National Diabetes Audit: Submitting Data on the Clinical Audit Platform Step 1: Log into your NHS Digital Account In order to access the Clinical Audit Platform and submit data, you must start by signing into your SSO account. Enter your Username (your registered email address), your password and click ‘Sign in’. The email address used for SSO must be the same as the one submitted on the User Registration Form. If these email addresses are not the same, you will not be able to access the Clinical Audit Platform. Please note this guide is for submitting individual records into CAP. For uploading bulk files, see the bulk file upload document. The ‘Add / Search for Patient Record’ screen is displayed. Enter the NHS Number and the Date of Birth of the patient. Click ‘Submit’. The ‘Patient Information’ screen will be displayed. Note: If the patient already exists in the NDA database, the patient’s record tree will be displayed, with the Step 4: Complete the Patient Information Complete the patient record by entering the Postcode and select the patient’s gender and ethnicity from the drop down list and enter Date of Diagnosis.

National Diabetes Audit: Submitting Data on the Clinical Audit Platform

Step 1: Log into your NHS Digital AccountIn order to access the Clinical Audit Platform and submit data, you must start by signing into your SSO account.

Enter your Username (your registered email address), your password and click ‘Sign in’.

The email address used for SSO must be the same as the one submitted on the User Registration Form. If these email addresses are not the same, you will not be able to access the Clinical Audit Platform.

Before you startSubmitting data to the National Diabetes Audit (NDA) is done via the Clinical Audit Platform (CAP); a secure, web-based platform supported and hosted by NHS Digital. Please note this guide is for submitting individual records into CAP. For uploading bulk files, see the bulk file upload document.

In order to submit data to the collection, you must first register by creating an NHS Digital Single Sign On (SSO) account and submitting the NDA User Registration Form.

Step 2: Access the National Diabetes Audit CollectionOnce you are signed in click on ‘National Diabetes Audit’ under ‘My Available Audits’.

The NDA Home screen will bedisplayed.

Step 3: Add Patient RecordIn order to create a new patient record, click on ‘Add/Search for Patient Record’ at the top of the screen.

The ‘Add / Search for Patient Record’screen is displayed.

Enter the NHS Number and the Date of Birth of the patient. Click ‘Submit’.

The ‘Patient Information’ screen will be displayed.

Note: If the patient already exists in the NDA database, the patient’s record tree will be displayed, with the option to edit the Patient or Care records.

Step 4: Complete the Patient InformationComplete the patient record by entering the Postcode and select the patient’s gender and ethnicity from the drop down list and enter Date of Diagnosis.

To enter GP Practice Code, select the field and search for the correct GP practice by either GP Practice Code or Name. Click on the correct GP Practice in the search results to make your selection. Click on ‘Submit’ to save the record.

Step 5: Add Data to the Record TreeOnce the patient record has been entered, the Record Tree will be displayed.

Click on ‘Add Record’ next to NICE Processes of Care which will open up the options to enter data for the 5 NICE Care Guidelines.

Click Serum Creatinine tab, enter Level and click Submit. Enter date of observation and click Submit. The Serum Creatinine level must be between 20 and 1000.

Click the Urinary Albumin tab and enter Level (which will bring up the date field) and select testing method and stage from the drop down options and click Submit:Urinary Albumin Level must be in the following format: 0000.00

Click Eye Examinations tab and select whether a review was carried out using the drop down option. Enter the Observation Date and click Submit:

Click Smoking Status and choose most appropriate option from the drop down list, enter the Observation Date and click Submit:

Click Foot Examinations and select whether a review was carried out using the drop down option. Enter the Observation Date and click Submit:

Once you have submitted any or all of the 5 items, it will display the message below:

The observation dates of the records you created will appear on the Record Tree. You are able to edit or delete this record if necessary:

If you wish to add a new record for all items or just an individual process, the record(s) will be displayed with the most recent update at the bottom of the list:

Click Smoking Status and choose most appropriate option from the drop down list, enter the Observation Date and click Submit:

5.1 - Add HbA1c informationClick ‘Add Record’ to add HbA1c information:

Add HbA1c level and Observation Date and click Submit: HbA1c level must be between 1 and 300

A confirmation message will be displayed on screen to confirm you have successfully submitted the record:

Once you have clicked Submit, the Observation Date, with the option to Edit or Delete the record will be displayed on the Record Tree:

If you wish to add a new record, each individual record will be displayed with the most recent entry at the bottom of the list:

5.2 - Add Blood Pressure informationClick ‘Add Record’ to add Blood Pressure information:

Add figures for Systolic and Diastolic Blood Pressure and the Observation Dates for each, and click Submit:

Systolic BP must be between 80 and 250, Diastolic BP must be between 30 and 150

A confirmation message will be displayed on screen to confirm you have successfully submitted the record:

Once you have clicked Submit, the Observation Date, with the option to Edit or Delete the record will be displayed on the Record Tree:

If you wish to add a new record, each individual record will be displayed with the most recent entry at the bottom of the list:

5.5 - Add Insulin Pump informationClick ‘Add Record’ to add Insulin Pump information:

Select from the drop-down box whether the patient is using an Insulin Pump. If Yes is selected, complete the Observation Date and other fields as displayed on screen:

If No is selected, enter the Observation Date. If the patient was not previously on a Pump, select No and click Submit:

5.6 - Add Insulin Pump information - contIf the patient was previously on pump, select Yes and enter the Year the patient stopped using it:

A confirmation message will be displayed on screen to confirm you have successfully submitted the record:

Once you have clicked Submit, the Observation Date, with the option to Edit or Delete the record will be displayed on the Record Tree:

If you wish to add a new record, each individual record will be displayed with the most recent entry at the bottom of the list:
