Prof. Nigel Ratcliffe Trust Chairman
This document outlines Staffordshire and Stoke on Trent Partnership NHS Trust’s
(The Partnership Trust) future plans and priorities, including its ambition to become
a Foundation Trust (FT). Most importantly, it asks for your views, ideas and support.

This document will help to explain the opportunities that
achieving Foundation Trust status will offer. It will explain
why we want to become a Foundation Trust and the
benefits Foundation Trust status can bring to our local
communities. It will also provide you with information
about the Trust and how being a Foundation Trust will
underpin our vision and plans for the future.
The Government wants all NHS trusts to become
Foundation Trusts by 2014 but regardless of this
deadline, only those that can prove they are well
managed and financially sound can apply. Our
readiness to undertake this challenging process is a
testament to the quality of our services and to the
commitment, care and skill of our staff.
The key difference of a Foundation Trust is that it is
accountable to local people and involves service users,
carers, staff and local residents in deciding the priorities
of the Trust. We believe that becoming a Foundation
Trust is the best way for us to fulfil our exciting plans for
community based health and social care services. It will
also allow us to be truly Ô better togetherÕ as it is the best
way for people with an interest in health and social care
services to become directly involved in influencing them.
Between July and October 2012, we will be consulting
with patients, service users, carers, staff, partners and
members of the public about our plans for future service
delivery and proposals to become a Foundation Trust.
Please take some time to look through this document.
It may answer many of your questions and if not will tell
you how to find out more. It also includes a freepost
form through which you can tell us what you think Ð
and apply to become a Member.
We think this is a wonderful opportunity for local people
and for health and social care services. We hope you will
agree and look forward to hearing from you.

Our Organisation

Staffordshire and Stoke on Trent Partnership NHS Trust was established on
1 September 2011, bringing together the provider arms of NHS North Staffordshire,
NHS Stoke on Trent and South Staffordshire Primary Care Trust (PCT).

In April 2012, Staffordshire Adult Social Services
successfully integrated into the Partnership Trust,
making it one of the largest Partnership Trusts in
England.
We are committed to providing high quality, safe and
effective services to patients and service users which are
simple to access.
We are located within the geographical boundaries of
Staffordshire County Council and Stoke-on-Trent City
Council and serve a diverse population of 1.1 million
people, covering a wide geographical area stretching
from the Staffordshire Moorlands which borders the
Peak District in the North to the conurbation of the
Black Country in the South.
Although largely rural we provide services in urban
centres such as Stoke-on-Trent, Leek, Burton upon Trent,
Cannock, Lichfield, Stafford, Tamworth and Wombourne.
Levels of deprivation are varied, however, Stoke-on-
Trent is in the 10 % most deprived areas. It is also
predicted that by 2020 the elderly population will
have risen significantly. All these factors will impact
on the demand for community health and social care
provision. We will be working closely with our partners
to understand this impact.
We employ approximately 6,000 staff and have an
annual turnover of £350 million.
We deliver services to a diverse population in a variety
of settings including community hospitals, clinics,
nursing homes, the community, prisons and in peopleÕ s
own homes. Our vast range of services include
district nursing, health visiting, nutrition and dietetics,
occupational and physiotherapy, school nursing, speech
and language therapy and an array of specialist teams.

Our Partners
Staffordshire and Stoke on Trent Partnership NHS Trust
has a broad spectrum of partners. In order to continually
improve our health and social care services, we link in
closely with our service users, carers, staff and partner
organisations such as Staffordshire’s police and fire
services and other NHS provider trusts in the region.
We also have strong links with local third sector
organisations, which provide support to service users
and carers in the local community Ð for example,
Community Health Voice and the Fifty Plus Forum.
Other NHS partners and key health commissioners (people
who pay us to deliver health services) are the Clinical
Commissioning Groups within the Staffordshire Cluster.
Our main commissioner for adult social care is Staffordshire
County Council through the Joint Commissioning Unit.
We also work closely with a number of other local
authorities covering the areas of Staffordshire
and Stoke-on-Trent and have relationships with
neighbouring Primary Care Trusts (PCTs). A small
number of our services are commissioned by
organisations beyond the Staffordshire border.
Through the development of a diverse membership and a representative council of governors (see p.19-24), we can work with our partners even more effectively with true local ownership of the Partnership Trust.

Hospital@Home
Baby Finley Shepherd was referred to the ChildrenÕ s
Hospital@Home service based in North Staffordshire just
before Christmas after struggling for breath as a result
of a cough.
Following a number of visits to his doctor it was
suggested the nursing team from Hospital@Home called
to see Finley and his mum Michelle at home.
Michelle said: “Finley is our first baby and sometimes he
was really struggling for breath. It was frightening to see
him like that and we were very concerned. The Hospital
@Home team came in morning and night and they were
absolutely brilliant. At the time Finley didnÕ t need to go into
hospital but the doctor didnÕ t want us to be on our own.
“Knowing someone would be visiting to check him or
on the end of the phone for reassurance was fantastic
and so reassuring.Ó
During 2011, the service was recognised nationally for
its exceptional efficiencies, delivered without sacrificing
their impeccably high standards of patient care.
Cathy Morris, Team Leader, said: “Our team provides
follow-up nursing support, in a childÕ s home, to children
who have experienced mild to moderate episodes of
minor illness.
“We work in partnership with the child, their parents or
carers and other professionals to enable the child to be
cared for at home with appropriate support and advice.
This prevents the child having to be admitted to Hospital
which causes anxiety for their families and in turn this
reduces the pressure on accident and emergency and
hospital wards.
Case Studies
Here are a couple of examples of the excellent level of care already being provided by Staffordshire and Stoke on Trent Partnership NHS Trust. We believe that by becoming a Foundation Trust we will have the freedom to develop our services in a way that will make them even better.

Musculoskeletal Service
Lynn Colley from Stoke was referred by her GP to the
Musculoskeletal Service based at Haywood Hospital as
she was experiencing a lot of pain throughout most of
her body. Lynn has been diagnosed with Fibromyalgia,
which is widespread musculoskeletal pain, the cause of
which is still unknown.
Lynn said: “The pain first started a couple of years ago,
just in my shoulder at first and I couldn’t move my neck.
It spread to my right arm and then my left one, then it
seemed to spread across my body. I had always suffered
from pain in my lower back, but this was very different
and my arm was swelling and I was in a lot of pain. My
appointment was great. It was quick but very thorough
and the staff were really professional and friendly.
“They talked me through what was wrong and assessed
what treatment would be best for me. In my case it will
be physiotherapy and advice, support and guidance on
how to manage my pain and manage my medication.
“I am just recovering from a hysterectomy, so I had to talk
through with staff when my treatment will start and what
will happen, but they have really put my mind at rest.Ó
Kay Stevenson, Consultant Physiotherapist said: “The
Musculoskeletal Service is a Ô one-stop-shopÕ for patients.
A variety of clinicians make up the team. They have rapid
access to equipment to quickly diagnose the problem,
including x-ray, MRI scanners and ultrasound. This means
we can assess the problem quickly and intervene to help
the patient, which may even be at the appointment itself
by giving medication, an injection or advice on how to
self-manage the pain.
“We have ensured that our clinicians are all appropriately
trained and supported. We listen to the patient, examine
them and suggest treatment in a short timescale. This
means less inconvenience and distress for our patients,
hopefully helping them to be independent and improve
their quality of life.Ó
Lynn added: “Other members of my family had talked
about the service, telling me how good it was, but now I
have experienced the friendliness for myself.Ó
Musculoskeletal Clinics are held in a variety of locations
across North Staffordshire including Haywood Hospital,
Longton Cottage Hospital, Milehouse Primary Care
Centre. Appointments are made through a referral by
your GP and may last up to 30 minutes. The patient could
be seen by any of our musculoskeletal practitioners, who
will ask about the problem, do an examination, request
appropriate investigation and suggest treatment.
Other members of my family had talked about the service, telling me how good it was, but now I have experienced the friendliness for myself.

