We found the session to be most informative and stimulating, and look forward to continuing this tradition of discussing issues of mutual concern. While minority recruitment and the declining applicant pool were expressed as major concerns, the large group gathered at the meeting that evening generally agreed that curriculum review was of the utmost importance. The following were identified as five key issues. (1) Need to define the product of the four years of medical school Amidst a heated discussion about medical education, it became apparent that medical educators were unsure of exactly what we hoped to transform the ideal student into within four years of medical school. Without a clear definition of the "end-product" of the medical school process, it was hard to analyze or review the educational process itself. (2) Need to define the critical minimum of information (core material) With the geometric explosion of information in medicine today, it was agreed that a concomitant expansion of curriculum could not be fit into the four-year medical school experience. It seemed imperative to identify "critical information and skills" that the medical student needed. This "core material" would then serve as a strong foundation with which the young physician could keep up with new medical breakthroughs during his or her lifetime. ( 3 ) Problem-oriented Examinations Multiple-choice examinations encourage pattern recognition, not necessarily a knowledge of the concepts. Several faculty members reported that students frequently experience "intellectual bulimia" with binging and purging of facts around the time of examinations, and no recollection of these facts post -exam. The group felt that methods of evaluation that stressed problem-solving skills would be beneficial to students. Specifically, essay -type questions could be encouraged even if accompanied by transient shifts in National Board Scores, because effective communication and conversance with concepts would serve a student well in medicine.

SUBJ: Issues raised during the joint Fireside Chat with COD, GSA, GME and OSRat the 1989 Spring AAMC Western Regional Meeting

The OSR members of the Western Region really enjoyed the provocative discussion we recently hadwith the Medical Dean, Student Affairs Deans, and representatives of the Group on MedicalEducation at the AAMC meeting at Asilomar on Monday, April 24, 1989. We found the sessionto be most informative and stimulating, and look forward to continuing this tradition of discussingissues of mutual concern.

While minority recruitment and the declining applicant pool were expressed as major concerns, thelarge group gathered at the meeting that evening generally agreed that curriculum review was of theutmost importance. The following were identified as five key issues.

(4) Reducing the Deleterious Effects of National Board Part I Scores on the PreclinicalCurriculum It is recognized that National Boards Part I, originally designed as a "licensure exam",measures several factors in addition to the student's knowledge of the basic sciences. Foreexample, students from schools with a month off for "studying for the boards" with formalBoard-review courses undoubtedly have a significant advantage over students given a weekoff with no structured review courses, often reflected in scores. Additionally, theoveremphasis on Part I scores dictates the depth and focus of the preclinical curriculum inmost medical schools, and leads to a dichotomy between the preclinical and clinical years withan inflexible preclinical curriculum with no room for growth. The group recognized theimportance of a national form of medical student evaluation, but it was felt that othercreative ways of evaluating a student's aptitude and knowledge for medicine should bepursued.

(5) Addressing the Psychosocial Aspects of Medicine While medical schools aspire to train competent physicians with desirable personal "caring"qualities, the psychosocial aspects of medicine are frequently not addressed in the four-yearmedical school curriculum. It was felt that courses in Bioethics, Proper Bedside Manners,Social Aspects of Medicine, Diversity in the Patient Population and their differentexpectations of medical encounter, etc. are important components of the medical curriculum.

The discussion was unexpectedly lively and productive. Several other issues were discussed byu thislarge audience, and some solutions were debated.
