouth.

Ann Williams, who is in charge of the Dental Service, sentan e-mail to say that they could not meet the needs ofpeople with a learning disability in the old building. Theservice is much better now, and people may be able toget help with transport.

The Health Action Group feel that they should have beentold about this sooner so they could have had a saybefore the move.

A letter was sent to Val Graves on behalf of the HealthAction Group in October. It was signed by all 4 Co-chairsof the Health Action Group.

Jannie said at the Poole Partnership Board meeting theyfound they had not received a reply.

Val Graves said she replied last Friday, and said they dorealise that they did not get it right when the change wasmade. She agreed that people were not involved enoughin the change, and she apologised for the mistake.

Val also said the facilities are much better in Boscombe,and they are happy for people to come and look around.

Rosemary said there has been some confusion over thisreport. Originally it was agreed by Judith Geddes thatNadine would give a report about the Joint ManagementBoard.

It was then said that the only thing they could report wasabout the Integrated Team Report to the JointManagement Board.

It was agreed that Kellie would bring her Report to ourBoard every other meeting.

Kellie was unable to attend this meeting, so KateCuthbertson gave the Integrated Team Report.

The Community Team for People with Learning Disabilitiesnow has 28 members of staff. 1 Social Worker is leavingin January, and interviews are taking place beforeChristmas.

2 Nurses have left, and the team are waiting for it to beagreed that they can find 2 team assistants.

One Physiotherapist has left and they have not been ableto replace her.

After the last Partnership Board, Kellie said she would sendeveryone a copy of the Team Plan for 2012/2013. Theteam are also working on some areas of the Big Plan.

Please see Previous Action on page 3 of these Minutes.

Kellie would like to know if the Partnership Board wouldlike the Big Plan and the Team Plan to link together better.This could be included in the report from the team. Kelliemeets regularly with Jo O’Connell and could talk aboutthe Team Plan and the Big Plan then.

The Team still run some groups. Some of the team areworking with colleagues from Poole to run a men’s group.

Sean, an Occupational therapist, has been part of theEmployment Forum with Helen Holland. This work hasnow stopped because there has been no one able toproduce a web page in order to launch the project. Seanis going to meet Helen to talk about what to do next.

The team have been working really hard to make surethat people have been able to have a personal budget,and to carry out reviews. There are lots of things that theteam need to do at the moment, and sometimes this hasmeant that they haven’t been able to do things quicklyfor people and their carers.

In January 2013, the team will be starting work on‘re-designing’ packages of care and support for peoplewho have moved on from hospital living. This is becausesome people might be able to be supported in a betterway.

The Local Authority and Primary Care Trust spend a lot ofmoney on this support and the team need to make surethat this is the right support for the people.

The team have a social care budget which is given bythe Local Authority. Kellie has been working with Vikkifrom the finance team and Nadine Evans to find out howthey could use their money in a better way and makesome savings over the next year.

Team members are spending lots of time doing tasks thattheir team clerk used to do. This means that they cannotdo things as quickly as they would like.

Kellie and Nadine have had a meeting with Tim Bransonto look at how Provider Services could support peoplerather than the team pay a lot of money for support fromother providers.

The team are still getting lots of new referrals and sinceJune have had 43 safeguarding investigations at level 3.There have been more safeguarding referrals over the lastfew months and in September the team got 28.

They also get referrals for assessments and reviews, andneed to make sure that they know who needs a review.Kate, Kellie and Anita Hazell are working together tomake sure that this happens.

Sometimes they also need to visit some homes moreoften. This is so that they can make sure people withlearning disabilities are supported properly. This can takea lot of time and they need to be able to do this moreoften.

The team are moving to Hillcrest next week. There havebeen some more delays with the move because of aproblem with the computers.

Working together needs the Local Authority and HealthCare Trust to agree how things will happen. This is bysomething called a Section 75 agreement. Thisagreement has now been signed by Eileen Dunnachieand Val Graves.

Kate was asked about the Physiotherapist that left. Shesaid It was decided not to replace her, but to have 2team assistants instead

Anna Harris asked if the packages of care for peoplemoving on from hospital living were being re-assessedto save money.

Kate said it was not to save money. They had a numberof issues with the providers, so they want to be sure theright care is being provided.

Anna asked if advocates might have to be involved.

Kate said there might be a need for advocates. Theyare not starting the re-assessments until January. It islikely the team will contact Citizen Checkers when theystart.

Anna Harris asked what “provider services” are.

Kate said that Tim Branson is the Manager of providerservices, and he is in charge of the Council-run dayservices, Choices, April Court, Wallfield, Shared Lives,and Community Support. It is important to make sureeveryone is working together.

we have now finished all of the visits topeople who have moved on from hospital living. We aremaking our final report.

41 of the 51 people who moved on have filled outquestionnaires.

- we have visited 50 people in 12residential homes. We still have 18 more people to visit.7 people have not wanted to take part.

We have made some changes to the way we work. Weask staff more questions, and we ask the home forcomments about our report before we send it to theCouncil.

We are still finding that, when staff support people toanswer questions, we get more ‘yes’ answers. (84% with,77% without)

“Do you like what you do in the daytime? In May 88%said ‘yes’, in November 91% said ‘yes’.Anna said they think this has increased because thepeople we have seen recently have a more structuredroutine, and do more.

“Is there anything stopping you from doing the things youwant to do?” - In May 60% said ‘yes, in November 66%said ‘yes’.Anna said about a third of the people said it is lack ofmoney or support to do what they would like to do.