Foundation Trusts are a specific type of NHS
organisation. They are more independent of
Government and have more financial freedom than
other NHS Trusts.
They are all accountable to service users, carers, staff and
the local community, through a membership made up of
these key stakeholder groups. They also have governors,
who are elected by the members or nominated by
partner organisations, such as local authorities. Through
members and governors, local people have a real say in
how the Foundation Trust operates and its priorities.
Foundation Trusts work in partnership with other
organisations to develop services that best meet the
needs of the local health communities.
Foundation Trusts remain part of the NHS, and continue
to provide services free to the people who use them, on
the basis of need not ability to pay. They are still subject
to the same regulations and standards as all other NHS
Trusts, including quality and effectiveness scrutiny by the
Care Quality Commission.
The Government has set up an independent body -
Monitor - to regulate Foundation Trusts, making sure
that they operate as they should and stepping in should
problems occur.
Foundation Trusts have more autonomy and freedom
for decision-making than other NHS organisations.
Therefore there is an extremely rigorous application
process before a Trust achieves Foundation Trust
status. Monitor is directly involved in this process and
they will examine our plans in detail.

What does Foundation Trust status mean?

Why do we want to become a Foundation Trust?Foundation Trusts have some major advantages in the way they are run, compared to other NHS Trusts.
In particular, becoming a Foundation Trust will ensure:
• Better…Accountability – Service users, carers, our
staff and the general public will have opportunity to
be directly involved in the running of the Trust.
• Better…Innovation – We will have greater
freedom to develop and innovate, using feedback
to design services to meet local needs. For example,
the Partnership Trust may wish to undertake
specialist research in a particular field, potentially
becoming national leaders in that area.
• Better…Quality – We will be able to use
information and involvement from the community
to further improve quality.
• Better…Investment – Should we work so
efficiently that we create a financial surplus, we will
be able to retain this at the end of the financial year
rather than it being taken back into a central budget
and reallocated nationally (as currently happens). This will give us more freedom to invest in our
services. For example:
- The Partnership Trust could save up surpluses
over number of years for a significant project
(building of new healthcare facility) rather than
budget being re-allocated annually
- The Partnership Trust could borrow money
providing it can show a sustainable and long-
term financial plan to achieve the level of surplus
required to pay this back. This gives scope
for bigger improvement projects to happen
immediately, depending on local need.
• Better…Viability – We will be able to focus on
efficiency and productivity to ensure that resources
are used effectively.
Why becoming a Foundation Trust will help
Although achieving Foundation Trust status is something
we are required to do by April 2014, we want to be clear
the emphasis will not be on completing this as a separate
programme of work or Ô tick box exerciseÕ .
We want to become a better organisation and to operate more
effectively on a day-to-day basis. We believe that by becoming
a Foundation Trust and adopting a new way of working
throughout the organisation, we can achieve this ambition.
Foundation Trust status will enable us to build on our
core vision and values because we will be able to use
the financial and operational freedom we will have as a
Foundation Trust to work differently. For example, with
more financial freedom, we can develop new services,
or deliver existing services in new ways.
We can also be more flexible in our relationships with
other organisations and engage in innovative partnerships
to deliver services in the most effective and equitable way.
Most importantly, we will be able to engage more
effectively with our communities to meet existing and
emerging needs.
On the next page are some comments from a service user, carer and member of our clinical staff on how they view our plans to become a Foundation Trust.

