6-week Self Management Programme

Aims of the programme:

• Increase patient knowledge, confidence & locus of control

• Increase fitness, flexibility & muscle strength

• Gain an awareness of nutrition and treatment

• Encourage a more active lifestyle

• Have strategies in place to reduce falls

• Introduce MDT

• Link into patient-led osteoporosis support group

• Facilitate effective self-management

Who should attend?

• Patients with a diagnosis of Osteoporosis and Osteopaenia with or without fracture

When & Where?

• 2-locations• Salford Royal Hospital

– Tuesday Mornings• Walkden Gateway –

Wednesday afternoons

How do I refer a patient?• Send a completed physio

referral form and attach a Bone Boosters Inclusion/Exclusion Form to:

Rheumatology Team, Rehabilitation Services, Salford Royal Hospital, Stott Lane, Salford M6 8HD or phone Will Gregory on:

0161 206 5328

• Patients will be offered a taster session and an initial patient assessment before starting the programme

• Some patients may be offered 1:1 depending on their needs

Patient Follow-Up

• All patients who attend the programme are given the opportunity to attend evening hydro or other exercise groups such as Healthy Hips and Hearts. Some join local gyms, exercise classes

• All patients are recalled on an annual basis for annual review and update

Patient Feedback 100% of patients enjoyed the programme. Patient’s favourite aspect of the programme

included Hydro Meeting people with similar problems Exercise

Patients felt that the educational content was at the right level. the level of exercise was just right. the program had increased their confidence