We asked questions to find out if people were happy. InMay 97% said ‘yes’, in November 96% said ‘yes’.

“Does anyone do anything to you that might hurt you orput you in danger?” - In May 28% said ‘yes’, inNovember 19% said ‘yes’.Anna said mostly the threats they feel are from otherresidents. Most of the people they have visited recentlyfeel safer.

“Does anything scare you?” - In May 52% said ‘yes’, inNovember 47% said ‘yes’.Anna said about two thirds of the people had everydayfears such as dogs or bees. The other third were scaredby the people they live with.

Eileen asked if the people who feel scared know what todo about it. Anna said they do check that people knowwho to talk to.

“Are you getting day opportunities that meet your needs?” -In May 66% said ‘yes’, in November 73% said ‘yes’.Anna said that people who were busier seemed happierand safer.

We will soon be visiting homes for a 2nd time and askingthe same questions. We can then see if things havechanged.

Overall people are happy, and the people we have visitedsince May seem to feel safer and are happier about thethings they do, and the support they get.

- we would like to work with more privatecare providers. We would also like to check other servicesas well.

We would like to work with the new care providers so thatCitizen Checks can carry on for people who have movedon from hospital living.

Brian asked if they had had any interest from privatehomes.

Anna said some of the homes were quite enthusiastic.1 said they would include it on their Website. Most of thehomes were happy with what has been done so far,even though the checks may have found some ‘bad’things.

Brian Langridge gave the up-date.

Bournemouth People First did a very goodjob with Mystery Shoppers on the buses a while ago. Itwas funded by the Council.

We would like it done again, but the bus companies havebeen told that they would have to pay for it.

Bournemouth People First contacted the buscompanies as if they had a complaint. This will berepeated.

- Bournemouth People First will ask membersfor their experiences of taxi journeys. If this shows a needfor further action, it will be looked at.

- There are a lot of peopleinterested in it.

Adult Education run a course about Independence whichincludes basic training about travel.

David Harrup, Manager of the new Passenger TransportUnit, will be looking to see if a local Transport Fund canbe used help more with travel training.

The Transport Group have £8,700 to help towards the costof travel training.

- the main bus stationin Bournemouth will be made better now we havefunding. The Travel Interchange Office will make sure theirstaff are trained, by Bournemouth People First, in the SafePlace scheme.

David Harrup has just beenemployed, and he is part of the Transport Sub Group.

it has been suggested that thegroup join with the Poole group. When the Poole grouphave their action plan the Bournemouth group will see iftheir aims are the same. They will then decide whether ornot to join together.

Anna Harris asked if there are any changes to theTransport that would affect Day Services.

Brian said that there should not be any changes, but if ourMembers are worried he would ask David Harrup toexplain to the next Sub Group meeting.

Eileen said that the very first draft is now out forconsultation. She apologised that there is not an easyready version.

It has been made by Bournemouth and Poole, and itcovers the changes that the Government are bringing in.

Public Health will now be under the Council instead ofthe NHS. Primary Care Trusts will be replaced by ClinicalCommissioning Groups led by GPs. LINks will bereplaced by Healthwatch.

Nationally there will be Commissioning Boards to lookafter more specialised services. We will come underWessex which includes Hampshire, Southampton,Portsmouth, Dorset, Bournemouth, and Poole.

There is a new local Health and Wellbeing Board forBournemouth and Poole which has to make a planshowing what its priorities (the things they think are mostimportant) are. This is a joint Board with the Councils andNHS.

They want to make health and wellbeing better, andmake sure everyone has equal access to health and care.

See draft plan for their priorities.

The key questions are:-

� “What do you think would be the best thing thatwould help improve the health and wellbeing oflocal people?”

� “How could you help the Health and WellbeingBoard to make this happen?”

� “What are the top 3 things you would like to see inthe Plan?”

This Plan, which is for everyone in Bournemouth andPoole, will last for 3 years. When the Big Plan for peoplewith a learning disability is finished, it will work with thisPlan.

Amanda Frost said that, when she went to anothermeeting at the Hospital she was told about lots of healthinformation being in easy read, but she had never seen it.

Eileen said that Amanda’s top priority would be for easyread information to be more available.

Anna Harris said that Bournemouth People First has beenasked to make an easy read version of the draft Plan, andalso an easy read feedback form. The ManagementCommittee will be looking at it this week.

Anna Harris asked how people can comment on the draft.Eileen said that Emma Leatherbarrow from Poole iscollecting all the replies, so any comments should go backto her or Hilary Evans.

Eileen said there was nothing specific aimed at peoplewith a learning disability, but lots of areas apply toeveryone. This includes early intervention, andSafeguarding adults and children from abuse.

Linda Reider said that improving services for children withdisabilities also covered those in Transition. Services theyget as children should not just disappear when theybecome adults.

Eileen said it was important that services for childrenshould be put in early, to make them more independent.It is no good putting in lots of services which make themmore dependent.

Jo said that she and Val are now involved in Transitions,which should help.

Les said that the health services are getting better.

Rosemary said that a Safeguarding report was due to begiven, but it is now going to be an update onWinterbourne View.

Anna said we may have had the meeting with the SubGroup Chairmen about the Big Plan. If so, there may bean update from that.

Amanda Frost said we will have had our Out Right event,and our Big Social Night.

Amanda Frost said that this would be Eileen’s last Boardmeeting as she is leaving Bournemouth Council.

Amanda thanked her on behalf of the Board for all shehad done.