We would like to hear your views on becoming a Foundation Trust.
My life
changed dramatically after I underwent
brain surgery to remove a tumour and I have been a service user
for over six years now. Three months after my operation it was discovered I had
suffered a stroke during surgery.
I knew something terrible had happened but felt like no one was listening to me. ThatÕ s when a
lifeline came into my painful, fearful life Ð I was visited by a stroke specialist nurse called Rita Tranter. Rita
helped me in my battle to obtain a diagnosis and at times IÕ ve felt like she has been the only constant with me on
this long and difficult journey.
I was eventually found to have central post stroke syndrome (thalamic pain). I still have days where I am in a lot of pain
and absolutely exhausted but when my husband and I feel stuck Rita always comes up with an answer.
ItÕ s great to have someone our lives that we can rely on.
I believe that achieving Foundation Trust status will help improve all aspects of care provided by Staffordshire and
Stoke on Trent Partnership NHS Trust. I feel very strongly that all members must be listened to and should be
able to voice their opinions in a variety of ways as leaflets can be confusing to some people. This should
include face-to-face contact with not only service users but their partners as well, many of whom
have become full-time carers and need consideration and support.

I am a carer to my husband and I am still finding my way around the
system. I think achieving Foundation Trust status will be a good thing, but itÕ s going to be important to reassure people that the standard of care will remain high and ensure
service users and carers understand how changes may affect them.
Providing the level of care continues to improve this could be positive. In particular, having a membership seems a very exciting idea and I think members of the public will want to get involved. I would definitely like to know more
about this as I feel it would help the local stroke group I attend in Tamworth. I would certainly like to get involved and put my degree to some use.

Achieving Foundation
Trust status will be enormously beneficial to our service users and

aspect of our services or proposals, but we are particularly
seeking your views on:
Our consultation will run from July 2012 to October
2012. During this period, we will explain our plans, listen
to your opinions and answer your questions.
During our consultation we will be out and about in
the community talking to local people. We will also
hold public meetings to provide information about our

Our aim is to fundamentally transform the way care is delivered to local people with an integrated and
proactive approach to supporting people with long term conditions, coupled with a genuine shift in
power to the patient or user, making shared decision making a reality.
Our Better Together Transformation Programme will introduce a new Model of Care across
Staffordshire and Stoke-on-Trent which will enable us to achieve our vision of delivering personalised
care of the highest quality with the best possible outcomes for users and carers. Its key focus is to
improve:

Our Future Plans and PrioritiesAs one of the largest providers of community health and adult social
care, we are in a strong position to be at the forefront of services
delivery to improve patient and service user experience and care.

• Identification of patients and service users
most likely to use NHS or adult social care
services in the future so we can proactively
manage their condition before they need to
access emergency care
• Integrated Health and Social Care Teams to
deliver the care required via a single key worker
• A Single Point of Access / Care will provide
one single contact point into the organisation
which links to care co-ordination and allows
us to direct any service according to the needs
of the service user or client. This is expected
to be available 24 hours a day, giving a clear
route into our services at all times.
• A Standardised Assessment Process and
simplified care planning are essential to building
integrated care plans around the individual
needs of our service users. This new process will
stop duplication of paper work and will save
staff and patient or service user time.
• We know we could be more productive if we
made data entry easy through remote devices.
Good IT remote working will also improve
communication between team members and
as a result deliver a better experience for the
service user.
• We are working closely with our commissioners
to implement a new model of clinical cover
for our five community hospitals. We
are committed to providing safe care and
improving the opportunities for people
to access local care which is focussed on
maintaining independence and rehabilitation.
Key projects are already underway to support the implementation of the new Model of Care. These include:
What does it mean for you – the patient or service user?
The new Model of Care proposed will
• increase independence
• improve experience

• deliver value for money

Staff will also be able to spend more time with patients
and service users as necessary and increase the number of
patients and service users they see. It is envisaged that, on
average, staff will be able to undertake between one and
two additional contacts per day.
It is this level of excellence that will play an integral part in
successfully becoming an Foundation Trust.

Our Membership ProposalsAs an aspirant Foundation Trust, we are developing a membership of local people, service users and carers, staff and
partners. Our membership will reflect the diverse communities we serve and members will be able to offer their comments
and views on Trust priorities, service developments and plans. The local community will therefore have a direct influence on
what the Trust does.
Membership Criteria
We are proposing to have three membership categories with no limits on the number of people who can register.
Our Public Category
You are eligible to be a member of our public category if you are over 16 years of age (there will be no upper age limit)
and a resident of one of the following constituencies:
Our Staff Category
You are eligible to be a member of our staff category If
you are:
• on a permanent contract
• on a fixed contract of at least 12 months or have
worked for the Trust for 12 months on a
temporary contract
• on a secondment of over 12 months at the Trust
• exercising functions for the purposes of the Trust e.g.
volunteers can become staff members due to their
contribution to services
We are proposing an Ô opt-inÕ scheme where staff will be
asked to make an active choice to become members of the
Trust. We feel this matches our commitment to ensuring
active staff engagement in our activities. Staff may change
their mind and opt-in or out at any time.

We are passionate about involving people with real
experience of using our services. Therefore, we are
proposing that in addition to a category for public
members, we have a specific category for patients, service
users and carers.
This category will include anyone who has been or who is
currently a service user. It will also include people who care
or have cared for someone using the TrustÕ s services.
The patient, service user, carer category will operate under
the same age restrictions as the public category. Once a
person has become a service user or carer they will remain
within this membership category even if they cease to use
services, unless they contact the membership office stating
that they would like to alter their membership status. This
is due to the additional insight people gain from having
interacted with services and the
value they could add to certain
involvement activities.

Our Council of Governor ProposalsAs an aspirant Foundation Trust, we will put in place a Council of Governors, the
majority of whom will be elected by members and some will be appointed from our
partner organisations. The Council of Governors will be available for consultation
and will represent the interests of members when providing views to the Board of
Directors. The Trust Chairman will chair the Council of Governors.

1. NHS Foundation Trusts will be part of the NHS and subject to NHS standards, performance ratings and systems of inspection. They will treat NHS patients according to NHS quality standards and principles - free care based on need, not ability to pay.
2. NHS Foundation Trusts will be established in law as independent public benefit corporations. This will mean far greater local ownership and involvement of patients, the public and staff rather than control from the Department of Health. The principles behind NHS Foundation Trusts build on the sense of ownership many local people and staff feel for their health and care services.
3. NHS Foundation Trusts will be democratic. Local people and staff will directly elect representatives to serve on the Council of Governors. The Council of Governors will work with the board of directors - responsible for day-to-day running of the Trust - to ensure that the NHS foundation trust acts in a way that is consistent with its terms of authorisation. In this way, the Council of Governors will play a role in helping to set the overall direction of the organisation.
4. NHS Foundation Trusts will not mean privatisation of the NHS. They will be required in law to use their assets - such as land and buildings - to promote their primary purpose of providing care to service users. A legal lock will protect these organisations from the sort of Ô de-mutualisationÕ we have seen in the Building Society sector and prevent any threat of future privatisation.
5. NHS Foundation Trusts will operate within a clear accountability framework. They will not be left to sink or swim, allowed to Ô cherry pickÕ services or set loose to pursue organisational goals at the expense of the needs of their local communities.
6. NHS Foundation Trusts will be there to treat service users, not to make profits or to distribute them. Most of their income will come through agreements reached with local Clinical
Commissioning Groups to provide locally relevant services care to service users at the national tariff rate. Private work will be strictly limited.
7. NHS Foundation Trusts are at the cutting edge of the Government’s commitment to devolution and decentralisation in the public services. They will not be subject to direction from Whitehall. Local managers and staff working with local people - rather than remote Civil Servants - will have the freedom to innovate and develop services tailored to the particular needs of their local communities.
8. NHS Foundation Trusts are not about elitism. All NHS trusts will get help and support so they too are in a position to apply for foundation status.
9. NHS Foundation Trusts will work in partnership with other NHS organisations. They will remain part of the NHS. They will have a duty in law to cooperate with other local partners using their freedom in ways that fit with NHS principles and are consistent with the needs of other local NHS organisations. They will be overseen by the Independent Regulator, Monitor, who is accountable to Parliament, and they will be inspected by the Healthcare Commission.
10. NHS Foundation Trusts will be able to direct their services more closely to the communities they serve with freedom to develop new ways of working that reflect local needs and priorities. This will be done within the NHS framework of standards and inspection that safeguards the quality of NHS care. Direct elections of Governors by local people and staff will ensure local health and social care is better focused on meeting the needs of local communities.

Board of Directors The Board of Directors of our
Foundation Trust will run the organisation day-to-day. It
will be chaired by the Chairman of the Trust, Nigel Ratcliffe,
and will comprise of our most senior staff including the
Chief Executive and seven other Executive Directors. In
addition, the Board will have four Non-Executive Directors.
These are people who do not work full-time for the Trust,
but bring a wealth of experience from other walks of life.
Council of Governors Having a Council of Governors will
be completely new to us if we become a Foundation Trust.
The Council of Governors does not run the Trust day-to-
day, but takes a very important role in scrutinising plans
and agreeing key decisions. The Council will also be a main
channel of accountability to local communities. It will be
made up of Governors elected by public and service user/
carer members (the majority); by our staff members; and
non-elected Governors appointed by partner organisations.
An important question for this consultation is around the
composition of the Council of Governors.
Care Quality Commission (CQC) The Care Quality
Commission is a Government agency which oversees the
quality of services within the NHS. Even if we become a
Foundation Trust, we will still be inspected and expected to
meet the same standards as all other Trusts.
Integrated Business Plan (IBP) The Integrated Business
Plan (IBP) is the central document to our Foundation Trust
application. It is a costed plan of our long-term strategy
and what we hope to achieve as an organisation. The areas
we see as priorities for our future will be written into the
IBP, so this consultation exercise is seeking your views on
these priorities.
Monitor Monitor is an organisation set up by the
Government to oversee all Foundation Trusts. It is known
as a Ô RegulatorÕ . Ultimately, Monitor will have to agree
our application if we are to become a Foundation Trust. If
we are successful, Monitor will continue to work with us,
ensuring that our plans remain on track.
Commissioners All Trusts are funded by commissioners,
who agree with us the health and social care needs of our
communities. They are the route by which the Government
finances health and social care. The Partnership Trust’s key
health Commissioners will be the Clinical Commissioning
Groups (CCGs) within the Staffordshire Cluster. In
addition to this the Partnership Trust provides services to
local authorities and neighbouring Primary Care Trusts
(PCTs) and a number of services are commissioned by
organisations beyond the Staffordshire borders. Our
main commissioner for social care is Staffordshire County
Council through the Joint Commissioning Unit.
Primary Care Trust (PCT) Primary Care Trusts (PCTs) are
the part of the NHS which agrees with Trusts how much
money they need to provide services. Our commissioners
provide the payments which enable us to run as an
organisation and buy the supplies we need. However, due
to recent NHS reforms the role of commissioner will soon
be taken over by Clinical Commissioning Groups.
Clinical Commissioning Groups A Clinical
Commissioning Group (CCG) is the name for the new
health commissioning organisations which will replace
Primary Care Trusts in 2013. The key difference between
a PCT and a CCG is that the commissioning of services
will be led by clinicians instead of managers. This is due to
clinicians being better placed to understand what service
users want and need. The Staffordshire CCG Cluster is
made up of seven CCGs which will commission services
covering the entire county. The Partnership Trust is working
closely with its CCGs to ensure the best quality care is
provided to service users.
Stakeholders As well as the NHS, we work with many
other agencies. Good health within the community
relies on many agencies working together, for example:
local authorities, housing providers, education, and
voluntary sector organisations. Our stakeholders include
organisations such as councils, universities, StaffordshireÕ s
police and fire services. As part of this consultation we will
be talking to many of these organisations so their ideas can
help shape our plans.
